Pulse pressure is an independent risk factor for cardiovascular disease. The aim of the present study was to assess the association between pulse pressure and metabolic syndrome in Korean men. The study subjects were 8,439 adults aged 40 to 64 years, who underwent health screening examination from January 2012 to December 2014 at the Health Promotion Center of one hospital in Gyeonggi-do for general health check-up. They include the metabolic syndrome absent group (Absent, n=3,078), the pre-metabolic syndrome group (Pre-MetS, n=4,242) and the metabolic syndrome group (MetS, n=1,119). Progressive increase in pulse pressure was demonstrated for increasing components of the metabolic syndrome (P<0.001). The pulse pressure according to the degree of the metabolic syndrome was higher in the pre-MetS and MetS groups compared to the Absent group (P<0.001). Systolic and diastolic blood pressure, total cholesterol, triglyceride, fasting glucose, and abdominal obesity positively correlated with pulse pressure (P<0.001).
Objectives This study was to identify the lifestyle associated with metabolic syndrome and to suggest a personalized health management according to the constitution to prevent disease by metabolic syndrome stage. Methods This study used the data of Korean medicine Data Center (KDC). A total of 8,985 data were searched for subjects who participated in Anseong and Ansan cohorts study from 2009 to 2012. We analyzed 2,602 participants that diagnosed with metabolic syndrome among the ages of 30 to 55. We divided into three groups, none, pre-metabolic syndrome (Pre-MetS), and metabolic syndrome (MetS), according to number of metabolic syndrome elements. Results The prevalence of metabolic syndrome was highest in Taeumin (Pre-MetS: 48.2%, MetS: 41.2%). The risk factors for metabolic syndrome are dietary amount, speed of eating, and sleep quality in Taeumin, and dietary amount and sleep quality in Soyangin. Objectives: This study established a practical direction for the prevention and management of metabolic syndrome by evaluating the health status, nutrition intake level, and diet quality according to metabolic syndrome and related drug treatment in Korean adults. Methods: The data from the 2017 KNHANES (Korea National Health and Nutrition Examination Survey) was analyzed. The analysis included 2,978 adults, classified into the normal, metabolic syndrome (MetS), metabolic syndrome with medicines (MetS-M), and without medicines (MetS-noM) groups. The nutrient intake, NAR (nutrient adequacy ratio), INQ (index of nutritional quality), and DDS (dietary diversity score) were analyzed. Results: The mean BMI was significantly higher in the MetS group than in the normal group for all subjects. Subjects of the MetS group tended to consume less energy and major nutrients, while males aged 50 ~ 64 and all females showed less intake of nutrients in the MetS-M group. The energy intake ratio was within 55 ~ 65 : 7~ 20 : 15 ~ 30 of KDRI (Korean Dietary Recommended Intake), but the carbohydrate energy ratio of all subjects aged 50 to 64 was over 65%. The NAR of the major nutrients was lower in the MetS-M group, the average INQ was around 0.8, especially the INQ of calcium and vitamin A was less than 1, and the total DDS score was less than 4 points. Conclusions: This study confirmed that the nutrient intake and diet quality differed among subjects diagnosed with metabolic syndrome and managed with medical care. The intakes of energy and many nutrients, the quality of diets, and the diversity of food groups in the MetS-M group were lower than in the normal group. Therefore, these will be an important basis for establishing a specific direction of diet education for preventing and managing metabolic syndrome according to gender, age, metabolic syndrome, and drug treatment.
BACKGROUND/OBJECTIVES: The Seoul Metabolic Syndrome Management (SMESY) program is a 1-yr lifestyle modification program targeting metabolic syndrome (MetS) in Seoul residents. This study investigated the associations between adherence to dietary guidelines and MetS among the SMESY program participants. SUBJECTS/METHODS: Data of 54,385 participants aged 20-64 yrs who completed the SMESY program in 2015, had information on adherence to dietary guidelines, and were not medicated for diabetes, hypertension, or dyslipidemia were analyzed. Participants underwent MetS screening and completed a lifestyle questionnaire including adherence to 10 dietary guidelines before and after participation. Participants were classified according to the number of MetS risk factors at baseline (MetS group, ≥ 3; risk group, 1-2; healthy group, none). Adherence to dietary guidelines was determined from the number of "yes" responses regarding the fulfillment of each guideline on ≥ 5 days/week. Multiple logistic regression was used to evaluate associations between newly diagnosed MetS and changes in adherence to dietary guidelines. RESULTS: In the MetS group, MetS prevalence decreased after the SMESY program (men, -41.9%p; women, -48.7%p), and all risk factors were significantly improved (P < 0.0001). All groups exhibited improved adherence to all dietary guidelines after participation (P < 0.0001). In the MetS group with positively changed adherence scores, the MetS prevalence decreased by -44.1%p for men and -49.5%p for women, whereas the prevalence in those with negative changes decreased by -38.1%p for men and -48.6%p for women. In the risk group, those with positively changed adherence scores had significantly decreased odds ratios (ORs) for newly diagnosed MetS compared with those with negative changes (OR, 0.70; 95% confidence interval [CI], 0.61-0.80 for men; OR, 0.88; 95% CI, 0.79-0.99 for women). CONCLUSIONS: The SMESY program may effectively reduce the risk of MetS among adults with risk factors by improving adherence to dietary guidelines.
Metabolic syndrome (MetS) involves multi-factorial conditions linked to an elevated risk of type 2 diabetes mellitus and cardiovascular disease. Pre-metabolic syndrome (pre-MetS) possesses two MetS components but does not meet the MetS diagnostic criteria. Although cardiac autonomic derangements are evident in MetS, there is little information on their status in pre-MetS subjects. In this study, we sought to examine cardiac autonomic functions in pre-MetS and to determine which MetS component is more responsible for impaired cardiac autonomic functions. A total of 182 subjects were recruited and divided into healthy controls (n=89) and pre-MetS subjects (n=93) based on inclusion and exclusion criteria. We performed biochemical profiles on fasting blood samples to detect pre-MetS. Using standardized protocols, we evaluated anthropometric data, body composition, baroreflex sensitivity (BRS), heart rate variability (HRV), and autonomic function tests (AFTs). We further examined these parameters in pre-MetS subjects for each MetS component. Compared to healthy controls, we observed a significant cardiac autonomic dysfunction (CAD) through reduced BRS, lower overall HRV, and altered AFT parameters in pre-MetS subjects, accompanied by markedly varied anthropometric, clinical and biochemical parameters. Furthermore, all examined BRS, HRV, and AFT parameters exhibited an abnormal trend and significant correlation toward hyperglycemia. This study demonstrates CAD in pre-MetS subjects with reduced BRS, lower overall HRV, and altered AFT parameters. Hyperglycemia was considered an independent determinant of alterations in all the examined BRS, HRV, and AFT parameters. Thus, hyperglycemia may contribute to CAD in pre-MetS subjects before progressing to MetS.
Metabolic syndrome and homocysteine are associated with increased independent risk factors of cardiovascular disease. We investigated the difference between the degree of obesity, metabolic syndrome risk factors, plasma homocysteine and anthropometric obesity factors. Totally 398 participated in a medical check-up program were selected for this study. Subjects were categorized into three groups according to the number of metabolic syndrome components present as defined by the NCEP-ATP III criteria; Absent (0 criteria, n=124), Pre-MetS (1-2 criteria, n=220) and MetS (${\geq}3$ criteria, n=54). Body mass index (BMI) is a measure used to distinguish between normal weight, overweight and obesity. MetS presented higher homocysteine than Absent (p<.05) and obesity higher than normal weight (p<.01). When Absent+Pre-MetS was used to classify obese or not, obesty presented higher homocysteine than non-obese (p<.05). Further homocysteine levels positively correlated with weight, BMI, waist circumference, hip circumference and waist-hip ratio (WHR). Especially WHR is not only MetS (r=0.378, p<.001) but also Absent+Pre-MetS (r=0.305, p=0.029) significantly positively correlated with homocysteine. The results of our study indicate that homocysteine is related closely to obesity. Although obesity has not been diagnosed with metabolic syndrome, obesity related with increased homocysteine.
This study was performed to investigate the characteristics of nutrient intake according to metabolic syndrome in Korean elderly. Data on 1,200 elderly (men 545, women 655) over 65 years was obtained from the Korea National Health and Nutrition Examination Survey 2010. The subjects were divided into metabolic syndrome (MetS) and normal groups based on NCEP-ATP III guidelines and by applying the Asia-Pacific definition with waist circumference. The prevalence of MetS was 48.9% in all subjects, 35.6% in men and 58.9% in women, respectively. All nutrient intake was significantly lower in MetS than normal subjects, except iron, vitamin A, carotene, and retinol. In the carbohydrate: protein: fat (C:P:F) ratio, MetS showed a higher rate for carbohydrate, but a lower rate for protein and fat than normal. MetS was lower in calcium, vitamin B1, and vitamin C by the nutrient adequacy ratio than normal. Mean adequacy ratio of MetS (0.69) was lower than that of the normal group (0.72). These results demonstrated that undernutrition, not excess intake of specific nutrients - energy, fat, or cholesterol was associated with the risk for MetS in Korean elderly with MetS. Therefore, it is recommended to have a healthy dietary pattern with diverse nutrients for the prevention of MetS.

Objectives : The study was done to verify the relation between early-onset androgenetic alopecia(AGA) and metabolic syndrome(MetS). Methods : Data were collected through electronic database including KoreaMed, National Assembly Library, KMBASE, NDSL, KCI, KERIS, Google Scholar, Pubmed, Cochrane CENTRAL and EBSCO MEDLINE. A total of 13 case-control studies related to the MetS of early-onset alopecia patients were used for the systematic review and meta-analysis. Risk of bias of included studies were assessed by RoBANS tool. RevMan5.3, CMA3 were used for the meta-analysis. Results : In 13 evaluated articles, most frequent bias was the participant selection bias that was found in 10 articles. Significant association between early-onset AGA and MetS was found in 10(76.9%) out of 13 articles in the systematic review. In meta-analysis, early-onset male AGA was associated with increased risk of metabolic syndrome(OR: 3.73, 95% CI:2.49 -5.61). Conclusions : AGA, particularly early -onset male AGA, is significantly associated with MetS. Therefore all patients with early onset male AGA should be suggested to take preventive treatment to reduce the risk of MetS and various problems associated with it.
Objectives: This study evaluated dietary behavior and nutritional status according to the metabolic syndrome status in Korean menopausal women. Methods: The subjects were 1,392 menopausal women aged 50 to 64 who took part in the Korea National Health and Nutrition Examination Survey of 2016 and 2017. Subjects were classified into normal (NOR) group, pre-metabolic syndrome (Pre-MetS) group, and metabolic syndrome (MetS) groups according to the number of metabolic syndrome risk factors present. Results: The overall prevalence of metabolic syndrome was 33.7%. Using the NOR group as a reference, the odds of belonging to the MetS group in Model 1 adjusted for age were higher at 53% (OR = 1.53, 95% CI:1.011-2.307) for 'not used' subjects compared to 'used' subjects of the nutrition labeling system. Using the NOR group as a reference, every 1g increase in the intake of monounsaturated fatty acids (MUFA) and polyunsaturated fatty acids (PUFA) decreased the odds of belonging to the MetS group in Model 1 adjusted for age by 3% (MUFA, OR = 0.97, 95% CI:0.946-0.991; PUFA, OR = 0.97, 95% CI:0.942-0.993). Conclusions: These results suggest that to reduce the number of risk factors of metabolic syndrome in menopausal women, nutritional education should emphasize the adequate intake of riboflavin, unsaturated fatty acids, protein, and calcium, and also encourage the recognition and use of nutritional labeling. Results of this study are expected to be utilized as basic data for the health management of menopausal women.
Purpose: The prevalence of metabolic syndrome (MetS), a cluster of metabolic abnormalities has rapidly increased in Korea. Sleep may play a role in determining its prevalence. However, the relationships between MetS and the duration and quality of sleep are not yet clear. This study aimed to examine the associations between the duration and quality of sleep and the prevalence of MetS. Methods: Study participants included 348 Korean blue-collar workers employed by six small-sized companies in Korea. The data were collected using an interviewer-administered questionnaire, and logistic regression analysis was conducted to estimate the effects of the factors related to MetS. Results: The multiple logistic regression analysis revealed that the independent factors that contributed to the prevalence of MetS were being male (adjusted odds ratio [aOR]=4.87, 95% confidence interval [CI]=1.58~15.0) and lower sleep quality (aOR=5.12, 95% CI=1.90~14.30). Sleep duration was related to the prevalence of some MetS components, but it was not associated with MetS prevalence. Conclusion: Sleep quality was negatively associated with MetS prevalence when covariates, such as sleep duration, were controlled. When developing a MetS risk-reduction program, focus should be given to sleep quality as well as sleep duration in an intervention for Korean blue-collar workers.
