공기청정용 가습액이 새집증후군 자각증상에 미치는 영향연구 (Effects of Humidification with an Air Purifying Substance on Sick Building Syndrome Symptoms)

  • Indoor air quality (IAQ) is recognized as a byproduct of industrialization that took place in the 20th century. The great attention to IAQ has been gained since 1980s, and led to numerous research studies that have mainly made in the developed countries. As the development of residential environment in Korea enters in the process of qualitative growth, IAQ has viewed as one of the core issues in the society, and research in relation to IAQ has been implemented until now in the fields of engineering and medical science. It is widely known that the sources and effects of indoor air pollution have been various, and IAQ research with multidisciplinary approaches is required. Nevertheless, few research embedded into a socio-behavioral approach to identifying interventive measures of residents and analyzing their choices is made. The purpose of this study is to examine the use of an interactive measure of residents in apartments that is air purification with phytoncide available in the market, and to investigate their self-reported effects of the air purifying substance on sick building syndrome symptoms. This study utilizes a field experiment design with pre-test and post-test, and the experiment is conducted in a natural field setting. A total of 87 test subjects from 32 households with already sick building syndrome are drawn via internet, and the participation is solely voluntary. The participants receive and use the air purifying substance, and are asked to use it continuously for two weeks and identify their self-reported symptoms of before- and after-use. The research findings indicate that air purification using phytoncide is effective to alleviate their symptoms in relation to sick building syndrome. Also it is noted that the participants report that the use of the air purification causes the positive impacts on their daily life. Further research is needed that compares the group with only humidifier and evaluates the mid-term and long-term use of the air purification and its effects.

Multiple brain abscesses treated by extraction of the maxillary molars with chronic apical lesion to remove the source of infection

  • Background: Brain abscess is a life-threatening condition that occurs due to complications during a neurosurgical procedure, direct cranial trauma, or the presence of local or distal infection. Infection in the oral cavity can also be considered a source of brain abscess. Case presentation: A 45-year-old male patient was transported with brain abscess in the subcortical white matter. Navigation-guided abscess aspiration and drainage was performed in the right mid-frontal lobe, but the symptoms continued to worsen after the procedure. A panoramic radiograph showed alveolar bone resorption around the maxillary molars. The compromised maxillary molars were extracted under local anesthesia, and antibiotics were applied based on findings from bacterial culture. A brain MRI confirmed that the three brain abscesses in the frontal lobe were reduced in size, and the patient's symptoms began to improve after the extractions. Conclusion: This is a rare case report about multiple uncontrolled brain abscesses treated by removal of infection through the extraction of maxillary molars with odontogenic infection. Untreated odontogenic infection can also be considered a cause of brain abscess. Therefore, it is necessary to recognize the possibility that untreated odontogenic infection can lead to serious systemic inflammatory diseases such as brain abscess. Through a multidisciplinary approach to diagnosis and treatment, physicians should be encouraged to consider odontogenic infections as a potential cause of brain abscesses.

다층모형을 적용한 조혈모세포이식 환자의 삶의 질 변화 영향요인 (Factors Influencing Changes in Quality of Life in Patients undergoing Hematopoietic Stem Cell Transplantation: A Longitudinal and Multilevel Analysis)

  • Purpose: This study was a prospective longitudinal study to identify changes in quality of life in patients undergoing allogeneic hematopoietic stem cell transplantation (HSCT). It was based on Roy's adaptation model. Methods: The questionnaires were administered before HSCT, 30 and 100 days after HSCT. Of the 48 potentially eligible patients, 44 (91.7%) participated in the study and 40 (90.9%) completed the questionnaires at 100 days after HSCT. Multilevel analysis was applied to analyze changes in quality of life. Results: Overall, quality of life showed a decreasing tendency from pre-HSCT to 100 days after HSCT. The adaptation level of participants was compensatory. Type of conditioning was the significant factor influencing quality of life before HSCT (${\beta}_{00}$=79.92, p <.001; ${\beta}_{01}$= - 12.64, p <.001) and the change rate of quality of life (${\beta}_{10}$= - 1.66, p =.020; ${\beta}_{11}$=2.88, p =.014). Symptom severity (${\beta}_{20}$= - 1.81, p =.004), depression (${\beta}_{30}$= - 0.58, p =.001), social dependency (${\beta}_{40}$= - 0.35, p =.165), and loneliness (${\beta}_{50}$= - 0.23, p =.065) had a negative effect on changes in quality of life. Symptom severity and depression were statistically significant factors influencing changes in quality of life. Conclusion: According to the results of this study, the development of nursing intervention is needed to improve quality of life in patients undergoing allogeneic hematopoietic stem cell transplantation in the early immune reconstruction period. The interventions should include programs to enhance coping capacity and programs to help control symptom severity and depression. Also these interventions need to be started from the beginning of HSCT and a multidisciplinary approach would be helpful.

NI-ELVIS를 활용한 센서계측공학의 실습교육 사례 (Application of practical education program of sensor instrumentation engineering using NI-ELVIS)

  • 본 논문에서는 센서공학분야의 실습교육에 NI-ELVIS(National Instrument Educational Laboratory Virtual Instrumentation Suite)를 활용하는 방안을 제시하였다. ELVIS는 LabView 기반의 계측시스템 설계와 프로토타이핑 환경을 제공하는 개발 플랫폼을 지칭한다. ELVIS는 가상 계측장치와 다기능 데이터 수집 장치(DAQ) 및 벤치탑 워크스테이션, 프로토타이핑보드 등으로 구성되어 있기 때문에 사용자가 의도하는 다양한 형태의 계측시스템을 PC에서 소프트웨어적으로 구성할 수 있다. 따라서 고가의 계측 장비를 이용하지 않고도 전기전자 분야의 효과적인 실험 실습 교육을 진행할 수 있다는 장점이 있다. 특히 센서계측공학의 경우 센서 기술, 전기전자공학, 신호처리, 데이터 분석 등 다양한 분야가 혼합된 영역임에도 복잡한 실험 장치에 의존하지 않아도 실습교육에 활용이 가능하다. 본 논문에서 제시한 계측공학 실습교육에서의 적용 외에도 전기전자 실험이 필요한 다양한 교과목에 높은 학습효과를 기대할 수 있다.

  • 이 논문은 공익명분에 입각한 공용수용 방식을 통해서 시행하는 미국 도시재생사업에서 사유재산권의 차별화를 분석한다. 도시재생사업이 강력한 수용방식을 활용하는 사업임에도 불구하고 실제로는 사회적 약자로부터 대자본 또는 민간개발업자에게 재산권을 이전시키는 재산권 차별화와 희생과정임을 밝힌다. 먼저 도시재생에 대해 다양한 학문적 관점에서 연구동향을 검토하고, 새로운 연구방향으로서 법제지리학적 관점을 제시한다. 다음으로 환경보호청과 회계검사원 자료를 통해 미국에서 이들 사업대상 지구의 규모를 추정하며, 이들을 탈공업화와 교외화의 유산으로 파악한다. 다음으로 사유재산권의 차별화 과정은 미국 연방대법원의 공적소유개념에서 경제적 공익개념으로의 공익개념 확장 과정, 공용수용의 민영화 과정, 개발주의 성장연합체제와 대자본 편향 및 인종 계층적 편견, 신자유주의적 재산권운동 등이 복합적으로 작용하여 발생하는 결과라는 것을 밝힌다.

북미지역 알코올중독 치료 프로그램 분석 - 재발방지를 중심으로 - (Analysis of Alcoholism Treatment Programs in North America - Focused on Relapse Prevention -)

  • Relapse is one of the most serious problem in alcoholism treatment area. But there is no proper post-treatment management program for preventing relapse in Korea. The purposes of this study are the analysis of several alcoholism treatment programs and recommendation about consideration for developing our alcoholism treatment program to prevent relapse in our country. Five different alcoholism treatment programs were collected for this study, two programs were collected by participant observation and three were collected by instruction manuals, references, and briefing materials. The followings are the criteria for this analysis of the five treatment programs. 1. the specific characteristics of the treatment program 2. the criterion of the patients, the duration and period (or the treatment, the special activities, and goals of the treatment. 3. the common characteristics of the five treatments. The common characteristics of those treatment programs are; 1 preparation for post-treatment from the beginning of the treatment (hospitalization) 2. assessment of relapse related factors 3. assessment of the relapse-warning signs 4. development and plan of individualized intervention strategy 5. practice of the plan The common characteristics of those treatment programs are 1. groups are classified the recovery training groups and the self-help groups 2 all kind of treatment programs carried out as groups rather than individuals. 3. usefulness which apply to various settings and various types 4. multidisciplinary and interdisciplinary approach As results, those programs were developed as post-detoxication management program, follow up program, and continued care program and their theoretical framework was from social learning theory. Those programs used the principles of cognitive-behavioral therapy for the classes, groups, and social skill training. These finding can be used for developing culturally appropriate alcoholism treatment programs focused on relapse prevention.

뇌졸중 환자에게 적용한 운동중재에 관한 논문분석 (An Analysis on the Research Papers about Exercise Interventions to the Stroke Survivors)

  • Purpose: This study was to suggest the direction to the exercise intervention development for the stroke survivors. Method: 12 domestic and 54 foreign research papers about the exercise intervention for the stroke patients published during 1998~2007 were analyzed. Result: Among the papers, quasi-experimental design(75.8%) and multidisciplinary approach(60.6%) were the most frequent ones. The mean age of the respondents was 63.3 and the intervention was supplied for 7.14 weeks. The frequency of exercise was 3.87 per week and the exercise time was 1.88 hours. And physical research variables - such as ambulation, muscle strength, activities of daily living and balance - were more than psychosocial variables. This study suggested that the exercise intervention for the stroke survivors need to include aerobic exercise, muscle strengthening exercise, flexible exercise, body coordination, balance exercise. In addition we confirmed that the exercise intervention was revealed as an effective physiological parameters; such as maximum oxygen uptake quantity, blood pressure. Conclusion: We suggest that the further researchs are needed which include the intensity of exercise into the quantity of the exercise. Also researchers need to extend the motion intervention time for an effective sociopsychological variable and to try the meta analysis for the effective exercise intervention for the stroke survivors.

Safety Management for MR-Guided Interventions

  • Purpose: Operating room management is the serious and complex task for hospital managers and the common approach is to develop relevant standard operational procedures. From patient and staff safety perspective, operating room management should be well-studied and hospital should identify and address any potential risks. Simultaneous usage of different imaging and less-invasive treatment technologies demands strong management control. Materials and Methods: We have formed the multidisciplinary expert panel (surgeons, anesthesiologists, radiologists, healthcare managers etc.) for hybrid theater management standard operational procedure development. On the first stage the general concept of hybrid room design and patient routing was developed. The second stage included the technical details discussion. For patient safety improvement we modified the Surgical Safety Check-list in accordance with potential MRI-related safety challenges and concerns. Results: WHO Surgical Safety Checklist is a simple and easy-to use tool which includes three blocks of question (grouped by the surgery process). We have developed two additional blocks of questions for the intraoperative magnetic resonance investigation. It is very important to have a special detailed routing with a strong control of ferromagnetic devices and anesthesiology care. Conclusion: High-energy MRI (1.5-3.0T) is characterized by potential influence on patient and staff safety in case of hybrid surgery. It is obvious to have a strong managerial control of ferromagnetic devices and anesthesiology care. Surgical Safety Checklist is the validated tool for improving patient safety. Modification and customization of this check-list potentially provides the opportunity for surgery processes improving.

Anorectal Malformations Associated with Esophageal Atresia in Neonates

  • Purpose: Anorectal malformations are often associated with other anomalies, reporting frequency with 40-70%. Gastrointestinal anomalies have been known to be relatively less common than associated anomalies of other organ system. This study was performed to assess a distinctive feature of cases associated with esophageal atresia. Methods: Clinical data (from January 2000 through December 2011) on the 196 subjects with anorectal malformations, managed in our Hospital, were reviewed. Total 14 neonates were identified with accompanying esophageal atresia and retrospective analysis was conducted. Results: The incidence was 7.1% and there were 8 male and 6 female subjects. Only 2 cases were associated with esophageal atresia without tracheoesophageal fistula. Although variable cases of anorectal malformation in female subjects, almost cases were anorectal malformations with rectourethral fistula in male. Other associated anomalies were identified in all cases, with more than 3 anomalies in 10 cases. There were 4 VACTERL (Vertebral abnormalities, Anal atresia, Cardiac anomalies, Tracheoesophageal fistula, Esophageal atresia, Renal and Limb anomalies) associations accounting for 28.6%, but could not identify chromosomal anomaly. Most cases were managed with staged procedure, usually primary repair of esophageal atresia and diverting colostomy. Overall mortality rate was 21.4%, mainly caused by heart problems. Conclusion: This study shows that early diagnosis and rational surgical approach with multidisciplinary plan are mandatory in managing anorectal malformations with esophageal atresia, when considering a high frequency of associated anomaly and a relative high mortality.

Prevention of Occupational Diseases in Turkey: Deriving Lessons From Journey of Surveillance

  • Introduction: To prevent and manage the societal and economic burden of occupational diseases (ODs), countries should develop strong prevention policies, health surveillance and registry systems. This study aims to contribute to the improvement of OD surveillance at national level as well as to identify priority actions in Turkey. Methods: The history and current status of occupational health studies were considered from the perspective of OD surveillance. Interpretative research was done through literature review on occupational health at national, regional and international level. Analyses were focused on countries' experiences in policy development and practice, roles and responsibilities of institutions, multidisciplinary and intersectoral collaboration. OD surveillance models of Turkey, Belgium and the Netherlands were examined through exchange visits. Face-to-face interviews were conducted to explore the peculiarities of legislative and institutional structures, the best and worst practices, and approach principles. Results: Some countries are more focused on exploring OD trends through effective and cost-efficient researches, with particular attention to new and emerging ODs. Other countries try to reach every single case of OD for compensation and rehabilitation. Each practice has advantages and shortcomings, but they are not mutually exclusive, and thus an effective combination is possible. Conclusion: Effective surveillance and registry approaches play a key role in the prevention of ODs. A well-designed system enables monitoring and assessment of OD prevalence and trends, and adoption of preventive measures while improving the effectiveness of redressing and compensation. A robust surveillance does not only provide protection of workers' health but also advances prevention of economic losses.
