Genetic variance of Tuchomonns uaginclis isolates by Southern hybridization (Southern hybridization에 의한 질편모충의 유전학적 다양성)

  • In the present study, genomic DNAs were purified from Korean isolates (KT8, KT6, KT-Kim and KT-Lee) and foreign strains (CDC85, IR78 and NYH 286) of 1Trichomonas voslnalis, and hybridized with a probe based on the repetitive sequence cloned from T. uqfinolis to observe the genetic differences. By Southern hybridization, all isolates of T. uoSinoLis except the NYH386 strain had 11 bands. Therefore all isolates examined were distinguishable into 3 groups according to their banding patterns; i) KT8, KT6 and KT-Kim isolates had 11 identical bands such as 1 kb, 1.2 kb, 1.6 kb, 1.9 kb, 2.3 kb. 27 kb, 3.2 kb, 2.4 kb, 3.8 kb, 4.9 kb and 6.0 kb, ii) The metronidazole-resistant IR78 strain had the some bands as KT-Lee isolate at bands of 1 kb, 1.2 kb, 1.6 kb. 1.8 kb, 2.1 kb, 2.5 kb, 2.7 kb, 2.9 kb, 3.4 kb, 5.0 kb and 6.0 kb, Bands of CDC85, metronidazole-resistant strain, were similar to those of IR78 and KT-Lee, except that 3.2 kb replaced 2.9 kb. iii) NYH286 particularly had 12 bands and bun patterns were similar to IR78 with a few exceptions as follows; i) 6.2 kb in place of 6.0 kb, ii) 2.0 kb and 2.2 kb instead of 2.1 kb. Through the results obtained, genetic variance of T. uoginnlis isolates was demonstrated by Southern hybridization.

Antimicrobial Effects of Lonicera japonica against Gram Positive and Gram Negative Anaerobic Bacteria

  • It has been shown that the butanol extract of Lonicera japonica has antimicrobial and other potentially useful biological activities. The purpose of this study was to determine the in vitro activity of Lonicera japonica compared to other antimicrobial agents against anaerobic bacteria. Specifically, the in vitro activity of the butanol extract was investigated against 104 clinical isolates of anaerobic bacteria using an agar dilution method and the results were compared to erythromycin, cefoxitin, imipenem, clindamycin, and metronidazole. It was found that Lonicera japonica and imipenem were the most active antimicrobial agents tested.

A case of amebiasis in the newborn period (신생아에서 나타난 아메바증 1례)

  • Entamoeba histolytica (E. histolytica) is a worldwide protozoan parasite causing a wide rage of intestinal disease in human, including asymptomatic cyst carriage, acute diarrhea, fulminant dysentery, and chronic nondysenteric colitis. Extraintestinal amebiasis such as hepatic abscess also occurs. Spread of the infection occurs due to consumption of food and water that is contaminated with the cyst. Amebiasis is rarely reported in the newborn period. We report a case of a 6-day-old female newborn with watery diarrhea that progressed to bloody mucoid diarrhea with persistent fever. The fresh stool microscopic examination disclosed E. histolytica trophozoites. She was born of spontaneous vaginal delivery at 41 weeks of gestation after premature rupture of membrane. She was given breast-feeding after birth, then entered postpartum care center. Her mother's stool microscopic examination showed negative result. Infection might have occurred from giving sweetened water following birth or contamination of the baby's bottles at the postpartum care center. She was treated with oral metronidazole for 24 days.

Frequency of Clostridium difficile-associated Diarrhea and Relevant Medical Treatment in a Tertiary Care Hospital in Korea (3차 의료기관에서 Clostridium difficile-associated Diarrhea의 발생빈도 및 치료에 관한 연구)

  • 배경: 약물로 인한 Clostridium difficile-associated diarrhea (CDAD)는 널리 알려져 있으며 우리나라에서 항생제와 프로톤 펌프 억제제 소모량을 고려할 때 질환 치료과정에서의 CDAD 발생빈도 및 CDAD 유발 이전에 투여한 약물의 사용빈도와 CDAD의 치료방법을 조사할 필요성이 있다. 방법: 경상대학교 병원에서 2011년 1월부터 6월까지의 입원환자를 대상으로 대변 독소 검사에 의해 CDAD로 판명된 환자의 성별, 연령분포, 질환명, 입원병동, 재발률을 조사하였으며 CDAD 판명이전에 투여한 약제 및 CDAD 판명후 치료약제를 조사하였다. 결과: 연구기간 동안 CDAD 대변 독소 검사 의뢰된 환자수는 1,500명이었으며 CDAD 양성은 111명(9.3%)이었고, 재발은 29명(26.1%)이었다. CDAD를 주소로 입원한 환자는 17명 (15.3%)이었고, 나머지는 입원기간 중에 발생하였다. CDAD 양성인 환자의 연령대는 60대에서 32.4% (36/111명) 이었고, 내과병동에서 34.2%를 나타내었고, 재발률은 외과계 병동에서 41.4%로 가장 높게 나타났다. CDAD 환자의 17% (19/111명)은 항암제 투여 동안 발생하였으며 CDAD 발생 전 사용약물로는 세팔로스포린계 항생제가 162회로 가장 빈번하게 사용 되었으며, 히스타민2 수용체길항제 107회, 스테로이드 82회, 비 스테로이드 항염제 79회, 프로톤 펌프 억제제 77회, 하제 59회, 항암제가 33회 처방되었다. CDAD 치료약제로는 8종의 약제가 241회 처방 되었으며 metronidazole이 99회로 가장 빈번하게 사용되었고, vancomycin이 37회로 나타났다. 결론: 입원환자에 있어서 CDAD양성은 특히 고령의 암환자가 많아 항암제 투여 시에는 CDAD 발생에 주의해야 할것으로 보인다. CDAD의 치료약제로는 metronidazole이 vancomycin 보다 많이 사용되는 것으로 나타났다.

Clinical Characteristics of Symptomatic Clostridium difficile Infection in Children: Conditions as Infection Risks and Whether Probiotics Is Effective

  • Purpose: This study investigated the clinical presentations of symptomatic Clostridium difficile infection (CDI) in children. Methods: We reviewed the medical records of 43 children aged <20 years who showed either positive C. difficile culture or C. difficile toxin test results between June 2010 and April 2014. Results: Of the 43 patients (mean age 6.7 years), 22 were boys. Sixteen patients (37.2%) showed both positive C. difficile culture and toxin test results. Seventeen out of 43 children (39.5%) had preexisting gastrointestinal diseases, and 26 children had other medical conditions that were risk factors for CDI. Twenty-eight children had a history of antibiotic treatment for >3 days, and the most frequently prescribed antibiotic was amoxicillin-clavulanate (35.7%). Twenty-eight patients were diagnosed with CDI despite taking probiotic supplements, most commonly Lactobacillus acidophilus (53.6%). The most common symptom was diarrhea (72.1%) at the time CDI was diagnosed. C. difficile was eradicated in 11 patients (25.6%) after treatment with oral metronidazole for 10-14 days, and in the two patients (4.6%) who required two courses of oral metronidazole. Sixteen patients (37.2%) showed clinical improvement without any treatment. Conclusion: This study showed the various clinical characteristics of CDI in children and that preexisting clinical conditions favored the development of CDI. In addition, CDI was found to occur in a number of patients even after probiotic prophylaxis given in conjunction with antibiotic therapy.

Isolation, In vitro Antibacterial Activity, Bacterial Sensitivity and Plasmid Profile of Lactobacilli

  • The present research work was conducted to evaluate the beneficial effects as well as the safety aspects of lactobacilli as probiotic. Lactobacilli were isolated from poultry faecal samples, feed samples and from some known preparations procured from poultry feed manufacturers. L. acidophilus and L. sporogenes were tested for the antibacterial activity against four poultry pathogens viz. Escherichia coli, Salmonella spp., Proteus spp. and Pseudomonas aeruginosa. Cell free supernatant (CFS) of L. acidophilus exhibited significantly higher antibacterial activity against Salmonella spp. at original pH (4.50${\pm}$0.02). At the adjusted pH (6.50${\pm}$0.02) significantly higher antibacterial activity was recorded against indicator organism except for P. aeruginosa. Likewise, L. sporogenes exhibited similar antibacterial activity at original as well as adjusted pH except for E. coli. Antibacterial activity against E. coli was significantly higher at adjusted pH than at original pH of CFS. The competitive exclusion of E. coli by lactobacilli over the intestinal epithelial cells (IEC) was checked. L. acidophilus strain I, which was of poultry origin, exhibited maximum attachment over IEC as compared to other three strains of non-poultry origin viz. L. acidophilus strain II, L. sporogenes strain I and II. Overall, L. acidophilus exhibited higher competitive exclusion as compared to L. sporogenes. All the lactobacilli of poultry origin were most sensitive to penicillin G, amoxycillin, ampicillin and chloramphenicol, least sensitive to sulphamethizole, ciprofloxacin, neomycin, norfloxacin and pefloxacin and resistant to metronidazole and nalidixic acid. The isolates from probiotic preparations were most sensitive to ampicillin, amoxycillin and tetracycline, least sensitive to sulphamethizole, norfloxacin, neomycin and ceftriazone and resistant to nalidixic acid and metronidazole. Eight of the multiple drug resistant lactobacilli isolates were studied for the presence of plasmids. Plasmids could be extracted from six isolates of lactobacilli. These plasmids could be responsible for bacteriocin production or for antibiotic resistance of the strains. The lactobacilli need further studies regarding their safety for use in the probiotic preparations.

Outcome of Regenerative Endodontic Treatment for an Avulsed Immature Permanent Tooth: A Case Report (완전 탈구된 미성숙 영구치의 치수재생치료 증례 보고)

  • Dental avulsion, defined as the complete displacement of a tooth from the alveolar bone with consequent loss of the blood and nerve supply, was reported as one of the most severe dental injuries. Avulsion can cause tissue ischemia, which leads to pulp necrosis. Apexification is a conventional treatment method that induces an apical calcified barrier in immature roots with pulp necrosis. However, root development characterized by an increase in the root thickness and length cannot be achieved by apexification. The purpose of this case report was to describe the radiographic and clinical outcomes of regenerative endodontic treatment for the avulsed and necrosed permanent tooth with an immature root after replantation in a 5-year-old girl; the treatment was performed using a mixture of ciprofloxacin, metronidazole and cefaclor, CollaTape and Biodentine.

A Case of Brain Abscess due to Parvimonas micra in a Healthy Child without Dental Disease (특이 과거력이 없는 소아에서 발생한 Parvimonas micra 뇌농양 1예)

  • Parvimonas micra is a non-spore-forming anaerobic gram-positive coccus and a known commensal of the skin, gums, vagina, and gastrointestinal tract. It is rarely associated with severe infections, which typically follow invasive procedures such as dental treatment. We describe a case of a brain abscess caused by P. micra in an immunocompetent 11-year-old boy without periodontal disease. He presented with a 7-day history of headaches and vomiting, and complained of diplopia that began on the day of presentation. He did not have any recent dental treatment or specific past medical history. A brain abscess in the left frontoparietal lobe was noted on brain magnetic resonance imaging. P. micra was cultured from brain abscess aspirate. He was successfully treated with surgical drainage and combined antibiotic therapy with ceftriaxone and metronidazole for 6 weeks.

Genotyping of Peroxisome Proliferator-Activated Receptor gamma in Iranian Patients with Helicobacter pylori Infection

  • Helicobacter pylori (H. pylori) infection as a serious problem in both adults and children can induce chronic gastritis, peptic ulcer disease (PUD), and possibly gastric cancer. The aim of the current study was to survey antibiotic resistance and also to determine influence of PPAR$\gamma$ polymorphism in patients with H. pylori infection. During an 11-month-period, 98 H. pylori isolates were collected from 104 biopsy specimens. In vitro susceptibility of H. pylori isolates to 4 antimicrobial agents metronidazole, clarithromycin, amoxicillin and tetracycline were assessed by quantitative method according to European Committee on Antimicrobial Susceptibility Testing (EUCAST) guideline. PPAR$\gamma$ polymorphism was determined using polymerase chain reaction-restriction fragment length polymorphism assay. The frequency of H. pylori infection in our study was 94.2%. In vitro susceptibility data showed that highest level of resistance was related to metronidazole (66.3%), and the majority of H. pylori isolates were highly susceptible to amoxicillin and tetracycline (94.9% and 96.9%, respectively). Genotypic frequencies were 25.5% for CC (Pro12Pro), 40.8% for GC (Pro12Ala) and 33.7% for GG (Ala12Ala). In our study, CG genotype had highest distributions among infected patients with H. pylori. The study suggests that the PPAR-$\gamma$ Pro12Ala polymorphism could be evaluated as a potential genetic marker for susceptibility to gastric cancer in the presence of H. pylori infection.

TREATMENT OF MINOCYCLINE STAINED NON-VITAL TOOTH (Minocycline에 의해 변색된 실활치의 치료)

  • A mixture of metronidazole, ciprofloxacin and minocycline(3Mix) in distilled water or in a root canal sealer were used to disinfect the infected endodontic and periapical lesions. It has been well acknowledged in recent literature that minocycline, a semisynthetic tetracycline derivative, causes discolorations in adult teeth and various other collagenous tissues. In these cases, pulp treatments were finished successfully using a combination of antibiotics but severe discolorations of the teeth were observed. Discolored non-vital posterior tooth was treated using the prosthodontic procedure. And anterior tooth was successfully treated using the walking bleach technique. Bleaching was performed by temporarily placing a mixture of sodium perborate and distilled water into the pulp chamber. There were significant esthetic improvement.
