동작관찰 훈련과 과제지향적 훈련이 뇌졸중 환자의 일상생활활동에 미치는 영향 (An Impact of Action-Observation Training and Task-Oriented Training on Activities of Daily Living of Stroke Patients)

  • Purpose : The purpose of this study was to find out the impact of action-observation training and task-oriented training on activities of daily living performance of stroke patients. Method : 30 stroke patients hospitalized in D hospital located in Busan and treated were randomly allocated to Action-Observation Training Group and Task-Oriented Training Group in fifteens. To compare activities of daily living performance before and after therapy intervention, Korea-modified Barthel index (K-MBI) was carried out. Result : In both groups, activities of daily living performance of stroke patients before and after therapy intervention showed statistically significant differences (p<.05) and activities of daily living performance between two groups after therapy intervention showed statistically significant differences. Conclusion : It was found that action-observation training and task-oriented training improved activities of daily living performance of stroke patients. It is considered that the application of action-observation training and task-oriented training to clinical occupational therapy will show a positive effect on the improvement of activities of daily living performance.

  • Neuromyelitis optica (NMO) is an autoimmune inflammatory disorder of the central nervous system characterized by optic neuritis and longitudinally extensive myelitis. Painful tonic spasm is a common complication of NMO, but there have been no reports about it in Korean medicine studies. In this case, we treated a 53-year-old woman diagnosed with NMO with paraplegia, painful tonic spasm, and decreased visual acuity using Korean medicine treatment, including acupuncture, herbal medicine, cupping, and moxibustion. We measured changes in clinical symptoms using the manual muscle testing (MMT), the numeric rating scale (NRS), the modified Barthel Index (MBI), and the Functional Independence Measure (FIM). After treatment, clinical symptoms were improved. The results indicate that Korean medicine treatment may be effective in the treatment of an NMO patient with paraplegia and painful tonic spasm.

  • Objective: This study is a report on a case of a Guillain-Barre syndrome patient with quadriplegia and paresthesia, whose condition was improved by Korean medicine treatment. Methods: A 71-year-old woman diagnosed with Guillain-Barre Syndrome was treated by acupuncture, electroacupuncture, Sambi-tang-gami, cupping, moxibustion, and rehabilitation. Clinical symptoms were measured with the Manual Muscle Test (MMT), the Numeric Rating Scale (NRS), the Modified Barthel Index (MBI), and the Functional Independence Measurement (FIM). Result: After treatment, clinical symptoms showed improvement. Conclusion: Korean medicine treatment could be effective in the treatment of Guillain-Barre syndrome patients with quadriplegia and paresthesia.

  • Objectives:This study was to investigate the post-stroke depression morbidity and to evaluate the post-stroke depression in relation to activities of daily living by using BDI(Beck's Depression Inventory)and MBI(Modified Barthel Index) in 45 patients with stroke. Methods:The subjects of this study were 45 stroke patients who were admittied to the Dong-Seo oriental hospital.The post-stroke depression was determined by using the BDI and the evaluation of activities daily living was measured by using the MBICollected data analysis were completed by using correlation analysis.Results:1. The post-stroke depression morbidity was 30%.2. The post-stroke depression have no significant correlation with activities of daily living skills.Conclusions:This study shows that the post-stroke depression have no significant correlation with impairment in activities of daily living.Advanced studies are required to investigate other factors that influence post-stroke depression and post-stroke depression follow-up studies.

골다공증을 동반한 하지부골절 수술 후 보행불리 환자 재활에 단순추나요법을 적용한 치험 1례 (Clinical Case Report on the Gait disturbance Patient with Osteoporosis and Lower Limb Fracture Surgery Treated by Chuna Therapy)

  • Objectives : The purpose of this study was to show the effectiveness of Chuna Manual Therapy applied to a osteoporosis patient with gait disturbance due to lower limb fracture surgery. Method : We focused on Chuna therapy accompanied with acupuncture, herbal medicine, moxibustion, cupping treatment and rehabilitation exercise. This case was assessed using the Visual Analogue Scale (VAS), Pain Free Walking Distance (PFWD), Berg Balance Scale (BBS), and Modified Barthel Index (MBI). Result : This patient showed improvement on the VAS from 6 to 3, PFWD from 0 m to 2000 m using a walker and 50 m by self-walking, BBS from 2 to 28, and MBI from 56 to 89. Moreover the walking state improved from gait disturbance to self-walking. Conclusion : This study suggests that Chuna therapy could be used to improve gait disturbance in patients with osteoporosis and lower limb fracture surgery.

두침경혈 매선요법을 이용한 중풍 편마비 환자 치험 3례 (A Case Study of Stroke Patients with Hemiparesis Treated by Catgut-Embedding Therapy on Scalp Acupoints)

  • Objectives : This study was to report the effect of catgut-embedding therapy on scalp acupoints in stroke patients with hemiparesis. Methods : Three stroke patients with hemiparesis were treated by catgut -embedding therapy on scalp acupoints twice a week. After three weeks, the score of manual muscle test (MMT) and modified Barthel index (MBI) were improved compared with the first examination. Results and Conclusions : After the treatment, MMT and MBI score improved in all three patients. These results showed that catgut-embedding therapy on scalp acupoints is possible to be effective for treating hemiparesis in stroke patients, and further studies should be conducted to provide more valuable information.

뇌졸중 환자의 재활에서 포괄적인 한방치료의 효과: 후향적 연구 (Effect of Comprehensive Korean Medicine with Rehabilitation in Stroke Patients: A Retrospective Study)

  • This study is to investigate the effect of the level of Korean medical intervention (KMI) on the recovery of activity of daily living (ADL) and the motor function in patients with stroke. A total of 43 stroke patients were recruited. The participants were divided into two groups, the basic Korean medical intervention (BKM) and the comprehensive Korean medical intervention (CKM) group. The patients in BKM group were treated with the basic acupuncture and the consultation of Korean medicine doctors. The treatments of CKM group patients included the comprehensive herbal medical therapies in addition to the treatments of BKM group. The western rehabilitation therapies were performed in all participants with KMI. The functional outcomes using modified Barthel Index of Korean version (K-MBI) and Fugl-Meyer assessment (FMA) were analyzed before and after the treatment between two groups. Significant differences between the BKM and the CKM group were demonstrated in both K-MBI and FMA (p<0.05). There were significant relationships between the level of the KMI and K-MBI/FMA (p<0.05). In conclusion, the comprehensive KMI was more effective to improve the ADL and motor function in patients with the stroke east-west integrative medical care.

뇌졸중 환자에서 자기력에 의해 유도되는 재활운동기기 (Outcome of Rehabilitation Device Driven by Magnetic Force in Stroke Patients)

  • PURPOSE: This study investigates the therapeutic effect of a prototype of a hand rehabilitation device based on magnetic forces. METHODS: Using an electromagnet and permanent magnets, we developed an end effector type device that induces various movements of the finger in accordance with the magnetic field direction. A total of 26 subacute stroke patients were enrolled and assigned to two groups in this randomized controlled trial. The intervention group received 30 minutes hand rehabilitation therapy per day for 4 weeks, using the device developed by us. Conventional physical therapies were conducted equally twice a day, 30 minutes per session, during the same period in both groups. RESULTS: After 4 weeks, rate of the Wolf Motor Function Test as a primary outcome measure showed significant improvement in the intervention group as compared to control group(p = .036). Scores of the Manual Function Test and Fugl-Meyer Assessment of upper limb were also significantly increased in the intervention group as compared to control group(p = .038 and p = .042, respectively). Moreover, the Korean version of Modified Barthel Index tended to improve after subjecting to physical therapy in both groups. CONCLUSION: Our results indicate that the novel hand rehabilitation device developed using a magnetic force, improves the hand motor functions and activities of daily life in subacute stroke patients.

The Effect of Upper Extremity Training with a Focus on Functional Reaching, on Trunk Control and ADL Performance in Post-Stroke Hemiplegic Patients

  • Purpose: In this study, we tried to determine the effect of upper extremity training such as functional reaching on improved trunk control and ADL performance in post-stroke hemiplegic patients. Methods: We randomly selected 11 stroke patients in the hospital, who had a problem with the upper extremity, trunk and ADL performance. The patients were divided into the conservative training group and the functional reaching training group. We applied general occupational therapy only in the conservative training group whereas we applied upper extremity training with a focus on functional reaching in the functional reaching training group. To compare the two groups we used several assessment tools such as Modified Barthel Index (MBI), total Trunk Impairment Scale (TIS), static TIS, dynamic TIS and coordinative TIS. Results: The results obtained were as follows: (1) In the functional reaching training group, there was a statistically significant difference in the total TIS score, dynamic TIS score, and MBI. (2) We compared the results obtained before training with the changes in the results obtained after training and found that there was a relation between the assessment outcomes. Especially, static TIS score showed a relation in both groups. Conclusion: Functional reaching training influenced both the trunk control and ADL performance. Especially, the functional reaching training group demonstrated better static trunk control ability than the conservative treatment group.

Effects of Respiratory Muscle Training on the Respiratory Function, Balance, and Activities of Daily Living in Patients with Stroke

  • Purpose: This study evaluated the effects of respiratory muscle training on respiratory function, balance, and activities of daily living (ADL) in patients with stroke. Methods: The study included 21 patients with stroke, divided into the experimental group and control group. Both groups underwent traditional physical therapy once a day for 30 minutes, five times weekly for 6 weeks. The experimental group underwent additional respiratory muscle training once a day, five times a week during the study period. Maximal inspiratory pressure (MIP), maximal expiratory pressure (MEP), Berg balance scale (BBS), and Korean-modified Barthel index (K-MBI) were measured before and after the study period. For statistical analysis, paired t-test was used to compare the difference between the pre and post values. Independent t-test was used compare the differences between groups. Results: Both groups had significantly improved MIP, MEP, BBS, and K-MBI scores after the study period. The experimental group had significantly improved MIP, MEP, BBS, and K-MBI scores. Conclusion: These results suggest that respiratory muscle training improves respiratory function, balance, and ADL in patients with stroke.
