성견 열개형 수평골 결손부에서 탈회냉동건조골과 Calcium Sulfate 차단막이 치주조직 치유에 미치는 영향 (The Effects of Graft of DFDB combined with Calcium Sulfate membrane on the Periodontal Wound Healing of Horizontal Dehiscence Defects in Dogs)

  • The present study investigates the effects of DFDB graft combined with Calcium sulfate membrane on the periodontal wound healing in dehiscence defects of dogs. Following the initiation of general anesthesia by I.V. administration of 30mg/kg of pentobarbital, first premolar was extracted and full-thickness flap was elevated from the second to the fourth premolar. The portion of premolars coronal to the alveolar crest was removed and mesial and distal roots separated to produce single rooted teeth. Exposed root canals were sealed with Caviton and covered completely with flaps sutured. Following the healing period of 12 weeks, the surgical sites were uncovered and $4{\times}4mm$ dehiscence defects were surgically created. Those defects with DFDB graft combined with Calcium sulfate membrane following root planing, were designated as test sites and those with flap surgery-only were designated as controls. 1. No foreign-body reaction or inflammation were observed in either groups. Calcium sulfate was completely resorbed in the test sites. 2. Significantly greater amounts of new cementum was observed in test sites compared with the controls. Significant amounts of functionally orientated collagens were observed in the test sites. 3. New bone formation was observed in significantly greater amounts in test sites. The results suggest that combined graft of DFDB and calcium sulfate is extremely biocompatible with a potential for new bone and cementum formation, and functional alignment of periodontal ligaments.

치은연하 치석제거와 치근면 활택술시 Gracey curet과 Mini-five curet의 치석제거 효과에 대한 비교 연구 (A Comparison of effectiveness of Gracey curet and Mini-five curet on subgingival scaling and root planing)

  • Removal of subgingival calculus is essential for the success in periodontal treatment. Subgingival instrumentation is used for the removal of all bacterial plaque and calculus. In this study, two types of anterior curet were used on ant. teeth to conduct subgingival scaling and root planing. The remaining amount of calculus was evaluated according to type of instrument, depth of pocket, and tooth surface. 24 teeth extracted from patients being treated at Dan Kook University dept. Perio. were used. 4 surfaces per tooth a total of 96 areas were evaluated. 12 teeth treated with Gracey No. 1-2 was used as the control group and 12 teeth treated with Mini-five curet No. 1-2 was the experimental group. The 4 surfaces of the teeth {buccal, mesial, lingual or palatal, distal) were observed under a stereomicroscope and the images were captured 3 times per surfaced with a CCD. The image were observed on the monitor using a $10{\times]10$ grid produced with the Microsoft power point. The amount of calculus remaining was evaluated 3 times per surface. The results were as follows. 1. There was no significant difference in remaining calculus according to the pre-treatment pocket depth, and tooth position{Mx. or Mn). 2. The Mini-five curet showed better results than the Gracey curet but there was no statistically significant difference. 3. In both Gracey curet group and Mini-five curet group the lingual(or palatal) surface showed significant difference compared to the other surfaces(p<0.05). From the results above, it is thought that when treating ant. teeth consideration of the tooth surface is more important than the choice of instrument.

Konus Telescopic Denture의 내관 각도 및 높이에 따른 지대치 및 지지조직의 응력 분석 (Stress Analysis of Abutment and Supporting Tissues by Changing Angles and Heights of Konus Telescope Inner Crown)

  • This study was to evaluate and to compare the compressive strength and the displacement effecting the abutment or the residual ridge which are transformed by the angle and the heights of the konus denture inner crown when restorating the unilateral konus denture by using the mandibular canine and the 1st premolar as an abutment. The author made 9 different models for different inner crown heights and konus angles. The inner crown height were divided to 5mm, 6mm, and 7mm and konus angles was divided to $4^{\circ}$, $6^{\circ}$, and $8^{\circ}$. And then in each model, 5kg of $15^{\circ}$ mesial load was stressed on the central fossa of the 1st premolar and the 1st molar. The stresses and displacement were measured using the finite element analysis. The results were as follows 1. The maximum compressive strength was shown on the connective area of the abutment and the denture base. 2. As the angle of the inner crown becomes increased, the compressive strength was shown smaller. 3. As the height of the inner crown becomes increased, the maximum compressive strength was shown smaller while the compressive strength of the root apex and the residual ridge showed larger. 4. When the stress was loaded only on the 1st premolar, the more compressive strength was concentrated on the root apex area of the 1st premolar. 5. When the stress was loaded only on the 1st premolar, the compressive strength was concentrated uniformly on the abutment and the residual ridge. 6. When the stress was loaded only on the 1st molar, the maximum displacement was shown on the distal part of the residual ridge.

수종 근관확대 기구의 만곡된 치근관 성형능력 (SHAPING ABILITY OF ROOT CANAL INSTRUMENTS ON CURVED ROOT CANALS)

  • Four endodontic instruments from different manufacturers(K - file, Hedstrom - file, K - flex - file, Cavi - Endo) were used to shape 80 mesial canals of extracted posterior teeth. The instrument's ability to enlarge and shape the canals as the original canal shape was assessed by superimposing radiographs of canals obtained before and after canal shaping. The results were as follows 1. Hedstrom - file enlarged canals greater than K - file, K - flex - file, and Cavi - Endo(P<0.05). There are no differences in canal enlargement among K - flex, K - flex - file, and Cavi - Endo(P<0.05). 2. All the instruments showed no difference in canal enlargement between convex side and concave side of B point in proximal and clinical radiographic views, but at the concave side of C point canals were enlarged greater than at the convex side(proximal view of K - file: P<0.05, the others: P<0.01). 3. There was no difference m canal enlargement between convex side and concave side in proximal view of A and D points of K - file and K - flex - file(P<0.05). But in clinical view, canal enlargement at convex side of A point and concave side of D point was greater than that at concave side of A point and convex side of D point respectively. 4. Hedstrom - files showed greater canal enlargement at convex side of A points and concave side of D points (P<0.05 or P<0.01). Cavi - Endo showed no difference in canal enlargement between convex side and concave side at A and D points in proximal view(P<0.05). Bur canal enlargement was greater at concave side than at convex side of D point in clinical view. 5. K - file and Cavi - Endo showed no differences in canal enlargement between convex and concave side in proximal and clinical view (P<0.05). K - flex - file showed greater canal enlargement at concave side in proximal and clinical view(P<0.0:5). Hedstrom file showed no difference in canal enlargement between convex side and concave side in proximal view, but showed greater canal enlargement at concave side in clinical view(P<0.05).

Multivariate analysis of the cleaning efficacy of different final irrigation techniques in the canal and isthmus of mandibular posterior teeth

  • Objectives: The aim of this study was to compare the cleaning efficacy of different final irrigation regimens in canal and isthmus of mandibular molars, and to evaluate the influence of related variables on cleaning efficacy of the irrigation systems. Materials and Methods: Mesial root canals from 60 mandibular molars were prepared and divided into 4 experimental groups according to the final irrigation technique: Group C, syringe irrigation; Group U, ultrasonics activation; Group SC, VPro StreamClean irrigation; Group EV, EndoVac irrigation. Cross-sections at 1, 3 and 5 mm levels from the apex were examined to calculate remaining debris area in the canal and isthmus spaces. Statistical analysis was completed by using Kruskal-Wallis test and Mann-Whitney U test for comparison among groups, and multivariate linear analysis to identify the significant variables (regular replenishment of irrigant, vapor lock management, and ultrasonic activation of irrigant) affecting the cleaning efficacy of the experimental groups. Results: Group SC and EV showed significantly higher canal cleanliness values than group C and U at 1 mm level (p < 0.05), and higher isthmus cleanliness values than group U at 3 mm and all levels of group C (p < 0.05). Multivariate linear regression analysis demonstrated that all variables had independent positive correlation at 1 mm level of canal and at all levels of isthmus with statistical significances. Conclusions: Both VPro StreamClean and EndoVac system showed favorable result as final irrigation regimens for cleaning debris in the complicated root canal system having curved canal and/or isthmus. The debridement of the isthmi significantly depends on the variables rather than the canals.

Evaluation of mandibular buccal shelf characteristics in the Colombian population: A cone-beam computed tomography study

  • Objective: To evaluate the mandibular buccal shelf (MBS) in terms of the angulation and bone depth and thickness according to sex, age, and sagittal and vertical skeletal patterns in a Colombian population using cone-beam computed tomography (CBCT). Accordingly, the optimal site for miniscrew insertion in this area was determined. Methods: This descriptive, retrospective study included 64 hemi-arches of 34 patients. On CBCT images, the angulation, buccal bone depth (4 and 6 mm from the cementoenamel junction [CEJ] of MBS), and buccal bone thickness (6 and 11 mm from the CEJ of MBS) were measured at the mesial and distal roots of the mandibular first and second molars. Results: There were no statistically significant differences in the angulation, depth, and thickness of MBS between male and female patients. The values for the bone around the distal root of the mandibular second molar were significantly greater than the other values. The osseous characteristics were significantly better in participants aged 16-24 years. Class III patients exhibited the best osseous characteristics, with the bone depth at 6 mm being significantly different from that in Class I and Class II patients. Although values tended to be greater in patients with low angles, the difference was not statistically significant. Conclusions: MBS provides an optimal bone surface for miniscrew insertion, with better osseous characteristics at the distal root of the mandibular second molar, 4 mm from CEJ. Adolescent patients, Class III patients, and patients with a low angle exhibit the most favorable osseous characteristics in the MBS area.

Shape and anatomical relationship of the mental foramen to the mandibular premolars in an Indian sub-population: a retrospective CBCT analysis

  • Objectives: This study assessed the shape and anatomical relationship of the mental foramen (MF) to mandibular posterior teeth in an Indian sub-population. Materials and Methods: In total, 475 existing cone-beam computed tomography records exhibiting 950 MFs and including the bilateral presence of mandibular premolars and first molars were assessed. Images were evaluated 3-dimensionally to ascertain the position, shape, and anatomical proximity of MFs to mandibular teeth. The position and shape of MFs were measured and calculated. The Pythagorean theorem was used to calculate the distance between the root apex of the mandibular teeth and the MF. Results: MFs exhibited a predominantly round shape (left: 67% and right: 65%) followed by oval (left: 30% and right: 31%) in both males and females and in different age groups. The root apices of mandibular second premolars (left: 71% and right: 62%) were closest to the MF, followed by distal to the first premolars and mesial to the second premolars. The mean vertical distance between the MF and the nearest tooth apex calculated on sagittal sections was 2.20 mm on the right side and 2.32 mm on the left side; no significant difference was found according to sex or age. The distance between the apices of the teeth and the MF was ≥ 4 mm (left; 4.09 ± 1.27 mm and right; 4.01 ± 1.15 mm). Conclusions: These findings highlight the need for clinicians to be aware of the location of the MF in treatment planning and while performing non-surgical and surgical endodontic procedures.

국소의치 최후방 지대치 유도면의 기울기와 형태가 지대치 및 지지조직의 응력분산에 미치는 영향 (A STUDY OF THE STRESS DISTRIBUTION OF THE ABUTMENT AND SUPPORTING TISSUES ACCORDING TO THE SLOPES AND TYPES OF CHIDING FLAMES OF THE LAST ABUTMENT IN DISTAL EXTENSION REMOVABLE PARTIAL DENTURE USING THREE DIMENSIONAL FINITE ELEMENT ANALYSIS METHOD)

  • 하악 우측 제1대구치 및 제2대구치가 손실된 2급하악 후방연장 국소의치에서 무치악 부위 지대치 유도면의 기울기와 형태가 지지조직에 미치는 응력 및 변위를 비교 분석하기 위하여 3차원 유한요소법을 이용하여 알아보았다. Kratochvil 형태의 유도면의 기울기가 잔존치조제에 대하여 $90^{\circ}$인 경우, $95^{\circ}$인 경우, 그리고 $100^{\circ}$인 경우와 Krol형태의 유도면의 기울기가 $90^{\circ}$인 경우에 있어서 제2소구치를 지대치로 하고 RPI형태의 유지장치를 사용하는 경우를 가정하여 모델링한 다음 제1대구치 및 제2대구치의 중심와에 150N의 하중을 가하여 응력 및 변위에 대하여 실험한 결과 다음과 같은 결론을 얻었다. 1. Kratochvil의 유도면 형태 중 기울기가 $90^{\circ}$인 경우와 Krol의 유도면 형태에서는 유사한 응력 분포를 나타내었으며 특히 지대치에서 치근단 방향으로 응력이 향하는 것으로 나타났다. 2. 유도면의 기울기가 $95^{\circ}$인 경우에서는 지대치의 치근단 부위에 응력이 증가하면서 근심방향으로 향하였으며, $100^{\circ}$인 경우에서는 치근단의 응력이 협측 및 근협측 방향으로 집중되어 나타났다. 3. 유도면의 기울기가 $90^{\circ}$인 경우에서는 지대치의치근이 조금 시계방향으로 비틀리는 양상으로 나타났으며, Krol의 유도면 형태에서는 지대치가 치근단 부위에서는 원설측으로 치경부에서는 근협측으로 변위되었다. 4. 유도면의 기울기가 $95^{\circ}$인 경우에서는 지대치의 치근은 치근단 부위에서 근설측으로 약간 변위되었고 치경부 방향으로 갈수록 반시계 방향으로 비틀리면서 보다 많이 변위되었으며, $100^{\circ}$인 경우에서는 지대치 치근단에서는 근협측으로 변위되었고 치경부방향으로 갈수록 시계방향으로 비틀리면서 보다 많이 변위되었다.

  • The purpose of this study is to observe how the Class III intermaxillary elastics act upon the craniofacial structures of mixed dentition with anterior crossbites. The cephalometric headplafes of 16 childrens treated only with Class III elastics (C III elastics group) and 23 childrens treated with Class III elastics and Reverse pull headgear simultaneously (C III elastics + RPHG group) were traced, digitized and statistically analyzed. The results were as follows. 1 . Anterior displacement of maxilla was observed in both groups. 2. Simultaneous Class III elastics and reverse pull headgear group showed the counterclockwise rotation of maxilla, however Class III elastics group manifested no rotational change of maxilla. 3. Simultaneous Class III elastics and reverse pull headgear group showed the downward and backward rotation of mandible, however Class III elastics group manifested no rotational change of mandible. 4. Counterclockwise canting of occlusal plane, labial tipping of upper incisor and lingual tipping of lower incisor and mesial displacement of upper molar to pterygoid root vertical were observed in both groups.

Miniplate를 골격성 고정원으로 사용한 심도있는 매복 하악 제2대구치의 맹출 치료: 증례보고 (Use of a Miniplate for Skeletal Anchorage in the Forced Eruption of a Severely Impacted Mandibular Second Molar: Case Report)

  • Tooth impaction represents the stop of eruption by clinical and radiographical disturbance in eruption path or the dislocation of tooth germ. The most common factor in tooth eruption disorders are spacial deficiency with other causes reported to be odontogenic tumors, periodontal ligament injury, etc. Impaction of the mandibular second molar is relatively rare and reported in about 3 out of 1,000 people. Because the second molars tend to erupt in a mesial direction, this situation can lead to serious problems if untreated, including dental caries, periodontal disease and root resorption of the first molar. Treatment of this problem includes, surgical repositioning and orthodontic forced eruption. Because each procedure have the definite advantages and disadvantages, and influenced by circumferential environment, these have limits for successfu1 recovery as independent treatment. In a case at St. Mary's Hospital, we performed successful correction of a horizontal impacted mandibular second molar using a miniplate skeletal anchorage system. We introduce this treatment as a valid method for an impacted second molar and consider a oromaxillofacial surgeon's role in tooth movement treatment.
