소아의 근골격계 감염에서 지역사회 관련 메치실린 내성 황색 포도알균의 증가: 2000년 9월-2012년 8월간의 단일기관 연구 (Increasing Rates of Community Associated Methicillin-Resistant Staphylococcus aureus in Children with Muscular-Skeletal Infections in Korea: A Single Center Experience from 2000 to 2012)

  • 목 적 : 본 연구에서는 CA-MRSA로 인한 소아의 세균성 근골격계 감염의 임상양상을 연구하기로 하였다. 방 법 : 2000년 9월부터 2012년 8월까지 12년간 분당 차병원에 입원한 18세 이하의 환아 중 혈액과 관절액, 조직이나 고름의 세균 배양검사에서 S. aureus가 동정된 환아를 대상으로 의무기록을 후향적으로 분석하였다. 결 과 : 대상 환아 수는 총 31례였으며 중간값은 7세(최소 17일, 최대 18세)이다. 화농성 관절염으로 진단된 경우는 11례(35.5%), 골수염은 16례(51.6%), 화농성 관절염과 골수염이 병발된 경우는 5례(16.1%)였다. 환아의 혈액이나 관절액, 조직이나 고름 등에서 동정된 S. aureus 31례 중 MSSA는 25례(80.6%), MRSA는 6례(19.4%)였다. 검출된 균주 중 다제내성균은 관찰되지 않았다. MRSA가 배양되었던 6례는 모두 2006년 이후 발생하였다. 모든 증례에서 항생제가 투여되었으며 항생제 투여 기간은 $26.4{\pm}12.7$일이었고, 첫 항생제로 반코마이신을 동반 사용한 경우는 4례(12.9%)였으며 10례(32.3%)에서 최종 항생제로 반코마이신이 투여되었다. 결 론 : 본 연구를 통하여 최근 12년간 세균성 근골격계 감염에서 MRSA의 발생은 연구기간 후반에 편중되었다.

  • Resistance to antibiotics is a problem all over the world, and this problem also is so extended in veterinary fields. Therefore, this study aimed to find out the antibacterial substances from natural medicinal herbs against bovine mastitis pathogens, especially methicillin-resistant Staphylococcus aureus (MRSA). Among seventy two medicinal herbs, Terminaliae chebula extract showed antimicrobial effect against MRSA isolated from mastitis milk in cow and patient in human. However, Terminaliae chebula extract didn't show antibacterial effects on various strains including other contagious and environmental pathogens related with mastitis of dairy cows.

Antimicrobial Susceptibility and Clonal Relatedness between Community- and Hospital-Acquired Methicillin-Resistant Staphylococcus aureus from Blood Cultures

  • We compared the antimicrobial resistance and clonal relationships among the community-acquired (CA) and hospital-acquired (HA) methicillin-resistant Staphylococcus aureus (MRSA) strains that were isolated from blood cultures in a university hospital over a 4-year period. A total of 131 MRSA isolates, including 28 CA-MRSA and 103 HA-MRSA strains, were identified; antimicrobial susceptibility testing indicated that the CA-MRSA isolates were more susceptible to erythromycin (21 % vs 6% ; P=0.02), clindamycin (46% vs 12%; P<0.01), ciprofloxacin (43% vs 11%; P<0.01), and gentamicin (43% vs 6%; P<0.01) than were the HA-MRSA isolates. Pulsed-field gel electrophoresis (PFGE) typing and antimicrobial resistance profiles separated the 20 CA-MRSA isolates into 14 and 10 different patterns, respectively, and the 53 HA-MRSA isolates were separated into 24 and 7 different patterns, respectively. Twenty-one (40%) of the 53 HA-MRSA isolates belonged to two predominant PFGE types, and most of them showed multi-drug resistant patterns. Four (20%) of the 20 CA-MRSA and 10 (19%) of the 53 HA-MRSA isolates fell into two common PFGE patterns, and each of them showed the same multi-drug resistant pattern. This study suggests that, although the CA-MRSA blood isolates showed diverse PFGE and antimicrobial resistance patterns, some of these isolates may have originated from the HA-MRSA strains.

  • At the present study, it was aimed to explore the states of antimicrobial resistant Staphylococcus aureus isolates from 1,360 chickens, pigs and cattle carcass (400 chickens, 480 pigs and 480 cattle) in Daegu province from January 2022 to December 2022. Among 1,360 samples, 81 of S. aureus were isolated cattle (1.4%), pigs (7.7%) and chickens (9.2%). In antimicrobial susceptibility test, all of the isolates were demonstrated susceptibility to rifampin. But the isolates were showed resistance other antibiotics in order of tetracycline (62.9%), ciprofloxacin (62.9%), tobramycin (58.0%), gentamicin (51.8%), amikacin (40.7%), penicillin (39.5%), clindamycin (35.8%), enrofloxacin (33.3%), trimethoprim/sulfamethoxazole (30.8%), oxacillin (30.8%), minocycline (29.6%), erythromycin (25.9%), quinupristin/dalfopristin (20.9%), chloramphenicol (12.3%), cefoxitin (9.8%). Among the 81 S. aureus isolates, 25 (30.8%) methicillin-resistant staphylococcus aureus (MRSA) were observed. Seven (28.0%) of 25 MRSA harbored mecA gene. About 96% of MRSA were multidrug resistance to at least 3 more drugs. A continuous monitoring and surveillance program to prevent antimicrobial resistance in livestock products is demanded.

임상검체에서 분리된 Methicillin Resistant Staphylococcus aureus의 다제내서양상과 내성 관련 유전자의 검색 (Detection of multidrug resistant patterns and associated-genes of Methicillin Rdsistant Staphylococcus aureus(MRSA) isolated from clinical specimens)

  • Methicillin Resistant Staphylococcus aureus (MRSA) was obtained from the clinical specimens at Pusan national university Hospital, Pusan, Korea. The sensitivities against various antibiotics were examined by using disc diffusion test and associated genes such as mecA, mecR1, mecI and femA were detected by polymerase chain reaction. Among Seventy-nine strains of MRSA, 38 strains(48.1%)were sensitive to streptomycin and 32 strains(40.5%) to cefoperazone, while one strain(1.3%) were resistant to vancomycin. In considering the result of this study, 7 strains showed resistance to 9 kinds of different antibiotics, 12 strains were to 8 kinds, 24 strains were to 7,25 strains were to 6, 9 strains were to 5, and 2 strains were to 4 antibiotics. Among 79 strains of MRSA, 67 strains were coagulase positive and 12 were coagulase negative. In the detection of MRSA associated genes by PCR method, mecA, mecR1, mecI, and femA genes were detected in 30 strains(44.8%), 28 strains(41.8%), 23 strains(34.3%) and 15 strains(22.4%), respectively. MecA type that is without femA were found in 21 strains(31.3%), femA type that is without regulator genes were shown in 4 strains(6.0%), while mecA-mecR1-mecI type with regulator genes were shown more to be 17 strains(25.4%). There was little statistical significance between multidrug resistance and MRSA associated genes. Considering these result, it is necessary to include moecular biological studies of related genes to the study drug resistance.

Community-Associated Methicillin-Resistant Staphylococcus aureus Colonization in the Upper Respiratory Tracts of Korean Military Recruits

  • Background: Several large outbreaks have demonstrated the threat of community-associated methicillin-resistant Staphylococcus aureus (CA-MRSA) in close-contact environments, such as occurs during training and quartering of military recruits training. In South Korea, which is a hospital or healthcare-associated MRSA prevalent area, military service is compulsory for all healthy young men. We surveyed and determined the extent of CA-MRSA colonization in the upper respiratory tracts of Korean military recruits. Methods: The Korean military recruits who were enrolled in a military training facility from November 2004 to March 2005 were eligible for this study. Sputum or nasopharyngeal swap was obtained from randomly selected subjects who displayed upper respiratory tract symptoms. Results: Of the 181 participants, 32 participants (17.7%) were colonized with S. aureus, and 12 participants (6.6%) were colonized with MRSA. Among the cases that were colonized with S. aureus, 37.5% (12/32) were colonized with MRSA. Antimicrobial susceptibility testing showed resistant patterns that were suggestive of the CA-MRSA strains for all of the MRSA isolates. Conclusion: This study of Korean military recruits found a great deal of showed MRSA colonization in them, and the antimicrobial resistant profile that was suggestive of a CA-MRSA strain. Further efforts to prevent the spread of MRSA infections and careful monitoring for CA-MRSA outbreaks are warranted, especially in a high risk group such as military recruits.

지역사회 내 일반인 및 동물에서 methicillin 내성 Staphylococcus aureus의 유전학적 특성 (Genetic characterization of methicillin-resistant Staphylococcus aureus from humans and animals within the community)

  • Methicillin-resistant Staphylococcus (S.) aureus (MRSA) is one of the most important nosocomial pathogens worldwide and the emergence of this strain has become a major clinical problem. In this study, we investigated the prevalence of MRSA and their genetic characteristics in 69 S. aureus isolated from humans and animals. In human isolates, higher antimicrobial resistance rates were observed against penicillin (80.6%), followed by erythromycin (11.9%) and tetracycline (9.0%). All of them were susceptible to clindamycin, enrofloxacin, novobiocin, pirlimycin, trimethoprim/sulfamethoxazole and vancomycin. The resistance patterns in animal isolates were similar to those of human isolates. Two (2.9%) MRSA strains were isolated from human (n = 1) and animal (n = 1), and these isolates were confirmed as carrying the mecA gene. One isolate originating from human was resistant to 7 drugs and the other isolate derived from animal was resistant to 11 drugs. Staphylococcal cassette chromosome mec (SCCmec) variant IIIB was identified in animal isolate but SCCmec type of an isolate from human was not exactly determined. Two MRSA isolates showed unrelated PFGE pattern between them. Our results indicated although the frequency of MRSA isolates from humans and animals was low, a continuous surveillance and monitoring should be called for to prevent the contamination and spread of MRSA in the community. To our knowledge, this is the first time that SCCmec type variant IIIB was detected from animals in Korea.

First Report on Multidrug-Resistant Methicillin-Resistant Staphylococcus aureus Isolates in Children Admitted to Tertiary Hospitals in Vietnam

  • The extensive distribution of multidrug-resistant (MDR) methicillin-resistant Staphylococcus aureus (MRSA) poses a threat to healthcare worldwide. This study aimed to investigate the MDR and molecular patterns of MRSA isolates in children admitted to the two biggest tertiary care pediatric hospitals in northern and southern Vietnam. A total of 168 MRSA strains were collected to determine antibiotic susceptibility by minimum inhibitory concentration tests. Antibiotic-resistant genes, pulsed-field gel electrophoresis, staphylococcal cassette chromosome mec (SCCmec) typing, and multilocus sequence typing were used for the molecular characterization of MRSA. Among the total strains, the MDR rate (51.8%) was significantly higher in the northern hospital than in the southern hospital (73% vs. 39%, p < 0.0001). The MDR-MRSA with the highest rates were "ciprofloxacin-erythromycin-gentamicintetracyclines" (35.6%), followed by "erythromycin-tetracycline-chloramphenicol" (24.1%), and "ciprofloxacin-erythromycin-gentamicin" (19.5%), showing an accumulative total of 79.3%. The most susceptible antibiotics were rifampicin (100%) and vancomycin (100%), followed by doxycycline (94.0%), meropenem (78.0%), and cefotaxime (75.0%). The SCCmecII strains showed greater resistance to gentamicin, ciprofloxacin, tetracycline, meropenem and cephalosporins compared with the other strains. The SCCmecII strains exhibited the highest rate in the tested genes (aacA/aphD: 55.2%, ermA/B/C: 89.7%, and tetK/M: 82.8%). ST5-SCCmecII was the predominant clone in the northern hospital, whereas SCCmecIVa was more pronounced in the southern hospital. In conclusion, our results raised concerns about the predominant MDR-MRSA strains in the pediatric hospitals in Vietnam. The north-south difference in the antibiotic resistance patterns and genetic structure of MRSA suggests different MRSA origins and various uses of antimicrobial agents between the two regions.

대황목단탕의 메티실린 내성 황색 포도상구균에 대한 항균활성 (The Antimicrobial Activity of Daehwangmokdan-tang against Methicillin-resistant Staphylococcus aureus)

  • Objective : Infectious diseases are a growing problem worldwide by Methicillin-resistant Staphylococcus aureus (MRSA). Daehwangmokdan-tang is one of the oriental medicine prescriptions contained in Principles and Practice of Eastern Medicine. This study investigated the antibacterial activity of EtOH 70% extracts of Daehwangmokdan-tang (DMT) which prescription is composed of oriental medicine against MRSA. Methods : The antimicrobial activity and active concentration of MRSA were verified by measuring the minimum inhibitory concentration (MIC) of DMT. In addition, the effects of the disease were checked by treating the existing antibiotics and large ethanol extract in parallel, and the extent of growth suppression was checked over time. In addition, cell membrane permeability experiment confirmed the effect of large DMT on the immunity mechanism of MRSA. Results : TThe minimum inhibitory concentration of DMT against MRSA is 500 ~ 2000 ㎍/㎖ by broth dilution method. In the checkerboard method, the combinations of DMT with antibiotics has partial synergistic effect or synergy effect and DMT markedly reduced the MICs of the antibiotics oxacillin (OX), gentamicin (GEN) against MRSA. In the inhibition of resistance mechanism of DMT against MRSA, the expression of resistance gene and protein about β-lactam antibiotic was reduced. Also, we observed the effect of DMT about cell membrane permeability against MRSA, and confirmed that DMT suppressed growth of strains by increasing cell membrane permeability and energy metabolism. Conclusion : Basis on the result, we speculate that DMT may be useful for the treatment of MRSA infections when used in combination with β-lactam antibiotic.

Rapid Detection of Methicillin Resistant Staphylococcus aureus Based on Surface Enhanced Raman Scattering

  • Methicillin-resistant Staphylococcus aureus (MRSA) is one of the severe nosocomial infectious agents. The traditional diagnostic methods including biochemical test, antibiotic susceptibility test and PCR amplification are time consuming and require much work. The Surface enhanced Raman spectroscopy (SERS) biosensor is a rapid and powerful tool for analyzing the chemical composition within a single living cell. To identify the biochemical and genetic characterization of clinical MRSA, all isolates from patients were performed with VITEK2 gram positive (GP) bacterial identification and Antibiotic Susceptibility Testing (AST). Virulence genes of MRSA also were identified by DNA based PCR using specific primers. All isolates, which were placed on a gold coated nanochip, were analyzed by a confocal Raman microscopy system. All isolates were identified as S. aureus by biochemical tests. MRSA, which exhibited antibiotic resistance, demonstrated to be positive gene expression of both femA and mecA. Furthermore, Raman shift of S. aureus and MRSA (n=20) was perfectly distinguished by a confocal Raman microscopy system. This novel technique explained that a SERS based confocal Raman microscopy system can selectively isolate MRSA from non-MRSA. The study recommends the SERS technique as a rapid and sensitive method to detect antibiotic resistant S. aureus in a single cell level.
