Chronic infantile neurological cutaneous articular (CINCA) syndrome periodically causes fever along with inflammation in multiple organs. Patients with this condition are vulnerable to dental problems due to systemic inflammation. For uncooperative patients, general anesthesia has been widely used to control negative behavior. However, caution should be exercised when administering general anesthesia in these patients because this syndrome is pro-inflammatory. The present case report describes the clinical considerations of the dental treatment of an uncooperative child with CINCA syndrome who was treated under general anesthesia.
Objectives : The purpose of this study was to investigate the factors related to the awareness level of community water fluoridation program. Methods : 700 subjects were surveyed among the residents living in Incheon Metropolitan City. Data were collected on awareness of community water fluoridation program, general factors, oral health behavior, self-reported oral health and oral health interest. The factors related to the awareness level of community water fluoridation program were analyzed by t-test, a one way ANOVA and multiple linear regression analysis. Results : 1. Subjects' ages were 40~59 years, monthly average incomes were more than 3 million won and higher their educational levels had a higher awareness level of community water fluoridation program(p<0.01). 2. People who brushed their teeth more than three times a day, used oral hygiene devices, and had periodic oral examination and removed plaques were had higher awareness level of community water fluoridation program(p<0.01). Especially, the use of oral hygiene devices was the strongest factor in relation with the awareness level of community water fluoridation program(p<0.01). 3. People who had a fine self-reported oral health, highly interested in dental hygiene and made an effort to keep oral health were had higher awareness level of community water fluoridation program(p<0.001). Conclusions : The awareness level of community water fluoridation program was related to oral health behavior, self-reported oral health and oral health interest. So, in order to expand the areas for community water fluoridation program, it is important to educate to the right information about objectives, safety, dental caries preventing effect of the community water fluoridation program. And the education and publicity on general oral health will have an affirmative effect on expanding community water fluoridation program.
The aim of this study was to analyze the factors that were associated to the approval for the community water fluoridation program. The subjects of this study were 751 mothers of elementary school student from September 1 to 30, 2012. Data were analyzed with chi-square, and multiple logistic regression analysis SPSS 12.0. Respondents approving and opposing for the implementation of water fluoridation program were 50.3% and 10.4%. The most associated factors of approval for the community water fluoridation program was recognition of water fluoridation program (odds ratio [OR], 2.98; 95% confidence interval [CI], 1.14~7.76), which was followed by length of residence (OR, 2.49; 95% CI, 1.39~4.47), and recognition of sealant (OR, 1.88; 95% CI, 1.02~3.50) in the order. And the approval opinion had relationship with district of residence. It is recommend that construction of public service system on education and information in order to most peoples can confidence the necessity of community water fluoridation program.

Cyclic neutropenia is a hematologic disorder characterized by a marked decrease in the number of circulating neutrophils occurring at regular intervals and after this period, the level of neutrophils usually recovers to a normal range. The clinical symptoms of cyclic neutropenia include fever, malaise, headaches and oral findings associated with painful soft tissue ulceration where lips, tongue and gingiva are typically involved. A 4 year 1 month old boy was presented to the hospital. His chief complaint was mobility of his teeth and swollen gums. The patient had suffered from cyclic neutropenia. Clinical examination revealed evident decay on all primary teeth except for the mandibular anterior teeth and localized alveolar bone loss around mandibullar right and left first primary molars which have mobility was notable. The patient was diagnosed with multiple dental caries, gingivitis and localized periodontitis associated with cyclic neutropenia and treated based on it. The dental treatment, including regular tooth care and appropriate treatment of dental caries or gingivitis, is essential for patients suffering cyclic neutropenia. Especially, proper care at an early stage is needed for young patients to minimize the unwanted consequences for permanent teeth development.

Lowe syndrome, also known as oculocerebrorenal syndrome, is a rare genetic disorder involving eyes, kidney and nervous system, and occurs predominantly in mostly males. The patients with Lowe syndrome are characterized with prominent forehead, thin and sparse hair, protruding ears, congenital cataracts, glaucoma, mental retardation, stunted growth, hypotonia, decrease in muscle mass and tendon reflexes, renal tubular dysfunction, and metabolic bone disease. A 6-year-old boy with Lowe syndrome was admitted to our clinic, with multiple caries and a chief complaint of intermittent pain on the left mandibular molar area. Because of difficulty in management of behavior and his medical problem, general anesthesia was performed for dental care. No specific complication was noticed during dental treatment procedure under general anesthesia and also during periodic recall-checks. General anesthesia itself, however, could be a potentially life-threatening procedure due to patient's biomedical problems. When a dental procedure under general anesthesia is to be required in patient with Lowe syndrome, it may be advisable being teamed with physicians, and general anesthesia duration should be as short as possible.

PURPOSE. The purpose of this study was to evaluate the effect of provisional cement removal by different dentin cleaning protocols (dental explorer, pumice, cleaning bur, Er:YAG laser) on the shear bond strength between ceramic and dentin. MATERIALS AND METHODS. In total, 36 caries-free unrestored human third molars were selected as tooth specimens. Provisional restorations were fabricated and cemented with eugenol-free provisional cement. Then, disc-shaped ceramic specimens were fabricated and randomly assigned to four groups of dentin cleaning protocols (n = 9). Group 1 (control): Provisional cements were mechanically removed with a dental explorer. Group 2: The dentin surfaces were treated with a cleaning brush with pumice Group 3: The dentin surfaces were treated with a cleaning bur. Group 4: The provisional cements were removed by an Er:YAG laser. Self-adhesive luting cement was used to bond ceramic discs to dentin surfaces. Shear bond strength (SBS) was measured using a universal testing machine at a 0.05 mm/min crosshead speed. The data were analyzed using a Kolmogorov Smirnov, One-way ANOVA and Tukey HSD tests to perform multiple comparisons (${\alpha}$=0.05). RESULTS. The dentin cleaning methods did not significantly affect the SBS of ceramic discs to dentin as follows: dental explorer, pumice, cleaning bur, and Er:YAG laser. CONCLUSION. The use of different cleaning protocols did not affect the SBS between dentin and ceramic surfaces.

CATCH22 syndrome or DiGeorge syndrome is a medical acronym of cardiac defects, abnormal facial appearances, thymic hypoplasia, cleft palate, and hypocalcemia. Patients with CATCH22 syndrome are susceptible to infection due to an absent or hypoplastic thymus and often have difficulties in maintaining good oral hygiene, which may require dental treatment. We present a case of dental treatment for the uncooperative child with CATCH22 syndrome under general anesthesia. A 4-year-old, 14.8 kg boy with CATCH22 syndrome visited Seoul National University Dental Hospital for dental check up. The patient had multiple caries requiring dental treatment. He experienced the corrective heart surgery due to Tetralogy of Fallot a few years ago. General anesthesia was planned because his heart rate and vital sign had shown unstable during the previous conscious sedation procedure. Dental restorative treatments were successfully performed and no complications were observed during and after the procedure. Safe and effective dental management of the patients with CATCH22 syndrome could be performed with the help of general anesthesia and careful monitoring.
The purpose of this study was to develop a standardized preschool oral health education program as one of educational media and to apply it to preschool education. It's ultimately meant to help improve the oral health education policy of the nation toward young children and to provide multiple information on that. The subjects in this study were 250 young children who were at the age of 7 and attended kindergartens in the city of S. To evaluate an oral health education program prepared in the study, the selected young children were taught by using it, and a survey was conducted twice before offering education and three weeks after it to assess their knowledge. Besides, their patient hygiene performance was tested. For data handling, SPSS program was utilized, and in order to see if there would be any changes in their oral health behaviors, statistical data on percentage were obtained and crosstab was employed. To track any possible additional changes in their relevant knowledge and Simplified Oral Hygiene Index, t-test was implemented. The findings of the study were as follows: 1. As for changes in oral health behaviors, there were significant differences in tooth brushing time(p<0.05), tooth brushing method(p<0.01) and toothbrush keeping method(p<0.001) between before and after the education. 2. Regarding changes in oral health knowledge, there was a significant gap between before and after the education(p<0.001). In detail, their knowledge about the cause of dental caries(p<0.05), the right choice of toothbrush(p<0.001) and toothbrushing after meals(p<0.01) became significantly different. 3. There was no gap in patient hygiene performance index between before and after the education. 4. To boost the effect of oral health education on kindergarteners, it seems necessary to give a lecture on oral health, to demonstrate tooth brushing by using dntiform, and to offer instruction by utilizing disclosing solution. 5. Repeated education should be provided on a regular basis by preparing standardized teaching plans tailored to the characteristics of kindergarteners, and a wide variety of teaching materials that could be easy to understand, authentic and provide motivation to them should be developed.

Oculo-cerebro-renal syndrome, known as Lowe syndrome, is an X-linked gene disorder characterized by congenital cataracts, brain abnormalities and renal dysfunction. Tooth crowding, taurodontism, delayed eruption in the permanent dentition and over-retained primary teeth with ectopic eruption of the permanent teeth have been reported as dental findings. Because of the high incidence of poor cooperation, patients with Lowe syndrome have difficulties in maintaining good oral hygiene, which may require dental treatment. We present a case of dental treatment for the uncooperative child with Lowe syndrome under general anesthesia. A 4-year-old, 11.2 kg boy with Lowe syndrome visited Seoul National University Dental Hospital for gingival swelling. The patient had multiple caries requiring dental treatment. Because of his past history of malignant hyperthermia during inhalation induction, anesthesia was induced and maintained with total intravenous anesthesia (TIVA) after medical consultation. Dental restorative treatments were successfully performed and no complications were observed during and after the procedure. Safe and effective dental management of the patients with Lowe syndrome could be performed with the help of general anesthesia and careful monitoring.

This in vitro study compared the remineralization of incipient interproximal caries in the presence of three glass ionomer cements(highly-filled glass ionomer cement, resin-modified glass ionomer cement, compomer) and a resin composite(control). Thirty-two extracted premolars were selected based upon the lack of any visible demineralization. The teeth were coated in a transparent acid resistant nail varnish leaving $3{\times}3$ mm square. The teeth were subjected to the demineralizing buffer for 3 days and quantitative light-induced fluorescence(QLF) images of the subjects were taken. Proximal restoration was simulated by placing tooth specimens and the various glass ionomer cements in closed containers with artificial saliva at $37^{\circ}C$ and pH 7.0 with constant circulation. Further QLF images were subsequently taken at 30, 60, and 90 days. The changes of mineral loss(${\Delta}Q$) were evaluated by QLF and the change of ${\Delta}Q$(${\Delta}{\Delta}Q$) were compared between groups in order to evaluate the effects of remineralization. All data were analyzed using ANOVA and the post-HOC Dunnett C multiple comparison test at p<0.05. While ${\Delta}Q$(changes of mineral loss) increased for all treatments, the increases for three glass ionomer groups were significantly higher than that for the resin group at first month period. As time went on, the amount of ${\Delta}{\Delta}Q$ decreased.
