Assessment of Hepatic Cytochrome P450 3A Activity Using Metabolic Markers in Patients with Renal Impairment

  • Background: The renal function of individuals is one of the reasons for the variations in therapeutic response to various drugs. Patients with renal impairment are often exposed to drug toxicity, even with drugs that are usually eliminated by hepatic metabolism. Previous study has reported an increased plasma concentration of indoxyl sulfate and decreased plasma concentration of $4{\beta}$-hydroxy (OH)-cholesterol in stable kidney transplant recipients, implicating indoxyl sulfate as a cytochrome P450 (CYP) inhibiting factor. In this study, we aimed to evaluate the impact of renal impairment severity-dependent accumulation of indoxyl sulfate on hepatic CYP3A activity using metabolic markers. Methods: Sixty-six subjects were enrolled in this study; based on estimated glomerular filtration rate (eGFR), they were classified as having mild, moderate, or severe renal impairment. The plasma concentration of indoxyl sulfate was quantified using liquid chromatography-mass spectrometry (LC-MS). Urinary and plasma markers ($6{\beta}$-OH-cortisol/cortisol, $6{\beta}$-OH-cortisone/cortisone, $4{\beta}$-OH-cholesterol) for hepatic CYP3A activity were quantified using gas chromatography-mass spectrometry (GC-MS). The total plasma concentration of cholesterol was measured using the enzymatic colorimetric assay to calculate the $4{\beta}$-OH-cholesterol/cholesterol ratio. The correlation between variables was assessed using Pearson's correlation test. Results: There was a significant negative correlation between MDRD eGFR and indoxyl sulfate levels. The levels of urinary $6{\beta}$-OH-cortisol/cortisol and $6{\beta}$-OH-cortisone/cortisone as well as plasma $4{\beta}$-OH-cholesterol and $4{\beta}$-OH-cholesterol/cholesterol were not correlated with MDRD eGFR and the plasma concentration of indoxyl sulfate. Conclusion: Hepatic CYP3A activity may not be affected by renal impairment-induced accumulation of plasma indoxyl sulfate.

대사증후군 환자의 영양소 섭취상태 및 식사의 질과 염증지표 농도의 상관성 (Relationship between Nutrients Intakes, Dietary Quality, and Serum Concentrations of Inflammatory Markers in Metabolic Syndrome Patients)

  • Elevated serum concentration of inflammation markers is known as an independent risk factor of metabolic syndrome (MS) and dietary intake is an important factor to control MS. The purpose of this study was to investigated the hypothesis that inflammatory indices are associated with dietary intake and diet quality index-international (DQI-I) in subjects with MS. A cross-sectional study was conducted on 156 men and 73 postmenopausal women with MS, defined by three or more risk factors of the modified Adult Treatment Panel III criteria. Serum levels of high sensitive C-reactive protein (hs-CRP), adiponectin were examined and nutrients intake and DQI-I were assessed using a semiquantitative food frequency questionnaire. The total DQI-I score was significantly higher in female subjects ($65.87{\pm}9.86$) than in male subjects ($62.60{\pm}8.95$). There was a positive association between hs-CRP and polyunsaturated fatty acid intake (p < 0.05) and a negative association between adiponectin and lipid (p < 0.05), total sugar (p < 0.01), and total fatty acids (p < 0.05). When the subjects were divided into 5 groups by quintile according to serum adiponectin and hs-CRP level, there was no association between DQI-I score and hs-CRP levels. Moderation score of DQI-I was significantly higher in highest quintile group than the lower quintile groups. Therefore, our results provide some evidence that dietary intake and diet quality are associated with inflammation markers and dietary modification might be a predictor to decrease risk for metabolic syndrome complications. However further research is needed to develop the dietary quality index reflecting the inflammatory change by considering the dietary habit and pattern of Koreans.

Capsaicin Ameliorates Cisplatin-Induced Renal Injury through Induction of Heme Oxygenase-1

  • Cisplatin is one of the most potent chemotherapy agents. However, its use is limited due to its toxicity in normal tissues, including the kidney and ear. In particular, nephrotoxicity induced by cisplatin is closely associated with oxidative stress and inflammation. Heme oxygenase-1(HO-1), the rate-limiting enzyme in the heme metabolism, has been implicated in a various cellular processes, such as inflammatory injury and anti-oxidant/oxidant homeostasis. Capsaicin is reported to have therapeutic potential in cisplatin-induced renal failures. However, the mechanisms underlying its protective effects on cisplatin-induced nephrotoxicity remain largely unknown. Herein, we demonstrated that administration of capsaicin ameliorates cisplatin-induced renal dysfunction by assessing the levels of serum creatinine and blood urea nitrogen (BUN) as well as tissue histology. In addition, capsaicin treatment attenuates the expression of inflammatory mediators and oxidative stress markers for renal damage. We also found that capsaicin induces HO-1 expression in kidney tissues and HK-2 cells. Notably, the protective effects of capsaicin were completely abrogated by treatment with either the HO inhibitor ZnPP IX or HO-1 knockdown in HK-2 cells. These results suggest that capsaicin has protective effects against cisplatin-induced renal dysfunction through induction of HO-1 as well as inhibition oxidative stress and inflammation.

Time-Dependent Hepatic Proteome Analysis in Lean and Diet-Induced Obese Mice

  • C57BL/6J mice have been widely used as a diet-induced obesity model because they trigger common features of the human metabolic syndrome. In the present study, C57BL/6J male mice were fed either a high-fat diet (HFD) or normal diet (ND) during a 24-week period, and then the age-dependent liver proteome of mice in two groups was analyzed using 2-DE combined with MALDI-TOF-MS. Among identified proteins, up-regulated proteins were subdivided to early (during the first 4 weeks) and late (20~24 weeks) markers that played a role in diet-induced obesity development. Important early markers included ketohexokinase and prohibitin, and late markers included the 75 kDa glucose-regulated protein, citrate synthase, and selenium-binding liver protein. Of these, the 75 kDa glucosere-gulated protein has already been linked to obesity; however, prohibitin protein involved in obesity was identified for the first time in this study. In order to validate the proteomic results and gain insight into metabolic changes between the two groups, we further confirmed the expression pattern of some proteins of interest by Western blot analysis. Combined results of proteomic analysis with Western blot analysis revealed that antioxidant enzymes were progressively decreased, whereas cytoskeletal proteins were time-dependently increased in HFD mice.

Subtle inflammation: a possible mechanism of future cardiovascular risk in obese children

  • Purpose: The risk of cardiovascular disease (CVD) has been shown to be associated with systemic inflammation in obese adults with metabolic syndrome (MetS). The aims of this study were to evaluate the prevalence of MetS and its relation to inflammatory markers in obese Thai children. Methods: A cross-sectional study was conducted. Children with history of endogenous obesity, chronic diseases, drug ingestion, and any acute illness within 2 weeks prior to enrollment were excluded. Their fasting blood glucose (FBG) levels, oral glucose tolerance tests, insulin, lipid profiles, and selected inflammatory markers, including interleukin-6, tumor necrosis factor-alpha, and high-sensitivity C-reactive protein (hs-CRP) levels, were tested. Results: In this study, 58 obese Thai children (female, 20; male, 38) with a mean body mass index z score of $5.1{\pm}2.2$ were enrolled. The prevalence of MetS and prediabetes was 31% and 17.2%, respectively. None of the children had diabetes. FBG levels, 2-hour glucose levels, and lipid profiles were not statistically different between those with and without MetS. However, obese children with MetS had higher insulin levels and homeostasis model assessment of insulin resistance values. Elevated hs-CRP levels were found in 69% of the cases, although it was not statistically different between the 2 groups. Conclusion: We described a substantial prevalence of MetS in Thai obese children. Regardless of MetS status, two-thirds of the obese children had elevated hs-CRP level, indicating subtle ongoing inflammatory process. This chronic inflammation feasibly predisposes them to CVD in the future, even in children without MetS.

Use of the Talk Test for Comparison of Exercise Intensity between Aerobic Activities with Treadmill and Bicycle

  • Purpose: The talk test (TT) is an alternative, self-reported method for prescribing and guiding exercise training in both healthy adults and patients with cardiovascular and pulmonary diseases. This study examined whether the TT is a valid tool for evaluating the exercise intensity during two different types of aerobic activity on a treadmill or stationary bicycle in a healthy population. Methods: A total of ten subjects (six males and four females) who had no medical history related to musculoskeletal, cardiovascular, and pulmonary disorders were enrolled in this study. They were evaluated using the TT, which consisted of three-level of difficulties demanding cardiac loads while performing aerobic activities on a treadmill and bicycle ergometer across two consecutive days in a counterbalanced manner. During the activities, the psychophysiological response markers were collected in terms of the heart rate, oxygen saturation, rating of perceived exertion, and metabolic equivalents. Results: Statistical analyses revealed a significant difference in the between-subject variance regarding the TT level effect (p<0.05). On the other hand, no significant findings were detected on the between-group variance(p>0.05) and the TT level×group interaction (p>0.05). The independent t-test indicated no significant differences in heart rate, oxygen saturation, rating of perceived exertion, and metabolic equivalents at any levels of the TT in the two groups. The TT showed a strong correlation with the rating of perceived exertion. Conclusion: This study showed that the TT is a valid and alternative tool for evaluating the aerobic exercise intensity in a healthy population. In addition, differences in the psychophysiological response markers between two aerobic activities, treadmill and bicycle ergometer, were detected in the same exercise intensity scaled with the TT. The TT can be used to evaluate and prescribe the exercise intensity of aerobic activity in cardiovascular and pulmonary physical therapy.

황백(黃柏)이 비만 유발 mouse의 대사기능에 미치는 영향 (Effects of Cortex Phellodendri on the Metabolic Function in Experimental Mouse Model of Obesity)

  • Objectives: This study was undertaken to investigate how Cortex Phellodendri affects metabolic functional change in an experimental rat model of obesity.Methods: An obesity model was induced in a C57BL/6 mouse with a high-fat diet. Mice were divided into three groups (n=6) of normal diet, high-fat diet (=control), and high-fat diet with Cortex Phellodendri. After 12 weeks, we measured the three mice groups’ body weight, FBG, FBI, HOMA-IR, OGTT, the weight of epididymal fat and liver, the percentage of ATM, and the gene expression of TNF-α, IL-10, and CD68.Results: Cortex Phellodendri significantly reduced blood glucose and oral glucose tolerance levels. It also reduced ATM numbers and TNF-α and CD68 gene expression and increased IL-10 gene expression.Conclusions: This study suggests that Cortex Phellodendri normalized the blood glucose and reduced the expression of inflammatory markers. However, with respect to other indicators of metabolic function in obesity, there were no significant results.

Effects of Panax ginseng on hyperglycemia, hypertension, and hyperlipidemia: A systematic review and meta-analysis

  • Panax ginseng is a medicinal plant is a material with various pharmacological activities and research suggests that it is particularly effective in representative metabolic diseases such as hyperglycemia, hypertension, and hyperlipidemia. Therefore, in this study, systematic review and meta-analysis were performed to investigate the comprehensive effect of P. ginseng on metabolic parameters representing these metabolic diseases. A total of 23 papers were collected for inclusion in the study, from which 27 datasets were collected. The investigational products included P. ginseng and Korean Red ginseng. Across the included studies, the dose ranged from 200 mg to 8 g and the supplementation period lasted from four to 24 weeks. The study subjects varied from healthy adults to those with diabetes, hypertension, obesity, and/or hyperlipidemia. As a result of the analysis, the levels of glucose and insulin area under the curves, % body fat, systolic and diastolic blood pressures, total cholesterol, triglycerides, and low-density lipoprotein cholesterol were significantly reduced in the P. ginseng group as compared with in the placebo group. In conclusion, P. ginseng supplementation may act as an adjuvant to prevent the development of metabolic diseases by improving markers related to blood glucose, blood pressure, and blood lipids.

한국 성인 남녀의 커피 섭취와 건강관련 삶의 질 및 대사증후군과의 관련성 : 2013 ~ 2016 국민건강영양조사 자료를 이용하여 (Association of coffee consumption with health-related quality of life and metabolic syndrome in Korean adults: based on 2013~ 2016 Korea National Health and Nutrition Examination Survey)

  • 본 연구에서는 2013 ~ 2016년 국민건강영양조사 자료를 이용하여 19 ~ 64세 성인 남녀의 커피 섭취 종류와 섭취량(정량적 식품섭취빈도조사 자료 활용)에 따른 일반사항, 총 당류를 포함한 영양섭취상태, 건강관련 삶의 질 및 대사증후군과의 관련성을 분석하였다. 남성과 여성 모두에서, 에너지 섭취량이 에너지필요추정량 (EER) 미만인 사람과 철분 섭취량이 평균필요량 (EAR) 미만인 사람의 비율이 3-in-1 커피 섭취군에서 낮았다. 커피를 섭취하지 않는 여성과 비교했을 때, 3-in-1 커피를 하루 2회 이하로 섭취하는 여성은 EQ-5D를 통해 조사한 건강 관련 삶의 질저하에 대한 위험이 19% 낮은 것으로 나타났으며, 남성과 여성 모두에서 섭취하고 있는 커피의 종류와 섭취량에 따라 대사증후군 관련 지표 및 대사증후군에 대한 위험에 차이가 없었다. 따라서 3-in-1 커피 섭취가 건강관련 삶의 질저하에 대한 낮은 위험과 관련이 있었으나, 대사증후군 및 관련 지표에 대한 위험과는 관련이 없다는 결론을 얻을 수 있었다. 앞으로 3-in-1 커피 섭취가 건강관련 삶의 질 및 대사증후군 나아가 사망률까지 이어지는 건강에 미치는 영향에 대한 기전을 파악하기 위해 이와 관련하여 잘 설계된 무작위 임상비교시험이나 코호트 연구가 필요하다.

한국인의 계란 섭취와 대사증후군과의 관계: 2007-2008 국민건강영양조사 자료를 이용하여 (Egg Consumption and Prevalence of Metabolic Syndrome in Korean Adults: Based on 2007-2008 Korean National Health and Nutrition Examination Survey)

  • There is a great public concern that higher consumption of eggs may increase the risk of chronic disease due to high levels of cholesterol in eggs. In contrast to this concern, limited studies have examined this relationship, and the study results have been inconsistent. This study was to evaluate the cross-sectional associations between egg consumption, blood markers and metabolic syndrome in Korean adults. We analyzed data from the combined 2007-2008 Korean National Health and Nutrition Examination Survey (KNHANES). Egg intakes were assessed using a food frequency questionnaire. Metabolic syndrome was ascertained using criteria of the NCEP ATP III (2001) and abdominal obesity criteria of the WHO. In 5,548 subjects, 937 subjects were defined as having metabolic syndrome. In linear regression models, subjects reporting one or more egg consumption per day had significantly lower levels of triglyceride, fasting blood glucose and blood pressure, and a higher level of HDL cholesterol, compared to those who rarely consumed eggs (P-trends < 0.05). After adjustments for potential confounding factors, the associations were no longer significant except for total cholesterol, showing higher levels with frequent egg consumption. More frequent intakes of egg (1 egg or more/day) were associated with lower risk of metabolic syndrome in a logistic regression model (OR: 0.69, 95% CI: 0.50-0.96), but this association also became insignificant after adjustments for multiple confounding factors (OR: 1.05, 95% CI: 0.71-1.57). In healthy Korean adults, frequent intakes of eggs did not adversely affect the risk of metabolic syndrome. Further studies are needed to confirm this finding.
