폐기능 장애와 대사 증후군 관련성: 2008-2013 국민건강 영양조사 자료 사용 단면연구 (Associations between pulmonary function disorders & Metabolic Syndrome: A Cross-Sectional Study Using Data from KNHANES 2008-2013)

  • 본 연구는 심혈관 질환을 공통적으로 동반하는 폐기능 장애와 대사증후군 관계에 대한 연구이다. 이들 질환이 상호 관련성이 있다는 가정 하에 폐기능 장애와 대사증후군간의 상호 관련성 여부를 확인하여 올바른 질환 관리를 위한 초석을 마련하고자 하였다. 자료는 국민건강 영양조사 자료중 2008년부터 2013년까지 6년간 자료를 통합한 53,829건에서, 폐기능 검사를 시행한 만 40세 이상으로 모든 혼란 변수를 통제한 정제된 자료 8,137명(남자, 3,951명, 여자 4,186명)의 자료를 사용하였다. 폐기능 장애는 GOLD(Global Initiative for Chronic Obstructive Lung Disease)기준에 따라 폐쇄성, 제한성으로 구분하였고, 대사증후군은 개정된 NCEP-ATPIII(National Cholesterol Education Program-Expert Panel-Adult Treatment Panel III)기준을 사용하였다. 연관성 분석은 로지스틱 회귀분석을, 유병률은 교차 분석을 시행하였다. 연구 결과, 연관성 분석에서 폐쇄성 폐질환은 대사증후군과 직접적인 관련성을 보이지 않았으며 제한성 폐질환은 대사증후군과 직접적인 관련성을 보였다. 유병률 분석에서는 폐쇄성 폐질환과 제한성 폐질환 모두 대조군보다 높은 대사증후군 유병률을 보였다. 그러나 제한성 폐질환의 대사증후군 유병률이 폐쇄성 폐질환의 대사증후군 유병률보다 더 높았다. 결론적으로 폐쇄성이 아니라 제한성 폐질환이 대사증후군과 유의한 연관성을 보였고 대사증후군 유병률도 더 높았다.

Subtle inflammation: a possible mechanism of future cardiovascular risk in obese children

  • Purpose: The risk of cardiovascular disease (CVD) has been shown to be associated with systemic inflammation in obese adults with metabolic syndrome (MetS). The aims of this study were to evaluate the prevalence of MetS and its relation to inflammatory markers in obese Thai children. Methods: A cross-sectional study was conducted. Children with history of endogenous obesity, chronic diseases, drug ingestion, and any acute illness within 2 weeks prior to enrollment were excluded. Their fasting blood glucose (FBG) levels, oral glucose tolerance tests, insulin, lipid profiles, and selected inflammatory markers, including interleukin-6, tumor necrosis factor-alpha, and high-sensitivity C-reactive protein (hs-CRP) levels, were tested. Results: In this study, 58 obese Thai children (female, 20; male, 38) with a mean body mass index z score of $5.1{\pm}2.2$ were enrolled. The prevalence of MetS and prediabetes was 31% and 17.2%, respectively. None of the children had diabetes. FBG levels, 2-hour glucose levels, and lipid profiles were not statistically different between those with and without MetS. However, obese children with MetS had higher insulin levels and homeostasis model assessment of insulin resistance values. Elevated hs-CRP levels were found in 69% of the cases, although it was not statistically different between the 2 groups. Conclusion: We described a substantial prevalence of MetS in Thai obese children. Regardless of MetS status, two-thirds of the obese children had elevated hs-CRP level, indicating subtle ongoing inflammatory process. This chronic inflammation feasibly predisposes them to CVD in the future, even in children without MetS.

우리나라 성인의 대사증후군 유병과 관련된 식습관 및 식행동 요인 (Dietary Behaviors Related to Metabolic Syndrome in Korean Adults)

  • This study was conducted to investigate the cross-sectional associations between dietary factors and the risk of metabolic syndrome (MetS) in 12,755 subjects (males 5,146, females 7,609) aged 19 years or above using data from the 4th (2007-2009) Korea National Health and Nutrition Examination Survey (KNHANES). The prevalence of MetS in Korean adults was 23.6% (males 26.1%, females 20.9%) with the criteria for modified National Cholesterol Education Program Adult Treatment Panel III. While males had a higher prevalence of abdominal obesity, hyperglycemia, hypertriglyceridemia, and high blood pressure than females, the prevalence of low HDL-cholesterol level was higher in females than in males. Among dietary guidelines, the response of 'yes' for asking practice of 'avoiding salty foods', and 'eating moderately and increasing physical activity for healthy weight' were significantly associated with the decreased risk of MetS in both males and in females. Especially, the risk of MetS was significantly lower in the subjects that responded the practice of all items of Korean Dietary Guidelines. Significantly negative associations with MetS were also found in the responding for practice of 'limiting consumption of alcoholic beverages' in males, and taking dietary supplements in females. Skipping breakfast was positively associated with the risk of MetS. In conclusion, dietary behaviors such as having breakfast, practice of dietary guidelines, and food consumption in moderation could modify the prevalence of MetS, and our findings could be useful for establishing guidelines for preventing MetS.

Dietary flavan-3-ols intake and metabolic syndrome risk in Korean adults

  • Flavan-3-ols are a subclass of flavonoids found in a variety of foods including teas. The effects of flavan-3-ols on the risk of metabolic syndrome (MetS) have been investigated, generally focusing on tea catechins or individual flavan-3-ol rich foods, but there is little information on dietary flavan-3-ols intake and risk of MetS in population-based studies. In this cross-sectional study, we examined the association between dietary flavan-3-ols intake and the risk of MetS in Korean adults. Subjects comprised 1,827 men and 2,918 women aged 20-69 years whose data was included in the 2008 Korean National Health and Nutrition Examination Survey. This survey was conducted between January 2008 and December 2008. Total flavan-3-ols intakes were calculated from 24-hour dietary recalls using a flavonoids database. Thirty percent of the male subjects and 24% of the female subjects were reported as having MetS. In the female subjects, flavan3-ols intake was inversely associated with the risk of MetS after adjusting for potential confounders (5th vs. 1st quintile, OR=0.64, 95% CI=0.45-0.91, P for trend=0.384). The main food source of flavan-3-ols was green tea followed by apples and grapes. Among MetS components, flavan-3-ols intake was inversely associated with the risk of high blood pressure after adjusting for potential confounders (5th vs. 1st quintile, OR=0.64, 95% CI=0.45-0.90, P for trend=0.005). No significant association between flavan-3-ols intake and risk of MetS was found in the male subjects. After stratified analysis by obesity (BMI ${\geq}$ 25 or BMI < 25), however, flavan3-ols intake was inversely related to the risk of hypertension in non-obese men. These results suggest that dietary flavan-3-ols intake may have beneficial effects on MetS risk by reducing the risk of hypertension. The effects of flavan-3-ols intake dependent on obesity need further investigation.

Validity and reproducibility of a food frequency questionnaire to assess dietary nutrients for prevention and management of metabolic syndrome in Korea

  • Little data exists on metabolic syndrome (MetS) related with intake, especially for the South Korean. The purpose of this study was to develop and evaluate a food frequency questionnaire (FFQ) for nutritional assessment in the population with MetS in South Korea. Randomly selected female participants, mean age 21.9 years (n = 38) were invited to answer the FFQ twice (FFQ1 and FFQ2) over a nine-month interval and to complete twelve-day diet records (DR) during the months between in South Korea. The correlation coefficients for nutrient intake between FFQ1 and FFQ2 varied from 0.253 (niacin) to 0.573 (cholesterol), and the energy intake-adjusted correlation coefficients ranged from 0.187 for protein to 0.662 for iron. The energy intake-adjusted and de-attenuated correlation coefficients for comparison of FFQ1 and the DRs ranged between 0.277 (vitamin $B_1$) and 0.768 (fiber), and between 0.229 (zinc) and 0.859 (fat) for comparison of DRs with FFQ2. The percentages of study subjects classified into the same quartiles in both the DRs and FFQ1 ranged from 15.8% (vitamin $B_6$) to 47.4% (calcium), and for the same quartiles in DRs and FFQ2 from 13.2% (vitamin $B_1$) to 44.7% (potassium). The FFQ has reasonably good validity and reproducibility. Further research is needed for an assessment of reproducibility and validation of present FFQ in the subjects with MetS.

The efficacy and safety of Dendropanax morbifera leaf extract on the metabolic syndrome: a 12-week, placebo controlled, double blind, and randomized controlled trial

  • BACKGROUND/OBJECTIVES: The extract from Dendropanax morbifera exhibited diverse therapeutic potentials. We aimed to evaluate the efficacy and safety of D. morbifera leaf extract for improving metabolic parameters in human. SUBJECTS/METHODS: A 12-week, double blind, placebo-controlled and randomized trial included a total of 74 adults, and they were assigned to the placebo group (n = 38) or 700 mg/day of D. morbifera group (n = 36). The efficacy endpoints were changes in glycemic, lipid, obesity, and blood pressure (BP) parameters, in addition to the prevalence of metabolic syndrome (MetS) and the numbers of MetS components. Safety was assessed by monitoring adverse events (AEs). RESULTS: After 12 weeks of treatment, the hemoglobin A1c (HbA1c) level significantly decreased in the D. morbifera group compared to that of the placebo group (difference: -0.13 ± 0.20% vs. 0.00 ± 0.28%, P = 0.031; % of change: -2.27 ± 3.63% vs. 0.10 ± 5.10%, P = 0.025). The homeostatic model assessment for insulin resistance level also decreased significantly from its baseline in the D. morbifera group. The systolic BP of D. morbifera group decreased significantly than that of placebo group (difference: -3.9 ± 9.8 mmHg vs. 3.3 ± 11.7 mmHg, P = 0.005; % of change: -2.8 ± 7.7% vs. 3.3 ± 10.2%, P = 0.005). However, the lipid parameters and body composition including body weight did not differ between the groups. The prevalence of MetS (36.8% vs. 13.9%, P = 0.022) and the incidence of MetS (10.5% vs. 13.9%, P = 0.027) at 12 weeks was significantly lower in the D. morbifera group than it was in the placebo group. No serious AEs occurred in either group. CONCLUSIONS: Supplementation with D. morbifera extracts over a 12-week period improved metabolic parameters such as HbA1c and BP and reduced the prevalence of MetS.

  • Objectives: This study was designed to investigate the relationship between metabolic risk factors, Index of Nutrition Quality, and the dietary quality index score of Korean adults. Methods: The subjects were 18,652 Korean adults aged 19 years or older (7,899 males, 10,753 females) who participated in the 2016-2019 Korea National Health and Nutrition Examination Survey. Subjects were divided into normal, pre-metabolic syndrome, and metabolic syndrome (MetS) groups according to the number of their metabolic risk factors. Data were analyzed using the SPSS program. Results: About 44.7% of men in the MetS group were at least college graduates (P < 0.001), whereas 52.0% of women in the MetS group were middle school graduates or lower (P < 0.001). The frequency of fruit and dairy products intake tended to decrease as the number of metabolic risk factors increased in both men and women (P for trend < 0.001). As the number of metabolic risk factors decreased, the frequency of grain intake tended to decrease in men (P for trend < 0.001) while the frequency of intake of red meat (P for trend = 0.001), poultry (P for trend < 0.001), and eggs (P for trend < 0.001) decreased in women. The total scores of Diet Quality Index-International (DQI-I) (men P < 0.001, women P < 0.01) and Korean Healthy Eating Index (KHEI) (men and women P < 0.001) were significantly lower in the MetS group compared to the other groups, and the total score of DQI-I and KHEI tended to decrease as the number of metabolic risk factors increased. Conclusions: Dietary quality evaluation using various indices can provide more information on the dietary problems related to metabolic risk factors. Nutrients and foods that have been confirmed to be related to metabolic risk factors can be used to develop dietary guidelines for the nutritional management of metabolic diseases.

Renal Cell Carcinoma is More Aggressive in Turkish Patients with the Metabolic Syndrome

  • Background: Metabolic syndrome (MetS) is a multifactorial disease characterized by impaired glucose tolerance/diabetes, obesity, high triglyceride levels, low HDL levels, and hypertension. In this study we evaluate the relationship between tumor size and grade, and presence of the metabolic syndrome in patients with renal cell carcinoma. Materials and Methods: Between 2007-2013, radical nephrectomy was performed for 310 patients with renal tumors in our clinic and those with pathology reported renal cell carcinoma were enrolled and divided into two groups, with and without metabolic syndrome diagnosed on the basis of the National Cholesterol Education Program (NCEP) Adult Treatment Panel III (ATP III) criteria. The relationship between tumor size and grade of the two groups (Fuhrman nuclear degree) was evaluated statistically. Results: The metabolic syndrome was found in 70 patients, with a mean age of 65.5 (40-87), as compared to 58.8 (31-84) years in the non-metabolic syndrome group. Tumor size over 7 cm was found in 54% and 33%, respectively, and tumor grade over Fuhrman 3 in 56% and 32% of patients. Patients with metabolic syndrome had significantly higher tumor size and grade (p<0.05). In the presence of hypertension, diabetes and high triglyceride levels, significant assocations were again observed (p<0.05). Tumor size and degree also increased with increasing body mass index but this was not statistically significant (p>0.05). Conclusions: Renal cancer is more aggressive in patients with metabolic syndrome. Lifestyle and risk factors were revealed to be significant influences in renal cancer patients.

청소년의 근감소성 비만과 비근감소성 비만이 대사증후군 구성요소에 미치는 영향 (Influences of Sarcopenic and Non-sarcopenic Obesity on the Components of Metabolic Syndrome in Adolescents)

  • Purpose: The purpose of this study was to investigate the influences of sarcopenic and non-sarcopenic obesity on the components of Metabolic Syndrome (MetS) in adolescents. Methods: This study used the fifth Korean National Health and Nutrition Examination Survey data from 2010 to 2011. The study included 859 adolescents aged 12 to 18 years. Descriptive statistics and simple and multiple logistic regression analyses were conducted using SAS 9.2. Results: Based on the results, adolescents with sarcopenic obesity had a higher risk of central obesity (AOR: 23.41, 95% CI: 12.76-43.97), high triglyceride (OR: 4.58, 95% CI: 2.69-7.79), low HDL-cholesterol (AOR: 2.66, 95% CI: 1.74-4.05), high blood pressure (AOR: 3.44, 95% CI: 1.37-8.68), and high fasting glucose (AOR: 2.37, 95% CI: 1.13-4.96) than their normal counterparts. Adolescents with non-sarcopenic obesity had a higher risk of central obesity (AOR: 19.75, 95% CI: 9.73-44.67), high triglyceride (OR: 3.09, 95% CI: 1.22-7.81), and low HDL-cholesterol (AOR: 2.73, 95% CI: 1.37-5.45) than normal youths, and these were not significantly related to high blood pressure and fasting glucose. Conclusion: Sarcopenic obesity was more related to the components of MetS than non-sarcopenic obesity. Since adolescents with sarcopenic obesity are a more vulnerable population, a prevention and management program for MetS and cardiovascular risk should be implemented in this population.

폐경 전, 폐경 후 여성의 혈청 25-hydroxyvitamin D 수준과대사증후군 위험도의 관계 (Relationship between Serum 25-hydroxyvitamin D Concentration and the Risks of Metabolic Syndrome in Premenopausal and Postmenopausal Women)

  • Metabolic syndrome (MetS) has become a global epidemic. In particular, it is known that there is a dramatic increase in the prevalence of MetS among women during the postmenopausal period. Recently, accumulating studies have suggested that vitamin D deficiency may be inversely associated with the risk factors regarding MetS. However, evidence from postmenopausal women is limited. In this study, we examined the association between the serum 25-hydroxyvitamin D [25(OH)D] and the MetS in Korean adult women aged 20-69 years (n = 2,618) by using the 2007-2008 Korean National Health and Nutrition Examination Survey data. The geometric mean of plasma 25(OH)D were $17.16{\pm}6.28\;ng$/mL and $20.20{\pm}7.69\;ng$/mL for premenopausal and postmenopausal women, respectively. The percentages of vitamin D deficiency [25(OH)D < 12 ng/mL] were 22.5% and 14.4%, respectively. MetS was more prevalent in postmenopausal women (43.0%) compared with premenopausal women (11.2%). When serum concentrations of 25(OH)D were categorized in quintiles, there was no relationship in the prevalence of MetS in both premonopausal and postmenopausal women. However, in premenopausal women, compared with the lowest 25(OH)D quintile, the odds ratio for hypertriglyceridemia in the highest quintile was 0.57 (95% CI 0.34-0.95, $P_{trend}$ = 0.041) and for low serum HDL cholesterol 0.60 (95% CI 0.42-0.85, $P_{trend}$ = 0.014) after adjusting for all potential confounders. On the other hand, we observed the tendency of an inverse relationship for 25(OH)D regarding low serum HDL cholesterol (OR 0.78, 95% CI 0.50-1.22, $P_{trend}$ = 0.029) and a direct relationship with abdominal obesity (OR 1.94, 95% CI 1.01, 3.74, $P_{trend}$ = 0.049) in postmenopausal women. Further studies are needed to confirm these findings in other research settings.
