DENTAL TREATMENT IN A PATIENT WITH PIERRE ROBIN SYNDROME UNDER GENERAL ANESTHESIA : A CASE REPORT (피에르 로빈 증후군 환아의 전신마취 하 치아우식 치료 증례 보고)

  • Pierre Robin syndrome (PRS) is characterized by the triad of congenital mandibular hypoplasia, glossoptosis and cleft palate. Infant PRS patients are frequently suffering from upper airway obstruction, gastroesophageal reflux and growth retardation caused by above mentioned problems. We report a dental caries treatment of 3-year old girl with Pierre Robin syndrome with multiple caries. The cause of multiple caries was mainly presumed as patient's eating habit caused by her general condition. She had some feeding problems and had history of gastric tube. She was still using milk bottle and took more than an hour to finish a meal. The treatment was performed under general anesthesia considering patient's condition; mild autism, poor cooperation and respiratory problem due to micrognathia. Severely affected upper incisors were treated with pulp treatment and restored with zirconia crown for esthetic purpose. Lower incisors were treated with pulp treatment and restored with composite resin. Upper right first primary molar was restored with stainless steel crown and other primary molars were treated with composite resin. There were no postoperative complications. According to her parents, the patient's compliance to oral hygiene management was greatly improved after the treatment since she was very pleased with the esthetic result and highly motivated by her looks. The treatment without sedation or general anesthesia would be possible once the airway is improved as the mandible grows.

CORRELATION OF CARIES ACTIVITY BETWEEN MOTHERS AND CHILDREN WITH CARIOGRAM AND EVALUATION OF CARIES RISK FACTORS (Cariogram을 이용한 모자간 우식활성도의 상관성 및 우식위험요소 평가)

  • Dental caries during infantile and early childhood period is a complex disease resulting from multiple caries factors. Streptococcus mutans(S. mutans) plays a critical role in the initiation of caries, and its early transmission through mothers provides a strong etiologic factor for future development of caries in the primary dentition. Therefore, early detection of caries risk factor is important for prevention of caries. Recent studies about etiologic factors of caries have brought advent of various tools for caries risk assessment in order to predict progress of caries more accurately. Cariogram is a common tool for caries risk assessment, which illustrates present caries risk assessment and correlation of caries risk factors for an individual graphically. The aim of this study was to assess if there is any correlation in the level of S. mutans and caries activity and to verify the effect of caries risk factors between children with age ranging from 3 to 5 years with severe early childhood caries(S-ECC) and their mothers using caries-related salivary test and Cariogram. The results of this study were as follows. 1. Children with S-ECC had a statistically higher level of Streptococcus mutans compared to caries-free children(p<0.05). 2. There was significant correlation between mothers and their children in the aspect of the level of Streptococcus mutans(p<0.05). 3. When caries activity was assessed using Cariogram, significant correlation between mothers and their children was found(p<0.05). 4. When each caries risk factor was evaluated for its affect on caries activity, host was more influential factor compared to microorganism and diet in children. Based on these results, we could conclude that assessing the level of S. mutans can be an effective tool for predicting possibility of caries formation in future. Since prediction of future caries progression and evaluation of caries risk factor became possible with Cariogram, we may conduct early and preventive measures for treatment of caries.

  • Moyamoya disease is a disorder in which certain arteries in the brain are constricted. Blood flow can be blocked by the constriction and blood clots. The patients frequently experience transient ischemic attacks (TIA), cerebral hemorrhage, or may not experience any symptoms at all. It is reported that they have a higher risk of recurrent stroke and a distinct underlying pathophysiology. A 3-year-8-month old boy with moyamoya disease experienced cerebral infarctions five times, and he underwent a cerebrovascular anastomosis surgery four years ago. He showed swallow disturbance, general delayed development, hemiplegia, and strabismus. Also he had hypocalcified teeth with or without multiple caries lesions in all dentitions. Dental treatment under general anesthesia using sevoflurane was performed due to his lack of cooperation. Moyamoya disease is associated with various medical conditions requiring a thoughtful deliberation and a careful examination before and during dental treatment. Pain and anxiety control during dental treatment is important because hyperventilation induced by crying has been seen to trigger TIA. Both isoflurane and sevoflurane are commonly used in patients with MMD, but dynamic autoregulation is better preserved during sevoflurane than isoflurane anesthesia. So sevoflurance general anesthesia may be recommendable to manage dental patients having multiple caries with moyamoya disease.

  • Lennox-Gastaut syndrome (LGS) is a type of childhood epilepsy, and this syndrome can compromise dental treatment. LGS represents variable etiologies, multiple types of intractable seizures, and cognitive impairment in most patients. A 7-year-4-month old girl with LGS was referred to our hospital for dental treatment under general anesthesia. Clinically, the patient presented with multiple dental caries. The patient had been prescribed a ketogenic diet for the management of repetitive seizures. In addition, she experienced severe types of hypoxia during medical examination using chloral hydrate. Therefore, dental treatment was performed under general anesthesia. LGS patients have a high risk of seizure during dental treatment and can induce a lot of complications. Dental treatment under general anesthesia for LGS patients can be carried out careful preoperative evaluation and in cooperation with medical care.

  • Atypical teratoid rhabdoidal tumor(AT/RT) is a rare and fast-growing tumor usually diagnosed in childhood. Routine magnetic resonance imaging(MRI) assessment of AT/RT patients is done to detect metastatic tumors and recurrent lesions. The purpose of this case report is to present a case about caries treatment of a 3-year-8-month old female patient with AT/RT under general anesthesia. A 3-year-8-month old patient with AT/RT visited Seoul National University Dental Hospital for caries treatment. At the first dental visit, she was receiving chemotherapy. Multiple caries were observed from clinical and radiographic examination. The dental procedure was successfully performed under general anesthesia. Patients with brain tumor may take routine MRI to detect metastatic tumors and recurrent lesions. Dentists should take into consideration of possible dislodgement of restoration by ferromagnetic properties and artifact formation on MRI image. Ready-made zirconia crowns can be a good restoration option for such patients.

  • Objectives : This study was performed analysis of Halitosis components following by subjective cognition of halitosis and Oral State in order to provide basic data for halitosis prevention and establish a device to efficiently eliminate halitosis and analyze the factors that affect the halitosis. Methods : 80 adults were participated who visited at the Dental Clinic in Kyoung-gi do, on the period of the March to September in 2010. The obtained results, through the oral examination, halitosis check and interview, Individually, were as followings. Results : According to the ammonia level was a high relationship between sex, CPI, Tongue plaque, DMFT index which were statistically significant (p<0.05). Total oral gas level was a relationship between VAS, tongue plaque, DMFT index(p<0.05). The Correlation coefficient between the Hydrogen sulfide level and Methyl mercaptan was estimated as r=0.534(p<0.01). As for the multiple regression analysis result, there have been selected dimethyl sulfide and Tongue plaque, Gingival index as factors affecting Bridge, Methyl mercaptan level is as factors affecting Hydrogen sulfide level. Conclusions : There was relationship between the periodontal disease and Ammonia gas component, hydrogen sulfide, dimethyl sulfide, methyl mercaptan. Which were statistically significant total oral gas level and oral variables. Such dental caries as the prevention or early treatment of periodontal disease.

  • The authors have interpreted one patient's full mJuth intra-oral films, oblique- lateral film of the left mandible and orthopantomograph which revealed 6 radicular and 1 residual cysts. As a results of interpretation of these serial films, we have drawn following conclusions: 1. Radicular cyst arose from the cell rests contained in an apical granuloma which was sequel to advanced pulpitis due to dental caries. 2. Residual cyst was developed from remaining cell rests after the extraction of a tooth with such a radicular cyst or apical dental granuloma. 3. Cyst grew in size by absorption of fluid into cystic cavity due to difference in osmotic pressure between the cystic fluid and adjacent tissue fluid.

  • Lennox-Gastaut syndrome (LGS) is a severe form of childhood epilepsy that is defined by generalized multiple type seizures, slowness of intellectual growth, and a specific EEG disturbance. Children affected might previously have infantile spasms or underlying brain disorder but etiology can be idiopathic. LGS seizures are often treatment resistant and the long term prognosis is poor. A 14-year-3-month old, 20.5 kg girl with LGS was scheduled for dental treatment under general anesthesia. The patient presented with multiple caries and heavy calculus. General anesthesia was induced and maintained with inhalation agent, Sevoflurane. And caries treatment, scaling, subgingival curretage were performed. LGS patients usually have poor oral hygiene due to mental retardation and have high risk of seizure during dental treatment. In dental treatment under general anesthesia for LGS, it must be considered of seizure control during peri-operative period. Also periodic recall check, instruction of oral hygiene must be performed.

The correlation between soft drink consumption and subjective oral symptoms in Korean adolescents (우리나라 청소년의 탄산음료 섭취와 주관적 구강증상과의 관련성)

  • Objectives: The purpose of this study was to investigate the correlation between soft drink consumption and subjective oral symptoms in Korean adolescents. Methods: Data of 65,528 adolescents were derived from the twelfth Korea youth risk behavior web-based survey, which was conducted from 1st June 2016 to 30th June 2016. Sociodemographic factors, dietary habit, and oral health behavior were collected as independent variables. The subjective oral symptoms were used as a dependent variable. The chi-square test and logistic regression analysis were performed to identify the correlation between dental caries and the other variables. Results: A multiple logistic regression analysis revealed that subjective oral symptoms were correlated with sex, age, subjective economic status, soft drink consumption, sweet drink consumption, tooth brushing frequency and dental sealant experience. Conclusions: Since the intake of soft drink adversely effects subjectively perceived oral symptoms in Korean adolescents, oral health education should include information on the pH levels of food and beverages including carbonated drinks, the effects of sugar on the teeth, and precautions to preventing tooth damage.

The relationship of snack habits, oral health behavior and oral health status in psychiatric patients (일부 정신병원 입원환자의 구강건강상태와 구강건강행위 및 간식습관과의 관련성)

  • Objectives : The purpose of the study is to investigate the relationship between variables associated with oral health status in psychiatric patients and to provide basic information on oral health education program development for the psychiatric patients. Methods : Subjects were 151 psychiatric patients who filled out the structured questionnaire. The data were analyzed for independent t-test, one-way ANOVA and hierarchical multiple regression by using SPSS Win Program 18.0 version. Results : Higher DMFT index was shown in the older age(p<0.001) and low perceived oral health group(p<0.001). Higher DMFT index included no tooth brushing after breakfast(p=0.045), wrong tooth brushing method(p<0.001). Chewing discomfort(p=0.027) and denture(p=0.027) were closely related to higher DMFT index. Bread(p=0.033) and snack(p<0.001) increased DMFT index. Factors affecting oral health status were age, denture, snack and bread. The explanation power of the final model was 29.4%. Conclusions : Dietary control for the psychiatric patients is very important to prevent dental caries. Tooth brushing and use of oral hygiene devices can improve good oral health care. Furthermore, this result can improve the oral health care for the disabled people.
