Objectives: This study reports the case of a patient with right hemiparesis after a left thalamic infarction, in which the patient experienced improved clinical symptoms (hemiparesis, constipation) after treatment with a complex Korean medicine treatment. Methods: A 81-year-old male patient diagnosed with acute left thalamic infarction was treated with daily acupuncture and herbal medicine (Boyanghwanoh-tang-gagam) three times a day. Jowiseunggi-tang extract granules were administered to treat constipation by adjusting the dosage according to the reported symptoms. The patient's clinical course was assessed using changes in Modified Barthel Index (MBI) score, Manual Muscle Testing (MMT) grade, and defecation frequency . Results: The patient's MBI score improved from 35 to 69 and was discharged with the ability to walk under supervision. The MMT grade also improved from 3/4 to 5/5 in the upper/lower extremities. Other clinical symptoms, including constipation, also improved, leading to the discontinuation of magnesium medication. Conclusion: These findings suggest that complex Korean medicine treatments, including herbal medicine and acupuncture, may be an effective treatment for post-stroke hemiplegic patients.
Objectives: The aim of this study is to describe the effects of treatment with traditional Korean medicine on a patient with voiding dysfunction caused by cerebral infarction. Methods: The patient was treated with herbal medicine, moxibustion, acupuncture, and electroacupuncture. The effects of these treatments were evaluated using the Korean version of the Modified Barthel Index and a post-void residual volume. Results: In this study, the Korean medicine Dangguishaoyao-san-hap-Jeoryeong-tang resulted in improvements in this patient with post-stroke urinary retention and successful removal of the indwelling catheter. Conclusion: The results suggest that traditional Korean medicine may be effective in ameliorating dysfunction caused by cerebral infarction.
The purpose of this study is to report the case of a patient with idiopathic Parkinson's disease treated with scalp acupuncture and Chuna manual therapy. A total of 10 sessions of scalp acupuncture and Chuna manual therapy were performed. At the beginning of the treatment, the Korean version of modified Barthel index (K-MBI) was 25 points, and the range of motion (ROM) of both knee joints was 150° in passive flexion, while active movement was impossible. In addition, both knee joints were flexed and contracted at -40° on the right and -30° on the left. Over the course of treatment, the ROM of both knee joints improved to -20° on the right and -10° on the left. The patient was even able to maintain a standing position and was allowed to walk approximately 10 steps with support. Temporary improvement was observed as a result of applying scalp acupuncture treatment and Chuna manual therapy to a patient whose symptoms were gradually worsening.
This study reported the case of a patient with quadriparesis due to cerebral aneurysm rupture, subarachnoid hemorrhage, and intraventricular hemorrhage (IVH) treated with Korean medicine. The patient was treated with acupuncture, herbal medicine (mainly Tonggyuhwalhyeol-tang-gagam), Western medicine, moxibustion, cupping, and rehabilitative therapy for 75 days, and improved after administration. After treatment, the Manual Muscle Test grade improved from 4/4-/3+/3 to 4+/4+/4+/4+, the Korean version of the modified Barthel Index score improved from 9 to 100, the National Institute of Health's Stroke Scale score improved from 2 to 0, the Global Deterioration Scale score improved from 3 to 2, and the Korean version of the Mini-Mental State Examination score improved from 22 to 30. During administration, the patient did not show seizures, shock, or loss of consciousness, and the vital signs were stable in the normal range. We followed up the brain computed tomography findings for 2 times and found that there was no definite evidence of intracranial hemorrhage or IVH or re-rupture or rebleeding after Korean medicine treatment. This study suggests that Korean medicine treatment with blood-invigorating and stasis-removing herbs could be a safe and effective intervention option for improving quadriparesis due to cerebral aneurysm rupture and subarachnoid hemorrhage.
Objectives: This study reports on the improvement of anti-N-methyl-D-aspartate (NMDA) receptor encephalitis with ovarian teratoma after Korean medicine treatment. Methods: A patient was treated with Korean medicine treatments, such as acupuncture and herbal medications (Gami-ondam-tang and Samulanshin-tang-gamibang). The patient's improvement was evaluated using manual muscle testing (MMT), the Mini-Mental State Exam-Korea (MMSE-K), the modified Barthel index (MBI), and the Clinical Assessment Scale in Autoimmune Encephalitis (CASE). Results: After using Korean medicine treatments, the patient's MMT, MMSE-K, and MBI scores increased, and the CASE score decreased. Conclusion: This case suggests that Korean medicine can be effective in treating the symptoms of anti-NMDA receptor encephalitis.
This case report describes a patient diagnosed with central post-stroke pain (CPSP) and hemiparesis due to anterior cerebral artery and middle cerebral artery infarction. The patient was treated with Korean medicines, including moxibustion, acupuncture, electroacupuncture, herbal medicine, Western medicine, and rehabilitation therapy for 67 days. CPSP improved based on scores on the Numeric Rating Scale (NRS) (from 8 to 0), paresthesia NRS (7 to 0), Manual Muscle Test (4/1 to 4+/4-),4+/4-), Korean version of the modified Barthel Index (24 to 73), National Institute of Health's Stroke Scale (7 to 0), Global Deterioration Scale (1 to 1), and Korean version of the Mini-Mental State Examination (27 to 29). No seizures, shocks, recurrent ischemic stroke, and liver dysfunction were recorded during the treatment. A moxibustion-induced burn healed within 7 days. This case suggests that moxibustion, combined with other treatments, has the potential to improve CPSP, without severe side effects.
Background: The purpose of this study was to identify the effect of home-based exercise program in elderly with chronic diseases on balance, activities of daily living, depression and quality of life. Design: One group pretest-posttest design Methods: The subjects consisted of 21 elderly people over the age of 65 years with chronic diseases lived in seo-gu, Gwangju. Home-based exercise program was performed twice a week, for an hour, during 6 weeks under supervision of an experienced instructor. The home-based exercise program was structured to last a total of 1 hour, consisting of 10 minutes of warm-up, 30 minutes of main exercises, and 20 minutes of concluding exercises. For the warm-up, we performed range of motion, stretching, and flexibility exercises. The main exercises comprised of strength, weight-bearing and balance exercises, and endurance exercises. The concluding exercises were tailored to the individual's home environment and needs, including task-oriented daily living training such as getting up from bed, moving to the restroom, and walking indoors. The subjects were assessed before and after the exercise program with berg balance scale(BBS), Korean-version modified barthel index(K-MBI), euro quality of life questionnaire 5-dimensional classification(EQ-5D), and geriatric depression scale short form-Korea(GDSSF-K). The collected data was analyzed using SPSS Win 18.0 program. Results: The results showed statistically significant difference in all domains of the BBS, K-MBI, EQ-5D, GDSSF-K after the exercise program. Conclusion: These results suggest that Home-based exercise program has resulted in a significant improvement on ADL, depression and quality of life in elderly with chronic diseases.
The objective of this study is to report the clinical effects of applying an integrated rehabilitation protocol to four patients with traumatic cervical spinal cord injuries. The treatments applied included acupuncture, cupping and moxibustion, chuna manual therapy, functional electrical stimulation, and other physical therapies. The evaluation methods included American Spinal Injury Association Impairment Scale, functional independence measure (FIM), modified Barthel index (MBI), numeric rating scale (NRS) and other scales. All patients presented marked improvements in FIM, MBI, NRS and other scales, along with increased muscle strength in the manual muscle test. The application of our protocol resulted in clear clinical benefits and enhanced the recovery and quality of life for the patients in this study.
목적 : 이 연구에서는 단일 뇌졸중환자를 대상으로 체간안정화 훈련 프로그램 적용하여 자세조절과 일상생활 동작에 미치는 영향을 알아보고 분석함으로써 뇌졸중환자의 효과적인 임상적 중재 방안을 제시하고자 한다. 연구방법 : 경북 경주시에 소재한 C병원에 입원중인 75세 여성 뇌졸중 환자 1명을 대상으로 하였다. 연구 설계는 단일사례연구(single subject design)로써 ABA반전설계를 사용하였고, 연구기간은 2014년 4월 21일부터 5월 23일까지 총 5주 동안 진행되었다. 실험 과정은 기초선(A) 3회, 중재 기간(B) 9회 유지(A') 3회로 총 15회 진행하였고 중재기간 동안 체간안정화 프로그램을 적용하였다. 연구 참여자의 자세조절과 일상생활동작의 평가를 위해 Postural Assessment Scale for Stroke(PASS)와 Modified Barthel Index(MBI)를 사용하였다. 수집된 자료는 마이크로 소프트 오피스 Excel 2013 을 사용하여 그래프로 나타내는 시각적 분석(visual analysis)을 사용하였다. 결과 : 자세조절 검사에서 평균 점수는 기초선 구간 11.7점, 중재 구간 14.8점, 유지 구간 15.3점으로 향상되었고, 일상생활동작 평가에서 기초선 기간 66점으로 보통의 의존을 보였지만 중재 후와 유지 구간에서는 80점으로 향상되어 가벼운 의존을 나타냈다. 결론 : 체간안정화 훈련 프로그램이 뇌졸중 환자의 신체의 자세조절이나 정렬에 효과를 나타내고 나아가 일상생활활동의 수행에도 유익한 향상을 보이는 것으로 확인되었다.
목적 : 본 연구의 목적은 뇌졸중 환자에게 일상생활활동 향상을 위해 중재를 시행한 국내 단일대상연구문헌의 특성과 질적 수준을 확인하는 것이다. 연구방법 : 학술연구정보서비스(Research Information Sharing Service), 누리미디어(DBpia), 학술교육원(e-article)의 원문제공 서비스를 통해 2009년부터 2023년까지 최근 15년간 발간된 논문 중 '뇌졸중', '일상생활활동', '단일대상연구'를 주요 키워드로 검색하였다. 총 9편의 논문을 대상으로 분석 대상 논문의 특징과 질적 수준을 알아보았다. 결과 : 9편의 분석 대상 논문 중 작업치료 분야 학회지에 실린 논문이 6편이었고, 분석 논문에서 적용한 연구 설계는 ABA 설계가 3편으로 가장 많았다. 대상자들의 성별은 남성과 여성이 각각 10명씩이었고, 대상자 수가 3명인 논문이 5편으로 가장 많았다. 연구 대상자들의 연령은 60대가 6명으로 가장 많았다. 일상생활활동 향상을 위해 적용한 독립변인은 강제유도운동치료, 상상연습, 시각되먹임을 차단한 자세 수직훈련, 양측성 상지운동, 가상현실을 이용한 과제훈련, 신경인지재활치료와 전통적인 작업치료, 체간 안정화 훈련, 완곡추적 안구운동과 경부신전근 진동자극법, 작업기반 지역사회 재활 각각 1편씩으로 다양한 중재들을 적용하고 있는 것으로 나타났다. 종속변인을 측정한 평가도구는 Assessment of Motor and Process Skills이 4편으로 가장 많았고, Modified Barthel Index와 Canadian Occupational Performace Measure가 각각 2편이었다. 분석 논문의 질적 수준을 평가한 결과 총 9편 중 높은 수준의 연구가 7편이었고, 중간 수준의 연구는 2편이었으며, 낮은 수준의 연구는 단 1편도 없었다. 결론 : 뇌졸중 환자의 일상생활활동 향상을 위한 중재법으로 여러 유형의 재활치료가 활발히 적용되고 있으며, 일상생활활동 중재를 적용한 단일대상연구들의 질적 수준은 신뢰할 수준인 것을 확인할 수 있었다.
