경도인지장애 환자의 수단적 일상생활활동에 대한 고찰 (Review of the instrumental ADL for the Patients With Mild Cognitive Impairment)

  • 본 연구는 최근 많은 관심이 되고 있는 경도인지장애 환자의 특징과 수단적 일상생활활동에 대해 알아봄으로써 임상가들에게 작업치료에서 경도인지장애환자를 평가 및 치료하는데 도움이 되고자 하였다. 현재 치매환자를 위한 작업치료는 매우 활발하게 이루어지고 있으나 치매로 전환될 위험이 높은 경도인지장애 환자에 대하여 작업치료적 접근은 아직 이루어지지 않고 있다. 본 고찰에서는 정상노인과 감별하기 어려운 경도인지장애 환자의 작업영역 중 수단적 일상생활활동에 대해 초점을 맞추어야 한다는 근거들을 다루었으며, 경도인지장애 환자가 치매로 발전하는 것을 예방하기 위해서는 수단적 일상생활활동에 대한 고려가 필수적으로 이루어져야 함을 고찰하였다. 결론적으로, 치매로 발전 될 가능성이 높은 위험 집단인 경도인지장애 환자군의 작업영역을 고려한 수단적 일상생활활동에 대한 주기적인 평가와 그에 따른 작업치료적 개입이 필요할 것으로 보인다.

경증 치매 노인을 위한 애플리케이션 디자인기획 (Application Design Planning for the Elderly with Mild Dementia)

  • 우리 사회는 고령화에 따라 치매 유병률이 지속적으로 증가하여 사회적으로 큰 문제가 되고 있다. 국가적으로 장기 요양 보험 제도를 시행하고 있으나, 조건이 안되는 치매 노인은 기본적인 요양서비스도 이용할 수 없는 상황이다. 이러한 사회적 배경 아래 스마트폰 사용이 보편화되는 환경 하에서 현재 서비스되는 애플리케이션의 기능 분석과 대상자 설문조사를 토대로 초기(경증) 치매 노인에게 질병 관리 및 가족 간의 소통에 도움을 주는 애플리케이션에 대해 연구하고자 하였다. 연구 방법은 연구주제와 관련된 이론 연구를 배경으로 경증 치매 노인을 위해 서비스되는 애플리케이션 사용 환경을 분석하였으며, 이 분석을 통해 치매 노인을 위한 앱이 없다는 사실을 알게 되었다. 이 기본연구를 바탕으로 경도 치매 노인에게 필요한 약물 복용 관리, 신체 활동 관리, 두뇌 활동 관리, 치매 정보, 메모 등의 기능이 포함된 애플리케이션의 디자인기획 방향을 제시하였다. 나라에서 시행하는 제도적 지원에 이와 같은 서비스가 확대된다면, 애플리케이션 사용을 통해 수집된 데이터를 기반으로 경증 치매의 제도적 관리 선진화에 기여를 할 것으로 기대한다.

협응이동훈련이 경도인지장애노인의 낙상물리요인에 미치는 영향 (The Effect of Coordinative Locomotor Training on Physical Factors for Falls in the Elderly with Mild Cognitive Impairment)

  • PURPOSE: This study examined the effects of coordinative locomotor training on the physical factors for falls in the elderly with mild cognitive impairment. METHODS: This study examined thirty subjects diagnosed with mild cognitive impairment by the radiologic findings, history, and physical examination. The subjects were assigned to a control group (n = 15) or experimental group (n = 15, coordinative locomotor training). The experimental group underwent coordinative locomotor training for four weeks, with training sessions two times per week. The control group was given a fall-prevention education for 60 minutes without coordinative locomotor training. To evaluate the physical factors for falls, the lower extremity strength and the Korean version of the Fullerton advanced balance scale and biorescue were measured for balance. These tests were conducted before and after training. RESULTS: Significant differences were observed between the two groups after the four weeks of coordinative locomotor training for the elderly with mild cognitive impairment the experimental group had a greater degree of improvement in the physical factors for falls. CONCLUSION: These findings suggest that coordinative locomotor training may have a functional effect on fall-prevention and the mobility of the elderly with mild cognitive impairment. In addition, it is expected to provide systematic and effective data that can be used as a fall prevention program for the elderly with mild cognitive impairment in each institution.

The Usefulness of Brain Magnetic Resonance Imaging with Mild Head Injury and the Negative Findings of Brain Computed Tomography

  • Objective : To investigate the cases of intracranial abnormal brain MRI findings even in the negative brain CT scan after mild head injury. Methods : During a 2-year period (January 2009-December 2010), we prospectively evaluated both brain CT and brain MRI of 180 patients with mild head injury. Patients were classified into two groups according to presence or absence of abnormal brain MRI finding even in the negative brain CT scan after mild head injury. Two neurosurgeons and one neuroradiologist validated the images from both brain CT scan and brain MRI double blindly. Results : Intracranial injury with negative brain CT scan after mild head injury occurred in 18 patients (10.0%). Headache (51.7%) without neurologic signs was the most common symptom. Locations of intracranial lesions showing abnormal brain MRI were as follows; temporal base (n=8), frontal pole (n=5), falx cerebri (n=2), basal ganglia (n=1), tentorium (n=1), and sylvian fissure (n=1). Intracranial injury was common in patients with a loss of consciousness, symptom duration >2 weeks, or in cases of patients with linear skull fracture (p=0.00013), and also more frequent in multiple associated injury than simple one (35.7%>8.6%) (p=0.105). Conclusion : Our investigation showed that patients with mild head injury even in the negative brain CT scan had a few cases of intracranial injury. These findings indicate that even though the brain CT does not show abnormal findings, they should be thoroughly watched in further study including brain MRI in cases of multiple injuries and when their complaints are sustained.

경도 인지장애 선별에 적용된 이중과제 평가에 대한 체계적 고찰 (A Systematic Review of the Application Dual Task Assessment for Screening Mild Cognitive Impairment)

  • Purpose: This study aims to systematically review the dual-task evaluation applied to the screening of mild cognitive impairment. It also aims to present various evaluation items and results analysis methods for dual tasks applied to patients with mild cognitive impairment. Methods: We conducted a systematic search of published studies in PubMed databases and KISS from January 2000 to August 2020 using the main keywords such as "Dual task," "Mild Cognitive impairment," "Elderly," and "Screening." We selected a total of 10 studies for the analysis from 1314 searched articles. Results: We analyzed the qualitative level of 10 studies that were nonrandomized two-group studies with evidence level II (100.0%). These results suggest that the evidence level of the studies was high. We analyzed 10 studies and identified 12 motor tasks and 19 cognitive tasks. Walking was the most commonly used evaluation motor task and counting backward by ones and naming animals were the most commonly used evaluation cognitive tasks. Moreover, the velocity speed was the most used result analysis method. The results indicate that there were significant differences in dual-task performance between patients with normal and mild cognitive impairment. Conclusion: The results of this study can be used as a basis for the selection of dual-task evaluation items and methods of analyzing the results for screening mild cognitive impairment. Furthermore, they are expected to be used for research on the development of dual-task evaluation tools. It is necessary to compare and analyze the usage trends of dual-task evaluation by cultural differences in future studies.

경두개 직류자극과 가상현실프로그램 적용이 경도인지장애 환자의 인지 및 우울에 미치는 영향 (Effects of Transcranial Direct Current Stimulation and Virtual Reality Program Application on the Cognition and Depression of Mild Cognitive Impairment Patients)

  • Purpose : The purpose of this study is to investigate the effects of transcranial direct current stimulation and virtual reality program application on cognition and depression of patients with mild cognitive impairment, and to find an intervention method that can enhance active participation of patients with mild cognitive impairment. Methods : In this study, 50 mild cognitive impairment patients were divided into a treatment group (25 patients) and a control group (25 patients). The treatment group was applied with a transcranial direct current stimulation and a virtual reality program, while the control group received a placebo transcranial direct current stimulation and a virtual reality program. Both groups received five 50-minute sessions per week (one session per day) for six weeks (total of 30 sessions). NCSE was used to evaluate the cognitive functions of the patients before and after treatment intervention. Moreover, K-BDI was conducted to examine the depression of the patients. Results : As a result of the transcranial direct current stimulation and a virtual reality program intervention, the cognitive function of both treatment and control group significantly (p<.05) improved, and the depression of both treatment and control group significantly (p<.05) decreased. Moreover, the changes in cognitive functions and depression were significant between the two groups¸ treatment and control group (p<.05). Conclusion : The results of the study showed that the application of the transcranial direct current stimulation and virtual reality program significantly improved the cognitive function of mild cognitive impairment patients and decreased the depression of them. Therefore, it could be concluded that the transcranial direct current stimulation and virtual reality program was an intervention method which positively affects the cognitive function and depression of mild cognitive impairment patients.

Euler-Lagrange 식을 사용한 확장형 완경사방정식 유도 (Derivation of Extended Mild-Slope Equation Using Euler-Lagrange Equation)

  • 본 연구에서 Euler-Lagrange 식을 사용하여 속도포텐셜로 표현되는 확장형 완경사방정식을 유도하였다. 먼저, Euler-Lagrange 식을 사용하여 흐름함수로 표현된 확장형 완경사방정식을 유도한 Kim과 Bai(2004)의 유도과정을 따라가면서 속도 표텐셜로 표현된 확장형 완경사방정식과의 관계를 찾았다. 속도포텐셜로 표현된 Euler-Lagrange 식을 찾아낸 다음 고차의 수심변화 항을 유도하였다. 본 연구에서 유도된 확장형 완경사방정식은 기존의 식인 Massel(1993)의 식과 Chamberlain과 Porter(1995)의 식과 정확히 일치하였다. 본 연구의 연구 성과는 확장형 완경사방정식의 유도 방법을 새로 제시하여 해안공학의 영역을 넓히는데 의의가 있다.

약독 담배 모자이크 바이러스 I. 약독바이러스의 유기 및 특성 (Studies on Mild Mutants of Tabacco Mosaic Virus I. Induction of Mild Mutants and Their Characteristics)

  • TMV-OM계통을 접종하여 고온처리시킨 Samsun으로부터 3종의 약독변이계통 Tg 5272, Tw 227 및 Tw 333을 유기시켰다. 이들 약독바이러스는 담배 및 고추에 가벼운 mottling 증상이나 무증징으로 감염되어 TMV-OM에 의한 모자이크, 기형, 위축현상 등을 나타내지 않고 건전식물과 비교하여 생육에 거의 차이를 보이지 않았다. 또한 국부병반기주에 감염되었을 때에도 원주에 비해 전반적으로 미약한 반응을 나타냈고, 특히 Tw 227 및 Tw 333은 N. glutinosa, D. stramonium 및 G. globosa에서 매우 특이한 병징을 나타냈다. 약독바이러스의 물리적 성질 및 TMV-OM 항혈청에 대한 혈청학적 성질의 차는 인정되지 않았다. 한편 약독바이러스를 둔화하였을 때, 약독바이러스의 농도는 TMV-OM에 비해 매우 낮았으나, 이들 바이러스의 자외선흡수 및 전기영동패턴에는 차이를 보이지 않았다.

금속 표면처리방법이 니켈-크롬 합금 의치상과 첨상레진간의 결합강도에 미치는 영향 (EFFECTS OF METAL SURFACE TREATMENTS ON THE SHEAR BOND STRENGTH BETWEEN NI-CR DENTURE BASE AND RELINE RESINS)

  • The purpose of this study was to evaluate the effects of four metal surface treatments on the shear bond strength of reline resin to Ni-Cr alloy. The denture base metal used in this study was Ni-Cr alloy(Ticonium Premium 100. Ticonium Co., U.S.A.). 120 specimens were divided into five metal surface treatments: sandblasting only, MR. BOND(Tokuyama Corp.. Japan), Cesead Opaque Primer(Kuraray Co., Japan), METALPRIMER II(GC Corp., Japan) and Super-Bond C&B(Sun Medical Co., Japan) after sandblasting. They were bonded with one of three reline resins Mild Rebaron(GC Corp., Japan), Mild Rebaron LC(GC Corp., Japan) and Meta Base M(Sun Medical Co., Japan). Then they were thermocycled 1,000 times at temperature of $4^{\circ}C$ and $60^{\circ}C$. The shear bond strengths were measured using the universal testing machine(Instron, Model 4301, England) with a cross-head speed of 2 mm/min. The results were as follows : 1. All metal primers and adhesive cement significantly improved the bond strength of reline resin to Ni-Cr alloy compared with sandblasted specimens. 2. In Mild Rebaron and Mild Rebaron LC. Cesead Opaque Primer showed the highest bond strength, but the differences among Cesead Opaque Primer, MR. BOND and METALPRIMER II were not significant. The bond strength of Cesead Opaque Primer was significantly different with that of Super-Bond C&B. 3. In Meta Base M, Super-Bond C&B showed the highest bond strength, but there was no difference between Super-Bond C&B and three metal primers. 4. There was no difference in the bond strength between Mild Rebaron and Mild Rebaron LC when metal surface was treated with the same method. 5. The bond strengths of Mild Rebaron and Mild Rebaron LC treated with Cesead Opaque Primer were higher than that of Meta Base M. The bond strengths of Mild Rebaron treated with MR. BOND and METALPRIMER II was higher than that of Meta Base M, However, there was no difference among three reline resins treated with Super-Bond C&B.
