Three-dimensional evaluation of tooth movement in Class II malocclusions treated without extraction by orthodontic mini-implant anchorage

  • Objective: The aim of this study was to analyze tooth movement and arch width changes in maxillary dentition following nonextraction treatment with orthodontic mini-implant (OMI) anchorage in Class II division 1 malocclusions. Methods: Seventeen adult patients diagnosed with Angle's Class II division 1 malocclusion were treated by nonextraction with OMIs as anchorage for distalization of whole maxillary dentition. Three-dimensional virtual maxillary models were superimposed with the best-fit method at the pretreatment and post-treatment stages. Linear, angular, and arch width variables were measured using Rapidform 2006 software, and analyzed by the paired t -test. Results: All maxillary teeth showed statistically significant movement posteriorly (p < 0.05). There were no significant changes in the vertical position of the maxillary teeth, except that the second molars were extruded (0.86 mm, p < 0.01). The maxillary first and second molars were rotated distal-in ($4.5^{\circ}$, p < 0.001; $3.0^{\circ}$, p < 0.05, respectively). The intersecond molar width increased slightly (0.1 mm, p > 0.05) and the intercanine, interfirst premolar, intersecond premolar, and interfirst molar widths increased significantly (2.2 mm, p < 0.01; 2.2 mm, p < 0.05; 1.9 mm, p < 0.01; 2.0 mm, p < 0.01; respectively). Conclusions: Nonextraction treatment with OMI anchorage for Class II division 1 malocclusions could retract the whole maxillary dentition to achieve a Class I canine and molar relationship without a change in the vertical position of the teeth; however, the second molars were significantly extruded. Simultaneously, the maxillary arch was shown to be expanded with distal-in rotation of the molars.

  • The purpose of this study is to observe how the Class III intermaxillary elastics act upon the craniofacial structures of mixed dentition with anterior crossbites. The cephalometric headplafes of 16 childrens treated only with Class III elastics (C III elastics group) and 23 childrens treated with Class III elastics and Reverse pull headgear simultaneously (C III elastics + RPHG group) were traced, digitized and statistically analyzed. The results were as follows. 1 . Anterior displacement of maxilla was observed in both groups. 2. Simultaneous Class III elastics and reverse pull headgear group showed the counterclockwise rotation of maxilla, however Class III elastics group manifested no rotational change of maxilla. 3. Simultaneous Class III elastics and reverse pull headgear group showed the downward and backward rotation of mandible, however Class III elastics group manifested no rotational change of mandible. 4. Counterclockwise canting of occlusal plane, labial tipping of upper incisor and lingual tipping of lower incisor and mesial displacement of upper molar to pterygoid root vertical were observed in both groups.

Management of open bite that developed during treatment for internal derangement and osteoarthritis of the temporomandibular joint

  • This case report describes the orthodontic treatment performed for open bite caused by internal derangement (ID) and osteoarthritis (OA) of the temporomandibular joint (TMJ). A Japanese woman, aged 31 years and 11 months, referred to our department by an oral surgeon had an open bite with clockwise rotation of the mandible and degeneration of the condyle. The overbite was corrected through intrusion of the maxillary and mandibular molars using mini-screw implants to induce counterclockwise rotation of the mandible. Then, the mandibular second premolars were extracted and comprehensive orthodontic treatment was performed to establish a Class I molar relationship with distalization of the maxillary arch and to eliminate anterior crowding. Following treatment, her facial profile improved and a functional and stable occlusion was achieved without recurrence of the TMJ symptoms. These results suggest that orthodontic intrusion of the molars is one of the safer and less stressful alternatives for the management of open bite due to degeneration of the condyles caused by ID and OA of TMJ.

  • There has been so much controversies about the position of upper and lower jaws, and their first permanent molars in normal occlusion and Angle's class $I{\cdot}II{\cdot}III$ malocclusions. So, the purpose of this study is to compare the position of upper and lower jaws, and their first molars in normal occlusion and Angle's class $I{\cdot}II{\cdot}III$ malocclusions by lateral cephalometric analysis. The sample consisted of one hundred and twenty girls(thirty in each group) who had completed growth. The findings of this study were as follows : 1. In class I malocclusion, both maxilla and mandible were slightly posterior position than normal occlusion, but they showed harmonious relationship. 2. In class II malocclusion, the mandible was greatly retruded, and the maxilla was also slightly retruded to the cranial base as compared with normal occlusion. 3. In class III malocclusion, the maxilla was significantly retruded to the cranial base, but no significant difference was found in mandibular position as compared with normal occlusion. 4. The maxillary first molar was located at posterior position in class II malocclusion, and anterior position in class III malocclusion to the cranium, so that the rotation of mandible was influenced by that. 5. The mandibular first molar showed constant relationship to the mandible in all four groups, but different position to the cranial base in direct proportion to the mandibular position. 6. On the treatment planning of class III malocclusion, it seems to be better to promote the mandibular horizontal growth by inhibiting the vertical growth of maxillary molar area, and on the treatment planning of class III malocclusion, it seems to be better to promote the antero-inferior growth of maxilla mi to promote the mandibular vertical growth by inducing the vertical growth of maxillary molar area.

디지털 공제방사선영상의 기하학적 보정에 관한 연구 (A study on the geometric correction for the digital subtraction radiograph)

  • Purpose : To develop a new subtraction program for registering digital periapical images based on the correspondence of anatomic structures. Materials and Methods: The digital periapical images were obtained by Digora system with Rinn XCP equipment after translation of 1-16 mm, and rotation of 2-20° at the premolar and molar areas of the human dried mandible. The new subtraction program, NIH Image program and Emago/Advanced program were compared by the peak-signal-to noise ratio (PSNR). Results : The new subtraction program was superior to NIH Images program and Emagol Advanced program up to 16 mm translation and horizontal angulation up to 4°. Conclusion: The new subtraction program can be used for subtracting digital periapical images.

Asymmetric Head-Gear의 견인력의 차이에 따른 상악 제 1 대구치에 나타나는 힘과 변위 및 초기 응력분포에 관한 유한요소법적 비교 연구 (A FEM comparison study about the force, displacement and initial stress distribution on the maxillary first molars by the application of Asymmetric Head-Gears with the different traction forces)

  • 편측성 II급 부정교합의 치료 시 다양한 mechanics가 사용되어지는데 그중 한가지가 asymmetric face-bow를 가진 head gear이다. 이 asymmetric head gear의 편측 효과에 대해 다양한 연구결과를 보고하고 있는데 이에 저자는 편측성 II급 부정교합의 상악 치열 모델링과 임상에서 자주 사용되는 Power arm asymmetric face-bow를 모델링하고, 견인력을 달리하여 유한 요소법을 통해 치근막에서 발생되는 응력의 분포와 그에 따른 각 방향의 반력, 그리고 그로 인한 변위를 관찰하여 역계를 이해하고 치료효과를 예견하고자 실험을 고안하였다. 발치나 교정치료의 경험이 없는 25세 남자 정상 교합자를 대상으로 컴퓨터 단층 촬영을 시행하여 얻은 방사선필름을 기초로 하여 유한 요소 상악 모델과 치주인대 모델을 제작하고 그후 다시 좌측 제 1 대구치가 근심 편위되어있는 편측성 상악 II급 부정교합 모델로 제작하고, RMO 사의 Face-bow (Penta-$^{TM}$/Medium size)를 기본 모델로 0.045 inch 직경의 inner-bow와 0.072 inch 직경의 outer-bow를 가지는 asymmetric face-bow를 우측을 25mm 짧게 하여 모델링한 후, 좌우측 제 1 대구치에 각각, 250g, 300g, 350g 씩 견인력을 부여하여 다음과 같은 결론을 얻었다. 1. 양쪽 제1 대구치가 받는 힘의 총합은 견인력이 증가함에 따라 증가하는 경향을 보였는데 대체로 근심위치된 치아가 정상 위치한 치아보다 더 많은 힘을 받는 것으로 나타났으며, 두치아 모두 윈심으로의 힘을 받으며, 원심으로 이동하는 양상을 보여주었다. 2. 측방력은 두 치아 모두 협측으로의 힘을 받는 것으로 나타났는데, 힘의 성분을 분석하여 보면 견인력이 증가함에 따라 X축으로의 힘이 근심 위치된 치아에서 점점 작아지며, 정상 위치한 치아에서 점점 증가하여 측방력의 성분이 힘을 많이 받는 쪽에서 적게 받는 쪽으로 이동하는 양상을 보여주었다. 3. 원심으로의 이동과 동시에 회전과 경사이동 양상을 보여주었는데 견인력의 증가시 함께 증가하는 양상을 보였고, 힘을 많이 받는 쪽, 즉 근심 위치된 치아가 더 많은 회전과 경사이동을 보였으며 적은 양이나마 정상 위치된 치아에 서도 같은 양상의 변위를 보였다.

TRANSPALATAL LINGUAL ARCH에 의한 골내 응력 분포에 관한 광탄성적 연구 (A PHOTOELASTIC STUDY OF THE STRESS DISTRIBUTION IN BONE BY THE TRANSPALATAL LINGUAL ARCH)

  • 본 연구는 TMA wire로 제작된 transpalatal lingual arch에 의한 골내 응력 분포를 파악하기 위하여 양측성 및 편측성 확장을 가하여 이때 발생하는 치아주위조직 응력 분포 양상을 광탄성 응력 해석법으로 관찰하여 다음과 같은 결과를 얻었다. 1. 양측성 확장시 수평 변형만 적용했을 때 탄선끝이 각진 경우 둥근 탄선끝을 적용한 경우보다 응력이 치근단으로 집중되며 회전 중심을 의미하는 흑색선이 변화하는 양태를 보였고 수직변형량이 증가되면서 전체적인 응력차수의 증가와 흑색선의 하방 이동 양상을 보였다. 2. 편측성 확장을 위해 수평 변형과 함께 확장측에만 수직 변형을 적용하였을 때 확장력이 확장측으로 편중되는 응력 분포를 관찰할수 있었다. 수직 변형량의 증가시 확장측 구치는 정출하는 응력을 보였고 수평변형량의 증가시 고정측 치아도 경사지려는 응력 분포가 관찰되었다. 3. 편측 구치의 회전을 개선하기 위해 철사끝을 비대칭적으로 toe-in한 경우 변형량이 적은 구치의 치근단 원심측에 응력 분포를 관찰할수 있었다. 4. 악궁 확장시 철사의 길이를 줄일수록 무늬차수가 증가하는 응력 분포를 보였다.

Treatment effects of mandibular total arch distalization using a ramal plate

  • Objective: The purpose of this study was to evaluate treatment effects after distalization of the mandibular dentition using ramal plates through lateral cephalograms. Methods: Pre- and post-treatment lateral cephalograms and dental casts of 22 adult patients (11 males and 11 females; mean age, $23.9{\pm}5.52years$) who received ramal plates for mandibular molar distalization were analyzed. The treatment effects and amount of distalization of the mandibular molars were calculated and tested for statistical significance. The significance level was set at p < 0.001. Results: The mandibular first molar distalization at the crown and root were 2.10 mm (p < 0.001) and 0.81 mm (p = 0.011), respectively. In the evaluation of skeletal variables, there was a significant increase in the Wits appraisal (p < 0.001). In the evaluation of the soft tissue, there was no significant effect on upper lip position, but the lower lips showed a significant retraction of 2.2 mm (p < 0.001). Conclusions: The mandibular molars showed a significant amount of distalization accompanied by limited extrusion and mesiobuccal rotation of the crowns. A ramal plate may be a viable device for mandibular total arch distalization in Class III patients who are reluctant to undergo orthognathic surgery.

저위교합된 제 2유구치에 대한 치험례 (MANAGEMENT OF INFRAOCCLUDED MANDIBULAR SECOND PRIMARY MOLARS: CASE REPORT)

  • 저위교합이란 치아가 출은한 이후 유착 등의 원인으로 맹출이 정지되어 주위 인접치 보다 교합평면이 낮아지는 것을 말한다. 저위교합은 인접치들의 경사, 저위교합된 부위의 공간상실, 대합치의 정출, 해당부위 치아우식 감수성의 증가, 후속 영구치의 맹출경로 변위, 매복, 회전 등의 문제점을 유발할 수 있으므로 조기 진단 및 정기검진을 통한 적절한 치료가 필요하다. 저위교합된 유구치는 대부분 영구 계승치가 존재하며 일반적으로 정상 탈락한다. 그러나 저위교합된 유구치의 탈락이 지연되고 저위교합의 정도가 심화되어 해당부위의 공간상실이 발생하면, 공간부족으로 인하여 후속 영구치의 매복을 야기할 수 있다. 이러한 경우 교정적으로 공간확장술을 시행한 후 외과적 발치를 고려해야 한다. 본 증례는 하악 제 2유구치의 저위교합이 관찰된 3명의 환아를 대상으로 공간에 대한 평가 후 필요한 경우 공간확장술을 시행하고 발치를 시행함으로써 저위교합으로 인해 발생된 문제점을 개선하고, 추후에 발생될 합병증을 예방하였기에 이를 보고하는 바이다.

Effect of archwire stiffness and friction on maxillary posterior segment displacement during anterior segment retraction: A three-dimensional finite element analysis

  • Objective: Sliding mechanics using orthodontic miniscrews is widely used to stabilize the anchorage during extraction space closure. However, previous studies have reported that both posterior segment displacement and anterior segment displacement are possible, depending on the mechanical properties of the archwire. The present study aimed to investigate the effect of archwire stiffness and friction change on the displacement pattern of the maxillary posterior segment during anterior segment retraction with orthodontic miniscrews in sliding mechanics. Methods: A three-dimensional finite element model was constructed. The retraction point was set at the archwire level between the lateral incisor and canine, and the orthodontic miniscrew was located at a height of 8 mm from the archwire between the second premolar and first molar. Archwire stiffness was simulated with rectangular stainless steel wires and a rigid body was used as a control. Various friction levels were set for the surface contact model. Displacement patterns for the posterior and anterior segments were compared between the conditions. Results: Both the anterior and posterior segments exhibited backward rotation, regardless of archwire stiffness or friction. Among the conditions tested in this study, the least undesirable rotation was found with low archwire stiffness and low friction. Conclusions: Posterior segment displacement may be unavoidable but reducing the stiffness and friction of the main archwire may minimize unwanted rotations during extraction space closure.
