Squamous Cell Carcinoma in Pere David Deer (사불상에서 자연발생한 편평상피암)

  • Metastatic squamous cell carcinoma was described in two female Pere David's deer reared at Everland zoo. Both deer with chronic emaciation had the mass on right maxilla(9$\times$11$\times$15cm or 20$\times$17cm) which was composed of miliary tan creamy contents and encapsulated by connective tissue. The undulating contents in the mass was a1so extended in the underlying or adjacent soft palate, maxillary and frontal sinus causing severe bone destruction. In one deer, two fistula were also noted in the right periocular area. Histologically, the neoplastic masses of both deer consist of anastomosing cords or nests of squamous epithelial cells with intercellular bridge or keratin pearl formation. The neoplastic cells invade deep into the subcutis and bony tissue. Mitotic figure was rare. Multifocal areas of necrosis and hemorrhages were also noted in the dermis. Metastasis to maxilla and ethmoid bones and/or to submandibular lymph node was noted in both cases. The diagnosis was based on the results of histopathology.

Two Cases of Mycotic Pneumonia in Dogs (개에서 곰팡이성 폐렴 2례)

  • A two-year-old male Laika weighing 19 kg and a two-year-old female Rottweiler dog weighing 33 kg were presented to the Veterinary Medical Center, Chungbuk National University. Chest radiographs showed miliary interstitial lung pattern in the overall lung fields and the ill-defined mass in the perihilar area, which is consistent with lymphadenopathy in both cases. In the tracheal wash fluid smears in two cases, 90% of cells were eosinophils. Also, Aspergillus fumigatus was isolated from the tracheal wash fluid in Czapek's agar in one case. Ketoconazol (10 mg/kg, PO, ql2hr) alleviated the clinical signs in two dogs. Follow up radiographs showed marked reduction of lung lesions in one case.

Early and Late Clinical Results of Cardiac Valvular Surgery (심장판막질환의 수술성적)

  • A total and consecutive 46 patients have undergone cardiac valvular surgery including 8 open mitral commissurotomy and 38 mitral, aortic, mitral-aortic, mitral-tricuspid, tricuspid valve replacements using 46 artificial valves in a period between September 1976 and July 1981. They were 19 males and 27 females with the age ranging from 16 to 50 (mean 32.6) years. Out of 46 valves replaced, 6 were prosthetic valves and 40 were tissue valves, and 33 were replaced in mitral, 9 in aortic and 3 In tricuspid position. Isolated replacements were 33 mitral valves, 6 aortic valves and 1 tricuspid valve; double valve replacements were 6 mitral-aortic valves and 2 mitral-tricuspid valves. . Early mortality within 30 days after operation was noted in 4 cases; 3 after MVR and 1 after open mitral commissurotomy. Causes of death were thrombus obstruction of Beall-Surgitool, Cerebral air embolism, acute renal shut down due to low output syndrome, and left upper pUlmonary vein rupture after open mitral commissurotomy (early mortality 8.7%). 3 late deaths were noted during the follow-up period from 2 to 59 months; 1 due to cerebral hemorrhage from warfarin overdose 3 months, 1 due to miliary tuberculosis 9 months, and another 1 due to cardiac failure after open mitral commissurotomy 42 months postoperatively. Total survival rate 59 months after valvular surgery was 84.8%; there were no early and late death in the group of AVR, TVR and double valve replacements. Preoperative NYHA Class III & IV were 35 cases (76%) out of total 46 cases, and 38 cases (94.8%) out of 39 survival cases were included In NYHA Class I & II during the follow-up period.

A Study on Punggi Rayon (풍기인견에 관한 연구)

  • This study uses a documentary survey, field survey and textile's characteristics to understand the history of the Punggi Viscose Rayon. Punggi's textile industry grew as people migrated from Pyeongan Province in the 1900s. Little is known regarding early production conditions, but documents show cottage industries in the 1940s made union cloth using silk and rayon. The Punggi weaving industry peaked after the Korean War and declined in the late 1950s. Punggi rayon at that time was mainly used for lingerie or lining, with miliary or polka dot patterns. The industry regained momentum when the power loom was introduced in the 1960s. Manufacturers also started weaving union cloth with synthetic fibers and rayon. In the 1990s, jacquard looms enabled the weaving of elaborately colored and patterned textiles for outer fabric that made Punggi rayon famous. Most Punggi rayon looms have developed in the order of manual handlooms, semiautomatic looms, Jokdapgi, to power looms. Looms equipped with dobbies or jacquard devices also changed from wooden dobby looms to wooden jacquard looms and then to iron jacquard looms. Punggi Rayon currently has its own trademark and tag, but lacks specific regulations on the blending of fibers.

  • Successful middle ear surgery requires the availability of al safe, effective bonding material. Side effect caused by synthetic materials have led to the use of biologic adhesive. However, they carry the risk of transmission of infectious diseases if they are prepared from pooled human blood. The adhesive strength of ammonium sulfate fibrin adhesive produce an adhesive strength that is half that of the homologous commercial product. It is, however, good enough for use in several otolaryngological operations, tympanoplasty, facial nerve repair, reconstruction of ossicles, reconstruction of posterior wall of ear canal and obliteration of frontal sinus and mastoid antrum using bone dust.

A Case of Complete Treatment of Multiple Intracranial Tuberculoma (범발성 두개내 결핵종 완치 1례)

  • Although the incidence of intracranial tuberculosis in children have been decreased with development of antituberculous therapy, mortality rate is still high as formerly if the diagnosis and treatment was not performed early. Authors report a case of a 5-month old male with multiple intracranial tuberculoma who have been accompanied by miliary tuberculosis and tuberculous meningitis. He was hospitalized at the pediatric department of Wonkwang University Hospital with mild fever and persistent cough. The brain MRI was performed and showed that there were multiple intracranial micronodular densities, perinodular edema and diffuse leptomeningeal enhancement which were disappeared completely after antituberculosis chemotherapy for 12 months and he had completely recovered without sequale.

Radiological Follow-up of a Cerebral Tuberculoma with a Paradoxical Response Mimicking a Brain Tumor

  • We report a case of a paradoxical response of a tuberculoma in the brain mimicking a brain tumor. A 76-year-old woman presented with a 2 week history of headache, dysarthia, and orthopnea. Brain magnetic resonance images (MRI) revealed two rim-enhancing lesions on the pons and occipital lobe, and chest computed tomography showed randomly distributed miliary nodules. The tentative diagnosis was tuberculosis (TB) of the brain and lung. She complained of right hemiparesis and worsening general weakness after taking the anti-TB medication. On the monthly follow-up images, the enhanced lesions were enlarged with increased perfusion and choline/creatinine ratio, suggesting a high grade glioma. A surgical resection was completed to diagnose the occipital lesion, and the tuberculoma was pathologically confirmed by a positive TB-polymerase chain reaction. The anti-TB medication was continued for 13 months. A follow-up MRI showed decreased size of the brain lesions associated with perilesional edema, and the clinical symptoms had improved. Brain tuberculoma could be aggravated mimicking brain malignancy during administration of anti-TB medication. This paradoxical response can be effectively managed by continuing the anti-TB drugs.

A Study for Aging Factor of Bulletproof Helmet : High Temperature Treatment (방탄헬멧의 노화인자에 관한 연구 : 고온 환경 처리를 중심으로)

  • This study was a prior study to establish the shelf life of bulletproof helmets, considered the aging factor of bulletproof helmets. To estimate the aging factor of bulletproof helmets, we established a hypothesis that the 'temperature' factor would have the greatest impact on the shelf life of the bulletproof helmet, considering the environmental treatment of the American army's bulletproof helmet's materials. To verify the hypothesis, high temperature acceleration environmental treatment of bulletproof helmets was performed, and the Arrhenius formula was applied to calculate the shelf life. The study result confirmed the negative correlation between bulletproof performance and high temperature, and the influence of temperature as an aging factor was not significant by deriving life by using the Arrhenius model. The limitation of this study is that we couldn't obtain enough samples due to the specificity (miliary supplies) of the test subjects. However, given that the life-related research on bulletproof helmets has not been carried out, this research has great implications and could be used as a reference for improving the Korean Army's bulletproof helmet specifications.

Reduction of RDX in Ground Water by Bio-Regenerated Iron Mineral: Results of Field Verification Test at a Miliary Shooting Range (생물환원 철광물촉매에 의한 지하수 내 RDX 환원:군사격장 현장적용 실증결과)

  • This study investigates the in-situ implementation of bio-regenerated iron mineral catalyst to remove explosive compounds in ground water at a military shooting range in operation. A bio-regenerated iron mineral catalyst was synthesized using lepidocrocite (iron-bearing soil mineral), iron-reducing bacteria Shewanella putrefaciens CN32, and electron mediator (riboflavin) in the culture medium. This catalyst was then injected periodically in the ground to build a redox active zone acting like permeable reactive barrier through injection wells constructed at a live fire military shooting range. Ground water and core soils were sampled periodically for analysis of explosive compounds, mainly RDX and its metabolites, along with toxicity analysis and REDOX potential measurement. Results suggested that a redox active zone was formed in the subsurface in which contaminated ground water flows through. Concentration of RDX as well as toxicity (% inhibition) of ground water decreased in the downstream compared to those in the upstream while concentration of RDX reduction products increased in the downstream.

Two Cases of Sebaceous Carcinoma on Scalp (두피에 발생한 피지선암 2례)

  • Purpose: Sebaceous carcinoma is a rare malignant tumor derived from the adnexal epithelium of sebaceous glands. This tumor usually occurs on the eyelids, but uncommonly it may occur on the extraocular sites. It is characterized by a tendency of local recurrence and occasional metastasis. Surgical excision is appropriate treatment for patients with sebaceous carcinoma. Because this kind of case is rare, we report two cases of sebaceous carcinoma developed on scalp. Methods: Case 1 was a 69 - year - old woman. She visited the hospital with a $1.5{\times}2.5cm$ sized reddish yellow - colored, slowly growing mass on left parietal scalp. The mass began at birth and started growing at 5 years ago. Case 2 was a 67 - year - old woman. She had $2.5{\times}3.0cm$ sized yellow - colored mass on right parietal scalp. It occured at birth and started growing at 3 years ago. And the masses had erythematous ulcer with sanguineous discharge. In the beginning, the masses were miliary nodule. Results: CT scan and fine needle biopsy were done. Case 1 and 2 were diagnosed as sebaceous carcinoma. Wide excision with safety margin of 10 mm and split thickness skin graft was done. Histological examination revealed well demarcated, irregular, variable sized tumor lobules. Each lobule was composed of sebaceous and undifferentiated cells. Postoperatively, the patients did well and the lesion had not recurred. Conclusion: Sebaceous carcinoma is an aggressive malignant tumor. It often can be mistaken for other skin tumors. Authors experienced two rare cases of sebaceous carcinoma developed on scalp. We recommend early wide excision with enough safety margin as treatment of sebaceous carcinoma.
