순환운동이 비만중년여성의 건강 체력 및 대사증후군 위험인자에 미치는 영향 (Effects of circulation exercise on Health Related Fitness and metabolic syndrome risk factors in obese middle age women)

  • 본 연구의 목적은 12주간 순환운동이 비만중년여성의 건강 체력 및 대사증후군 위험인자에 미치는 영향을 알아보기 위하여 실시하였다. $BMI\;25kg/m^2$ 이상인 40대 비만중년여성 중 순환운동군(10명), 대조군(10명)으로 총 20명을 대상으로 실시하였으며, 모든 검사항목은 동일한 방법과 조건으로 건강 체력 및 대사증후군 관련인자 대하여 사전, 사후 총 2회 측정하였다. 순환운동은 12주간, 주3회, 30~40분, 60~80%HRR 강도로 실시하였다. 측정변인에 대해 평균값과 표준편차($M{\pm}SD$)를 산출한 후 사전 운동군과 대조군의 신체적 특성과 각 측정변인에 대한 동질성 검정을 위하여 독립 t-test를, 집단 내의 사전 사후 평균치 변화에 대한 차이 검증은 종속 t-test를, 집단 간 차이에 대한 주효과 검정 및 집단 간 시기 간 상호작용 효과는 반복측정 분산분석을 이용하였다. 건강 체력에서 체지방률(p<.01)은 유의하게 감소하였으며, 제지방량(p<.05), 근지구력(p<.01), 유연성(p<.01) 및 심폐지구력(p<.001)은 유의하게 증가하였다. 대사증후군 위험인자 중 허리둘레(p<.05), SBP(p<.05), Glucose(p<.05) 및 TG(p<.001)는 유의하게 감소하였으며, HDL-C(p<.001)는 유의하게 증가하였다. 순환운동이 비만중년여성의 건강 체력 및 대사증후군 위험인자의 개선에 도움을 줄 수 있을 것으로 사료된다.

건강검진 수검자들의 대사증후군 위험인자별 유병률 및 비만지표와의 관련성 (Prevalence Rates of Risk Factors of Metabolic Syndrome, and Its Related with Obesity Indices Among the Health Checkup Examinees)

  • 본 연구는 일반 성인을 대상으로 대사증후군 위험인자의 유병률을 파악하고, 각 대사증후군 위험인자들과 비만지표들과의 관련성을 파악하며, 비만지표들의 대사증후군을 예측하기 위한 관련성과 타당도를 검토하고자 시도하였다. 조사대상은 2014년 2월부터 11월까지의 기간에 한국건강관리협회 D지부 건강검진센타에서 종합건강검진을 받았던 20세 이상 1,051명을 대상으로 하였다. 연구결과, 조사대상자의 대사증후군 유병률은 21.5%이었으며, 각 위험인자의 유병률은 허리둘레의 경우 남자 31.8%, 여자 41.6%이었으며, TG는 남자 35.6%, 여자 17.3%이었고, HDL-C는 남자 17.6%, 여자 34.2%이었다. 혈압은 남자 53.5%, 여자 35.9%이었으며, FBS는 남자 14.9%, 여자 6.8%이었다. 이 같은 유병률은 연령이 높은 군일수록, BMI가 높은 군일수록, 음주횟수가 많은 군일수록, 유의하게 높았다. 비만지표별 대사증후군 위험요인 개수별 예측도를 보면 허리둘레/신장비가 높게 나타났다. 이상과 같은 결과를 볼 때, 다른 비만지표에 비해 허리둘레/신장비가 대사증후군 위험요인을 선별하는데 좀 더 예측력이 있다고 나타났다. 허리둘레/신장비는 간단하고 실용적인 복부비만의 지표가 될 수 있으며 대사증후군에 대한 선별검사로서 체질량지수와 허리둘레보다 우수한 것으로 나타났다.

  • Type 2 diabetes mellitus in children and adolescents has been increasing for last 10 years. The increase in frequency of type 2 diabetes appears to parallel the increase in prevalence and severity of obesity in children and adolescents. The metabolic syndrome, cluster of potent risk factors for atherosclerotic cardiovascular disease and type 2 diabetes, consists of insulin resistance, obesity, hypertension and hyperlipidemia. The atherosclerotic cardiovascular disease are rarely seen in the young, but the pathologic processes and risk factors are associated its development have been shown to begin during childhood. In pediatrician it is important to recognize early and treat aggressively for prevention of future cardiovascular disease in children and adolescents with metabolic syndrome.

Relationship between inflammation biomarkers, antioxidant vitamins, and bone mineral density in patients with metabolic syndrome

  • Few studies have shown the correlation between metabolic syndrome and bone mineral density (BMD). The main pathogenic mechanisms of metabolic syndrome rely on chronic low-level inflammatory status and oxidative stress. There are few studies that examine the gender-specific effects of inflammation and antioxidants on BMD. In this study, we evaluated the relative contribution of these factors in patients with metabolic syndrome. We conducted a cross-sectional study of 67 men and 46 postmenopausal women with metabolic syndrome; metabolic syndrome was defined as having three or more metabolic syndrome risk factors. BMD, body fat mass, and lean body mass were evaluated. We also examined the levels of high sensitive C-reactive protein (hs-CRP), interleukin-6 (IL-6), adiponectin, vitamin E, and C in serum. Log-transformed hs-CRP levels were significantly higher in lumbar spine osteoporotic subjects than in normal subjects for women but not for men. There was no significant difference between the normal group and the osteoporotic group in other inflammatory markers. Stepwise regression analyses for BMD of the lumbar spine showed that lean body mass and vitamin E were significant determinants in men. Lean body mass and log-transformed hs-CRP were significant determinants in women Analysis for BMD of the femoral neck showed that lean body mass was a significant determinant for both men and women. There was no significant factor among the inflammatory markers or antioxidant vitamins affecting the femoral neck BMD for either gender. In conclusion, while hs-CRP is an independent predictor of the BMD of the lumbar spine in women, vitamin E showed profound effects on BMD in men but not women with metabolic syndrome.

Comparison of Metabolic and Anatomic Response to Chemotherapy Based on PERCIST and RECIST in Patients with Advanced Stage Non-small Cell Lung Cancer

  • Background: The aim of this study was to explore the prognostic role of metabolic response to chemotherapy, determined by FDG-PET, in patients with metastatic non-small-cell lung cancer (NSCLC). Materials and Methods: Thirty patients with metastatic NSCLC were analyzed for prognostic factors related to overall survival (OS) and progression free survival (PFS). Disease evaluation was conducted with FDG-PET/CT and contrast-enhanced CT prior to and at the end of first-line chemotherapy. Response evaluation of 19 of 30 patients was also performed after 2-3 cycles of chemotherapy. Morphological and metabolic responses were assessed according to RECIST and PERCIST, respectively. Results: The median OS and PFS were 11 months and 6.2 months, respectively. At the end of first-line chemotherapy, 10 patients achieved metabolic and anatomic responses. Of the 19 patients who had an interim response analysis after 2-3 cycles of chemotherapy, 3 achieved an anatomic response, while 9 achieved a metabolic response. In univariate analyses, favorable prognostic factors for OS were number of cycles of first-line chemotherapy, and achieving a response to chemotherapy at completion of therapy according to the PERCIST and RECIST. The OS of patients with a metabolic response after 2-3 cycles of chemotherapy was also significantly extended. Anatomic response at interim analysis did not predict OS, probably due to few patients with anatomic response. In multivariate analyses, metabolic response after completion of therapy was an independent prognostic factor for OS. Conclusions: Metabolic response is at least as effective as anatomic response in predicting survival. Metabolic response may be an earlier predictive factor for treatment response and OS in NSCLC patients.

다중인자 차원 축소 방법에 의한 대사증후군의 위험도 분석과 오즈비 (The study on risk factors for diagnosis of metabolic syndrome and odds ratio using multifactor dimensionality reduction method)

  • 대사증후군은 심혈관질환의 발생을 증가시키는 주요 요인으로 알려져 왔다. 특히 만성적인 대사장애로 여러 질병이 한 개인에게서 복합되어 나타나는 대사증후군의 경우 우리나라에서도 유병률이 점차 증가하는 추세이다. 이에 본 연구는 다중인자 차원 축소 방법을 이용하여 대사증후군의 위험도를 확인하고 여러 대사증후군 진단 조합 중에서 가장 위험한 조합을 제시하는 데 목적을 둔다. 자료는 질병관리본부에서 실시한 제 5기 국민건강영양조사 1차년도 (2010년)자료를 이용하여 성인 중에서 분석방법에 적용이 가능한 3,990명을 대상자로 결정하였다. 다중인자 차원 축소 방법을 적용시킨 결과 대사증후군에 가장 위험한 단일 요인은 복부비만이었고, 복부비만을 포함한 대사증후군 진단 조합은 복부비만과 고지혈증, 고혈압이 가장 위험한 것으로 나타났다. 이것은 대사증후군의 새로운 진단 결과이다. 특히, 남성의 경우는 복부비만, 저 HDL-콜레스테롤혈증, 고혈압이 가장 위험한 조합으로 확인되었고 체질량 지수가 $25kg/m^2$이상인 사람에게서는 복부비만, 고지혈증, 고혈당의 조합이 가장 위험한 대사증후군 조합이었다.

Metabolic Risk Profile and Cancer in Korean Men and Women

  • Objectives: Metabolic syndrome is a cluster of risk factors for type 2 diabetes mellitus and cardiovascular disease. Associations between metabolic syndrome and several types of cancer have recently been documented. Methods: We analyzed the sample cohort data from the Korean National Health Insurance Service from 2002, with a follow-up period extending to 2013. The cohort data included 99 565 individuals who participated in the health examination program and whose data were therefore present in the cohort database. The metabolic risk profile of each participant was assessed based on obesity, high serum glucose and total cholesterol levels, and high blood pressure. The occurrence of cancer was identified using Korean National Health Insurance claims data. Hazard ratios (HRs) and 95% confidence intervals (CIs) were estimated using Cox proportional hazards models, adjusting for age group, smoking status, alcohol intake, and regular exercise. Results: A total of 5937 cases of cancer occurred during a mean follow-up period of 10.4 years. In men with a high-risk metabolic profile, the risk of colon cancer was elevated (HR, 1.40; 95% CI, 1.14 to 1.71). In women, a high-risk metabolic profile was associated with a significantly increased risk of gallbladder and biliary tract cancer (HR, 2.05; 95% CI, 1.24 to 3.42). Non-significantly increased risks were observed in men for pharynx, larynx, rectum, and kidney cancer, and in women for colon, liver, breast, and ovarian cancer. Conclusions: The findings of this study support the previously suggested association between metabolic syndrome and the risk of several cancers. A high-risk metabolic profile may be an important risk factor for colon cancer in Korean men and gallbladder and biliary tract cancer in Korean women.

  • Objective: A case report on the improvement of metabolic syndrome by Ortho-Cellular Nutrition Therapy (OCNT). Methods: A 50-year-old Korean male with chronic fatigue and overlapping risk factors impaired fasting glucose, hypertension, and hypertriglyceridemia. Results: Diabetes and blood triglyceride levels improved after Ortho-Cellular Nutrition Therapy (OCNT). Conclusion: Ortho-Cellular Nutrition Therapy (OCNT) is effective in relieving the symptoms of metabolic syndrome patients.

Analysis of the association between health-related and work-related factors among workers and metabolic syndrome using data from the Korean National Health and Nutrition Examination Survey (2016)

  • BACKGROUND/OBJECTIVE: This study analyzed health-related factors for metabolic syndrome (Mets) among workers in South Korea. SUBJECTS/METHODS: This analysis included 4,666 adults aged 19-64 years to analyzed health-related risk factors for Mets from the Korean National Health and Nutrition Examination Survey (2016). The sociodemographic, working, health-related, and biochemical characteristics were presented as percentages (%) by chi-square tests. Multiple logistic regression analysis was performed to analyze the 95% confidence intervals (CIs) and associations between health-related factors characteristic of workers and the odds ratios of Mets. RESULTS: An increased prevalence of Mets was associated with male day workers compared to male shift workers (1.726-fold increase, 95% CI: 1.077-2.765), and with ${\geq}8hrs/day$ sleep duration compared to < 6 hrs/day sleep duration in female workers (2.133-fold, 95% CI: 1.041-4.368). In addition, reduced odds of high Mets were associated with male workers consumed of breakfast 5-7 times/wk compared to those consumed < 1 time/wk (0.593-fold decrease, 95% CI: 0.372-0.944). CONCLUSIONS: Health-related factors (sleep duration, frequency of breakfast) and working type in Korean workers may affect the prevalence of Mets.

Effects of a Self-Care Reinforcement Program for Socially Vulnerable Elderly Women with Metabolic Syndrome in Korea

  • Purpose: This study evaluates the efficacy of a Self-Care Reinforcement Program (SCRP) based on the Selection Optimization Compensation (SOC) model, in socially vulnerable elderly women with metabolic syndrome. Methods: This study adopts a pretest-posttest nonequivalent control group design. The participants were 64 socially vulnerable elderly Korean women with metabolic syndrome (experimental group: 31, control group: 33). Participants' body composition analysis, nutrient intake, risk factors of metabolic syndrome, depressive symptoms, and social network were measured. Data were analyzed with an independent t-test; statistical significance levels were set at p<.05. The SCRP, including metabolic syndrome education, nutritional education, exercise, and social network, was performed three times a week for 8 weeks. Results: There were statistically significant differences between the experimental and control groups in terms of systolic blood pressure, diastolic pressure, fasting blood sugar, triglycerides, sodium intake, depressive symptoms, and social networks. Conclusion: The SCRP is effective and can be recommended as a community health nursing intervention for socially vulnerable elderly women with metabolic syndrome.
