A Study on the Customer's Use of and Satisfaction with oriental medical services by local resident - Focused on Asan City - (지역 주민의 한방 의료 서비스에 관한 조사연구 - 아산시를 중심으로 -)

  • The purpose of this study is to survey the current status of the oriental medical service satisfaction and needs that the people who utilizing the medical services and suggest the primary factors of consumption and satisfaction in the oriental medical services the city of Asan. We analyze the use and satisfaction of the customers for the oriental medical services with using the questionnaires to the citizens of Asan. The method of this study is that the participants of the survey was 556 people of Asan City. The survey was conducted from August 15th, 2009 to September 15th, 2009(during 30 days) with disease index that specially developed as oriental medicine. The results of this study are as follows: 556 people were responded the survey. 471(84.7%) of respondents were who have ever been treated with oriental medical service and 85(15.3%) of respondents were who have never been treated with oriental medical service. 25(4.7%) of respondents were who doesn't believe the treat of oriental medicine. 76(13.6%) of respondents prefered the oriental medical service. 413(50.1%) of respondents prefered the acupuncture and moxa treatment and 280(33.9%) prefered the herbs. 227(39.6%) of respondents were treated for musculoskeletal disorders The conclusions from this study are as follows: Firstly, it was found that the people who prefer to use oriental medical services usually have one or more of these following diseases: musculoskeletal disorders, paralysis, unclear diseases or injuries. Secondly, the main factor of customers' satisfaction with oriental medical services lies in the age of the user. The more aged, the more high in customers' satisfaction with oriental medical services was found through the survey. The results of this study can be used to develop marketing strategies for oriental medical institutions in the city of Asan.

Depressor Responses to Intravenously Administered Artemisia asiatica Nakai Juice in Cats (애엽(艾葉) (Artemisia asiatica Nakai)의 혈압강하작용(血壓降下作用))

  • The wormwood is one of the plants which occur widely throughout the world. Though the precise data on the entire chemical composition of mugwort leaves are not available, the major principles which have been found so far include inulin, alkaloid, thujon, sesquiterpene and several vitamins. Santonin, a parasiticide, is one of the glucosides extracted from the limited species of wormwood. It has long been known in herb medicine that the plants of this family has not only strong hemostatic, analgesic and parasiticidal actions but also therapeutic effects for diarrhea, stomachache and asthma. In recent pharmaceutical botany the wormwood is introduced to have antipyretic and astringent actions also. The mugwort(Artemisia asiatica Nakai) is the most common species of wormwood that occurs in Korea. The usage of this edible leaves of mugwort is rather various. It is used not only for wormwood bath but also as forage, moxa and medicinal agents. Recently Kim et al reported from their study on the effect of mugwort on the motility of isolated intestine of rabbits that tonus and motility were markedly enhanced by mugwort but this effect of mugwort on intestinal motility was almost completely blocked by atropine suggesting that activity of mugwort was exerted through its cholinergic effect. It was the findings of Kim et al that prompted the authors to do the present experiment. The present study was undertaken to investigate effects of mugwort(Artemisia asiatica Nakai) juice on the respiration and blood pressure in cats. And also studied was the mechanism of depressor action of Artemisia asiatica Nakai Juice (AAJ). The results obtained are as follows; 1) It was observed that mean arterial blood pressure and heart rate were decreased markedly by AAJ. Following administration of 0.15 ml/kg and 0.3 ml/kg AAJ into cats the maximum depressor responses observed were $77.5{\pm}2.2\;mmHg$ and $94.0{\pm}3.7\;mmHg$ respectively. 2) Depressor responses to AAJ were blocked markedly by atropine whereas the responses were not affected by propranolol and dibenamine. Therefore it is strongly inferred that depressor action of AAJ results mainly from its cholinergic effect. This inference was further substantiated by the fact that heart rate change which invariably accompanies depressor responses to AAJ was almost completely abolished by atropinization. 3) After administration of AAJ into cats frequency of respiration was markedly increased while depth of respiration decreased during first 2-3 seconds.

A Study on "Beijijiufa" ("비급구법(備急灸法)"에 대한 연구(硏究))

  • "Beijijiufa" is a medical book republished by Sun Ju Qing in 1245. He compiled this book having added "Qizhumajiufa" and "Zhugejingyanbeijiyaofang" to the contents of "Beijijiufa" authored by Wen Ren Qi Nian. In "Beijijiufa" the author described treatment methods making use of moxibustion methods in connection with 22 cases of acute diseases. The author had collected the moxibustion methods used to treat acute diseases, which had been practiced by the medical practitioners of many generations, and quoted total 13 medical men's practices. In the book, the greatest parts of details were quoted especially from the writings of Sun Si Miao and Ge Hong, and this shows that the medical philosophies of both Sun Si Miao and Ge Hong were reflected onto "Beijijiufa". He had differed on his moxibustion practice: the size of moxa wool, the number of moxibustion treatment, and method of moxibustion for male and female were differed from one another according to the disease. As to the area of moxibustion, he chose the body parts around under four limbs and joints, and mostly used extraordinary acupoints rather than twelve main meridians. In his descriptions of finding meridian points, he did not describe it by its specific name of the reaction point, but explained the location of moxibustion points in detail through pictures. "Qizhumajiufa" is related to moxibustion method and prescriptions to treat surgical diseases, like skin boils or furuncle on the back, etc. He easily explained the method to find the meridian points for moxibustion treatment by using particular way through diagrams and pictures. Eight prescriptions he used were the collections among the historical practices of medical practitioners of many generations for skin boils which showed excellent therapeutic actions. In "Zhugejingyanbeijiyaofang", there are prescribed for 36 disease, also is the records of treatment methods for medical emergency which would be encountered easily in everyday life. As to therapeutic remedies, varied treatment methods, including the treatment by means of pasting and attaching medicinal substance to the spot, the treatment by means of mixing medicinal substance with alcoholic beverage, cleansing method, smoke inhalation remedy etc. were introduced. In "Beijijiufa" moxibustion was regarded as a top priority for treatment of acute disease, and the author strived to present remedies to the readers as easily as possible through 19 pictures. Regarding prescriptions, the author introduced diverse treatment methods with respect to various disease symptoms, and described the method to treat disease symptoms making use of medicinal ingredients which can easily be found in daily life. Likewise, "Beijijiufa" compiled by Sun Ju Qing was intended for clinical practice, and was indeed a medical book having been utilized for treatment of acute diseases in those days.

Effect of Tinnitus after Bleeding, Acupuncture, Moxa, and Laser treatment (부황, 뜸, 침 레이저요법 시술이 耳鳴에 미치는 영향)

  • Although tinnitus is a common disease, it's reason, pathogenesis and treatment are not examined clearly. As tinnitus is a subjective complaint, it is not easy to evaluate objectively and to compare with another. In this study, we intend to estimate the effect of a variety of oriental medical treatments to tinnitus. We inquired about 30cases of tinnitus patients visited the Kwang-ju Oriental Medical Hospital of Wonkwang University from Apr. 1998 to Sep. 1998. We examined the 30cases in the view of sex, age, occupation, duration, etiologic factors, past history, associating symptoms, quality of tinnitus, and recovery rate after treatments. The results were as follows: 1. In distribution of sex, the ratio of male was $63.3\%$(19cases)and\;female\;was\;36.6\%$(11cases). 2. In distribution of age, the ratio of the 2th decade was $6.6\%(2cases),\;the\;3th\;decade\;was\;20.0\%(6cases),\;the\;4th\;decade\;was\;16.6\%(5cases),\;the\;5th\;decade\;was\;23.3\%(7cases),\;the\;6th\;decade\;was\;30.0\%\;(9cases),and\;the\;7th\;decade\;was\;3.3\%(1case)$. 3. In distribution of occupation, the ratio of employee was $23.3\%$(7cases), house-keeper was $63.3\%(11cases),\;farmer\;was\;16.6\%(5cases),\;teacher\;was\;13.3\%$ (4cases), and student, merchant, soldier was each $3.3\%$(1case). 4. In distribution of duration, the ratio of under 1month was $6.6\%$ (2cases), 1 month -6 months was $20.0\%$(6cases), 6 months- 12months was $30.0\%$(9cases), 12months-36months was $23.3\%$(7cases), and over 36months was $20.0\%$(6cases). 5. In etiologic factor of tinnitus, the ratio of unknown reason was $40.0\%$ (12cases), overwork was $16.6\%$(5cases), emotional stress was $10.0\%$(3cases), noise was $6.6\%$(2cases), cases of laying overwork upon stress was $13.3\%$(4cases), head trauma was $3.3\%$(1case), gun report was $3.3\%$ (1case), and after sickness was $6.6\%$(2cases). 6. 9 cases had experienced such a disease as meniere's disease, tympanitis, labyrinthitis, trauma of tympanum, hypertension, etc. 7. Commonly associated symptoms were dizziness, deafness, uneasiness, headache, ear fullness, fatigue, insomnia, nausea, vomiting, and forgetfulness. 8. Most frequent quality of tinnitus were buzzing, whistling, humming etc. 9. In $76.6\%$(23cases) of tinnitus patients, it was improved, but in $23.3%$(7cases) of them, not improved. Total recovery rate was $42.2\%$.

  • The subjects of this clinical study are 17 patients with ALS(Amyotrophic Lateral Sclerosis), who were diagnosed in other or our hospital from January 1987 to Nomember 1997. The age of onset, clinical signs, type of ALS, methods of treatment and outcomes, etc. were studied and analyzed. The data are summarized as follows ; 1) The mean age was $52.4{\pm}11.5$ years old, and the ratio of male to female was 2.4:1. There is increasing frequency with rising age. 2) Pseudopolyneuritic type(67%) is the most in male patients, bulbar type(60%) is the most in female patients, and totaly Pseudopolyneuritic type(54%) is the most. 3) In the period of hospitalization after onset, 1 year(11 cases, 64.8%) is the most frequent, and 2 year(2 cases, 11.7%), 1 month(2 cases, 11.7%), 4 years(1 case, 5.9%) and 6 months(1 case, 5.9%) in orders. 4) In the duration of admission, 10 days(8 cases, 47.2%) is the most frequent, and 30 days(4 cases 23.5%), 2 months(2 cases, 11.7%), 20 days(2 cases, 11.7%) and 3 months(1 case, 5.9%) in orders. 5) In the signs of patients, muscle weakness(17 cases, 100%), bulbar signs(14 cases, 82.4%), increased reflex of deep tendon(10 case, 58.8%), muscle atrophy (7 cases, 41.2%) and fasciculations(7 cases, 41.2%) were in orders. 6) In the methods of treatment, herb-medication(17 cases, 100%), acupuncture therapy(16 cases, 94.1%), physical therapy(7 cases, 41.2%), moxibustion therapy(5 cases, 29.4%), cupping therapy(4 case, 23.5%) and moxa-pack(1 case 5.9%) were administered in orders. 7) As to the outcome of treatment, 8(47.1%) were no changed, 4(23.5%) improved, 4(23.5%) aggravated, 1(5.9%) expired in orders.

  • I studied on the influence of BUDDEUMI(moxa and cupping a boil) in the live blood condition and the favorable turn up a subjective symptom of chronic irritable bowel syndrome patient. The results are as follows : After the perform of BUDDEUMI, in the Erythrocyte Aggregation, the efficiency is 87%(3persons decrease and 10 persons destroy/15 persons). In the Rouleau, the efficiency is 78%(2persons decrease and 5 persons destroy/9 persons). In the Target Cells, the efficiency is 90%(3persons decrease and 6 persons destroy/10 persons). In the Ovalocytes, the efficiency is 88%(3persons decrease and 4 persons destroy/8 persons). In the Poikilocytes, the efficiency is 76%(1persons decrease and 2 persons destroy/4 persons). After the perform of BUDDEUMI, in the Cholesterol Crystals, the efficiency is 82%(3persons decrease and 6 persons destroy/11 persons). In the Atherosclerotic Plaque, the efficiency is 80%(2persons decrease and 2 persons destroy/5 persons). In the Chylous, the efficiency is 86%(3persons decrease and 9 persons destroy/14 persons). On the favorable turn up a subjective symptom, the efficiency of symptom of appetite and digestion is 91%(5persons turn up exceedingly favorable, 9 persons turn up favorable, 7 persons turn up good, 2 persons turn up invalid / 23persons). In the efficiency of symptom of fatigue and feeble is 85%(3persons turn up exceedingly favorable, 6 persons turn up favorable, 8 persons turn up good, 3 persons turn up invalid / 20persons). And the efficiency of symptom of abdominal swelling and displeasure is 88%(2persons turn up exceedingly favorable, 10 persons turn up favorable, 8 persons turn up good, 3 persons turn up invalid / 24persons). In the efficiency of symptom of abdominal pain is 88%(4persons turn up exceedingly favorable, 10 persons turn up favorable, 8 persons turn up good, 3 persons turn up invalid / 25persons). The efficiency of symptom of constipation and diarrhoea is 90%(5persons turn up exceedingly favorable, 9 persons turn up favorable, 6 persons turn up good, 2 persons turn up invalid / 22persons).

A Literature Study on the Korean Acupuncture for Oral, Glottal, Labial and Dental diseases (구설순치(口舌脣齒)질환에 대한 한국 침구서적의 치료법 비교 연구)

  • Background : The varieties of manifestations referred to oral, glottal, labial and dental diseases has been long complicated physicians in the efficient diagnosis and treatment of these diseases. Acupuncture has been widely used in Korea throughout the history and provides an efficient method in the treatment of them Objectives : Establish a distinctive and efficient acupuncture method for the treatment of oral, glottal, labial and dental diseases based in literature research Method : We reviewed four Korean medical literature, "Guide to Swollen Sore Treatment", "Treasured Mirror of Eastern Medicine", "Experiential Prescriptions of Acupuncture and Moxibustion", and "Essential Rhymes on Acupuncture and Moxibustion by Master Sa-am", and analyzed the therapeutic characteristics in the treatment of oral, glottal, labial and dental diseases Result : 1. According to "Guide to Swollen Sore Treatment", we could noted frequent application of blood letting methods in the affected area, then rinse ones mouth with salty water and further application of taro plaster. Also found acupuncture methods utilizing heated mole cricket or silkworm in the sublingual region. Regarding herbal method, Realgar was rubbed in the affected area. Mainly used acupuncture points in the treatment of oral, glottal, labial and dental diseases were GV20, GB20, LU5, and auricular anterior hairline 2. In "Treasured Mirror of Eastern Medicine", blood letting method in the sublingual region and burning needle searing method were the most frequently applied in the treatment of oral, glottal, labial and dental diseases. Moxibustion was also applied in the treatment of labial and dental diseases. Particularly, said to apply 3 moxa cones in ear zones of both sides, when no medicine is effective. This demonstrates how emphasized the importance of moxibustion in this kind of disease. Mainly used acupuncture points were GV16, CV24, LI4, EX-HN12, and EX-HN13 3. In "Experiential Prescriptions of Acupuncture and Moxibustion", no other methods beside wrist. Superstitious methods like applying moxibustion on the tooth picture drawn on the roofing tile need further confirmation. Mainly used acupuncture points were LI4, ST36, and HT7. 4. "Essential Rhymes on Acupuncture and Moxibustion by Master Sa-am", identified the cause of diseases according to visceral pattern identification. Stomach and Spleen in charge of vocal, lingual, and labial disease, Kidney for dental disease, and further scrutinizing identification according to Liver, Heart, Stomach, Lung, and Kidney manifestations. Used supplementation and draining needling methods of self meridians and other correlating meridians. Conclusions : After previous study on stroke and eye diseases, we could also find various efficient methods according to oral, glottal, labial and dental diseases, through literature research of korean medical classics. This study will concurrently result in establishing distinctive therapeutic method characteristic of Korea.

A Review on the Characteristics of Temperature Variation in Warm Needle (온침의 온도변화 특성에 대한 문헌 고찰 연구)

  • Objectives: The purpose of this study is to organize the research methods and results of studies related to the temperature of the warm needle for systematic utilization of warm needling technique. Methods: This study used the databases of nine (Pubmed, Science Direct, Cochrane Central, 4 Korean databases, CNKI, CiNii) to analyze temperature-related studies of the warm needle from 2000 to June 2019. Results: A total of 19 papers were included. Of these, 15 were used for mugwort, 2 for high frequency, and 1 for both mugwort and high frequency, and the other one for a ceramic heater. The maximum temperature rises as the amount of moxibustion increases. It is also affected by the density of moxa and the ignition part. There were 16 papers using stainless steel needles and 4 papers using a needle made of gold or silver to compare. In the area of the needle, the closer it is to moxibustion, the hotter it is. Compared to stainless steel needles, gold and silver needles showed almost twice the temperature. The effects of environment and radiant heat should be considered during warm needle procedures. Conclusions: There are various experimental methods such as warm needle technique materials, methods, measuring parts, measuring instruments, etc. The results were also very diverse. When setting the heating source, ignition part, size of moxibustion, etc. of warm needles, it should be implemented in a way that takes safety and validity into account. Considerations for temperature characteristics, radiant heat, etc. of warm needles will be needed when making warm needle apparatus.

A Study on Satisfaction level with Herbal Public Health Services and its Improvement Plans (한방공중보건서비스 만족도와 개선방안)

  • Objective : In order to investigate and improve public Korean medical health service satisfaction level, this study was designed. Method : A questionnaire has been conducted on 212 patients who received treatments at six public health centers in the northern part of Gyeongbuk during 15 days between Sep. 24 and Oct. 8, 2011. Result : 1. An Investigation on the usage of herbal clinics in public health centers reveals that 63.7% have received three times or more medical treatments previously and 61.8% have had their illness treated at other medical institutions. In regard to illness 32.1% have had arthritis or muscle aches. 50.9% have taken insurance medication after having had treatments at the public health centers. 66% have assessed acupuncture and moxa cautery the most satisfying. 2. To a question regarding whether herbal health treatment costs higher than that of physician's, the highest response at 31.6% is 'No'. And to a question regarding whether herbal medicines administered at public health centers have more side effects than that of physician's, the highest response at 39.6% is 'No'. 3. To a question regarding whether herbal treatment of public health centers has little effect against acute disease, 48.1% of responses are 'Fair'. To a question regarding whether herbal treatments, when compared with physician's treatments, boost better recovery of patients, 48.1% of responses are 'Fair'. To a question regarding whether herbal medicine is unscientific, when compared with that of western medicine, 38.2% of responses are 'Fair', To a question regarding whether herbal medicine has faster effect on disease than western medicine, 41.0% of responses are 'Fair'. To a question regarding whether herbal medicine is more effective on disease prevention and promotion of health than disease treatment, 38.2% of responses are 'Fair'. And to a question regarding whether the lack of various types of physical therapy devices in herbal medicine, when compared with western medicine causes inconvenience in herbal treatment, 42.0% of responses are 'Fair'. Those responses take up highest portion at each questionnaire. 4. A comparative study between herbal treatments and physician's treatments has also been conducted. To questions regarding which one of the two considering types of disease is the better, responses are the latter accounted for 43.9% against 'Cancer', the latter accounted for 45.3% against 'Endocrine disorders', the former accounted for 30.7% against 'Psychiatric disorders', the latter accounted for 38.2% gainst 'Otolaryngological(ENT) disease', the former accounted for 47.6% against 'Post traumatic stress disorder', and the former accounted for 52.4% against 'Muscle-skeletal disease'. 5. An investigation on frequency of patients' visits via (p<0.05) of subjects show a statistically significant difference. 6. First, an investigation on frequency of reasons of medical treatments reveal that age, occupation, monthly income, and insurance type (p<0.05) of subjects show a statistically significant difference. Secondly, an investigation on frequency of subjects taking insurance medicines after herbal health treatments reveal that monthly income (p<0.05) of subject shows a statistically significant difference. 7. First, an investigation on frequency of a claim that herbal treatments of public health center does not have great effect on acute disease reveals that age, education, and insurance type (p<0.05) of subjects show a statistically significant difference. Secondly, an investigation on frequency of analysis that herbal treatments has faster effect on disease compared with western treatments reveals that education level, religion, monthly income, and insurance type (p<0.05) of subjects show a statistically significant difference. 8. When herbal clinics of public health centers and general herbal medicine institutions are compared, a survey on additional treatments that herbal clinics need the most reveals that education level, monthly income, and insurance type (p<0.05) of subjects show a statistically significant difference. Secondly, an investigation on frequency of subjects who want various forms of herbal medicines reveals that occupation and insurance type (p<0.05) of subjects show a statistically significant difference. Conclusion : In order to improve efficiency of treatments and enhance patient's satisfaction level, this study suggests measures such as providing a differentiated acupuncture treatments as a whole, streamlining an reception procedure, adopting more elaborated computer system for a patient to get proper medical attention, standardizing a treatment duration in order for a maximum result, keeping regular office hours, and optimizing a consultation time for a patient.

  • Radiation exposure to humans can be caused by the gamma rays emitted from natural radioactive elements(such as uranium, thorium and potassium and any of their decay products) of Naturally Occurring Radioactive Materials(NORM) or Technologically Enhanced Naturally Occurring Radioactive Materials(TENORM) added consumer products. In this study, assume that activity of radioactive elements is $^{238}U$, $^{235}U$, $^{232}Th$ $1Bq{\cdot}g^{-1}$, $^{40}K$ $10Bq{\cdot}g^{-1}$ and the gamma rays emitted from these natural radioactive elements radioactive equilibrium state.
