여닫이형 방사선 치료의 검증: 잔여 움직임의 선량적 영향 (Verification of Gated Radiation Therapy: Dosimetric Impact of Residual Motion)

  • 여닫이형 방사선 치료에서, 잔여 움직임으로 인하여 방사선은 실직적인 질병의 부위 뿐만이 아니라 주변 정상조직까지 투여 되도록 되어 있다. 비록 표적이 방사선 투여 중 움직이지만, 방사선이 최소한도로 실질적인 부위 (임상적 표적 체적)에 조사되기를 원한다. 본 연구의 목적은 여닫이형 치료에 있어서 방사선이 실질적인 표적에 투여되는지를 검증하고, 여닫이 범위, 움직임의 정도 및 임상적 표적체적의 크기의 변화에 따라, 표적및 주변 조직에 투여되는 방사선의 경향을 연구하는 데 있다. 이 목적을 달성하기 위하여, 실험 및 이론적인 연구를 고안하여 수행하였다. 직육각형 및 피라미드형의 표적 체적을 내포하는 팬텀을 만들어 움직이며 4차원 영상을 얻었다. 여러 여닫이 범위를 얻어진 영상에 적용하여 치료계획용 내부표적(표적체적 및 내부 움직임범위포함)을 만들었다. 직육각형 표적에는 전통적인 치료계획을 그리고 피라미드형 표적에는 세기 변조형 치료계획을 세웠다. 평판형 다이오드에 치료계획에서 얻어진 여닫이형 방사선을 수직으로 조사하여 실험적으로 선량평가를 수행하였고 또한 움직이는 상황에서 선량투여를 전산적으로 모사하였다. 본 연구는 두 표적에 대한 반음영 영역의 확장 및 움직임으로 인하여 방해 받았으나 확실하게 수행된 표적 선량투여 그리고 주변 조직에 투여된 상당량의 선량등을 수반하는 잔여움직임의 영향을 정량적으로 그리고 해석적으로 분석하였다. 선량-체적 히스토그램 분석에 따르면, 내부표적에는 여닫이 범위 또는 움직임 정도가 감소함에 따라 또한 표적체적이 증가함에 따라 선량이 증가함을 보였고, 내부 움직임 범위에 해당하는 체적에 대하여는 여닫이 범위 또는 움직임 정도가 감소함에 따라 선량이 증가하였고, 마지막으로 주변 정상조직에 대하여는, 내부 움직임 범위와는 반대의 경향을 보였다. 본 연구는 잔여움직임의 영향에 대하여 확실한 이해를 주었고 호흡행태가 재생되는한 불연속적인 투여과 표적의 움직임에도 불구하고 여당이형 방사선 치료는 안전함을 입증하였다. 본연구에서 수반된 절차와 전산적 모델은 여닫이형 치료의 시작점 검증, 주기적인 품질관리 및 환자별 검증에 사용될 수 있다. 환자별 영상에 선량을 재구성하는 방향으로 추후 연구가 필요하다.

호흡동조 양성자치료의 Quality Verification (Quality Verification for Respiratory Gated Proton Therapy)

  • 목 적: 호흡동조 시스템을 이용한 양성자치료 시 생성되는 양성자빔을 측정하고 분석함으로써 호흡동조 양성자치료의 정확성을 검증하고자 한다. 대상 및 방법: 호흡동조 양성자치료를 실시한 환자 세 명의 치료계획을 이용하여 양성자 치료 시스템에서 양성자빔을 생성했다. 이때 자체 제작한 구동 팬톰을 사용하여 호흡동조 시스템을 적용해 분할 조사되는 양성자빔을 재현하였다. 양성자빔의 중요특성인 Range, Spread-Out Bragg Peak (SOBP), Output factor를 각 5회씩 측정하였으며 호흡동조 시스템을 적용하지 않은 연속 양성자빔에서도 동일한 항목들을 측정했다. Range와 SOBP 측정에는 Multi-Layer Ionization Chamber, Output factor 측정에는 Scanditronix Wellhofer와 Farmer chamber를 사용했다. 결 과: 호흡동조 시스템을 적용한 경우 세 환자의 평균 Range는 7.226, 12.216, 19.918 $g/cm^2$, SOBP는 4.950, 6.496, 8.486 $g/cm^2$, Output factor는 0.985, 1.026, 1.138 cGy/MU였으며, 적용하지 않은 경우 평균 Range는 7.230, 12.220, 19.920 $g/cm^2$, SOBP는 4.940, 6.512, 8.490 $g/cm^2$, Output factor는 0.984, 1.027, 1.136 cGy/MU로 측정됐다. 평균 Range의 차는 -0.004, -0.004, -0.002 $g/cm^2$, SOBP는 0.010, -0.016, -0.004 $g/cm^2$, Output factor는 0.001, -0.001, 0.002 cGy/MU로 나타났다. 결 론: 호흡동조 양성자치료 시 분할 조사된 양성자빔과 연속 조사된 양성자빔의 Range, SOBP 및 Output factor 모두 차이가 미미했다. 따라서 본 연구를 통해 호흡동조 시 발생된 양성자빔의 선질을 검증하였고 이를 이용한 양성자치료의 정확성 또한 확인할 수 있었다.

위상기반 호흡연동 방사선치료 시 오차 분석 프로그램 개발 (Development of Error Analysis Program for Phase-based Respiratory Gating Radiation Therapy)

  • 방사선 치료 시 환자 호흡에 의한 치료부위의 움직임 영향을 줄이기 위해 환자 복부에 배치한 적외선 반사체 표식자의 호흡에 따른 주기적 움직임을 분석하여 안정된 일정 영역에서만 방사선이 조사되도록 하는 호흡연동 방사선치료 방법이 임상에서 활용되고 있다. 이러한 호흡연동 방사선치료에서 표식자의 운동 주기 중 안정된 특정 위상영역을 기반으로 방사선 조사 구간을 설정하는 방법은 진폭변위를 기반으로 하는 방법보다 안정적으로 치료를 수행할 수 있는 장점이 있으나, 치료 시 방사선 조사 구간 내 표식자 진폭의 변화가 발생해도 설정 위상 기반영역에만 포함되면 방사선이 조사되는 경우가 발생할 수 있어, 실제 호흡량의 변위에 따른 내부 장기의 움직임 정도를 정확히 반영하기에는 한계가 있다. 본 연구에서는 이와 같은 위상기반 호흡연동 방사선치료 시 발생할 수 있는 표식자 진폭의 변화와 이로 인한 치료 부위의 움직임에 의한 오차를 분석할 수 있는 프로그램을 개발하여 정확성과 임상 적용 타당성을 분석하였다. 모의치료 시 설정된 방사선 조사 구간 내 가상의 진폭 변위를 주어 작성한 치료 기록 파일과 팬텀을 사용하여 임상에서 발생할 수 있는 진폭 변위의 크기와 불규칙한 변화가 발생하였을 경우의 환경 설정 후 방사선을 조사하고 생성된 치료 기록 파일들을 개발한 프로그램으로 분석하였을 때, 정확한 오차 구간의 판별과 오차 계산이 수행됨을 확인할 수 있었다. 실제 위상기반 호흡연동 방사선 치료 환자에 적용하였을 때에도 설정된 허용기준을 벗어나는 지점의 오차 영역 인식과 오차 계산이 정확히 수행됨을 확인하여, 향후 위상기반 호흡연동 방사선치료 시 발생할 수 있는 방사선 조사 구간 내 환자 호흡량의 변화로 인한 치료 표적의 변위 오차 분석에 유용한 도구로 사용될 것으로 생각된다.

소동물영상을 위한 마이크로 컴퓨터단층촬영장치 (Micro-CT System for Small Animal Imaging)

  • 살아있는 마우스 영상화를 목적으로 겐트리 회전형과 평판영상검출기를 기반으로 한 고분해능 마이크로 컴퓨터단층촬영 장치를 개발하였다. 이 장치는 주로, 마이크로 크기 광원사이즈를 갖는 X-선 광원, Csl (TI)과 결합된 평판형 상보성 금속산화 반도체 영상검출기(CMOS), 선형이송 카우치, 위치정보 엔코더와 결합된 겐트리, 그리고 영상데이터 처리를 위한 병렬처리 시스템으로 구성되었다. 본 장치는 겐트리 회전형으로 설계되었는데, 이는 살아있는 마우스를 CT 영상을 얻는데 있어서 마우스 움직임에 기인한 영상결점의 최소화에 유리하고 촬영하는 동안 쥐의 호흡마취시행에 여러 가지 장점을 갖기 때문이다. CT팬텀을 이용하여 개발한 CT장치의 공간해상도, 영상대비도 그리고 영상균일도를 평가하였다. 결과로써, 본 장치의 공간해상도는 MTF 곡선으로부터 10%에 해당하는 약 11.3 cycles/mm을 얻었으며, 마우스에 대한 방사선 피폭선량은 81.5 mGy의 결과를 얻었다. 저대비 영상팬텀을 이용한 영상실험에서 분해가능 최소영상대비차는 약 46 CT였다. $55{\times}55{\times}X100\;{\mu}^3$의 복셀(voxel) 크기에서 영상의 불균일도는 약 70 CT 임을 얻었다. 또한 본 연구에서는 살아있는 마우스의 몸체, 뼈, 그리고 간에 대한 영상 테스트 결과를 제시하였다.

이차원 양성자 선량 분포 확인을 위한 즉발감마선 이차원분포 측정 장치 개발 (Development of Two-dimensional Prompt-gamma Measurement System for Verification of Proton Dose Distribution)

  • 양성자 치료 시 양성자 빔을 사용하여 정밀한 치료를 수행하고 환자의 안전을 제고하기 위해서는 인체 내 양성자 빔의 선량 분포를 확인하는 것이 중요하다. 이를 위해 본 연구팀은 이전 연구에서 1차원 배열형 검출기를 종형으로 배치하여 빔 진행방향으로 이동시켜가면서 2차원적인 즉발감마선 분포를 측정하여 양성자 선량 분포와 유사한 분포를 갖는 것을 확인하였다. 본 연구에서는 이를 바탕으로 즉발감마선 이차원분포를 실시간으로 측정하기 위해 이차원 평행다공형 집속 장치, 이차원 배열의 NaI(Tl) 섬광체와 MA-PMT로 이루어진 즉발감마선 이차원분포 측정 장치를 개발하였다. 개발한 즉 발감마선 이차원분포 측정 장치의 성능을 평가한 결과 $^{22}Na$ 감마선원의 에너지 분해능은 $10.9%{\pm}0.23p%$ (0.511 MeV)와 $6.4%{\pm}0.24p%$ (1.275 MeV)로 평가되었으며, $^{137}Cs$$9.8%{\pm}0.18p%$ (0.662 MeV)로 평가되었다. 고에너지 Am-Be 선원의 double escape 피크인 3.416 MeV의 에너지 분해능은 $11.4%{\pm}3.6p%$로 평가되었다. 본 측정 장치를 이용하여 45 MeV 양성자 빔을 PMMA에 조사하여 발생한 즉발감마선 이차원분포를 측정한 결과 0.5 nA의 세기의 양성자 빔에서 $9{\times}10^9$ 양성자 조사 시 양성자 선량 분포와 유사한 즉발감마선 이차원분포를 측정할 수 있음을 확인하였다. 추가로 측정한 즉발감마선 이차원분포의 프로파일 분포를 이용하여 양성자 빔의 비정을 평가해 본 결과 $17.0{\pm}0.4mm$로 평가되었고, 실제 비정인 17.4 mm과 굉장히 밀접한 관련이 있음을 확인하였다.

호흡연동 래피드아크 치료 시 빔 조사 구간 설정에 따른 선량 변화 분석 (Dosimetric Analysis of Respiratory-Gated RapidArc with Varying Gating Window Times)

  • 호흡연동 래피드아크 치료는 무건운 하중의 선형가속기 갠트리의 회전과 정지의 반복과정에서 갠트리 회전 재시작점의 오차와 다엽조리개의 정확한 움직임 및 갠트리 속도와 같은 래피드아크의 선량정확도를 결정하는 요소들의 오차 가능성으로 인한 선량 오류가 발생할 수 있다. 본 연구에서는 이러한 갠트리의 회전과 정지의 반복적인 동작이 호흡연동 래피드아크 치료의 정확도에 어떠한 영향을 끼치는 지 치료 시 총 회전과 정지 동작 수를 결정하는 빔조사 구간 길이의 변화를 통해 분석하였다. 총 10명의 간암 환자를 대상으로 래피드아크 치료계획을 수립하였고, RPM 호흡연동 장치와 정확한 빔조사 구간 길이를 설정하기 위해 동적 팬텀을 사용하였다. 각 래피드아크 치료계획 당 EPID를 사용한 portal dosimetry delivery quality assurance (DQA) 계획과 이차원 다이오드 검출기배열장치인 MapCHECK2를 사용한 DQA 계획을 수립하여 호흡연동 방사선 치료과정에서 누적된 선량분포의 정확도를 분석하였다. 모든 환자의 호흡주기는 4초로 설정하였고, 수립한 DQA 계획들을 호흡연동 없이 연속적으로 조사하는 것과, 1초의 빔조사 구간과 2초의 빔조사 구간, 총 3가지의 경우에 대해 실제 방사선량 측정과 감마평가를 통해 선량의 정확도를 분석하였다. Portal dosimetry DQA 경우 평균 감마평가의 합격률은 호흡연동 없이 연속적일 때 $98.72{\pm}0.82%$ 였고, 1초의 빔조사 구간의 경우 $94.91{\pm}1.64%$, 2초의 빔조사 구간의 경우 $98.23{\pm}0.97%$이었다. MapCHECK2 DQA경우 평균 감마평가의 합격률은 호흡연동 없이 연속적일 때 $97.80{\pm}0.91%$였고, 1초의 빔조사 구간의 경우 $95.38{\pm}1.31%$, 2초의 빔조사 구간의 경우 $97.50{\pm}0.96%$였다. 본 연구 결과를 통해 빔조사 구간의 길이가 증가하여 갠트리 정지 동작 수가 감소할수록 호흡연동 래피드아크의 선량 정확도가 증가함을 확인할 수 있었으며, 이러한 특성을 호흡연동 방사선치료 대상 환자의 선정 및 호흡방식에 대한 교육과정에 고려되어야 할 것으로 판단되었다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Our department has developed an information system for routine hematology using down-sized computer system. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms.
