리튬 디실리케이트 최종 결정화 과정이 CAD/CAM으로 제조된 수복물의 적합도에 미치는 영향 (Influence of final crystallization process on precision of fit of monolithic CAD/CAM-generated restorations by lithium disilicate: A comparative study)

  • Purpose: To quantify the effect of the crystallization process on lithium disilicate ceramic crowns that are fabricated using a computer-aided design/computer-aided manufacturing(CAD/CAM) system, and to determine whether they are clinically acceptable by comparing values before and after the crystallization process. Methods: The maxillary first molar was selected as the abutment for the experiments. Ten working models were prepared. Marginal and internal gap of 4 groups of lithium disilicate crowns(n=10) fabricated with conventional method. Comparison was performed using the silicone replica technique and 3D superimposition analysis. The marginal and internal gaps of the restoration were measured before and after the crystallization process of this prosthesis. The average value of each part(the average of values measured before and after the crystallization) was statistically analyzed using paired t-test(α=0.05). Results: The results from the second phase of this research, which compared the average value of the gap between the marginal and internal fits of the lithium disilicate single crown before and after the crystallization process, indicated that the marginal gap was larger and the internal gap was smaller after the crystallization process, and this difference was statistically significant (P<0.05) in all the parts evaluated. Conclusion: While the shrinkage that occurs during crystallization does affect the marginal and internal fit of the prosthesis, it cannot be concluded to be a major effect because the resultant distortion was within the clinically acceptable range.

Influence of abutment height and convergence angle on the retrievability of cement-retained implant prostheses with a lingual slot

  • PURPOSE. Cement-retained implant prostheses can lack proper retrievability during repair, and residual cement can cause peri-implantitis. The purpose of this in vitro study was to evaluate the influence of abutment height and convergence angle on the retrievability of cement-retained implant prostheses with lingual slots, known as retrievable cement-type slots (RCS). MATERIALS AND METHODS. We fabricated six types of titanium abutments (10 of each type) with two different heights (4 mm and 6 mm), three different convergence angles ($8^{\circ}$, $10^{\circ}$, and $12^{\circ}$), a sloped shoulder margin (0.6 mm depth), a rectangular shape ($6mm{\times}6.5mm$) with rounded edges, and a rectangular ledge ($2mm{\times}1mm$) for the RCS. One monolithic zirconia crown was fabricated for each abutment using a dental computer-aided design/computer-aided manufacturing system. The abutments and crowns were permanently cemented together with dual-curing resin cement, followed by 24 hours in demineralized water at room temperature. Using a custom-made device with a slot driver and torque gauge, we recorded the torque ($N{\cdot}cm$) required to remove the crowns. Statistical analysis was conducted using multiple regression analysis and Mann-Whitney U tests (${\alpha}=.05$). RESULTS. Removal torques significantly decreased as convergence angles increased. Multiple regression analysis showed no significant interaction between the abutment height and the convergence angle (Durbin-Watson ratio: 2.186). CONCLUSION. Within the limitations of this in vitro study, we suggest that the retrievability of cement-retained implant prostheses with RCS can be maintained by adjusting the abutment height and convergence angle, even when they are permanently cemented together.

Wear of primary teeth caused by opposed all-ceramic or stainless steel crowns

  • PURPOSE. This study was conducted to evaluate the effects of full-coverage all-ceramic zirconia, lithium disilicate glass-ceramic, leucite glass-ceramic, or stainless steel crowns on antagonistic primary tooth wear. MATERIALS AND METHODS. There were four study groups: the stainless steel (Steel) group, the leucite glass-ceramic (Leucite) group, the lithium disilicate glass-ceramic (Lithium) group, and the monolithic zirconia (Zirconia) group. Ten flat crown specimens were prepared per group; opposing teeth were prepared using primary canines. A wear test was conducted over 100,000 chewing cycles using a dual-axis chewing simulator and a 50 N masticating force, and wear losses of antagonistic teeth and restorative materials were calculated using a three-dimensional profiling system and an electronic scale, respectively. Statistical significance was determined using One-way ANOVA and Tukey's test (P<.05). RESULTS. The Leucite group ($2.670{\pm}1.471mm^3$) showed the greatest amount of antagonist tooth wear, followed by in decreasing order by the Lithium ($2.042{\pm}0.696mm^3$), Zirconia ($1.426{\pm}0.477mm^3$), and Steel groups ($0.397{\pm}0.192mm^3$). Mean volume losses in the Leucite and Lithium groups were significantly greater than in the Steel group (P<.05). No significant difference was observed between mean volume losses in the Zirconia and Steel groups (P>.05). CONCLUSION. Leucite glass-ceramic and lithium disilicate glass-ceramic cause more primary tooth wear than stainless steel or zirconia.

Correlation between microhardness and wear resistance of dental alloys against monolithic zirconia

  • Purpose. The aim of this study is to compare the hardness according to the conditions of metal alloys. Moreover, the correlation between the cast crown hardness before and after wear testing and the degree of wear for each dental alloy was assessed. Materials and Methods. Cast crowns of three metal alloys (Co-Cr, gold, and Ni-Cr alloys) opposing smooth-surface monolithic zirconia were used. The Vickers microhardness of the ingot (which did not undergo wear testing) and the cast crown before and after wear testing were measured for each alloy. Two-way ANOVA and Scheffé tests were used to compare the measured hardness values. Moreover, the Pearson correlation coefficient was used to evaluate the relationship between the surface hardness and the wear of the cast crown (α=.05). Results. There was no significant difference in the hardness before and after wear testing for the gold alloy (P>.05); however, the hardness of the worn surface of the cast crown increased compared to that of the cast crown before the wear tests of Ni-Cr and Co-Cr alloys (P<.05). Furthermore, there was no correlation between the wear and hardness of the cast crown before and after wear testing for all three metal alloys (P>.05). Conclusion. There was a significant difference in hardness between dental alloys under the same conditions. No correlation existed between the surface hardness of the cast crown before and after wear testing and the wear of the cast crown.

상악 완전 무치악 및 하악 부분 무치악 환자에서 임플란트 지지형 고정성 보철물을 이용한 전악 수복 증례 보고 (Full-mouth rehabilitation with implant-supported fixed dental prostheses for the edentulous maxilla and partially edentulous mandible: A case report)

  • 넓은 영역의 무치악 부위를 수복하기 위한 전통적인 방법은 의치를 이용한 치료다. 하지만, 임플란트를 활용할 경우 보철물의 안정과 유지, 그리고 지지의 측면에서 더 유리하며, 저작효율이 향상되는 등 기능적으로 더 양호한 결과를 얻을 수 있다. 이 같은 임플란트를 이용한 치료방법은 크게 가철성과 고정성 방식으로 분류될 수 있으며, 치조골의 흡수정도, 악간관계, 환자의 선호도, 사회 경제적 요인 등을 고려하여 적절한 접근 방식을 선택하여야 한다. 한편, 지르코니아의 물성이 크게 개선됨에 따라, 임플란트 지지형 고정성 보철물의 재료로서 단일구조 지르코니아가 각광을 받고 있다. 단일구조 지르코니아는 지르코니아 코어에 세라믹을 비니어링 한 보철물이나 금속 도재관보다 파절과 chipping의 빈도가 적으며 지르코니아 자체가 충분한 두께를 가질 수 있어 구조적으로 좋은 물성을 지닐 수 있다. 최근에는 투명도가 증진된 단일구조 지르코니아도 출시되어, 전치부 보철물을 위한 재료로도 널리 사용되고 있다. 본 증례의 남환은 상악 양측 구치부의 결손 및 다수 잔존치의 중등도 이상의 전반적 만성 치주염 이환으로 인해, 하악 양측 견치 및 좌측 제1, 2 소구치를 제외한 모든 치아를 발치한 후, 상악 8개 및 하악 3개의 임플란트 식립을 동반한 전악 수복 치료를 받았다. 보철물 장착 후 1년 간의 경과 관찰 기간 동안 기능적, 심미적으로 만족할만한 결과를 얻어 이를 보고하는 바이다.

CAD-CAM을 이용한 RPD 지대치의 retrofit crown 제작 증례 (Fabricating retrofit crowns to an existing removable partial denture by CAD-CAM: a case report)

  • 가철성 국소의치를 사용하는 환자의 잔존 치아가 우식, 마모, 부식, 파절 등의 이유로 보철 수복이 필요한 경우가 있다. 가철성 국소의치의 구조물과 접촉하는 부분이 많은 지대치의 보철 수복이 필요한 경우, 서베이드 크라운을 제작 후 가철성 국소의치를 새로 제작하는 것이 통상적인 방법이다. 이럴 경우, 환자가 적응하기까지 오랜 시간이 걸리고, 시간적, 경제적 비용이 발생한다. 한편, 손상된 치아 외에 구강 내 다른 부분에서 가철성 국소의치의 지지, 유지 및 안정을 일부분 제공하는 경우 기존 국소의치의 사용은 임상적으로 양호하다. 이 때 기존 국소의치를 유지하면서 해당 치아를 보철 수복한다면, 환자는 불편과 비용을 줄이면서 기존 가철성 국소의치를 사용할 수 있다. 이처럼 국소의치에 맞추어 보철을 제작하는 retrofit crown 제작과정에서 Computer Aided Design-Computer Aided Manufacturing(CAD-CAM)의 발전과 지르코니아의 물리적 성질이 개선되면서 디지털 방식이 적용되고 있다. 본 증례는 하악에서 가철성 국소의치를 사용하는 환자의 지대치가 심하게 마모되어 국소의치에 맞추어 지르코니아 크라운을 제작한 케이스로, 환자와 술자 모두 만족하는 결과를 얻었기에 본 증례를 보고하는 바이다.

부분 무치악 환자에서 고기능성 폴리머(PEKK) 지대주를 이용한 임플란트 고정성 보철물 수복 증례 (Implant-supported fixed prostheses with high-performance polymer (PEKK) abutments in partial edentulous patients: A case report)

  • 완전무치악 환자한테 처음으로 시도되었던 임플란트 보철치료는 현재는 단일치아 상실된 경우까지 그 활용범위가 넓어졌으며, 임플란트 보철치료는 현대 치과에서는 거의 필수적인 치료방법으로 자리매김 하였다. 임플란트 보철물을 제작하는데 사용되는 재료도 초기에 비해 다양해졌는데, 그 중 하나가 고기능성 폴리머의 적용이다. 예전부터 의과에서 많이 사용되어 왔던 고기능성 폴리머를 치과에서도 시도하는 빈도가 점점 늘어나고 있다. 이에 고기능성 폴리머를 구치부 임플란트 지대주로 사용한 3가지 증례를 보고하고자 한다. 첫번째 증례는 하악 좌측 제2소구치가 상실된 부위에 PEKK 지대주 및 PFG 크라운(시멘트-나사-유지형)을 제작, 장착하였으며, 두번째 증례는 상실된 상악 좌측 제1대구치 부위에 PEKK 지대주 및 monolithic zirconia 크라운(시멘트-나사-유지형)을 제작, 장착하였으며, 마지막 증례는 상악 우측 제1,2 대구치 상실부위에 PEKK 지대주 및 splinted PFM 크라운(시멘트-나사-유지형)을 제작하여 장착하였다. 4년의 추적 관찰 결과 상기 환자 모두 심미적 및 기능적으로 만족스러운 결과를 얻었기에 증례를 보고하고자 한다.

The effect of thermo-mechanical fatigue on the retentive force and dimensional changes in polyetheretherketone clasps with different thickness and undercut

  • PURPOSE. Esthetic expectations have increased the use of polyetheretherketone (PEEK) clasps as alternatives to Cr-Co in removable partial dentures (RPDs). The objective of this study was to evaluate the retentive force and dimensional change of clasps with different thickness and undercut made from PEEK by the thermo-mechanical fatigue. MATERIALS AND METHODS. PEEK clasps (N = 48) with thicknesses of 1 or 1.50 mm and 48 premolar monolithic zirconia crowns with undercuts of 0.25 mm or 0.50 mm were fabricated. Samples are divided into four groups (C1-C4) and were subjected to 7200 thermal aging cycles (at 5 - 55℃). The changes in the retentive force and dimensions of the clasps were measured by micro-stress testing and micro-CT devices from five measurement points (M1 - M5). One-way ANOVA, paired t-test, two-way repeated ANOVA, and post-hoc tests were used to analyze the data (P < .05). RESULTS. The retentive forces of C1, C2, C3, and C4 groups in initial and final test were found to be 4.389-3.388 N, 4.67 - 3.396 N, 5.161 - 4.096 N, 5.459 - 4.141 N, respectively. The effects of retentive force of all PEEK clasps groups were significant decreased. Thermo-mechanical cycles caused significant dimensional changes at points with M2, M4, and M5, and abraded the clasp corners and increased the distance between the ends of the clasp, resulting in reduced retentive forces (P* = .016, P* = .042, P < .001, respectively). CONCLUSION. Thermo-mechanical aging decreases the retentive forces in PEEK clasps. Increasing the thickness and undercut amount of clasps decreases the amount of dimensional change. The values measured after aging are within the clinically acceptable limits.
