Transformational Leadership and Depressive Symptoms in Germany: Validation of a Short Transformational Leadership Scale

  • Objectives: The objective of the present study was to validate a shortened transformational leadership (TL) scale (12 items) comprising core TL behaviour and to test the associations of this shortened TL scale with depressive symptoms. Methods: The study used cross-sectional data from 1632 employees of the overall workforce of a middle-sized German company (51.6% men; mean age, 41.35 years; standard deviation, 9.4 years). TL was assessed with the German version of the Transformational Leadership Inventory and depressive symptoms with the Hospital Anxiety and Depression Scale (HADS). The structural validity of the core TL scale was assessed with confirmatory factor analysis. Associations with depressive symptoms were estimated with structural equation modelling and adjusted logistic regression. Results: Confirmatory factor analysis and structural equation modelling showed better model fit for the core TL than for the full TL score. Logistic regression revealed 3.61-fold (95% confidence interval [CI], 2.20 to 5.93: women) to 4.46-fold (95% CI, 2.86 to 6.95: men) increased odds of reporting depressive symptoms (HADS score >8) for those in the lowest tertile of reported core TL. Conclusions: The shortened core TL seems to be a valid instrument for research and training purposes in the context of TL and depressive symptoms in employees. Of particular note, men reporting poor TL were more likely to report depressive symptoms.

Factors Affecting the Preparation of Advance Directives of Older People (노인의 사전연명의료의향서 작성에 영향을 미치는 요인)

  • Purpose : This study intended to provide essential data for developing measures for the stable settlement and expansion of the life-sustaining treatment decision system by identifying the effects on preparing advanced directives. Methods : The effects on preparing advance directives of older people were identified based on Andersen's behavioral model, using the 2020 Korean national survey on elderly. Data were statistically analyzed with SPSS Statistics ver 25.0 and the significance level (α) was set to .05. Results : For factors that influence the preparation of advance directives, the predisposing factor was .769 times less for women than men (p=.026). By age, it was 1.410 times higher (p=.006) for people in their 70s compared to people in their 60s and 1.675 times higher (p=.003) for those in their 80s. By the level of education, it was 1.617 times higher (p=.026) for those who have elementary school education compared to those who have no education, 1.596 times higher (p=.048) for those who have a middle school education, 2.313 times higher (p<.001) for those who have a high school education, and 3.827 times higher (p<.001) for those who have a college education. By religion, it was 1.328 times higher (p=.008) for those who have faith compared to those who do not. For possible factors, it was 2.325 times higher (p=.003) for those who spend 100,000 won or more on healthcare (monthly average) compared to those who do not spend. For necessary factors, it was 1.439 times higher (p=.041) for those with the chronic disease compared to those without. Conclusion : It is deemed a measure that can increase the preparation of advance directives, considering the characteristics of each cause, for the stable settlement of the life-sustaining treatment decision system.

Changes in Depression and Stress of the Middle-Aged and Elderly through Participation in a Forest Therapy Program for Dementia Prevention

  • Various health and social issues related to the elderly are emerging in line with the rapid aging of the population. In particular, dementia currently has a prevalence of about 10 percent of the elderly population in South Korea, which increases financial and social burdens to not only individual patients but also their caring family. To assess the effects of participating in the forest therapy programs for dementia prevention, this study recruited participants aged 50 and above and tested their depression (Korean form of Geriatric Depression Scale; KGDS) and stress response (Modified form of Stress Response Inventory; SRI-MF), which are emotional aspects of dementia. As a result, KGDS showed a significant decrease of 3.2 points from 8.4 to 5.2 points, and SRI-MF showed a significant decrease of 7.2 points from 40 to 32.8 points, indicating a statistically significant improvement in both. In addition, participants with minor depression and high level of stress in the pretest showed statistically significant improvements in the SRI-MF for men, and the KGDS and SRI-MF for women. Furthermore, there were statistically significant improvements in KGDS for participants in their 60s and in SRI-MF for those in their 70s in terms of age, and in both KGDS and SRI-MF for participants with chronic diseases and in KGDS for participants without chronic diseases. This study confirmed the effects of forest therapy on the prevention of the emotional aspects of dementia and laid the groundwork for increasing the applicability of forest therapy by obtaining a place for dementia prevention as a field of forest therapy.

Life Stage Preparation Program in an Aged Society (고령사회를 대비한 노년준비교육 프로그램에 관한 연구)

  • Korea is expected to become an "aged society" in approximately fifteen years. In order to prepare Korean society for this "aged society", the government and social organizaitons need to offer a life stage preparation manual for the young, middle-aged, and aging populations, as well as providing support structures to guide them through this transition. Researchers have developed a Life Stage Preparation Program, based on surveys and experiments conducted to assess needs and appraise effectiveness. To accomplish the goal of this research, the team has been composed of specialists from the fields of social welfare, family studies, occupational studies, and gerontology. The Life Stage Preparation Program consists of ten areas: understanding ageing issues, sustaining and improving health, psychological issues, continuing education, family relations, social activities, financial preparation, occupations, shelter, and mortality. The effectiveness of the program surveyed on age group of 20s and 60s resulted in nine areas. The Life Stage Preparation Program needs to be operated as a life long education program.

Effect of Demographic Factors, Radiation Knowledge Level, Radiation Awareness on Radiation Benefit by Hierarchical Regression Analysis Model (위계적 회귀분석 모형에 의한 인구학적 요인, 방사선 지식수준, 방사선 인식도가 방사선 이익성에 미치는 영향)

  • The purpose of this study was to analyze the factors that demographic factors, radiation knowledge level, and radiation awareness could be affecting the benefits of radiation. From July 2022 to July 2023, after receiving consent to participate by using the link of Naver through Social Network Service (SNS) for the general public, 312 people were surveyed by self-registration method without collecting personal information. The questionnaire consisted of a total of 25 questions following demographic factors (5 questions including age group by life cycle, sex, monthly household income, residence), radiation knowledge level (8 questions including basic physical, biological effects, radiation protection technology), radiation awareness (12 questions including risk, management, benefit). Independent sample T-test and ANOVA tests were performed for significant differences in the average radiation awareness between variables, and hierarchical regression was performed to identify influencing factors on radiation benefits. As a result, the benefit of radiation was significantly high among the radiation awareness, but the awareness of the danger of radiation was insufficient to the level of recognizing it as safe. Men had significantly higher awareness of radiation management and benefits than women, and the awareness of radiation management was significantly higher in the middle class with a monthly household income of 4.31 million won or more. The higher the knowledge level of radiation, the higher the awareness of the benefits of radiation. The factors that had a positive effect on radiation benefits were the high level of radiation knowledge and awareness of radiation management.

Estimated pulse wave velocity as a forefront indicator of developing metabolic syndrome in Korean adults

  • Background/Aims: The predictive value of the estimated pulse wave velocity (ePWV) for the development of metabolic syndrome has not yet been extensively explored. This study aimed to fill this gap by evaluating ePWV as a potential predictor of metabolic syndrome development in middle-aged Korean adults. Methods: Using prospective data obtained from the Ansan-Ansung cohort database, participants without metabolic syndrome at baseline were studied. ePWV was calculated using specific equations based on age and blood pressure. The primary outcome was the incidence of metabolic syndrome during a median follow-up period of 187 months. Results: Among the 6,186 participants, 2,726 (44.1%) developed metabolic syndrome during the follow-up period. ePWV values were categorized into tertiles to assess their predictive value for the development of metabolic syndrome. An ePWV cut-off of 7.407 m/s was identified as a predictor of metabolic syndrome development, with a sensitivity of 0.743 and a specificity of 0.464. Participants exceeding this cut-off, especially those in the third tertile (8.77-14.63 m/s), had a notably higher risk of developing metabolic syndrome. Specifically, the third tertile exhibited a 52.8% cumulative incidence compared with 30.8% in the first tertile. After adjustments, those in the third tertile faced a 1.530-fold increased risk of metabolic syndrome (95% confidence interval, 1.330-1.761). Conclusions: ePWV is a significant predictor of the development of metabolic syndrome. This finding underscores the potential of ePWV as a cardiometabolic risk assessment tool and can thus provide useful information for primary prevention strategies.

Relation between Health Status and Intake of Soy Isoflavone among Adult Women in Seoul (서울 거주 성인 여성의 대두 이소플라본 섭취와 건강과의 관련성)

  • This study was conducted to survey isoflavone intake among adult women in menopause with diseases such as metabolic syndrome and osteoporosis and to analyze the relationship between each of these chronic diseases followed by isoflavone intake and the related health risk index. The average age of the subjects was 49.97 years old, while that of the pre-menopausal subjects was 45.14 years, and the post-menopausal subjects was 55.99 years. The average body mass index (BMI), waist-hip circumference, body fat percentage, blood pressure, blood sugar and blood lipid content of the post-menopausal subjects were higher in significant difference than those of the pre-menopausal subjects. The bone density of the hip and spine in post-menopausal subjects was lower in significant difference than that of the pre-menopausal subjects. After menopause, the subjects had a lower ratio of individuals at risk of anemia when compared with the subjects before menopause, but had higher health risk ratio related to each type of chronic disease, including obesity, hypertension, high cholesterol and osteoporosis than the subjects before menopause. The intake frequency of each soybean food was similar among subjects before/after menopause. The most common soybean based foods consumed by the subjects were soybean, soybean curd and soybean paste. The average daily intake level of isoflavone among subjects before menopause was 25.48 mg, while that of subjects after menopause was 32.25 mg. Evaluation of the distribution of the isoflavone level revealed that the pre-menopausal subjects consumed 3.29~78.36 mg and the post-menopausal subjects consumed 3.18~116.59 mg. The intake level by each individual varied greatly. The pre-menopausal subjects had a low BMI index and systolic blood pressure as much as their isoflavone intake level was high. Additionally, the post-menopausal subjects had a low menarche age and high menopause age when their isoflavone intake level was high, the BMI index and waist-hip circumference ratio was highest among individuals with lowest isoflavone intake level. This study showed that there was a possible relationship between soybean isoflavone intake and health problems such as obesity, high cholesterol, and osteoporosis in women after menopause with diseases such as metabolic syndrome and osteoporosis, even if this relationship was not great.

Middle-aged Women's Health Behavior and Its related Factors in Rural Area (농촌 중년여성의 건강행위와 관련요인)

  • This study was conducted to identify the health behavior of middle-aged rural women and the factors that have an effect on them. For the purpose of the study, examinations were made from March 01, 2000 to March 31, 2000 with 468 women aged 40 to 64 out of 2,263 people whom four Primary Health Posts located in Yechon County, Kyongsangbuk-do Province, are in charge of. The results are summarized as follows. 17.5% of the subjects responded that the extent of their own interest in health were high. For the subjects having a chronic disease, a nuclear family, or an open family atmosphere, the extent appeared to be relatively higher, 15.4% responded that the extent of family's interest in their health was high. It was significantly high if the extent of education was high or if the family atmosphere was open. The subjects' average score of self-efficacy was 49.9 out of 68. The score significantly varied depending on religion, education, living together with a spouse or not, and the extent of the subjects' interest in health. The family pattern, family atmosphere, family's interest in the subjects' health were the variables that significantly influenced the self-efficacy. The average score of family function was 5.51 out of 10. The score significantly varies depending on age, education, occupation, financial status, the extent of the subjects' own interest in health, family atmosphere and family's interest in the subjects' health. In the practice of health behavior, the nonsmoking rate was 89.5%, the nondrinking rate 63.0%, the rate of exercising practice 6.6%, the rate of normal sleeping 75.6%, the rate of eating breakfast 91.7%, the rate of not eating between meals 18.2%, and the standard BMI 69.2%. In the frequency of health behavior, the subjects with the Breslow Index of 0-3, 4-5 and 6-7 accounted for 4.5%, 53.2%, and 42.3%, respectively. The average score of health behavior was 5.20 out of 7, in which significant variables were living together with a spouse or not, financial status, absence or presence of a chronic disease, and family atmosphere. In the multiple regression analysis with health behavior as a dependent variable, it was shown that living together with a spouse or not, financial status, and family atmosphere were the significantly substantial variables. The subjects were found to do health behavior well if they had not a spouse, a good financial status, or an open family atmosphere. They were also found to do health behavior well if the extent of self-efficacy was high or if the extent of family function was low, but these were not the significant variables. It is needed to develop a standard measuring tool fit for our environment and perform more studies in the future because the measuring tool used in this study was a tool developed in a foreign county. In promoting community health projects, it is required not to provide all community people with a uniform health program but to identify the health behavior of individuals and other variables such as living together with a spouse or not, financial status and family atmosphere before arranging for a proper health program.

Cadever dissection and Dynamic CT for Vascular Anatomy of Rectus Abdominis Muscle (배곧은근의 혈관 주행에 관한 시신해부 및 컴퓨터단층촬영)

  • Purpose: Pedicled transverse rectus abdominis myocutaneous(TRAM) flap has been a gold standard for breast reconstruction and one of surgical techniques preferred by many surgeons. The authors examined the course of deep epigastric artery focusing on distance from margins of rectus abdominis to pedicle and location of choke vessels to get minimal muscles during pedicled TRAM flap operation. Methods: Eleven rectus abdominis muscle from nine cadavers were used in this study. Rectus abdominis was separated from the cadavers, deep inferior and superior epigastric artery were isolated and then 8 anatomical landmarks in medial and lateral margins of rectus abdominis were designated. Distance to a pedicle meeting first horizontally was measured and vertical location from umbilicus to choke vessel was determined. In addition, 32 rectus abdominis images of 16 women(average age: 37.2 years old) from 64 channel abdomen dynamic computerized tomography were also examined with the same anatomical landmarks with those of cadavers. Results: Average distance from four landmarks on lateral margin of rectus abdominis to pedicle was 1.9 - 3.4cm and 1.8 - 3.8 cm on medial margin. Choke vessel was located between middle and inferior tendinous intersection in all cases and average distance between two tendinous intersection was 6.7 - 7.0 cm on medial margin and 6.2 cm on lateral margin. Location of inferior tendinous intersection was on umbilicus or superior of it in all cases and its average distance from umbilicus was 1.8 - 5.6 cm on medial margin and 2.7 - 6.2 cm on lateral margin. Conclusion: Distance from medial and lateral margins of rectus abdominis muscle to pedicle was the shortest in inferior tendinous intersection and that was averagely 1.8 cm on medial margin and 1.9 cm in average on lateral margin. All choke vessels were located between middle and inferior tendinous intersection.

The Effects of 12-Week of Proprioceptive Exercise Programs on Chronic Low Back Pain and Sensorimotor Control in Middle-Aged Females in Rural Areasa (12주간의 고유감각자극운동이 농촌지역 중년여성의 만성요통과 감각운동 조절능력에 미치는 효과)

  • This study applied a props-based exercise program for 12 weeks to women in their mid-50s in rural areas who complained of back pain, and applied VAS (visual analogue scale), static balance ability left and right, front and back, and sensory movement. Ability Physical stability The left and right, before and after physical changes were investigated. The study subjects were randomly assigned to an experimental group (EG) of 24 people and a control group (CG) of 20 people. For the homogeneity test between the experimental group and the control group, an independent t-test was conducted to analyze the significant difference in average. When homogeneity was secured, two-way ANOVA repeated measures ANOVA was performed. As a result, there was an interaction effect between the group and the 12 weeks period. However, the VAS between periods within the group, static balance ability left and right, before and after, and sensorimotor ability and body stability left and right, before and after showed improved results with significant average differences. After 12 weeks between groups, the experimental group showed significant improvement effects in all variables.
