본 연구는 MRI 검사 시 금속 인공물을 억제하기 위한 목적에서 합리적인 수신대역폭 적용 기준을 제시하고자 하였다. 이를 위해 척추 수술용 나사로 제작된 팬텀을 대상으로 고속 스핀 에코기법을 적용한 T2 대조도 영상을 획득해 금속 인공물을 재현하였다. 그리고 수신대역폭을 100 Hz/PX에서 800 Hz/PX까지 100 Hz/PX씩 증가시키며 영상을 획득하였다. 금속 인공물은 신호 감쇄 영역과 신호 누적 영역의 면적을 측정하여 합으로 정하였다. 더불어 영상 변수에 따른 금속 인공물의 양상을 분석하기 위해 Pearson 상관 분석을 시행하였다. 그 결과, 신호 누적 영역은 수신대역폭 증가에 따라 유의한 변화가 없었지만(p>0.05) 신호 소실 영역과 금속 인공물의 면적은 수신대역폭의 증가에 따라 감소하였다(p<0.05). 흥미로운 점은 금속 인공물의 면적은 수신대역폭을 100 Hz/PX에서 200 Hz/PX로 증가시킨 구간에서 최대로 감소하였는데 이는 수신대역폭 증가에 따른 에코 간격이 최대로 줄어든 구간과 일치하였다. 더불어 상관 분석 결과에서도 에코 간격은 수신대역폭과 비교하여 신호 감쇄 영역 및 금속 인공물의 면적과 더 높은 연관성이 있는 것으로 나타났다. 따라서 금속 인공물을 줄이기 위한 목적으로 수신대역폭을 증가시킬 경우, 화질 요소를 고려해 에코 간격이 최소화되는 수치를 기준으로 정하는 것이 합리적이라 생각된다.
본 연구는 금속 인공물을 감소시키는 데 있어 synthetic MR 기법의 효과를 평가해보고자 하였다. 실험은 척추 수술용 나사로 제작된 팬텀을 대상으로 synthetic MR 기법과 고속 스핀 에코 기법을 적용하여 in-plane과 through-plane 영상을 획득하고 금속 인공물의 면적을 비교해 보았다. 금속 인공물은 signal-loss와 signal pile-up 영역으로 구분하여 측정하였고 둘의 합을 통해 최종 인공물의 면적을 계산하였다. 그 결과, in-plane, through-plane 모두 synthetic MR 기법을 적용했을 때 상대적으로 금속 인공물이 감소하였다. 시퀀스 별로 비교하면 in-plane의 경우 T1 영상은 23.45%, T2 영상은 20.85%, PD 영상은 19.67%, FLAIR 영상은 22.12% 감소하였다. 또한 through-plane의 경우 T1 영상은 62.95%, T2 영상은 73.93, PD 영상은 74.68%, FLAIR 영상은 66.43% 감소하였다. 이러한 결과의 원인은 synthetic MR 기법 적용 시 signal pile-up에 의한 왜곡이 발생하지 않아 전체 금속 인공물의 크기가 감소하였기 때문이다. 따라서 synthetic MR 기법은 매우 효과적으로 금속 인공물을 감소시킬 수 있어 영상의 진단적 가치를 높이는 데 도움을 줄 수 있다.
의료영상에서 인공물(Artifacts) 이라 함은 영상이 얻어지는 신체부위와 아무런 관련이 없으나 얻어진 영상에는 마치 영상의 일부분으로 나타나는 모든 것을 가리킨다. 따라서 영상에서 이들 인공물들은 실제 조직의 해부학적인 구조를 나타내지 않으므로 영상 판독에 영향을 주어 잘못된 진단을 초래할 수도 있다. 그러나 MR 영상이 가능한 이래로 새로운 여러 종류의 MR 인공물들이 많이 발견 되었으나 다행스럽게도 거의 모든 MR 인공물들은 쉽게 설명이 가능하며, 따라서 이들 인공물들에 의한 진단 오류의 가능성은 매우 희박한 실정이다. 그러나 새로운 영상방법이나 혹은 새로운 펄스대열이 계속 고안됨에 따라 새로운 종류의 인 공물들이 생겨날 가능성은 항상 존재하고 있다. 지금까지 알려진 여러 MR 인공물들은 그 생겨난 원인에 따라 다음과 같이 크게 세 가지로 분류가 가능하다. Woolen tapestry curtains possessed by the Seoul Museum of Craft Art were used to hung on the wall or used for carpets in the winter season in the late Joseon dynasty. Since similar artifacts were only used for the curtain as functional aspects, woolen tapestry curtains were a rare case. In this study, scientific analysis on the accessory ornament of woolen tapestry curtains such as components of metal accessories and frame bar were conducted with the microscope, p-XRF, and SEM. Result of frame bar pigments, organic pigments such as ink stick were likely been used in woolen tapestry curtain 1. In woolen tapestry 2, lead red (Minium) was used in the frame bar. The result of metal parts, copper, and zinc were analyzed by p-XRF. This suggests that metal accessories were crafted using brass. Frame bar of woolen tapestry curtain 2 was made of soft pine (Pinus spp.) analyzed with the scanning electron microscope. Artifacts like woolen tapestry curtains are rare in Korea and scientific analysis databases were scarce, so it is important to construct components analysis data of woolen tapestry curtains. It is expected that additional scientific analysis and interpretation on the artifact's crafting technique can be merged with the analytical data gathered in this study to be utilized on the conservation and restoration of not only woolen curtains but curtain artifacts of the late Joseon dynasty in general.
Purpose: This study was conducted to assess the effectiveness of a metal artifact reduction (MAR) algorithm activated at different times during cone-beam computed tomography (CBCT) acquisition on the magnitude of artifacts generated by a zirconium implant. Materials and Methods: Volumes were obtained with and without a zirconium implant in a human mandible, using the OP300 Maxio unit. Three modes were tested: without MAR, with MAR activated after acquisition, and with MAR activated before acquisition. Artifacts were assessed in terms of the standard deviation (SD) of gray values and the contrast-to-noise ratio (CNR) in 6 regions of interest with different distances (10 to 35 mm, from the nearest to the farthest) and angulations(70° to 135°) from the implant region. Results: In the acquisitions without MAR, the regions closer to the implant(10 and 15mm) had a higher SD and lower CNR than the farther regions. When MAR was activated (before or after), SD values did not differ among the regions (P>0.05). The region closest to the implant presented a significantly lower CNR in the acquisitions without MAR than when MAR was activated after the acquisition; however, activating MAR before the acquisition did not yield significant differences from either of the other conditions. Conclusion: Both modes of MAR activation were effective in decreasing the magnitude of CBCT artifacts, especially when the effects of the artifacts were more noticeable.
심장CT를 스캔하여 MAR알고리즘 적용 시 자체 제작한 Pacemaker삽입팬텀의 금속인공물 감소 정도를 비교 분석해 보았다. 에너지별 CT value를 비교한 결과 BKI에서 CT value의 증감이 있었으며 WSA에서 40 KeV의 경우 663.2%로 최대감소, BHA에서 140 KeV의 경우 56.2%로 최대로 증가 하였다. 또한, 70 KeV기준으로 인공물 형태별 CT value 비교결과 WSA에서 최대 약 145% 감소하였고, BHA에서는 최대 약 46.38%증가 하였다. MAR알고리즘 적용 은 에너지 변화 또는 인공물의 종류와 무관하게 금속 인공물 발생을 감소시켜주는 효과가 있어 Pacemaker삽입 후 심장CT로 추적검사 시 MAR알고리즘을 적용한다면 보다 질 좋은 심장CT 영상을 제공할 것으로 사료된다.

Purpose: This study aimed to quantify the influence of tooth position within the field-of-view (FOV) on cone-beam computed tomography (CBCT) imaging artifacts' intensity when assessing teeth restored with various intracanal materials. Materials and Methods: Seventy single-rooted teeth were divided into 7 groups (10 teeth per group): NiCr post (NC), AgPd post (AP), metal core fiberglass post (MCFG), fiberglass post (FG), anatomical fiberglass post (AFG), fiberglass post cemented with core build-up cement (FGCo), and anatomical fiberglass post cemented with core build-up cement (AFGCo). All posts were cemented using a regular dual-curing resin cement (Allcem), except FGCo and AFGCo which were cemented with a core build-up dual-curing resin cement (AllcemCore). Each tooth was scanned on a CS9000 in 5 positions within the FOV: a central position, anterior horizontal peripheral, peripheral superior, peripheral inferior, and posterior horizontal peripheral position. Hyperdense, hypodense, remaining teeth areas and ROI areas were quantitatively analyzed using ImageJ software. Results: Posterior horizontal peripheral position increased the intensity of artifacts on FGCo and AFGCo post groups (P<0.05), and specifically the hypodense artifact intensity on FG and AFG post groups (P<0.05). NC and AP groups presented greater intensity of artifacts than any other post groups(P<0.05). Conclusion: Artifact intensity increases in the presence of high atomic number materials and when the object is not centered within the FOV. The impact of positioning within the FOV on artifact was greater for fiberglass posts cemented with core build-up dual-curing cement than for metal posts and fiberglass posts cemented with regular dual-curing cement.
목적: 금속에 의한 영상 왜곡에 대한 정확한 계산하고 영상으로부터의 금속 물질의 위치 결정한다. 대상 및 방법: 주자기장과 일정 각도를 이루는 무한히 긴 비자성 금속 실린더에 대한 라플라스 방정식을 풀고, 이 결과를 이용하여 절편선택 경사자계와 주파수 부호화 경사자계에 의한 영상에 왜곡을 계산한다. 계산 결과를 바탕으로 하여 왜곡된 영상으로부터 원통형 보철물의 위치를 계산한다. 결과: Folded point와 금속 실린더의 중심 사이의 거리를 영상으로부터 측정하여 계산 결과와 비교한다. 측정 결과와 계산 결과 간의 퍼센트 오차는 한 경우를 제외하고 5% 이내였다. 결론: 금속 실린더가 자기장 하에 있을 때, 영상의 왜곡을 시뮬레이션 하였고, 이 기술은 생검술 또는 외과 수술 등을 자기공명영상법을 이용여 실시간 모니터링하는데 적용할 수 있을 것으로 기대한다.

Purpose: To evaluate the effect of different numbers of basis images and the use of metal artifact reduction (MAR) on the production and reduction of artifacts in cone-beam computed tomography images. Materials and Methods: An acrylic resin phantom with a metal alloy sample was scanned, with 450 or 720 basis images and with or without MAR. Standard deviation values for the test areas (around the metal object) were obtained as a way of measuring artifact production. Two-way analysis of variance was used with a 5% significance level. Results: There was no significant difference in artifact production among the images obtained with different numbers of basis images without MAR (P=.985). MAR significantly reduced artifact production in the test areas only in the protocol using 720 basis images (P=.017). The protocol using 450 basis images with MAR showed no significant difference in artifact production when compared to the protocol using 720 basis images with MAR (P=.579). Conclusion: Protocols with a smaller number of basis images and with MAR activated are preferable for minimizing artifact production in tomographic images without exposing the patient to a greater radiation dose.
Objective: The aim of this study was to assess artifacts induced by metallic restorations in three-dimensional (3D) dental surface models derived by cone-beam computed tomography (CBCT). Methods: Fifteen specimens, each with four extracted human premolars and molars embedded in a plaster block, were scanned by CBCT before and after the cavitated second premolars were restored with dental amalgam. Five consecutive surface models of each specimen were created according to increasing restoration size: no restoration (control) and small occlusal, large occlusal, disto-occlusal, and mesio-occluso-distal restorations. After registering each restored model with the control model, maximum linear discrepancy, area, and intensity of the artifacts were measured and compared. Results: Artifacts developed mostly on the buccal and lingual surfaces. They occurred not only on the second premolar but also on the first premolar and first molar. The parametric values increased significantly with increasing restoration size. Conclusions: Metallic restorations induce considerable artifacts in 3D dental surface models. Artifact reduction should be taken into consideration for a proper diagnosis and treatment planning when using 3D surface model derived by CBCT in dentofacial deformity patients.
