Influencing Factors on Care Burden among Family Caregivers for Elders with Dementia: Focusing on Family Caregivers using a Support Center for Dementia (치매노인을 돌보는 가족부양자 부양부담의 영향 요인: 치매지원센터 이용자를 중심으로)

  • Purpose: This study aimed to explore influencing factors on care burden among family caregivers for elders with dementia living at home. Methods: Participants in this study were 211 family caregivers who were taking care of elders registered at a support center for dementia located in S-gu, Seoul. Data were collected using a structured questionnaire. Data were analyzed with descriptive statistics, t-test, ANOVA, Pearson correlation coefficients, and multiple regression in SPSS WIN 23.0. Results: The care burden of family caregivers was mild to moderate ($40.31{\pm}21.50$) in this study. The factors influencing care burden among family caregivers for elders with dementia were the age, behavioral problems, and dependency in instrumental activities of daily living of demented elderly, in addition to the perceived health status and resting hours of family caregivers. Overall, these factors explained 46.5% of the total variability in care burden in this sample (F=13.01, p<.001). Conclusion: Findings from this study suggest that the characteristics of demented elderly and family caregivers can influence care burden differently. Individually tailored strategies based on the various caregiving contexts need to be developed to reduce the level of care burden among family caregivers for elders with dementia.

Time Perception and Memory in Mild Cognitive Impairment and Alzheimer's Disease: A Preliminary Study

  • Background and Purpose: Episodic memory is a system that receives and stores information about temporally dated episodes and their interrelations. Our study aimed to investigate the relevance of episodic memory to time perception, with a specific focus on simultaneity/order judgment. Methods: Experiment 1 employed the simultaneity judgment task to discern differences in time perception between patients with mild cognitive impairment or dementia, and age-matched normals. A mathematical analysis capable of estimating subjects' time processing was utilized to identify the sensory and decisional components of temporal order and simultaneity judgment. Experiment 2 examined how differences in temporal perception relate to performance in temporal order memory, in which time delays play a critical role. Results: The temporal decision windows for both temporal order and simultaneity judgments exhibited marginal differences between patients with episodic memory impairment, and their healthy counterparts (p = 0.15, t(22) = 1.34). These temporal decision windows may be linked to the temporal separation of events in episodic memory (Pearson's ρ = -0.53, p = 0.05). Conclusions: Based on our findings, the frequency of visual events accumulated and encoded in the working memory system in the patients' and normal group appears to be approximately (5.7 and 11.2) Hz, respectively. According to the internal clock model, a lower frequency of event pulses tends to result in underestimation of event duration, which phenomenon might be linked to the observed time distortions in patients with dementia.

The Effects of Aroma Foot Reflex Massage on Sleep, Depression and Problem Behaviors on Elderly with Dementia (아로마 발반사 마사지가 경증 치매노인의 수면, 우울 및 문제행동에 미치는 효과)

  • Purpose: The purpose of this study was to evaluate the effects of aroma foot massage and reflexology on sleep, depression and problem behaviors on elderly with dementia. Methods: The study was a none equivalent control group pre-post test quasi-experimental design. The participants were 62 elderly with mild mentia. Using a coin toss, 31 participants were assigned to the experimental group and 31 to the control group. The aroma reflex foot massage comprised of aroma foot massage and foot reflexology. The intervention was conducted twice a week for six weeks. Results: There were significant differences in reported sleep and depression between the two groups. However, the issue of problem behaviors was not significant. Conclusion: Aroma reflex foot massage can be utilized as an effective intervention for sleep and depression in conjunction with drug therapy for the management of elderly with mild dementia.

  • The brain magnetic resonance images (MRI) is an important imaging biomarker in Alzheimer's disease (AD) as the cerebral atrophy has been shown to strongly associate with cognitive symptoms. The decrease of volume estimates in different structures of the medial temporal lobe related to memory correlates with the decline of cognitive functions in neurodegenerative diseases. During the past decades several methods have been developed for quantifying the disease related atrophy of hippocampus from MRI. Special effort has been dedicated to separate AD and mild cognitive impairment (MCI) related modifications from normal aging for the purpose of early detection and prediction. We trained a multi-class support vector machine (SVM) with probabilistic outputs on a sample (n = 58) of 20 normal controls (NC), 19 individuals with MCI, and 19 individuals with AD. The model was then applied to the cross-validation of same data set which no labels were known and the predictions. This study presents data on the association between MRI quantitative parameters of hippocampus and its quantitative structural changes examination use on the classification of the diseases.

  • Purpose : This study is a descriptive research study to establish basic data for stable operation of dementia relief center as a part of national responsibility system for dementia and to establish the role of the professional workforce. Methods : This study is a survey study involving 126 nurses working at 19 dementia relief centers. The collected data were analyzed using the SPSS/WIN 23.0. Results : The importance tasks that nurses think of were Dementia Diagnosis, Counseling, Register and Classification and Dementia Preventive Projects. In addition, tasks that showed high performance were in the order of Dementia Diagnosis, Counseling, Register and Classification, Management of Dementia subjects. As a result, the final items of work areas and contents included 45 subcategories of work content for eight work areas. Conclusion : Through this study, it was possible to investigate the overall work area and content of nurses at the Dementia relief Center. It is necessary to develop a work description suitable for nurses who are medical personnel specializing in dementia safety centers.   • One of the most negative social changes of the last decade is population aging which leads to 19 times more patients with Mild Cognitive Disorder(MCI). It is well established that MCI is the most important state that can prevent dementia with early diagnosis and intervention. However, the social security system for patients with dementia is not working properly due to the coronavirus pandemic and the limited human power. This article proposes a form of workbench and design principles for dementia training programs of extended reality devices. and the findings in this study provide a guide for considering the cognitive and physical and social functions of patients.

  • The purpose of this study is to contribute to research on dementia and policy making by reviewing research trends of dementia for 15 years and it was collected from 12,588 dementia - related research data for 15 years at Google 's scalar site. The results of this study confirm that the research on dementia, one of the most common geriatric diseases, is continuously increasing. Second, research on dementia is increasing due to social influences such as government support. Third, we suggested that dementia is changing from a viewpoint of diseases to be managed at the facility and treatment of severe dementia patients to a paradigm from the viewpoint of management of mild patients and community participation. Fourth, dementia seems to be shifting from a serious and negative awareness to a social atmosphere that is accepted as a slow but manageable disease. This is changing to a dementia-friendly social environment, which is making a very positive environmental change. This study contributes to the field research by presenting the trend of domestic research on dementia.

A Study of the Family Caregiver's Burden for the Senile Dementia in a Rural Area (일부 농촌 지역 치매노인 가족의 부양부담에 관한 연구)

  • The purpose of this study was to analysis level on family caregiver's burden for the elderly with senile dementia in a rural area and to choose priority care group, thereby facilitating the development of interventions to reduce the caregiver's burden. For this purpose, data were collected by questionaire from June 10 to October 8, 1994. The instruments for data collection were Caregiver Burden Inventory by Novak(1989) and Zarit et al(1982), severity of dementia by Hughes Scales (1982), ADL by Lawton (1971), patients' family caregiving activity by pre-survey and reference review(Lee, 1993 ; Jang, 1990 ; Yoo, 1982). The subjects were 60 family caregiver of senile dementia in a rural area. The data was analysed by the use of t-test, ANOVA, correlation and multiple regression. The results were as follows : 1. Total burden was evaluated over average, the mean of family burden was 61.24. 2. Significant variables which were correlated to the family caregiver's burden were the severity of dementia(F=30.52, p<0.001), ADL(F=5.43, p<0.01), PADL(F=6.14, p<0.01), caregiver's health status(F=6.05, p<0.01), a view of patient's prognosis(F=4.08, p<0.05), the number of hours per day spent on caregiving(F=19.64, p<0.001), level of intimacy of caregiver and patients(F=7.16, p<0.001), the frequency of caregiving activity(F=5.31, p<0.01). 3. ADL was the most important predictor to family caregiver burden(R2=0.6611). In addition to this, the severity of dementia, level of intimacy of caregiver and patient, relationship with the patient accounted for 79% of family caregiver burden. 4. The criteria of priority care group were as follows ; the mean of family caregiver burden was above 58, above of moderate ADL, the number of hours per day spent on caregiving above of 8 hours, above of moderate dementia. By the the severity of dementia, number of priority care group, mild dementia was 10(43.4%), moderate dementia 23 (92.0%), sever dementia 12(100.0%).

A Study on the Development of a Korean Medicine Clinical Pathway for Primary Care of Patients with Dementia Based on Clinical Pathway Methodology (한의표준임상경로에 기반한 치매 안심 한의주치의 모형 개발 연구)

  • Objectives: This study aims to establish a Korean medicine doctor's range of services in the dementia relief primary care system based on the previously developed dementia clinical practice guidelines (CPGs). Developing a dementia relief primary care Clinical Pathway (CP) can aid clinically when the Korean medicine primary care doctor conducts treatment. Methods: We analyzed Dementia Korean Medicine Primary Care Model Data and then applied CP Methodology to develop the configuration of the Korean Medicine Primary Care Model. For patients with Alzheimer's dementia (AD), vascular dementia (VD), and mild cognitive impairment (MCI), the Korean Medicine Primary Care Model focuses on improving cognitive function, everyday living abilities and easing symptoms through interventions described in CPGs. The contents of the draft model later include references to already-existing CPs. Results: The study sites were chosen as Korean medical clinics connected to primary care physicians in the dementia-friendly model. The CP used a time task matrix version to arrange the clinical chronology, which included all examinations, diagnoses, and treatment procedures, from the initial appointment to follow-ups and the end of therapy. Conclusions: It anticipates that Korean primary care doctors familiar with dementia can use the offered therapies for the first time by creating the dementia Korean medicine primary care model in this study. This is expected to maximize the range of medical services provided by Korean medicine and improve the standard of medical treatment.
