Objectives: Metabolic syndrome is considered a coronary heart disease risk factor and its prevalence rate is increasing in Korea. Because obesity is relevant to metabolic syndrome, we investigated the relationship between metabolic syndrome and the Obesity Pattern Identification Questionnaire in middle-aged health check-up examinees. Methods: This was a cross-sectional study with 125 patients who visited a health promotion center of university hospital from October 2012 to January 2013. We analyzed the association of Obesity Pattern Identification Questionnaire and the diagnostic criteria of metabolic syndrome. Results: Pi deficiency (脾虛), phlegm (痰飮), liver stasis (肝鬱) and food accumulation (食積) pattern showed significantly highs score in the group with hypertriglyceridemia. Also, females demonstrated significantly high scores of liver stasis (肝鬱) and food accumulation (食積) in the group with hypertriglyceridemia. The questions of Pattern Identification that showed especially significant high score in the group of hypertriglyceridemia are as follows: 'Easily get annoyed', 'Usually worried', 'Frequently overeating or bingeing', and 'Having more after getting full'. There are positive correlations between triglyceride and the score of Pi deficiency (脾虛), phlegm (痰飮) and food accumulation (食積) pattern. Conclusions: Obesity Pattern Identification Questionnaire can be used for the management of hypertriglyceridemia in an effort to prevent metabolic syndrome.

Purpose: Finger injury by green onion cutting machine is one of the common hand injuries in the kitchen. It has a unique feature: there are multiple parellel laceration 3 - 5 mm wide. There are two directions of injuries(vertical, oblique). It may involve bone, tendon, nerve, and vessel injuries. We discuss its management and the long - term progress. Methods: We have treated six patients from 2003 to 2007. We carried out low tension approximation with thin suture materials to avoid ischemia and performed the additional operation as nail bed repair, tenorrhaphy, open reduction, vessel anastomosis, and composite graft. We reviewed the record of initial injury and collected the follow - up record. Results: They were all middle aged - women who had worked in the kitchen. Right hand was dominent over left hand. The ratio of the directions was 3 : 3 (vertical : oblique). They were all competely healed although there were three atrophy, four hyperesthesia, and one nail deformity. Conclusion: Finger injury by green onion cutting machine is a unique pattern of laceration with various accompanied injuries. It may look like a severe form of injury, but in most cases have relatively favorable progress. We have to perform careful examination of accompanied injuries and carry out the proper management. First and foremost, the user especially in the middle aged women should be warned to be careful in handling this risky machine.
Purpose: The purpose of this study was to evaluate the effectiveness of aromatherapy on headache, anxiety, and serum cortisol level in middle-aged women with recurrent headaches. Methods: Nineteen patients in the intervention group and 21 in the control group participated in the exercise. They were randomly selected volunteers whose average headache score over 6 months with above 4 points on the visual analogue scale (VAS). The experimental group received aromatherapy for 5 days; inhalation (3 times per day) and an application on the neck and both shoulders (one time per day). Prior and post treatment scores in headache, anxiety, and measurement of serum cortisol in experimental group were measured. Then, these were compared with the control group. The headache score was measured by VAS; anxiety score was measured by State Trait Anxiety Inventory; serum cortisol was measured by Radio-Immune-Assay method. Results: Decrease in headache, anxiety, and serum cortisol level in the experimental group was greater than those in the control group. Conclusion: It confirmed aromatherapy is effective in reducing headache, anxiety, and serum cortisol level.
Pupose. This study aimed to analyze the relationship between metabolic syndrome and glaucoma in a middle-aged men over the age of 40's by using the data from the 5th Korean National Health and Nutrition Examination Survey(2010~2012). Methods. The subjects for the analysis were comprised of 410 patients who were at least 40 years or more with metabolic syndrome and glaucoma. The comparison between the metabolic syndrome and glaucoma according to the health demographic characteristics, health behavior and health conditions was analyzed with ${\chi}^2$-test. The complex samples logistic regression analysis was used to calculate the odds ratio of glaucoma according to the health demographic characteristics, health behavior and health conditions and the combination of the comprising factors of the metabolic syndrome. Results. The prevalence of glaucoma among the subjects with metabolic syndrome and glaucoma was significantly higher in the case of females, as their age increased, educational level decreased, income level decreased, who are non-smokers, non-drinkers, with exercising habits of less than 3 days a week (p<0.001). However, the glaucoma OR was significantly high in case of increasing age, with exercising habits of less than 3 days a week, and hypertension (p<0.01), while the OR was significantly low in case of the drinking group (p<0.05). The results of analyzing the odds ratio for the occurrence of glaucoma with the components of metabolic syndrome showed that the OR for glaucoma is significantly low (OR 0.63) in case of concurrently high fasting blood glucose, high triglycerides and low HDL cholesterol levels (p=0.05). Conclusions. Therefore the thorough management of metabolic syndrome and components is needed to prevent glaucoma.

Overweight and abdominal obesity caused by sedentary lifestyle may induce deconditioned and atrophied extensor musculature of the lumbar spine and be a potential risk factor for low back pain (LBP). Therefore, this study was conducted to evaluate the validity of high intensity circuit training (HICT) on weight loss and subsequent alleviation of spine curvature (SC) and visual analog scale (VAS) among middle aged men with abdominal obesity. The training program (1 exercise session for 30 minutes, three times a week for 12 weeks) consisted of 12 different functional exercises based on core strengthening multiple joint circuit training. Portions of the obesity index related to body composition were positively changed, which improved the angles of thoracic kyposis and lumbar lordosis, which appeared to effectively reduce lower back pain. Taken together, HICT specifically designed for LBP effectively decreased obesity related body composition and was superior to other treatments for decreasing aggravation of the spine curvature and LBP caused by abdominal obesity; however, weight loss should be the primary treatment target for LBP patients.

Background: This study was carried out to contribute to effective management of operated patients by analyzing cooperative details about patients who were taken elective operation and consulted to the department of anesthesiology. Patients and Methods: One thousand patients who have been consulted to department of Anesthesiology, Chonnam National University Hospital from 2008. November to 2009. March were analyzed. The gender, age, cooperative departments, cooperative reason, the number of cancelled cases and cancelled reason were evaluated. Results: Among 1000 patients, there were 470 females and 530 males. After adjusting for age, middle-aged people which range 45 to 64 year-old are 39.9% and over 65 year-old people are 30.3%. Cooperative department is distributed to orthopedics which is highest occupying 20.6%, neurosurgery, otorhinolaryngology, obstetrics and gynecology in order. Dentistry occupied 2.0%. By specifying reasons of cooperation, cardiovascular diseases distribute to 38.4% which is the highest, the respiratory diseases, 17.5% and the endocrine diseases including diabetes 13.2%. Arranging details, hypertension is 23.9%, ECG abnormality 14.5%, chronic bronchitis in chest radiographic interpretation 14.2%. Of 1,000 consulted patients, delayed or cancelled cases are 58. And the reasons were 26 cases of need of appropriate the blood-sugar level. 26 cases needed appropriate treatment for comorbidities and others 6. Conclusion: Thoroughly evaluating patients before operation and maintenace of active cooperative system between operative department and anesthesiology department can contribute to reduction of cancellation rate and effective management of both hospitalizing and operating rooms.
The purpose of this study was to determine the association between drinking behaviors and the need for periodontal treatment. The data of 5,995 adults from the 2012 Korea National Health and Nutrition Examination Survey were categorized into young adulthood (19~39 years), middle age (40~64 years), and old age (${\geq}65$ years) and analyzed. Chi-square test was performed to determine those who needed periodontal treatment by the respondents' general characteristics and drinking behaviors, and logistic regression analysis was carried out to determine the impact of each drinking behavior on the need for periodontal treatment. The results of this study have confirmed that alcohol intake is a factor adversely affecting periodontal status; therefore, patients should know the fact that excessive alcohol intake and increased frequency of drinking may affect the prevalence rate of periodontal diseases. Moreover, moderate-drinking programs that can reinforce prevention of drinking so that dental hygienists can provide good oral health care should be developed. A systematic oral health promotion program should be introduced for constant prevention with the objective of activating oral health education that focused on middle-aged and young-adult people in pursuit of healthy aging society.

Purpose: To evaluate clinical features and surgical results for rotator cuff tear secondary to shoulder dislocation in middle-aged and elderly patients. Materials and Methods: We reviewed 19 patients over 50 years of age who had rotator cuff tears combined with shoulder dislocation between October 2004 and October 2008. There were 7 males and 12 females with a mean age 64.7 years (range, 50 to 78 years). The average follow-up duration was 22 months (range, 8 to 56 months). We investigated the number of dislocations, the size of the cuff tear, the presence of Bankart lesions and the time interval from dislocation to surgery. We also investigated the ASES score, UCLA score, SST score, and shoulder range of motion before and after surgery. We analyzed clinical outcomes and contributing factors. Results: ASES scores improved from 30.2 preoperatively to 72.3 postoperatively; UCLA scores improved from 12.9 to 26.5; SST scores improved from 2.4 to 7.3. Range of motion improved significantly: forward flexion, abduction, external rotation and internal rotation were, respectively, $110.8({\pm}39.3)^{\circ}$, $107.7({\pm}40)^{\circ}$, $22.5({\pm}17.6)^{\circ}$ and L5 level preoperatively; postoperatively they were $153.6({\pm}20.6)^{\circ}$, $152.1({\pm}20.8)^{\circ}$, $36.4({\pm}22.7)^{\circ}$ and L1 level. Age, the presence of Bankart lesions and the number of dislocations were not correlated with clinical outcomes. But the size of the cuff tear was correlated with clinical results. Also, the duration from dislocation to surgery was correlated with postoperative UCLA and SST scores (p=0.039, p=0.038). Conclusion: For shoulder dislocation, it is important to achieve early diagnoses of rotator cuff tears in middle-aged and elderly patients. If these injuries are both present, early rotator cuff repair should be performed for better clinical results.
Objectives : Middle aged women with depression often experience hot flush symptoms. It is still unclear about the association between depression and hot flush symptoms. Therefore, we investigated hormonal profile, functional somatic symptoms, coping styles and attitude for menopause between depressed women with hot flush and those without hot flush. Methods : This study included 33 depressed patients with hot flush symptoms and 33 depressed patients without hot flush symptoms. Hot flush was confirmed through prospective daily symptom ratings for a week. Subjects' coping styles were assessed by the stress coping checklist. Somatic symptoms were assessed by the Patient Health Questionnaire-15(PHQ-15). Sex hormonal levels were assayed by a radioimmunoassay. The Brief World Health Organization Quality of Life Assessment Instrument(WHOQOL-BREF) was used to evaluate quality of life. Results : In coping style, depressed women with hot flush symptoms used less 'problem-centered coping'($13.15{\pm}3.17$) and 'search for social support'($11.83{\pm}2.84$) than those without hot flush symptoms($15.17{\pm}3.1$, p=0.028 ; $14.25{\pm}3.22$, p=0.009 ; respectively). Depressed women with hot flush symptoms showed more negative attitude toward post-menopause, but its statistical significance was marginally insufficient(p=0.059). We did not find any group differences in sex hormonal levels and somatic complaints assessed by the PHQ-15. The score of social relationship domain of WHOQOL-BREF was significantly lower in subjects with hot flush symptoms($8.62{\pm}2.04$) than subjects without hot flush symptoms($9.71{\pm}1.65$ ; p=0.044). Conclusions : Among middle aged women with depression, the manifestation of hot flush symptoms was associated with coping styles and attitude for menopausal transition. Hot flush symptoms in depressed women negatively influence quality of life, so clinicians actively perform therapeutic approach in case of depressed patients with hot flush symptoms. In case of depressed patients who present hot flushes, cognitive behavior therapy or stress management might be an effective treatment option in company with antidepressants or hormonal treatment. Later, longitudinal study will be needed to evaluate risk factor, cause and effect associated with hot flush and depression.
The purpose of this study was to examine the anatomic changes in the upper airway with a dental orthosis. the effectiveness and side effects of orthosis in the treatment of snoring and obstructive sleep apnea. To meet this puppose a dental orthosis, designed to increase the size of the upper airway by advancing the mandible, was used in 42 patients (30 M, 12 F), aged 29 - 69 years, to treat snoring and varying decrees of obstructive sleep apnea. Cephalometric study of anatomic featured was made with and without a dental orthosis, and the evaluation of the effectiveness and side effects of orthosis was done by questionnaires. The obtained results were as follows : 1. All subjects were habitual snorers and 32 patients comp1ained the loudness of snoring as severe as be heard outside of the patient's room. 2. According to the degree of respiratory distirbance index(RDI) and aprea index(Al) from the polysomnograph in 34 patient, mild obstructive sleep apnea patients were 5, moderate 6 and severe 16. 3. Various anatomic changes in the upper airway with denta1 orthosis were as follows : (1) More superioly positioned hyoid bone ( p<0.001) (2) Enlarged oropharyngeal (superior p<0.01, middle p<0.01. inferior p<0.01) and hypopharyngeal (P<0.05) airway space. 4. According to the results of the changes of clinical syptoms after the usage of the dental orthosis acquired from questionnaires, there was significant improvement in the frequently, the loudness and the severity of snoring, cessation of breathing and awakening from the difficulty of breathing during sleep. 5. The effectiveness and side effects of dental orthosis by questionnaires were as follows ; (1) Dental orthosis satisfied almost all the patients (68±20%). (2) Snoring was improved in all the patients (73±19%). (3) Obstructive sleep aphea was improved in all the patients (61 ± 37%) (4) Sleepiness in the daytime was significantly improved (61 ±37%). (5) The sleep quality was significantly improved (61±37%). (6) The discomfort of the dental orthosis was minor (33±18%) and no serious complications were observed. 6. The dental orthosis is an effective treatment for the symptom of snoring, and it can also effectively treat varying degrees of obstructive sleep apnea.
