진행성 핵상 마비에서의 국소 뇌혈류 (Regional Cerebral Perfusion in Progressive Supranuclear Palsy)

  • Progressive supranuclear palsy (PSP) is a parkinson-plus syndrome characterized clinically by supranuclear ephthalmoplegia, pseudobulbar palsy, axial rigidity, bradykinesia, postural instability and dementia. Presence of dementia and lack of cortical histopathology suggest the derangement of cortical function by pathological changes in subcortical structures in PSP, which is supported by the pattern of behavioral changes and measurement of brain metabolism using positron emission tomography. This study was done to examine whether there are specific changes of regional cerebral perfusion in PSP and whether there is a correlation between severity of motor abnormality and degree of changes in cerebral perfusion. We measured regional cerebral perfusion indices in 5 cortical and 2 subcortical areas in 6 patients with a clinical diagnosis of PSP and 6 healthy age and sex matched controls using $^{99m}Tc$-HMPAO SPECT. Compared with age and sex matched controls, only superior frontal regional perfusion index was significantly decreased in PSP (p<0.05). There was no correlation between the severity of the motor abnormality and any of the regional cerebral perfusion indices (p>0.05). We affirm the previous reports that perfusion in superior frontal cortex is decreased in PSP. Based on our results that there was no correlation between severity of motor abnormality and cerebral perfusion in the superior frontal cortex, nonmotoric symptoms including dementia needs to be looked at whether there is a correlation with the perfusion abnormality in superior frontal cortex.

Proton MR Spectroscopic Changes in Parkinson′s Disease

  • Purpose: To investigate whether there are significant changes in regional brain metabolism in patients with Parkinson's disease after thalamotomy using proton magnetic resonance spectroscopy (1H MRS). Materials and methods: Fifteen patients with Parkinson's disease of mean age 56.5 years (7 males and 8 females; mean age, 56.5 years) that have treated with levodopa were included. All patients with tremor experienced amelioration of their symptoms on the side contralateral to the thalamotomy. As a single-voxel technique, 1H MR spectra were obtained from the volume of interested regions in thalamus and primary motor cortex. Spectral parameters were: 20 ms TE, 2000 ms TR, 128 averages, 2500 Hz spectral width, and 2048 data points.

실시간 기능적 자기공명영상 유도 기능적 자기공명분광법: 손운동 수행중 운동피질의 젖산변화 관찰 (Real-Time fMRI-Guided Functional MR Spectroscopy: The Lactate Peaks at the Motor Cortex during Hand-Grasping Tasks)

  • 목적: 특정 임무 부여시 특정 뇌피질의 대사물질의 변화를 자기공명 분광법으로 관찰할 수 있는지에 대해서는 논란이 있다. 이 연구의 목적은 실시간 기능적 자기공명영상 유도하의 기능적 자기공명분광법으로 손운동 수행중 운동피질의 젖산의 변화를 관찰하는 것이다. 대상 및 방법: 7명의 지원자(23-28세)를 대상으로 1.5 테슬라 자기공명영상 기계를 이용하여 우측 손을 쥐었다 폈다 반복하는 운동을 하면서 실시간 기능적 자기공명영상을 시행하여 뇌의 활성 영역을 확인한 후, 뇌의 활성 피질 부위에서 1)운동 전, 2) 운동 중, 3)운동 후에 단체적 자기공명분광법을 시행하였다. 연속적인 3회의 분광스펙스럼에서 운동에 의한 젖산의 변화를 비교 관찰하였으며, 이들 분광스펙트럼으로부터 Cho/Cr 비, NAA/Cr 비, Lac/Cr 비를 측정하였다. 결과: 실시간 기능적 자기공명영상에서 확인된 뇌의 활성 영역부위의 운동중 자기공명 분광에서 1.33 ppm 부위에 젖산 피크들이 현저하게 관찰되었으며, 운동후 스펙트럼에서 젖산 피크는 소실되어 운동전 상태로 회복되었다. 운동전, 운동중, 운동후에 Cho/Cr 비는 각각 0.81, 0.76, 0.77이고, NAA/Cr 비는 1.68, 1.65, 1.72이며, Lac/Cr 비는 0.28, 0.41, 0.30 이였으며, 운동중에 젖산은 46% 증가하였다. 결론: 저자들은 실시간 기능적 자기공명영상에서 확인된 뇌의 활성 영역부위에서 운동중 자기공명 분광법으로 현저한 젖산 피크들을 관찰하였으며, 이 기능적 자기공명분광법은 활성 뇌영역의 대사물질 변화를 실시간으로 관찰하는 수단으로 이용될 수 있을 것으로 생각된다.

Results: We found that NAA/Cho ratios showed generally low levels in thalamus in Parkinson's disease patients with clinical improvement following thalamotomy. Conclusions: 1H MRS may be a useful utility for the aid in better understanding the pathophy-siologic process in Parkinson's disease patients on the basis of the variation of NAA/Cho ratio. 신경병성 통증이 있는 당뇨 환자에서 반복 경두개 자기자극치료의 효과 및 피질 탈억제 현상: 환자 대조군 연구 (Effects of Repetitive High Frequency Motor Cortex Transcranial Magnetic Stimulation and Cortical Disinhibition in Diabetic Patients with Neuropathic Pain: A Case Control Study)

  • Objective: To investigate the cortical disinhibition in diabetic patients with neuropathic pain and without pain. In addition, we assessed the cortical disinhibition and pain relief after repetitive transcranial magnetic stimulation (rTMS). Method: We recruited diabetic patients with neuropathic pain (n = 15) and without pain (n = 15). We compared the TMS parameters such as motor evoked potential (MEP) amplitude, cortical silent period (CSP), intracortical inhibition (ICI %) and intracortical facilitation (ICF %) between two groups. Moreover, we evaluated the changes of pain and TMS parameters after five consecutive high frequency (10 Hz) rTMS sessions in diabetic patients with neuropathic pain. The neuropathic pain intensity (visual analog scale) and TMS parameters were assessed on pre-rTMS, post-rTMS 1day, and post-rTMS 5 day. Results: The comparison of the CSP, ICI % revealed significant differences between two groups (p<0.01). After rTMS sessions, the decrease in pain intensity across the three time points revealed a pattern of significant differences (p<0.01). The change of CSP and ICI % across the three test points revealed a pattern of significant differences (p<0.01). The ICI % revealed immediate increase after first rTMS application and significant increase after five rTMS application (p<0.01) in diabetic patients with neuropathic pain. The MEP amplitude and ICF % did not reveal any significant changes. Conclusion: Our findings demonstrate that cortical inhibition was decreased in diabetic patients with neuropathic pain compared with patients without pain. Furthermore, we also identified that five daily rTMS sessions restored the defective intracortical inhibition which related to improvement of neuropathic pain in diabetic patients.
