Effects of Combined Exercise and Creatine Intake to Muscular Function and Pain in Back Pain Patients (복합운동과 크레아틴 복용이 요통환자의 근기능과 통증에 미치는 영향)

  • The purpose of this study was to examine the effects of combined exercise and creatine consumption to muscular function and pain in 24 middle-aged men in back pain patients. The patients were categorized into three groups; 8 patients underwent combined exercise alone, 8 patients underwent creatine consumption as well as combined exercise and the control group was 8 patients without these options. The follow-up period was 10 weeks doing aerobic exercise, resistance exercise or creatine consumption. The group that underwent creatine intake as well as combined exercise simultaneously showed significant increase on the 0°, 12°, 24°, 36°, 48°, 60° and 72° of back muscular power in the muscular function changes compared to control group, furthermore, it showed statistically significant improvement on the 24°, 36°, 48° and 60° in the combined exercise group compared to control group. However, between the group that underwent creatine consumption as well as combined exercise and the group that underwent combined exercise, the back muscular power was high in the all angles, but it was not statistically significant. The group that performed both creatine consumption and combined exercise showed pain relief relatively, comparing with either combined exercise or control group and the pain was also alleviated in the combined exercise group.

Esophageal Leiomyoma: Radiologic Findings in 12 Patients

  • Objective: The aim of our study was to describe and compare the radiologic findings of esophageal leiomyomas. Materials and Methods: The chest radiographic (n = 12), esophagographic (n = 12), CT (n = 12), and MR (n = 1) findings of surgically proven esophageal leiomyomas in 12 consecutive patients [ten men and two women aged 34 - 47 (mean, 39) years] were retrospectively reviewed. Results: The tumors, surgical specimens of which ranged from 9 to 90 mm in diameter, were located in the upper (n = 1), middle (n = 5), or lower esophagus (n = 6). In ten of the 12 patients, chest radiography revealed the tumors as mediastinal masses. Esophagography showed them as eccentric, smoothly elevated filling defects in 11 patients and a multilobulated encircling filling defect in one. In 11 of the 12 patients, enhanced CT scans revealed a smooth (n = 9) or lobulated (n = 2) tumor margin, and attenuation was homogeneously low (n = 7) or iso (n = 4). In one patient, the tumor signal seen on T2-weighted MR images was slightly high. Conclusion: Esophageal leiomyomas, located mainly in the middle or distal esophagus, are consistently shown by esophagography to be mainly eccentrically elevated filling defects and at CT, lesions showing homogeneous low or isoattenuation are demonstrated.

RADIOGRAPHIC STUDY OF SOFT TISSUE CALCIFICATION IN THE ORAL AND MAXILLOFACIAL AREA (구강악안면영역의 연조직 석회화의 방사선학적 연구)

  • The radiographic findings of soft tissue calcification in the oral and maxillofacial area was analysed using panoramic, skull P-A and intraoral radiographs in 250 patients. The obtained results were as follows ; 1. Salivary stone had the highest rate of occurrence at 46%(116 cases), followed by lymph node calcification(97 cases), phleholith(21 cases), multiple miliary osteoma(15 cases), antrolith(l case), vessel calcification(l case) and cysticercosis(1 case). 2. The prevalence of salivary stone was slightly higher in females, on the right side and in the middle-aged group and was especially higher in the submandibular gland(83%). The majority of them were round-shaped, homogeneously radiopaque and associated with sialodochitis. 3. The prevalence of lymph node calcification was higher in the female and old-aged group. Irregular shape and radiopaque bodies were seen in the cervical area, bilaterally. 4. The prevalence of phlebolith was slightly higher in the male, and the third decade group. The radiopaque bodies were 4-8 mm in diameter and had laminated appearance. 5. The prevalence of multiple miliary osteoma was higher in old-aged females. The numerous doughnut-shaped radiopaque bodies, sized 2-4 mm in diameter, were seen bilaterally in the cheek and were usually associated with the edentulous ridge.

The Perceived Social Support of the Family with Pneumoconiosis Patient (진폐가족의 사회적지지 특성)

  • Purpose: The study has planned to find out the perceived social support of the families with pneumoconiosis patients. Method: The subjects of the study were the 300 family care givers of the pneumoconiosis patients who were hospitalized in Taeback, Donghae and Jeongsun Occupational Medical Center. The Social Support Survey Instrument developed by Park(1985) was adopted. Results: The Direct Perceived Supports showed statistically differences by the age(F=1.70 p=0.01) and the state of the disease(F=3.09 p=0.027) of the patients. The Health Situation Centered Support was different by the marietal situation(F=2.29 p=0.48) of the pneumoconiosis patients. The Indirect Perceived Supports were statistically different by sex(t=3.76 p=0.043) and relation with the patient (F=2.49 p=0.048), group joining(t=3.79 p=0.042) of the family care givers. The DPSs were statistically different by family income(F=2.25 p=0.025), family authority(F=2.81 p=0.031) and health insurance status(F=2.13 p=0.026). Recommendation: It is recommended to develop an active social support program at the pneumoconiosis care centers for the middle aged female family care givers of the pneumoconiosis patients with the support of Ministry of Labor, Ministry of Health and Welfare and other NGOs of pneumoconiosis.

Qualitative Analysis of the Hwa-byung Recovery Process and Influencing Factors

  • Objectives: To describe the progress and influencing factors of Hwa-byung. Methods: Data were collected from semi-structured interviews. The progress of and recovery from Hwa-byung were examined using the collected experiences of eight middle-aged women who suffered from Hwa-byung. For analysis of the data collected, a Consensual Qualitative Research (CQR) methodology was used. Results: Of the eight participants who thought they had Hwa-byung, six were actually Hwa-byung patients and two were non-Hwa-byung patients. Four years later, four of the six Hwa-byung patients recovered, while the Hwa-byung of one patient persisted, and the remaining patient had circular Hwa-byung. Over time, the symptoms of Hwa-byung declined substantially. However, the patients recognized that their disease continuously persisted. Moreover, they thought their symptoms had worsened. Many types of changes were observed during the recovery period, including environmental, physical, cognitive, and emotional changes, as well as changes in attitudes toward others. Recovery factors included the reduction of stimuli, social support, exercise, treatment, control of rumination, and some cognitive changes. Conclusions: To recover from Hwa-byung, it is necessary for patients to experience changes in environment, cognition, and emotion, and to employ self-management in everyday life.

The Effect of 8 Weeks of Aerobic Exercise on Serum Lipids and Kidney Function in Middle-Aged T2DM Patients (8주 유산소 트레이닝이 중년의 제 2형 당뇨(T2DM)환자의 혈청지질 및 신장기능에 미치는 영향)

  • The purpose of this study was to examine the effect of 8 weeks of aerobic exercise on serum lipids and kidney function in middle-aged T2DM patients. Subjects participated in aerobic exercise training during 8 wk. They started with an exercise intensity of $HR_{max}$ 60-75%, 20~45 minutes per day, 3~5 times a week. The results were compared to the baseline after 8 weeks. After the 8-week aerobic exercise routine, the body fat percentage and fasting glucose were significantly decreased, and the total cholesterol (TC) andtriglycerides (TG) were significantly decreased. Additionally, kidney function (Blood Urea Nitrogen (BUN], uric acids, and creatinine) was not significantly changed. In conclusion, a regular 8-week aerobic exercise training routine may improve serum lipids; however, kidney function (BUN, uric acids, and creatinine) did not change in middle-age type 2 diabetes mellitus (T2DM) and T2DM in diabetic nephropathy patients.

Case Report of Three Cases of Obstructive Sleep Apnea Improved with Oriental Medicine Treatment (한방치료를 통해 호전된 폐쇄성수면무호흡증 환자 3례에 대한 임상 보고)

  • Objectives: Obstructive sleep apnea (OSA) is prevalent disorder, particularly among the middle-aged. In this study, we report a new approach to improve OSA using oriental medicine and acupuncture. Methods: Three men diagnosed with OSA were enrolled in this study. They took oriental medicine 3 times a day and were given acupuncture twice a week for about 4 months. Results: We analyzed apnea-hypopnea index (AHI), oxygen desaturation index (ODI), average saturation (AS) and lowest saturation (LS) before and after treatment. AHI highly decreased in all patients and ODI also decreased in two of the three patients. Conclusions: These results indicate that oriental medicine and acupuncture therapy was effective in the improvement of OSA. Further study in a larger group with severity grades is recommended.

Cardiac Resynchronization Therapy Defibrillator Treatment in a Child with Heart Failure and Ventricular Arrhythmia

  • Cardiac resynchronization therapy (CRT) is a new treatment for refractory heart failure. However, most patients with heart failure treated with CRT are adults, middle-aged or older with idiopathic or ischemic dilated cardiomyopathy. We treated a 12-year-old boy, who was transferred after cardiac arrest, with dilated cardiomyopathy, left bundle-branch block, and ventricular tachycardia. We performed cardiac resynchronization therapy with a defibrillator (CRT-D). After CRT-D, left ventricular ejection fraction improved from 22% to 4 4% assessed by echocardiogram 1 year postoperatively. On electrocardiogram, QRS duration was shortened from 206 to 144 ms. The patient's clinical symptoms also improved. For pediatric patients with refractory heart failure and ventricular arrhythmia, CRT-D could be indicated as an effective therapeutic option.

  • Aspergilloma of the maxillary sinus is considered rare in immunocompetent patients, but a considerable increase has recently been seen in the incidence of reported cases. Dental procedures involving the antral region are thought to predispose individuals to this form of aspergillosis. Because aspergilloma shares similar clinical features with other sinus pathologies, its diagnosis may be delayed. Thus, an early diagnosis confirmed by a histopathological examination plays a crucial role in the adequate management of aspergilloma. This article provides a concise review of the reported cases of aspergilloma associated with dental procedures and reports 2 new cases of aspergilloma in middle-aged female patients, with a presentation of their cone-beam computed tomographic findings.

Paraplegia due to Acute Aortic Coarctation and Occlusion

  • Coarctation and occlusion of the aorta is a rare condition that typically presents with hypertension or cardiac failure. However, neuropathy or myelopathy may be the presenting features of the condition when an intraspinal subarachnoid hemorrhage has compressed the spinal cord causing ischemia. We report two cases of middle-aged males who developed acute non-traumatic paraplegia. Undiagnosed congenital abnormalities, such as aortic coarctation and occlusion, should be considered for patients presenting with nontraumatic paraplegia in the absence of other identifiable causes. Our cases suggest that spinal cord ischemia resulting from acute spinal subarachnoid hemorrhage and can cause paraplegia, and that clinicians must carefully examine patients presenting with nontraumatic paraplegia because misdiagnosis can delay initiation of the appropriate treatment.
