Relationship between Center of Pressure and Local Stability of the Lower Joints during Walking in the Elderly Women

  • Objective: The purpose of this study was to determine the relationship between center of pressure (CoP) and local stability of the lower joints, which was calculated based on approximate entropy (ApEn) during walking in elderly women. Method: Eighteen elderly women were recruited (age: $66.4{\pm}1.2yrs$; mass: $55.4{\pm}8.3kg$; height: $1.56{\pm}0.04m$) for this study. Before collecting data, reflective marker triads composed of 3 non-collinear spheres were attached to the lateral surface of the thigh and shank near the mid-segment to measure motion of the thigh and shank segments. To measure foot motion, reflective markers were placed on the shoe at the heel, head of the fifth metatarsal, and lateral malleolus, and were also placed on the right anterior-superior iliac spine, left anterior-superior iliac spine, and sacrum to observe pelvic motion. During treadmill walking, kinematic data were recorded using 6 infrared cameras (Oqus 300, Qualisys, Sweden) with a 100 Hz sampling frequency and kinetic data were collected from a treadmill (Instrumented Treadmill, Bertec, USA) for 20 strides. From kinematic data, 3D angles of the lower extremity's joint were calculated using Cardan technique and then ApEn were computed for their angles to evaluate local stability. Range of CoP was determined from the kinetic data. Pearson product-moment and Spearman rank correlation coefficient were applied to find relationship between CoP and ApEn. The level of significance was determined at p<.05. Results: There was a negative linear correlation between CoP and ApEn of hip joint adduction-abduction motion (p<.05), but ApEn of other joint motion did not affect the CoP. Conclusion: It was conjectured that ApEn, local stability index, for adduction/abduction of the hip joint during walking could be useful as a fall predictor.

  • 제주시 봉개동의 고냉이술 일대의 지질은 오래된 것으로부터 영평동현무암, 신안동현무암, 봉개동현무암 및 분석층으로 구성된다. 영평동현무암과 신안동현무암은 고냉이술 오름의 남쪽 중산간 지역에 위치하고, 고냉이술 오름과 그 북부 저지대 일원에는 봉개동현무암이 분포한다. 봉개동현무암은 아아 용암으로 주로 구성되며, 용암 켜 사이에 두터운 클링커를 협재하고 있다. 고냉이술 오름은 화산지형학적으로 하나의 독립된 소화산으로 만들어져 있으며, 고냉이술 오름의 산정부에는 낮은 외륜산릉으로 둘러싸여 있는 넓은 분화구를 가지고, 그 내부에는 분지형 함물지인 여러 개의 피트 분화구(pit carter)를 가지는 용암순상화산이다. 고냉이술 오름의 저면 고도는 295 m에서 300 m (평균 297.5 m)이며, 최고봉의 고도는 304.5 m, 저면의 지름은 평균 597.5 m, 비고는 7.9 m로 사면경사각은 $1.5^{\circ}$이다.

Effects of a Real-time Plantar Pressure Feedback during Gait Training on the Weight Distribution of the Paralyzed Side and Gait Function in Stroke Patients

  • PURPOSE: This study was conducted to investigate the effect of a real-time pressure feedback provided during gait training on the weight weight distribution of the inner part of mid-foot in paralyzed side and gait function in stroke patients. METHODS: A total of 24 patients with hemiplegic stroke in a rehabilitation hospital were randomly assigned to the experimental and control group. All participants (n = 24) performed 15 min of comprehensive rehabilitation therapy 5 times a week for a period of 4 weeks. Additionally, the experimental group and control group underwent gait training with a real time feedback and general gait training, respectively, for 15 min five times a week for 4 weeks. Weight distribution and gait function were measured before and after the 4-week training. RESULTS: Significant increases in the weight distribution (WD), stance time (ST) and step length (SL) of the paralyzed side, and a significant decrease in the 10 m walking test (10 MWT) observed after training in the two groups (p < .05). The experimental group showed larger changes in the all variables than the control group (WD, +10.5 kg vs. +8.8 kg, p < .05; ST, 12.8 s vs. 4.9 s, p < .05; SL, 4.9 cm vs. 1.7 cm, p < .05; 10 MWT, -3.5 s vs. -1.0 s, p < .05, respectively). CONCLUSION: Gait training with a real-time feedback might be effective in improving the normalization of weight bearing of the paralyzed lower extremity and gait function of stroke patients, and be considered to be a more effective gait training for improving the abilities than the general gait training.

  • 우리나라 낙동강 하류에 서식하는 뻘조개, Anodonta (Sinanodonta) woodiana의 생식소발달 과정 및 생식주기 그리고 임란기 등을 조사하였다. 1. 생식소는 내장낭의 간중사선 하방으로부터 족부의 외벽근층내에 있는 섬유성 망상결제조직까지 분포되어 있다. 난소는 많은 난소소낭으로 구성되어 있는데, 이들 소낭의 내공상피가 생식상피 기능을 하고 있다. 2. 초기난소 몇 정소의 생식상피상에는 호산성 과립세포와 부분화 간충조직들이 풍부하게 나타나고 있으며, 이들은 생식소 발달과 함께 그 량이 감소하고 있다. 3. 산란은 고수온기인 8월과 9월을 제외하고 년중 일어나고 있는데 1월에서 3월과, 6월에서 7월의 2회에 걸쳐 성기를 나타내고 있다. 4. 보육낭내에 trochophore 유생을 임란한 개체는 8월과 9월을 제외하고 년중 나타났으며 glochidia 유생을 임란한 개체는 9월을 제외하고 연중 나타났는데, 5월이 $72.7\%$ 년중 가장 높은 임란율을 나타내고 있다.

  • 개량조개, Mactra chinesis의 자원관리 및 증양식을 위한 기초생물학적 연구로서, 서해안 옥구군 비웅도 근해에서 1984년 1월부터 12월까지 월별로 채집한 재료를 대상으로 생식소발달, 생식년주기, 그리고 군성숙도를 조직학적 방법에 의해 조사하였다. 본 종은 자웅이체형 2매패로서, 생식소는 내장낭의 간중장선 하방으로부터 족부의 섬유성망상결체조직까지 불규칙하게 분포되어 있었다. 완숙란의 난경의 크기는 $50\~60{\mu}m$ 이었으며 젤라틴상의 피막으로 둘러 싸여 있었다. 산란은 수온이 $18.5\~27.0^{\circ}C$인 5월부터 9월에 걸쳐 일어나며 주산란기는 6월과 7월이었다. 개량조개의 생식년주기는 분열증식기, 성장기, 방출기, 퇴화 및 휴지기의 연속적인 5단계로 나눌 수 있었다. 비남도지수의 월별변화는 생식년주기와 밀접하게 상호관련되어 있었다. 암${\cdot}$수 개체들의 군성숙도는 각장 $3.6\~3.9cm$인 개체들은 $50\% $이상이었고, 각장이 5.0cm 이상인 개체들은 전 개체가 재생산에 참가하였다.

달리기 속도의 증가에 따른 운동화 중저의 경도와 신발바닥의 두께가 신발의 볼 굴곡각도에 미치는 영향 (The Influence of Midsole Hardness and Sole Thickness of Sport Shoes on Ball Flex Angle with the Increment of Running Velocity)

  • The purposes of this study were to determine the influence of midsole hardness and sole thickness of sports shoes on ball flex angle and position with increment of running velocity. The subjects employed for this study were 10 college students who did not have lower extremity injuries for the last one year and whose running pattern was rearfoot striker of normal foot. The shoes used in this study had 3 different midsole hardness of shore A 40, shore A 50, shore A 60 and 3 different sole thickness of 17cm, 19cm, 21cm. The subjects were asked to run at 3 different speed of 2.0m/sec, 3.5m/sec, 5.0m/sec and their motions were videotaped with 4 S-VHS video cameras and 2 high speed video cameras and simultaneously measured with a force platform. The following results were obtained after analysing and comparing the variables. Minimum angle of each ball flex position were increased with the increment of running velocity and shoe sole thickness(P<0.05), but mid-sole hardness did not affect minimum ball flex angle. The position which minimum angle was shown as smallest was 'D'. Midsole hardness and sole thickness did not affect time to each ball flex minimum angle, total angular displacement of ball flex angle, and total angular displacement of torsion angle(P<0.05). The position which minimum angle was appeared to be earliest was similar at walking velocity, and E and F of midfoot region at running velocity. Total angular displacement of ball flex position tended to increase as shifted to heel. It was found that running velocity had effects on ball flex angle variables, but shoe sole thickness partially affected. It would be considered that running velocity made differences between analysis variables at walking and running when designing shoes. Also, it was regarded that shoes would be developed at separated region, because ball flex angle and position was shown to be different at toe and heel region. It is necessary that midsole hardness and thickness required to functional shoes be analyzed in the further study.

저신장과 비만도의 상관성 분석 - 저신장을 주소로 내원한 환아 236명을 대상으로 (Correlation between Short Stature and Obese Degree - Intended for the Case of 236 Patient in the Name of Short Stature)

  • Objectives The purpose of this study was to investigate correlation between short stature and obese degree. And we also wanted to know the characteristics of patients visited clinic in the name of 'short stature'. Methods Height, body weight, BMI, fat mass, lean body mass, percent body fat were taken intended for 236 patients in the name of short stature. To all the patient questionnaire for growth clinic was drawn up, and ultrasound scan was taken through calcaneous of the right foot. The patients were classified to three groups - short, average, tall group - according to relative position of mid-parental height. It was analysed that the differences between groups in obese degree. Also was investigated correlation between position of short stature and obese degree, and between obese degree and bony maturity. Results & Conclusion 1. The average ages of patients in the name of 'short stature' were $12.69{\pm}3.93$ years old in boys, $10.66{\pm}3.67$ years old in girls. And it seemed to be just before second rapid maturing period. 2. The average BMI were $20.58{\pm}4.07kg/m^2$ in boys, $18.65{\pm}2.85kg/m^2$ in girls, and average percent body fat were $21.99{\pm}7.35%$ in boys, $26.01{\pm}6.35%$ in girls. 3. The numbers of obese children were 34(31.2%) in boys, 19(14.9%) in girls on the basis of BMI. And the numbers were 39(35.8%) in boys, 53(41.7%) in girls on the basis of percent body fat. There was a big difference in case of girls. 4. The numbers of AG(average group) were 48(44.0%) in boys, 60(47.2%) in girls, the numbers of SG(short group) were 35(32.1%) in boys, 31(24.4%) in girls, and numbers of TG(tall group) were 26(23.9%) in boys, 35(27.6%) in girls. 5. There were no significant differences among the groups in BMI, lean body mass, fat mass, percent body fat. Only significant difference in DI(disease index). 6. There were no significant correlation between PH(percent height) and BMI, leanbody mass, fat mass, percent body fat. Only significant correlation in DI(disease index). 7. There were no significant differences between DA(difference between bone age and chronorogical age) and BMI, leanbody mass, percent body fat. Only significant correlation in fat mass.

The Effects of Pilates Exercise on Static and Dynamic Balance in the Elderly

  • Objective: Pilates is a low/mid-intensity exercise that can be easily performed by elderly individuals as it is an individual body-oriented exercise. It is also a cardio workout that can be performed anywhere to develop strength and flexibility. Therefore, we investigated the effects of 8 week Pilates program on the balancing ability of elderly individuals. Method: The research participants were selected from elderly residents in B city. Ten individuals voluntarily signed an agreement to undergo free measurements as well as to participate in the workout program. (Height: $157.1{\pm}11.9cm$, Weight: $61.7{\pm}8.0kg$). The Pilates exercise was performed 60 minutes a day, three times a week for a total of eight weeks. The measurement variables used to test balance were the vestibular test, 5 m habitual and maximum walk test and 3 m tandem walk test. A series of paired t-test were conducted using IBM SPSS Statistics 23.0 to analyze all the research data collected in order to determine the balance ability of the participants before and after the Pilates program. Additionally, the statistically significant level for all analysis was set to ${\alpha}=.05$. Results: In the vestibular test, some meaningful changes were observed in the length envelope area (ENV) while standing on one foot, but there were no significant differences in the ENV, rectangle(REC), root mean square, and total length. Results also revealed that statistically significant differences existed in the 5 m habitual and maximum walk test, as well as the 3 m tandem walk test. Conclusion: To summarize the findings, the 8 week Pilates program employed in this study significantly improved the dynamic balance of the elderly participants. Thus, elderly individuals that frequent perform Pilates are expected to enjoy positive benefits such as increased balance and fewer falling accidents.

가상현실과 힘판을 통한 시각적 되먹임 트레드밀 보행훈련이 뇌졸중 환자의 보행능력과 삶의 질에 미치는 영향 (Effects of Visual Feedback Treadmill Gait Training Program Combined with Virtual Reality Technology and a Force Plate Measurement System on Gait Ability and Quality of Life in Stroke Patients)

  • 본 연구는 뇌졸중 환자에게 엔터테인먼트요소가 가미된 가상현실과 시각적 되먹임 보행 훈련 프로그램을 병행하여 보행능력과 삶의 질을 향상시키고자 하였다. 이에 휘돌림 보행을 하는 뇌졸중 환자 10명을 선정하여, 가상현실과 힘판을 통한 시각적 되먹임 트레드밀보행훈련을 일일 30분씩, 주5회, 5주간 총 25회 실시하였다. 이러한 보행훈련의 효과를 알아보기 위해 관절가동범위검사, 근활성도 검사, 버그 균형 척도(BBS), 보행분석, 삶의 질(SS-QOL) 평가를 중재 전·후 시행하였다. 본 연구결과, 보행 흔듦기 시기에 마비측 관절가동범위와 근활성도, 동적균형능력, 보행능력, 삶의 질이 중재 후 통계학적으로 유의하게 차이가 있었다(p<0.05). 본 연구결과를 통해 본 연구의 보행훈련이 휘돌림 보행을 하는 뇌졸중 환자의 발처짐, 근활성도, 동적 균형 및 보행능력을 향상시키고, 이로 인한 삶의 질도 개선하는 것을 알 수 있었다. 따라서 휘돌림 보행을 하는 뇌졸중 환자의 보행능력 향상과 삶의 질 개선을 통한 라이프케어증진을 위해 가상현실과 힘판을 통한 시각적 되먹임 트레드밀보행훈련 프로그램 적용을 권장한다.

빌라 데스테의 정원건축적 특성에 관한 연구 (A Study on the Characteristics of Garden Architecture in Italian Renaissance Villa d'Este)

  • 본 연구는 이탈리아 르네상스 정원의 조영배경, 입지 및 공간구성특성 구명에 있어 주요한 역할을 점유하고 있는 빌라 데스테의 정원건축적 특성을 구명하는데 목적을 두고 수행되었다. 연구를 위한 조사는 문헌조사와 현지조사로 구분하여 이루어졌으며, 자세한 내용은 다음과 같다. 첫째, 조영의 경우 16세기 중후반 설계가 피로 리고리오(Pirro Ligorio)가 이폴리토 데스테(Ippolito d'Este) 추기경의 지시 하에 빌라 조영이 시작되었으며, 이후 데스테 가문 소유로 계승되다 재정난으로 1850년 추기경 구스타프 폰 호엔로헤(Gustav von Hohenlohe)에게 소유권이 이전되어 훼손된 정원 복구에 힘썼고 세계 1차 대전을 겪으며 이탈리아 정부 소속으로 소유권이 넘겨진 빌라 데스테는 전쟁으로 인해 무너진 빌라의 대규모 복구 작업이 시작되고 세계 2차 대전을 겪었다. 또 한번 피해를 입은 빌라 데스테는 복구 과정까지 거친 후 현재는 많은 방문객을 받으며 운영되어지고 있다. 둘째, 입지의 경우 도심에 위치하는 우르바나 성격을 가지며, 산기슭에 위치한 계곡형의 지형조건을 가지고 있다.셋째, 공간구성의 경우 16세기 초기 코르틸레 델 벨베데레와 빌라 마다마의 노단의 설치, 기하학적 대칭 등의 조영원칙들에 영향을 받아 축선과 직교하며 정원의 각 부분이 전개된다. 주요 건물은 구릉에 위치하여 한눈에 정원과 주변 경관을 조망할 수 있도록 하였다. 넷째, 구성요소 중 건조물의 경우 계단, 로지아, 중정, 석굴 등이 있었고 구성요소 중 빌라 데스테의 가장 두드러지는 특징인 수경관에는 분수, 캐스케이드, 벽천, 연못 등이, 식생으로는 월계수, 가시나무, 감탕나무 등이 있다.
