Congenital Cleft of Anterior Arch and Partial Aplasia of the Posterior Arch of the C1

  • Congenital anomalies in arches of the atlas are rare, and are usually discovered incidentally. However, a very rare subgroup of patients with unique radiographic features is predisposed to transient quadriparesis after minor cervical or head trauma, A 46-year-old male presented with a 2-month history of tremor and hyperesthesia of the lower extremities after experiencing a minor head trauma. He said that he had been quadriplegic for about 2 weeks after that trauma. Radiographs of his cervical spine revealed bilateral bony defects of the lateral aspects of the posterior arch of C1 and a midline cleft within the anterior arch of the atlas. A magnetic resonance imaging revealed an increased cord signal at the C2 level on the T2-weighted sagittal image. A posterior, suboccipital midline approach for excision of the remnant posterior tubercle was performed. The patient showed significant improvement of his motor and sensory functions. Since major neurologic deficits can be produced by a minor trauma, it is crucial to recognize this anomaly.

Postoperative Results of Kyphoplasty for Osteoporotic Vertebral Compression Fractures

  • Objective: We analyze pain relief, deformity correction and complication rate after percutaneous kyphoplasty for osteoporotic vertebral compression fractures. Methods: The authors retrospectively reviewed medical records and radiological findings of 32 patients who underwent percutaneous kyphoplasty for osteoporotic vertebral compression fractures. Results: The patients had significant pain improvement with the procedure. The visual analogue scale score reduced from 8.6 to 3.4 significantly after the procedure. The midline vertebral body height significantly increased postoperatively, but mean kyphotic angle did not. There was no serious complication except one case of epidural cement leakage without neurological impairment. Conclusion: Balloon kyphoplasty safely can reduce severe back pain and returned geriatric patients to higher activity levels. The midline vertebral height is restored significantly. However kyphotic deformity correction is not significant as contrary to what we expected from the present study before it was carried out.

A Neurolinguistic Study of Korean Scrambling: An Event-related Potentials(EPR) based Study (한국어 어순재배치(scrambling) 문장의 신경언어학적 연구)

  • 본 연구는 한국어 어순재배치(scrambling) 문장의 이해 과정에서 발생되는 대뇌 활동을 사건관련전위(event-related Potentials; ERPs) 이용하여 살펴보기 위하여 실시되었다. 네 개의 어절로 구성된 표준 어순 문장(일년만에 마님이 영감을 만났어요.)과 어순재배치 문장(일년만에 영감을 마님이 만났어요.)을 어절별로 제시하고 첫 번째 명사구(NP1), 두 번째 명사구(NP2), 동사(Verb)의 시작점(onset)에서 측정한 뇌파를 비교하였다. 뇌파의 분석은 대뇌 영역을 중심선(midline), 중앙(medial), 편측(lateral)로 나누어 전후 분포(anterior-posterior distribution)와 정중선(midline)의 열에 의해 좌우 반구(hemisphere)로 분리하여 분석하였다. 분석 결과 중심선 영역에서 표준 어순에 비해 뒤섞기 어순에서 300-500ms 시간 창(time window)에서 큰 부적 전위(negative potential)가 관찰되었으며 이는 어순재배치로 인한 N400효과로 해석되며 P600효과는 관찰되지 않았다. 특히 첫 번째 명사구에서 문장유형(표준 어순 vs. 어순재배치)의 차이가 가장 크게 관찰되었으며 두 번째 명사구에서는 중앙에서 문장유형과 반구(좌우반구)의 상호작용이 관찰되었고, 동사에서는 문장유형과 반구, 문장유형과 전극 위치의 전후 분포와의 상호작용이 관찰되었다. 본 연구 결과에서 관찰된 N400효과는 독일어와 일본어를 대상으로 한 어순재배치 연구 결과와 유사하며 한국어 어순재배치 문장에 관한 사건관련 전위를 고찰하였다는 점에서 의의가 있다.

Rheological Properties of Flour Dough Added Power of Poria Cocos Wolf (백복령분말을 첨가한 밀가루 반죽의 물성특성)

  • The effects of flour dough by addition of Poria Cocos Wolf powder were investigated in dough added with $0{\sim}5%$ powder by evaluation of dough. Rheological properties of dough by mixograph, farinograph, alveograph and extensograph, and scanning electron microscope. The water absorption rate and resistance were increased by Increase of Poria Cocos Wolf powder. Mixograph for dough stability and v/v point dough were increased in dough added $1{\sim}2$ percent of Poria Cocos Wolf powder. In water absorption, stability and valorimeter value of the dough increased in weakness of the dough, as revealed through farinogram, extensiblity, resistance to extension, and energy increased in R/E ratio, as revealed through extensogram. Scanning electron microscope of bread wasn't changed by Poria Cocos Wolf powder.

The Effects of 4-Week Serratus Anterior Strengthening Exercise Program on the Scapular Position and Pain of the Neck and Interscapular Region

  • The purpose of this study was to investigate the effects of serratus anterior strengthening exercises on scapular position and Visual Analog Scale (VAS) pain measurements taken at the resting position in young adults with adducted scapular. The exercise program included stretching of the scapular retractor and strengthening of the serratus anterior muscle. We measured the distance from the midline of the thorax to the vertebral border of the scapular with a tape line (Superior Kibler), and the distance from the 7th cervical spinous process to the acromial angle with 3-dimension motion analysis system, to compare the resting scapular position before and after exercise. Fifteen subjects with adducted scapular were recruited to compare the resting scapular position and VAS. The distance from 7th cervical spine process to acromial angle of the scapular and VAS decreased significantly (p<.01) after exercise, while the distance from the midline of the thorax to vertebral border of the scapular increased (p<.05). The conclusion is that the serratus anterior exercise program altered the resting scapular position and decreased VAS.

  • Background and Objectives: Silicone, Gore- Tex, Gelfoam, Collagen and autologous fat are used for thyroplasty in case of vocal fold paralysis or paresis. These implants have many advantages, such as biocompatibility, low price and easy handling and accessibility. But additional voice modification is impossible using these materials. So, we design new thyroplasty technique, called balloon thyroplasty using Foley catheter. Materal and Methods: The fresh human larynx was dissected in midline posteriorly. Minithyrotomy hole was created using 5mm cutting burr in the midline of thyroid cartilage. Subperichondrial dissection was done using Duckbill elevator up to vocal process. Balloon catheter(1.5cm balloon size) was inserted through the subperichondrial tunnel. The balloon was inflated to medialize the vocal cord. Results: After ballooning, the true vocal cord medialized mimicking thyroplasty. Conclusion: The authors found that Balloon thyroplasty could be a good candidate for vocal fold medialization technique. The technical refinement and in vivo safety are reserved for the ongoing study.

Combined Repair of Pectus Excavatum and Open Heart Surgery in Marfan's Syndrome (Marfan 증후군 환자에서 개심수술 및 누두흉의 교정)

  • The presense of pectus excavatum in Marfan's syndrome may complicate cardiac operation by making midline sternotomy technically more difficult and limiting the operative exposure of the heart. We operated on a 33 year old male patient with Marfan's syndrome and severe pectus excavatum who had severe mitral regurgitation and moderate aortic regurgitation with 52mm aortic root dilation. The operative field was adequately exposed through a midline sternal incision with two sternal retactors. The patient underwent Bentall operation and mitral valve replacement. The repair of pectus excavatum was performed after completion of CPB and the administration of protamin. Permanent internal stabilization achieved by overlapping of the ends of lower ribs and reinforced with sternal closure wire.

A Case of Extranodal NK/T-cell Lymphoma, Nasal Type of the Oropharynx and Supraglottis (구인두와 성문상부에 발생한 결절 외 비성 NK/T 세포 림프종 1예)

  • The extranodal nasal NK/T-cell lymphoma was formerly known as lethal midline granuloma due to its most common clinical pattern like a destructive midline facial tumor. It often spread to other extranodal sites, such as skin, soft tissue, testis, upper respiratory tract, and gastrointestinal tract etc. For this reason, the term of extranodal NK/T-cell lymphoma, nasal type is preferred. Its disease entity may have a prominent admixture of inflammatory cells and necrotic tissues, further causing difficulty in diagnosis. A 44-year-old man was visited to our clinic with complaints of dysphagia and odynophagia during six months. He underwent three times punch biopsies and then, two times incisional biopsies. Finally, He was diagnosed as extranodal NK/T-cell lymphoma, nasal type involving oropharynx and supraglottis. We report an extremely rare case of extranodal NK/T-cell lymphoma, nasal type occurred in oropharynx and supraglottis with a brief review of literature.

Thyroglossal Duct Lesions in Childhood (소아에서의 갑상선설관낭종의 임상적 고찰)

  • Thirty-four consecutive cases of thyroglossal duct lesions in children were reviewed at the Department of Surgery, Chonbuk National University Hospital. Twenty patients were males, and the most prevalent age of discovering was 2 to 4 years (52.9%). Resection was performed within 2 years after discovering the lesions in 19 cases, but was delayed untill 4 to 10 years in 6 cases. Cystic lesions (85.3%) were 5.8 times more common than fistulas. A midline upper neck mass was found in every cystic cases, and a draining sinus at hyoid region was noticed in fistula patients. The location of the lesion was on the hyoid bone in 34 cases, at the midline in 31 cases, slightly to the left in 2 cases, and slightly to the right in 1. Two cases were misdiagnosed as lymphadenopathy, and a single case of ectopic thyroid gland was misinterpreted as a thyroglossal duct cyst. Modified Sistrunk operation was performed in twenty-three cases(67.6%), Sistrunk operation in 9(26.5%), and cyst excision in 2(5.9%). Postoperative complications occurred in 4 cases(11.8%); 2 wound infections and 2 recurrences. One of recurrences was a fistula treated by modified Sistrunk operation, and the other was a cyst treated by cyst excision.

Projections from the Prefrontal Cortex to the Dorsal Raphe Nucleus of the Rat

  • Projections from the prefrontal cortex to subdivisions of the dorsal raphe nucleus were investigated in the rat using retrograde and anterograde tracing methods. A retrograde tracer, gold-conjugated horseradish peroxidase (WGA-apo-HRP-gold), was injected into each subdivision of the dorsal raphe including lateral wing, dorsomedial, and ventromedial areas. The majority of retrogradely labeled cells were located in the prelimbic, infralim-bic, and dorsal peduncular areas of the medial prefrontal cortex. A few cells were also identified in the cingulate, various regions of the orbital, and agranular insular cortices. Secondly, an anterograde tracer, Phaseolus vulgaris leucoagglutinin (PHA-L), was injected into the medial prefrontal cortex involving the prelimbic or infralimbic areas. Axonal fibers with varicosities were identified in all subdivisions of the DR including the lateral wing, dorsomedial, and ventromedial areas. Projections were bilateral, with ipsilateral predominance. Axonal fibers were observed at the lateral border of medial longitudinal fasciculus or in the interfascicular region at the midline. The present findings demonstrate that both the midline and lateral wing regions of the dorsal raphe nucleus receive excitatory input from cognitive and emotional centers of the cerebral cortex.
