Paraaortic node를 포함하는 자궁 경부암 치료시 효율적인 MLC사용에 대한 고찰

  • I. 목적 : Primus의 Multi Leaf Collimator는 X측이 29쌍으로 이루어 져 있으며 그중27쌍의 leaf은 1cm이고 2쌍은 6.5cm의 leaf으로 구성되어있다. 이러한 이유로 Paraaortic node를 포함하는 자궁경부암 치료시 현재는 Y가 27cm 이상인 경우는 차폐블럭을 제작하여 치료를 시행하나 차폐 블럭의 제작에 따른 업무의 지연과 차폐 블럭이 무거워져 치료시 환자에게 떨어질 위험을 제거 하기 위해 Asymmetric Field로 Multi Leaf Collimator를 사용한 결과를 보고하고자 한다. II. 재료 및 방법 : 모의 치료(Ximatron, Varian, USA)시 $Y_1$측을 15cm을 기준으로 하여 $Y_2$측을 변경하면서 Field size를 결정한다. ($Y_2$측은 20cm가 최대, 즉 Y측은 35cm까지 적용) 이러한 방법으로 Multi Leaf Collimator를 사용한 환자와 기존의 차폐블럭을 제작하여 치료한 환자와의 업무 개선사항을 확인하기 위하여 실제 공작실 업무 담당자의 블럭 제작 시간과 Beam Shaper를 이 용해 Multi Leaf Collimator를 입력하는 시간을 상호 비교하여 단축된 시간을 조사하였다. III. 결과 : 차폐블럭을 대신해 Multi Leaf Collimator를 이용함으로써 치료실에서 환자에 대한 위험요소(차폐블럭이 무겁다)를 사전에 제거 할 수 있었고 공작실에서 블럭 제작 시간과 LANTIS를 이용해 MLC를 입력하는 시간을 실측 한 결과업무의 시간이 120분에서 5분으로 단축되는 효과가 있었다. 전산화 계획 실에서 선량 계산시 OAR Factor값을 고려하여야 한다. IV. 결론 : Paraaortic node를 포함한 자궁경부암 환자의 차폐부위는 모양이 거의 일직선이기 때문에 Mu]ti Leaf Collimator를 사용하기에 용이 한 치료 부위이다. 하지만 큰 Field size로 인한 불편함이 있었다. 이러한 제 약성을 Asymmetric Field를 이용해서 Multi Leaf Collimator의 사용을 가능하게 하고 차폐 블록의 제작과정과 치료 시에 발생되는 근무자의 업무의 손실을 줄이고 환자에 대한 위험성 을 해결하였다.

Development and utility evaluation of new Multi-Leaf Collimator for Diagnostic X-ray Equipment

  • The diagnostic multi-leaf collimator preventing unnecessary dose from entering into patients during the diagnostic examination was made in this study. The movement of the entire 50 leaves was embodied with the group of 25 ones thereof configured in a pair facing each other on the left and right of the median line. Dimensions of the length, width, and height of each shielding leaf were $5{\times}0.5{\times}0.5cm^3$ resulting in the maximum boost field of $10{\times}10cm^2$. The material of multi-leaf collimator had the excellence on the machinability with the use of the SKD-11 alloy tool steel having the high wear resistance against frequent movement, and it was devised to control both-side's shielding leaves by moving 2 motors unlike existing remedial multi-leaf collimator that use as many motors as the number of 50 shielding leaves. Thereafter, the transmission dose of leaves, cross-leaf leakage dose, and inter-leaf leakage dose were measured by the developed multi-leaf collimator attached to X-ray equipment. An ionization chamber was used to detect doses there from, and the comparative analysis was carried out by means of the radiographic film that was easy to detect the dose leakage in between each leaf. Results obtained from the test conducted in comparative analysis yielded approximately 98%, 96%, and 94% of shielding efficiency realized at each level of energy of 80kV, 100kV, and 120kV it was confirmed there was no dose leakage resulted from the varied level of irradiation energy. Thus the multi-leaf collimator to be developed based on this study is thought that it could fully reduce the unnecessary dose to patients in the diagnostic test and the shielding efficiency thereof is expected to be increasing if it is made in a miniaturized form with a way of increasing the thickness of each leaf later for an extended application to general diagnostic purposes.

  • We have developed the prototype Multi-leaf Collimator(MLC) for Medical linear acclerator. In this study, we have examined the mechanical characteristics of newly developed multileaf collimator. The leaf movement in clinical situation and the modification of accessories is good. such as the block tray mount and the wedges, due to the new MLC installation are efficient. But it was that bolts and nuts for the leaf control should have better performance.

Performance Evaluation of the Developed Diagnostic Multi-Leaf Collimator and Implementation of Fusion Image of X-ray Image and Infrared Thermography Image (개발한 진단용 다엽조리개 성능평가 및 X선영상과 적외선체열영상의 융합영상 구현)

  • We have developed and applied a diagnostic Multi-Leaf Collimator (MLC) to optimized the X-ray field in medical imaging and the usefulness evaluated through the fusion of infrared image and X-ray image acquired by infrared camera. The hand and skull radiography with multi-leaf collimator(MLC) showed significant area dose reductions of 22.9% and 31.3% compared to ARC and leakage dose was compliant with KS A 4732. Also scattering doses of 50 cm and 100 cm showed a significant decrease to confirm the usefulness of MLC. It was confirmed that the fusion of infrared images with an adjustable degree of transparency was possible in the X-ray images. Therefore, fusion of anatomical information with physiological convergence is expected to contribute and improvement of diagnostic ability. In addition, the feasibility of convergence X-ray imaging and DITI devices and the possibility of driving MLC with infrared images were confirmed.

Evaluation of Clinical Risk according to Multi-Leaf Collimator Positioning Error in Spinal Radiosurgery (척추 방사선수술 시 다엽콜리메이터 위치 오차의 임상적 위험성 평가)

  • The purpose of this study is to evaluate the clinical risk of spinal radiosurgery by calculating the dose difference due to dose calculation algorithm and multi-leaf collimator positioning error. The images acquired by the CT simulator were recalculated by correcting the multi-leaf collimator position in the dose verification program created using MATLAB and applying stoichiometric calibration and Monte Carlo algorithm. With multi-leaf collimator positioning error, the clinical target volume (CTV) showed a dose difference of up to 13% in the dose delivered to the 95% volume, while the gross tumor volume (GTV) showed a dose difference of 9%. The average dose delivered to the total volume showed dose variation from -8.9% to 9% and -10.1% to 10.2% for GTV and CTV, respectively. The maximum dose delivered to the total volume of the spinal cord showed a dose difference from -14.2% to 19.6%, and the dose delivered to the 0.35 ㎤ volume showed a dose difference from -15.5% to 19.4%. In future research, automating the linkage between treatment planning systems and dose verification programs would be useful for spinal radiosurgery.

Variation of Image Quality and Dose by Applying Multi-Leaf Collimator for Digital Mammography (디지털 유방촬영장치에서 다엽 조리개 적용으로 인한 화질과 선량의 변화)

  • Collimator has important functions with control primary X-ray that decrease radiation exposure dose for patients and reduce scatter ray and make better quality of image. But there are no regulations for X-ray mammography device of collimator, so widely used device adopt rectangularly controlled collimator. Though digital X-ray mammography device expand supply recently, rectangularly controlled collimator of film/screen mode still used. After searching for real condition of beam field with digital mammography, we made a multi-leaf collimator which is able to adjust the beam field in accordance with size and shape of breast, and we measuring up the transitions of image quality, average glandular dose(AGD) and, Dose area product(DAP). There are no significant differences between rectangularly controlled collimator and multi-leaf collimator, and DAP value decreased by 50.72%. As conclusion, there needs to expand the use of multi-leaf collimator for optimum adoption of beam field in digital mammography, and also need to develop an automatic regulation of beam field for reduce of exposure dose to patients.

The Dosimetric Effects on Scallop Penumbra from Multi-leaf Collimator by Daily Patient Setup Error in Radiation Therapy with Photon (광자선 치료시 Setup 오차에 따르는 Multi-leaf Collimator의 Scallop Penumbra 변화 효과)

  • Purpose : To evaluate the clinical implications of scallop penumbra width that comes from multileaf collimator(MLC) effect by the daily routine patient setup error. Materials and Methods : The anales of $0^{circ},{\;}15^{circ},{\;}30^{circ},{\;}45^{circ},{\;}60^{circ},{\;}and{\;}75^{circ}$ inclined -radiation blocked fields were generated using the both conventional cerrobend block and the MLC. Film dosimetry in the phantom were performed to measure penumbral widths of differences between the dose distributions from the cerrobend block and those of respect the MLC. The patient setup error effect on scallop penumbra was simulated with respect to the table of setup error distribution. Same procedures are repeated for the cerrobend block generated field. Results : There are penumbral widths of to 3mm difference between the dose distributioins from two kinds of field shaping tools, the conventional block and the MLC with 4mm setup error model and resolution of 1cm leaf at the isocenter. Conclusion : We need not additive margin for MLC, if planning target volume is selected according to the recommendation of ICRU 50. For particular cases, we can include the target volume with less than 3mm additive margin.

  • We have developed the prototype Multi-leaf Collimator(MLC) for medical linear acclerator. In this study, we have examined the clinical effectiveness of our newly developed MLC by analysing treatment pattern of radiation therapy and studied the radiation safety by measuring the transmitted radiation and the leakage radiation. Over 81% patients can be treated with new MLC(20 pairs of leaves) and 98% with 30 pairs of leafed-MLC, while 95% with commercial MLC. New MLC showed superior properties of radiation leakage and transmission to commercially available MLC.
