An Outlook of the Oriental and Western Medical Diagnosis and Treatment on Large Bowel Cancer (대장암(大腸癌)의 동서의(東西醫) 결합(結合) 진치근황(診治近況))

  • Large bowel cancer shows the 4-5th frequency in cancers that occurs in Korea. The western medicine cures the Large bowel cancer by radiation, surgery and chemotherapy. While, Oriental medicine cures the Large bowel cancer by Herb-drugs, acupuncture, moxa and et al. With just one way of treating Large bowel cancer can't be effective remedy. Because each medicine has a strength and weakness, it is effective treatment when two medicine combines and supplement each other. We got the following result about a trend of oriental and western combination treatment for Large bowel cancer through studding records. 1. In Large bowel cancer, colon cancer is referred hematochezia(腸風下血), rectal cancer is refereed enterotoxin(腸毒), and anal cancer is accumulation of pathogens in yin(結陰). 2. The western medicine treats Large bowel cancer patient with surgery first. They need on assembly treatment such as chemical, radiation and immune treatment. In oriental medicine, they treats Large bowel cancer patients with differentiation of symptom and signs and treatment(辨證施治) for example, insufficiency of spleen and stomach(脾胃虛弱), collapse of the spleen-ql(脾氣下陷), stagnation of blood stasis and toxic agent(瘀毒內結), reinforcing both qi and blood(脾血下陷), stagnation of damp-phlegm(痰濕凝結) and cure for them by acupuncture and moxa too. 3. In combination with oriental and western medical treatment princple of Large bowel cancer by each stage is as follows. First stage is cured with radical surgery and herb-drugs without chemotherapy. The intermediate and terminal stage patients is used radiation before surgery, or after palliative surgery cour with chemotherapy, radiation and Herb-drugs. In terminal stage patients, unable for surgery, is used combination between chemotherapy, palliative radiation and Herb-drugs. 4. After radiation surgery, the terminal stage patients who have extensively lymph node metastasis or local contraindication is able to undergo combination of Herb-durgs and chemotherapy. 5. The cure-effect with oriental and western medicine combination treatment was better than that just with oriental or western medical treatment. 6. The merits of oriental and western medicine combination treatment lengthen one's life and diminish the bad effect of chemotherapy and complete radiation treatment, prevent from relapsing, maintain the balance in their environment of body and improve immunity.

Protective Effects of Folium Artemisiae Argyi Herbal Acupuncture on Transient Forebrain Ischemic Injury in Rats (흰쥐의 일과성 전뇌 허혈 손상에 대한 애엽 약침의 신경보호 작용)

  • Objectives : Recently, the new therapeutic tool, that is herbal acupuncture, has been developed and applied to various diseases including the cerebrovascular accident. The main characteristics of herbal acupuncture are a combination of acupuncture and herbal medicine. It was not well known the therapeutic effect and the mechanism of herbal acupuncture on transient forebrain ischemic injury, although it has been used frequently in clinics. The objective was to determine the effect of folium artemisiae argyi (艾葉) herbal acupuncture on the trasient forebrain ischemic injured rat. Methods : In this study, the effects of folium artemisiae argyi (艾葉) herbal acupuncture on the $LR_3$ named Taechung, on neuroprotection after the transient forebrain ischemia were investigated in Sprague-Dawely rats. Expressions of cFos, FosB and BDNF protein in the hippocampus and cortex were observed at 2 hrs and 48 hrs after transient forebrain ischemia by immunohistochemistry and ELISA technique. Results : Expression of cFos protein was increased slightly in the hippocampus and cortex at 2 hrs after transient forebrain ischemia, but FosB protein was increased highly comparing to cFos protein. However, pretreatment with folium artemisiae argyi' herbal acupuncture on $LR_3$ significantly increased expression of cFos protein and significantly decreased expression of FosB protein compared to control group. These features were observed in the retrosplenial granular cortex as well as the hippocampus. Also, pretreatment with folium artemisiae argyi' herbal acupuncture on $LR_3$ significantly increased the expression of BDNF protein in the hippocampus ($263.26{\pm}44.56{\;}pg/ml$ extracted by water, $275.35{\pm}51.47{\;}pg/ml$ extracted by moxa tar)and the cortex ($102.33{\pm}13.65{\;}pg/ml$ extracted by water, $109.54{\pm}9.37{\;}pg/ml$ extracted by moxa tar) compared to the hippocampus ($134.07{\pm}2.96{\;}pg/ml$) and the cortex ($61.16{\pm}4.11{\;}pg/ml$) in control group at 48 hrs after transient forebrain ischemia. Conclusions : These results suggest that pretreatment with folium artemisiae argyi' herbal acupuncture on $LR_3$ has neuroprotective effect on transient forebrain ischemia and the herbal acupunture on $LR_3$ may be related to antioxidative function of folium artemisiae argyi.

An in-depth interview for use of moxibustion therapy in Korea (국내 뜸 요법 임상 실태 파악을 위한 면접조사)

  • Objectives : Moxibustion therapy is one of the most popular oriental treatments in Korea. In this study, we operate the in-depth interview for use of moxibustion therapy in Korea. Methods : Survey questions were developed based on consensus of acupuncture professors. The list of the Korean medical doctors with experiences more than 10 years is provided by the Association of the Korean Oriental Medicine. The in-depth interview was conducted in 30 Korean medical doctors who answered in the previous telephone survey that they use moxibustion therapy for more than 30 % of patients everyday. Interview survey with them were conducted by the well-trained interviewers of Korea Institute of Oriental Medicine in medical researcher from 22th May to 28th June 2007. Results : Korean medical doctors prefer to use the moxibustion with moxa stick (83.9%). The most common treatment disease was musculo-skeletal disorder (28.9%), gynecology (14.1%), digestive disorder (28.6%). The most common treatment area was the abdomen (35.3%), back (22.5%), lower extremity (15.8%) and upper extremity (16.9%). Indirect moxibustion’s was as frequent as 73.5% of moxibustion method. The most common instrument for treating was mini-moxa of manufactured goods (39.3%). Most Korean medical doctors (48.8%) took up the position that bring symptom relief following good treatment area. 'High preferences of patients' (32.7%) was one of moxibustion strong points but 'Lots of smell and smoke' (54.7%)' was a weak points. The most common side effects was the 'slight burn' (34.9%), 'skin rash' (22.5%), 'skin pruritus' (23.8%) Conclusions : This survey provides unique insight into the perception of the Korea medical doctor at moxibustion therapy. Future research needs to provide more in-depth insight into doctor views of the experience.

Effects of Folium Artemisiae Argyi' Herbal Acupuncture of $LR_3$ on Transient Forebrain Ischemic Injury in Rats (태충(太衝) 애엽(艾葉) 약침(藥鍼)이 일과성(一過性) 전뇌(前腦) 허혈(虛血) 손상(損傷)에 미치는 효과(效果))

  • Herbal acupuncture has been used to prevent and treat the cerebrovascular accident, such as a stroke, and many studies of acupuncture and moxibustion concerning to the stroke have been undertaken in the human and various animals. The main characteristics of herbal acupuncture are a combination of acupuncture and herbal medicine. Study on Developing Assessment Guideline for Safety and Performance of Electric Moxibustion Apparatus (전기식 온구기에 대한 안전성 및 성능 평가 가이드라인 개발 연구)

  • Objectives : We developed and proposed a guideline for safety and performance assessment of electric moxibustion apparatus (class II medical device). Methods : We drafted the guideline for safety and performance assessment of electric moxibustion apparatus by referring the existing standards, guidelines and measurement data from commercially available products. Temperature characteristics such as maximum temperature and ramp time, and physical characteristics such as weight, noise and diameter were measured. User friendliness was also evaluated for commercial devices. Results : This guideline only can be applied to the electric moxibustion apparatus where moxa is being heated by electricity for medical proposes. Maximum temperature of higher than $50^{\circ}C$ can be achieved mostly. Ramp rate of temperature seems to be reliable. Control of temperature is needed to be improved. Moxa and its derivative products seem to be regulated for reliable temperature performance for clinical application. Requirements for design and development of electric moxibustion apparatus are suggested : temperature indicator, temperature control and its accuracy, safety measure, surge protection, user friendliness and instruction for use (IFU). Design recommendation of the reduction of noise level and energy loss are suggested for better products. Conclusions : We proposed a guideline for safety and performance assessment of electric moxibustion apparatus to improve the quality of relating products and aid their commercialization by aiming higher industrial competitiveness of the medical device sectors in Korea. Discussion with related institutes such as industry, academy and government is further required. Public hearings also need to be held prior to the establishment of a final guideline and standard.

  • Background : Stroke occurrences are prevalent and require intensive care for its treatment and rehabilitation. Acupuncture has been widely used in Korea throughout the history and provides an efficient method in the treatment of it. Objectives : To establish a distinctive and efficient acupuncture method for the treatment of stroke based in literature research. Method : We reviewed four Korean medical literature, $ll$治腫指南Guide to Swollen Sore Treatment$gg$, $ll$東醫寶鑑Treasured Mirror of Eastern Medicine$gg$, $ll$鍼灸經驗方Experiential Prescriptions of Acupuncture and Moxibustion$gg$, and $ll$舍岩鍼法Essential Rhymes on Acupuncture and Moxibustion by Master Sa-am$gg$, and analyzed the therapeutic characteristics in the treatment of stroke. Result : 1. In $ll$治腫指南Guide to Swollen Sore Treatment$gg$, various types of treatment can be found. Besides acupuncture, cupping, moxibustion with moxa tube, and bath therapy using Duchesneae Indicae Herba and Sal was applied. Needling in sublingual and vocal region, and also points like GB20, GB31, LI15, BL60, GV20, GV20, TE4, GB39, LU5, ST36, GB30 were frequently inserted. 2. In $ll$東醫寶鑑Treasured Mirror of Eastern Medicine$gg$, moxibustion treatment were in top priority. Points like GV20, LI15, LI11, GB31, ST36, GB39, LI4, GB20 pertinent to Governor, Conception, Gallbladder, Large Intestine, and Stomach meridian were most frequently needled. Selection of adjacent point was widely applied. 3. $ll$鍼灸經驗方Experiential Prescriptions of Acupuncture and Moxibustion$gg$ has some similarity compared to $ll$東醫寶鑑 Treasured Mirror of Eastern Medicine$gg$ in considering the moxa in top priority. But selected points far from the disease site. Main points were LI4, ST36, GB39, PC5, GV20, LI11, LR3, BL40, HT7. 4. $ll$校勘 舍岩道人鍼法Essential Rhymes on Acupuncture and Moxibustion by Master Sa-am$gg$ having the same content orders with $ll$鍼灸經驗方Experiential Prescriptions of Acupuncture and Moxibustion$gg$, it may had some influence from it. But the differences are also apparent. It emphasized in using mother-supplementing child-draining method, experiential prescriptions and visceral pattern identification. Conclusions : We could find various efficient methods through literature research of medical classics. This will not be limited in stroke alone but also will be applied in other diseases. This study will concurrently result in establishing distinctive therapeutic method characteristic of Korea.

  • Objective : This study was designed in order to support the standpoint that dementia will be treated better by remedy for heart rather than for brain, liver and kidney in Korean Medicine. Methods : We investigated which of five viscera make dementia from original texts about it. Then, 15 aged men living alone in Yeongdong-gun, Chungcheongbuk-do were treated for dementia by herb remedies, acupuncture and moxa. After it, we verified through method, for example, SMMSE-DS, CDT, and etc. Results : We had results that the main viscera causing dementia is heart, and heart is related with gall bladder, spleen, and kidney according to philological method. Clinical testing in this viewpoint showed satisfactory effects on the diseases. Conclusion : From the results, we proved it could be a new way of remedy for dementia that main the viscera causing dementia is heart in Korean Medicine.

  • Objectives The purpose of this case study was to report the main effect of Yangdokbaekho-tang to a Soyangin atopic dermatitis patient. Methods This patient was treated by Yangdokbaekho-tang, topical herbal extract, moxa and acupuncture according to the result of Sasang constitutional diagnosis. Results and Conclusions The patient's subjective symptoms of atopic dermatitis and general condition were improved after using Yangdokbaekho-tang in spite of patient's loose stool. The SCORAD index (Scoring Atopic Dermatitis Severity Index) which was used to evaluate the therapeutic effect of Sasang constitutional treatment on atopic dermatitis was also decreased from 86.3 to 42.8 without using anti-histamine or steroid medications.

  • Purpose: This study was conducted to examine the effects of Pericardium Kimek (K9) and Nei-Guan (P6) acupressure on relief of the anxiety, nausea, retching and physiologic changes (oxygen saturation, blood pressure, heart rate, temperature and respiration) in the patients undergoing unsedated esophagogastroduodenoscopy. Methods: A quasi-experimental research design was used. The participants were assigned into the listed three groups, i.e., the control group, the first experimental group (E1: K9 acupressure group) and the second experimental group (E2: P6 acupressure group). A total of 60 participants were included for analysis. Results: The severity of nausea (F=3.503, p=.037), the oxygen saturation (F=5.417, p=.007). and the respiration rate (F=7.270, p=.002) showed statistically significant differences in three groups. Conclusion: Based on the results of the present study, K9 acupressure with moxa pellets and P6 acupressure with wristbands (Sea-Band) are considered to be safe and an easy-to-apply intervention to relieve nausea and maintain the level of oxygen saturation and respiration rate in patients undergone unsedated esophagogastroduodenoscopy.

  • Objectives: The purpose of this study is to show a case of patient with epigastralgia which appeared after Stroke improved by Baekho-tang. Methods: We treated the patient with Herbal med, Acupuncture, Moxa. Results: Every Herbal med was no effect on Epigastralgia of patient. But The epigastralgia of patient was disappeared after using Baekho-tang. Conclusion: We can find out the effect of the Baekho-tang for epigastralgia which appeared after Stroke in this study.
