Effectiveness of Intra-anal Biofeedback and Electrical Stimulation in the Treatment of Children With Refractory Monosymptomatic Nocturnal Enuresis: A Comparative Randomized Controlled Trial

  • Purpose: To compare the effects of intra-anal biofeedback (BF) and intra-anal electrical stimulation (ES) on pelvic floor muscles (PFMs) activity, nocturnal bladder capacity, and frequency of wet night episodes in children with refractory primary monosymptomatic nocturnal enuresis (PMNE). Methods: Ninety children of both sexes aged 8-12 years with refractory PMNE participated in this study. They were randomly assigned to 3 groups of equal number: control group (CON) that underwent behavioral therapy and PFM training, and 2 study groups (BF and ES) that underwent the same program in addition to intra-anal BF training and intra-anal ES, respectively. PFMs activity was assessed using electromyography, nocturnal bladder capacity was evaluated by measuring the first morning voided volume, and a nocturnal enuresis diary was used for documenting wet night episodes before treatment and after 3 months of treatment. Results: After training, all groups showed statistically significant improvements in all measured outcomes compared to their pretreatment findings. The ES group showed significantly greater improvements in all measured outcomes than the CON and BF groups. Conclusions: Both intra-anal BF training and ES combined with behavioral therapy and PFMs training were effective in the treatment of PMNE, with intra-anal ES being superior to BF training.

A Case Report of Esophageal Ulceration after Taking Doxycycline (독시사이클린 복용 후 발생한 식도궤양형성 사례)

  • Doxycycline is frequently prescribed to treat urogenital infections or acne vulgaris. A 39-year-old man was prescribed doxycycline as a treatment for his skin infection. He took each tablet on an empty stomach in the morning, and just before going to bed with insufficient water. After taking the prescribed medication for approximately 3 weeks, he was diagnosed with esophageal ulcer. He had not experienced these symptoms before, and the drug label says that doxycycline rarely causes esophageal ulcer. After discontinuing the medication, the symptoms disappeared. Using the World Health Organization-Uppsala Monitoring Center criteria, the causality for doxycycline-induced esophageal ulcer was evaluated as "possible." Additionally, using the Naranjo scale and Korean causality assessment algorithm (Ver. 2), the causality was evaluated as "probable." Previous reports have stated that most patients who took doxycycline with insufficient water just before going to bed or in the supine position developed esophageal ulcer. Therefore, all patients taking doxycycline must be provided with detailed instructions about the appropriate administration methods.

Analysis of the Correlation between Urban High Temperature Phenomenon and Air Pollution during Summer in Daegu

  • Recently, summer high temperature events caused by climate change and urban heat island phenomenon have become a serious social problem around the world. Urban areas have low albedo and huge heat storage, resulting in higher temperatures and longer lasting characteristics. To effectively consider the urban heat island measures, it is important to quantitatively grasp the impact of urban high temperatures on the society. Until now, the study of urban heat island phenomenon had been carried out focusing only on the effects of urban high temperature on human health (such as heat stroke and sleep disturbance). In this study, we focus on the effect of urban heat island phenomenon on air pollution. In particular, the relationship between high temperature phenomena in urban areas during summer and the concentration of photochemical oxidant is investigated. High concentrations of ozone during summer are confirmed to coincide with a day when the causative substances (NO2,VOCs) are high in urban areas during the early morning hours. Further, it is noted that the night urban heat island intensity is large.. Finally, although the concentration of other air pollutants has been decreasing in the long term, the concentration of photochemical oxidant gradually increases in Daegu.

Physiotherapy Approach to Patients with Chronic Plantar Fasciitis : Comparison of the Effects of Specific Stretching Exercise and High-Load Strengthening Exercise (만성 발바닥 근막염 환자에 대한 물리치료적 접근법: 특이적 뻗침운동과 고부하 강화 운동의 효과 비교)

  • Purpose : This study was to investigate the effectiveness of ESWT and plantar fascia-specific stretching exercise vs ESWT and high-load strengthening exercise in patients with chronic plantar fasciitis. Methods : The subjects were randomized to extracorporeal shock wave therapy ( ESWT, for 3 weeks) and daily plantar-specific stretching (Group I: Stretch group) or ESWT and high-load progressive strength (Group II: Strength group) performed every second day. The main outcome measures were ultrasound, visual analogue scale (VAS), and Korean Foot Function Index (KFFI). The ultrasound (plantar fascia thickness), pain intensity I, II (the most painful of the day?, the pain when you first step in the morning?) and KFFI (functional performance) were compared between the groups. Results : No significant difference was observed between the groups in the plantar fascia thickness but pain intensity I, II was significantly lower in Group 2 than in Group 1 at only 12weeks and functional performance was also significantly increased in Group 2 compared to Group 1 at only 12 weeks. Conclusion : The high-load strengthening exercise consisting of the progressive exercise protocol, resulted in superior after 12 weeks compared with plantar-specific stretching. High-load strength exercise may aid in a quicker reduction in pain and improvements in functional performance.

  • Objectives The purpose of this report was to study the circulation of meridian WiGi, YoungGi, from the viewpoint of manual medicine. Methods First, the Korean Medical approach analyzes documents about the circulation of meridian WiGi, YoungGi, and the biomechanical approach is to analyze documents about kinetic force and kinematic movement. The third inherent energy approach is to analyze documents about craniosacral rhythm and visceral motility. Finally, it is to study the correlation between the circulation of meridian WiGi, YoungGi, and the viewpoint of biomechanics force and movement, the inherent energy of manual medicine. Results Meridian WiGi is fast, powerful, and changeful. It circulates through the head and extremities in the daytime and visceral organs at night. The deviation pelvis and distorted thoracic cage create kinetic force and kinematic movement. Meridian YoungGi is very small and soft energy and circulates meridians and visceral organs permanently. Craniosacral rhythm and visceral motility radiate continuously from cranial and visceral organs to the whole body. Conclusions Circulation of meridian WiGi is closely related to the biomechanical approach. In addition, circulation of meridian YoungGi is closely related to the inherent energy approach.

Treatment of Cushing's disease with macroadenoma through transsphenoidal surgery (나비굴경유 수술을 통해 완치된 뇌하수체 거대선종에 동반된 쿠싱병 1예)

  • Cushing's disease (CD) is a rare illness characterized by chronic hypercortisolism secondary to the overproduction of adrenocorticotropic hormone by a pituitary adenoma, which is associated with a high risk of developing serious complications, such as diabetes mellitus, cardiovascular disease, and emotional disorders. Endoscopic transsphenoidal surgery is performed for the treatment of CD, and was initially preferred over other types of treatments. However, the recurrence after pituitary surgery for CD is a common problem after an initial successful surgery. In microadenomas, the remission rates were higher than those of macroadenoma. This patient had a giant tumor that was greater than 4 cm in length on sella magnetic resonance imaging, and panhypopituitarism was detected using a combined pituitary stimulation test. After transsphenoidal surgery, the patient required temporary hormone replacement for a short period of time. After 1 year, he showed a normal cortisol response on the overnight dexamethasone suppression test and low morning cortisol levels. Therefore, we indicated that the patient was cured of giant macroadenoma with panhypopituitarism before surgery, and thus, reported this case.

Travel Behavior Analysis using Origin-Destination Data for the Subway Line No.7 (수도권 지하철 7호선 주요역 통근통행특성 분석 연구)

  • Recent data development has made it possible to analyze each individual's daily commuting by using transportation card transaction. This research utilizes about 1 million observations from the subway line no.7 of Seoul metropolitan transportation data. By using such a massive dataset, the authors try to identify daily travel behavior of morning commute and its possible relationship between subway usage and socio-economic factors. There are 4 main types of users and their travel behavior, and top 15 stations with the most users for arrival and departure are selected. Accordingly, 15 stations have distinctive characteristics including population density and the number of businesses around stations. To identify this fact, the 4 most populated stations are selected and their socio-economic factors are examined. According to the analysis, the most departure stations are generally surrounded by hihgly populated residential areas, whereas the most arrival stations are stood within the job concentrated districts.

Factors Associated With Quitting Smoking in Indonesia

  • Objectives: The aim of this study was to identify factors associated with quitting smoking in Indonesia Methods: Data on 11 115 individuals from the fifth wave of the Indonesia Family Life Survey were analyzed. Quitting smoking was the main outcome, defined as smoking status based on the answer to the question "do you still habitually (smoke cigarettes/smoke a pipe/use chewing tobacco) or have you totally quit?" Logistic regression was performed to identify factors associated with successful attempts to quit smoking. Results: The prevalence of quitting smoking was 12.3%. The odds of successfully quitting smoking were higher among smokers who were female (adjusted odds ratio [aOR], 2.69; 95% confidence interval [CI], 2.08 to 3.33), were divorced (aOR, 2.45; 95% CI, 1.82 to 3.29), did not chew tobacco (aOR, 3.01; 95% CI, 1.79 to 5.08), found it difficult to sacrifice smoking at other times than in the morning (aOR, 1.29; 95% CI, 1.14 to 1.46), and not smoke when sick (aOR, 1.32; 95% CI, 1.14 to 1.54). About 59% of variance in successful attempts to quit smoking could be explained using a model consisting of those variables. Conclusions: Female sex, being divorced, not chewing tobacco, and nicotine dependence increased the odds of quitting smoking and were associated with quitting smoking successfully. Regular and integrated attempts to quit smoking based on individuals' internal characteristics, tobacco use activity, and smoking behavior are needed to quit smoking.

Analysis of the effect in the city due to the bridges incidents in Songdo International City (송도국제도시 연결도로의 유고상황 발생에 따른 신도시 내부 영향 분석)

  • The purpose of this study is to analysis the impact on the inside of the new city when an incidents occurs on the Songdo International City connecting road, which has a limited access. The analysis data used KTDB's O/D and network data of the Seoul metropolitan area. In addition, the scenario composition applied a method of reducing the number of lanes on the road according to the situation of incidents, targeting bridges advancing from Songdo International City to the outside in the morning peak hours. The analysis method analyzed the traffic volume, total travel time, total travel kilometer, and route change in the new city based on the results of the traffic allocation model. As a result of the analysis, the range of influence was shown to two types. First, of the seven bridges, Aam 3, Aam 2, and Aam 1 were analyzed to have an impact only in some areas of the northwestern part of the new city. On the other hand, the remaining bridges were analyzed to affect the new city as a whole. The analysis results of this study are expected to be used as basic data to establish the scope of internal road network management when similar cases occur in the future.

Correlation Analysis of Pressure Pain Threshold and Muscle Thickness in Individuals with Non-Specific Low Back Pain

  • Objective: Low back pain (LBP) is a symptom that accounts for a large proportion of musculoskeletal pain. Among them, non-specific LBP (NSLBP) means that the pathological cause is unknown, and belongs to the most common LBP. Studies on the mechanism of muscle control in LBP are insufficient, and quantitative studies are needed. Design: Observational cross-sectional study design Methods: A Thirty participants with NSLBP symptoms were enrolled, and their pressure pain thresholds (PPT) and muscle and fat thickness were measured. Participants measured the paraspinal muscles (PM) of the thoracic and lumbar spine and medial hamstring (semitendinosus) on the dominant and non-dominant sides in the prone position. Results: Among the variables that were significant in the correlation analysis, PM of the thoracic and lumbar spine showed a significant relationship in the PPT ([thoracic spine PM]=1.141+0.912 [lumbar spine PM]). Also, there was a significant relationship between the lumbar spine PM in the PPT and the thoracic spine PM in the muscle thickness ([lumbar spine PM of PPT]=4.057+0.117 [thoracic spine PM of muscle thickness]) Conclusions: Although there was no muscle imbalance according to the dominant and non-dominant side, there is a correlation between the pressure pain threshold and the muscle thickness between the paraspinal muscles of the thoracic spine and the lumbar spine.
