An unusual exophytic pleomorphic adenoma on the upper lip skin without mucosa involvement: a case report

  • Pleomorphic adenoma is a benign tumor that can occur in the salivary glands, most commonly in the parotid gland. While it primarily occurs in the major salivary glands, it can sometimes be found in the minor salivary glands. Within the minor salivary glands, it most often originates in the hard palate and soft palate, and less frequently in the upper lips. Due to its location in the minor salivary glands, most pleomorphic adenoma involve and protrude on the mucosa. A 61-year-old man presented with 1.5 cm exophytic mass on the skin of his upper lip. This mass was exophytic on the skin and did not involve or protrude into the inner lip mucosa. The mass was entirely excised, and a subsequent permanent biopsy diagnosed it as a pleomorphic adenoma. In such situations, it can be challenging to suspect pleomorphic adenoma during a physical examination, leading to potential diagnostic confusion. It might also be mistaken for an inclusion cyst or another type of mass, making it tempting to treat without verifying the pathological results.

  • Basal cell adenocarcinoma is an epithelial neoplasm which is cytologically and histomorphologically similar to basal cell adenoma but is different because of the infilitrative growth. This tumor, a rare salivary gland tumor newly classified as basal cell adenocarcinoma by the WHO in 1991, is infiltrative, locally destructive and tends to recur but metastasis is less common. The differential diagnosis includes basal cell adenoma, adenoid cystic carcinoma, and basaloid squamous carcinoma. Nearly 90 percent of these tumors occurr in the parotid gland and can be classified into low grade carcinomas with a relative good prognosis. Basal cell adenocarcinoma of minor salivary gland is very rare and has a less favorable clinical course compared with that of the major salivary glands. This is a case of basal cell adenocarcinoma occurring at the minor salivary gland of the soft palate. We treated this patient with block excision and adjunctive radiation therapy.

소타액선 종양에 관한 임상적 고찰 (A Clinical Analysis of Minor Salivary Gland Tumors)

  • 최근 7년간 저자들이 치험한 소타액선 종양 26례에 대하여 임상양상과 치료 결과를 분석하여 다음과 같은 성적을 얻었다. 1) 발생부위를 구개 10례 (38.5%), 비강 및 부비동 8례 (30.8%), 설근부 2례 (7.7%), 협부 점막 2례 (7.7%), 구순 1례(3.8%), 후두개곡 1례(3.8%), 구강저 1례(3.8%), 비인강 1례(3.8%)이었다. 2) 병리조직학적 소견은 양성 종양 10례, 악성종양 16례이었으며, 양성 종양은 전례가 다형성선종이었으며, 구개에서 6례로 가장 많았고, 악성종양은 비강 및 부비동 5례로 가장 많았으며, 선양낭성암종 9례, 악성혼합종 2례, 점액표피암종 2례, 다형성선암종 2례, 상피근상피성암 1례이었다. 악성화율은 61.5%이었다. 3) 치료는 양성 종양은 모두 적출술을 시행하였고 악성 종양은 광범위 적출술 4례(25.0%), 수술 및 방사선요법의 병합요법 9례(45.2%), 항암화학요법 및 방사선요법의 병합요법 1례(5.25%), 방사선 요법 2례(12.3%)등을 시행하였다. 4) 악성 종양에 대한 치료 결과는 근치적 치료를 시행한 8례에서는 무병생존 6례, 유병생존 1례, 유병사망 1례를 보였으며, 고식적 치료를 시행한 6례에서는 유병생존 2례, 유병사망 4례를 보였다. 이상의 성적에서 소타액선 종양은 대타액선 종양에 비하여 악성화 비율이 높고 선양낭성암종의 발생 빈도가 높아 그 침범 부위를 정확히 파악하는 것이 치료에 많은 도움을 줄 수 있으며, 고식적 치료의 범위를 확대하여 보다 적극적인 치료가 필요할 것으로 생각된다.

비인두에 발생한 상피-근상피암종 1예 (A Case of Epithelial-Myoepithelial Carcinoma in the Nasopharynx)

  • Epithelial-myoepithelial carcinoma is rare, low grade malignant tumor of the salivary glands that exhibits a dual composition of epithelial and myoepithelial cells. Most of these tumors arise in the parotid gland, and only few occur in the submandibular gland or minor salivary glands. We describe here a rare case of epithelialmyoepithelial carcinoma arising from a minor salivary gland in the nasopharynx, one of the most unusual locations. The clinical and biological behavior of this tumor is not yet known.

타액선 상피성 종양 387례에 대한 연구 (Histopathological and Clinical Studies of the 387 Cases of Salivary Gland Epithelial Tumors)

  • There have been few large scale surveys of salivary gland tumors, because the salivary gland tumors comprised less than 3 percent of all neoplasms of the head and neck. Also there is a problem that the incidences of specific types of tumors have the variation according to the geographic regions. ]'nother Korean reports about salivary gland tumor, they only treated about 100 cases only. Objectives : The purpose of this study is to identify the frequency and clinical characteristics of salivary gland tumors and to compare with other Korean reports. Materials and Methods : We had performed to analyze 387 cases of salivary gland tumors from 1983 to June 1997, were reviewed in histopathological and clinical aspects. The diagnoses of individual tumors were based on the World Health Organization classification. Tumors were analyzed according to the histologic type site, age and sex. Results : The majority (79.1%) of cases occurred in the major salivary glands and 20.9% in the minor. The parotid gland was the most frequent site with 52.4% and submandibular glands with 26.3%. Benign tumors were 79.3% of the tumors and malignant tmon were 20.7%. The most frequent tumor was pleomorphic adenoma occupied 63.8% of the total and frequency percent of histologic type of 98 malignant tumors showed 35.7% of mucoepidermoid carcinomas, 24.5% of adenoid cystic carcinomas. Overall gender with female predominance with 1 : 1.4 of male to female ratio. The average age of patients was 42.8 years. The most frequent chief complaint was a palpable mass. The duration of the symptom was shorter in the malignant salivary gland tumors (26.7 months) than in the benign salivary gland tumors (38.2 months). Conclusion : Comparing to the previous Korean reports, the present study confirms that the incidence of salivary gland tumor was lower in the parotid gland, and was higher in the submandibular and minor salivary glands.

Necrotizing Sialometaplasia: A Diagnostic Puzzle for Clinicians

  • Necrotizing sialometaplasia is a rare, benign, self-limiting inflammatory process, with most cases occurring in the palatal region. Although the pathophysiology of this condition remains unknown, it has generally been considered an ischemic reaction involving the minor salivary glands. Due to its clinical and histologic similarities to malignant salivary gland tumor, diagnostic biopsy is required. Careful monitoring is also recommended, and clinicians should provide sufficient information for pathologists to ensure the histologic diagnosis.

소타액선에 발생한 다형성선종의 임상 및 병리학적 고찰 (CLINICOPATHOLOGIC STUDY OF PLEOMORPHIC ADENOMA IN MINOR SALIVARY GLANDS)

  • Pleomorphic adenoma is the most common salivary neoplasm mainly occurring in the major salivary glands - especially in parotid gland, which is characterized by variable histopathologic appearances and high recurrence rate with malignant transformation according to surgical situations. And this benign mixed tumor occurring in minor salivary glands is believed to shows same clinicopathologic appearances and relatively low recurrent rate compared with the case in major salivary glands. But there are few comparative studies of large series of pleomorphic adenoma occurring in minor salivary glands which includes different histopathologic appearance, clinical characteristics, treatment methods, recurrence rate, and malignant transformation. We retrospectively studied the 54 patients who were pathologically confirmed with pleomorphic adenoma occurring in minor salivary glands, and analyzed the clinico-histopathological appearance, surgical methods, recurrent cases. The results obtained are as follows. 1. The incidence of the tumor was most frequent in 4th & 5th decade, and in female. 2. Palate(90%) including hard & soft palate was the most frequent site for pleomorphic adenoma in minor salivary glands. 3. The exact duration could not be known due to asymptomatic slow growth patterns of the tumor. 4. The mean tumor size was 2.3cm. 5. 28 (52%) pleomorphic adenomas were classified as Cellular type (cell-rich), 17 (31%) specimen as Intermediate type(equal cell to stroma ratio), and 9 (17%) as Myxoid type(stroma-rich). 6. Surgically 51 cases (94%) were showed well-encapsulated tumors, but histopathologically only 34 specimen (63%) were wellencapsulated. Therefore pleomorphic adenomas in minor salivary glands also have to be excised more widely, not enucleated. And in case of suspicious malignancy or large tumor, preoperative incisional biopsy can be applied in the center of the tumor for prevention of rupture of tumor cell, and total excision with use of frozen biopsy for detection of malignancy and confirming the excision margin, and closed follow-up according to final histopathologic results is recommended.

이하선의 다발성 소관선종(Canalicular Adenoma) 1예 (A Case of Multifocal Canalicular Adenoma of Parotid Gland)

  • Canalicular adenoma is a uncommon benign salivary gland tumor and it most frequently involves minor salivary gland of upper lip. It rarely occurs in parotid gland. The canalicular tumor of parotid gland can be manifestes clinically and pathologically as a multifocal lesion, which is not generally seen with other intraoral salivary gland tumors. Recently, we experienced a case of multifocal canalicular adenoma occurred on parotid gland in a 65-year-old woman and report it to support the view that canalicular adenoma occur rarely in parotid gland, and is recognizable entities.

Multiple bilateral malar mucinous cystadenomas in the minor salivary glands

  • Mucinous cystadenoma, one of the subtypes of cystadenomas, is a rare benign salivary gland tumor. Most of the cases reported tumors presenting as asymptomatic, slow growing, single masses, primarily occurring in the parotid glands, buccal mucosa, and hard palate. This report describes a case of multiple mucinous cystadenomas that presented as subcutaneous swellings in both cheeks, which were mistaken for a benign subcutaneous tumor. A complete surgical excision was performed through an intraoral incision. There were no recurrences or complications. A diagnosis of mucinous cystadenoma was made by histopathological examination. Mucinous cystadenomas are differentiated from other salivary gland tumors based on the pathological findings. A malignant transformation is also possible. Preoperative imaging cannot identify mucinous cystadenomas because of their small size and atypical features, and needle biopsy has its limitations in such salivary gland tumors. Therefore, accurate diagnosis and treatment through surgical excision become important.

구강설에 발생한 점액선암종 1예 (A Case Report of Mucinous Adenocarcinoma of the Oral Tongue)

  • Mucinous adenocarcinoma (MAC) is a rare malignant neoplasm that occasionally occurs in the large intestine (colon), followed by the pancreas, ovary, lung, prostate, and breast. It is characterized by large amounts of extracellular epithelial mucin that contains tumor cell nests. We herein present a unique case of MAC originating from minor salivary gland, the second to be reported in literature in South Korea. We report a case of MAC in the tongue considered to be developed from minor salivary gland with a review of literature.
