육총혈에 배속되는 경혈에 관한 연구 (Research for the Construction of Six Command Points)

  • Objectives : Six command points are individual points that have strong and broad effects on their related part of the body, but there is a controversy about which acupoints are should be included in the six command points. This research was reviewed which acupoints are suitable for the construction of the six command points. Methods : We searched acupoints mentioned six command points in literatures, then investigated the effects of the acupoints to identify that they can be included in the six command points. And we deliberated which acupoints are suitable for the six command points. Results : Acupoints mentioned the six command points are LU7, LI4, ST36, BL40, PC6, TE6, GV26, ouch point, GB34 and SP6. Among the acupoints, LU7, LI4, ST36, BL40 and PC6 was included in the six command points in all of searched literatures. And GV26, ouch point, GB34 and SP6 were excluded because GV26 and ouch point were not fit the definition of the six command points, ST36 can be an alternative to SP6 based on their effects, and GB34 has a similar but little effect compared to TE6. Conclusions : Based on the definition of six command points and the effect of the acupoints, we suggest that the six command points should be composed of LU7, LI4, ST36, BL40, PC6 and TE6.

드퀘르벵 증후군의 온침 및 화침 치료에 대한 최신 연구 동향 (A Review of Recent Studies about Fire and Warm Needling for De Quervain Syndrome)

  • Objectives : The purpose of this study is to examine the efficacy of fire needling and warm needling for De Quervain Syndrome by reviewing clinical studies for recent 10 years. Methods : Randomized controlled trials, non-randomized controlled trials, and case series containing more than 20 cases about fire needling and warm needling for De Quervain Syndrome published since 2011 were searched through four foreign online databases (CNKI, Pubmed, EMBASE, Cochrane Library) and five Korean online databases (OASIS, Science ON, DBPIA, KISS, RISS). The number and characteristics of participants, treatment points and main treatment methods involving other combination treatments, treatment cycle or total periods of treatments, evaluation indices, efficacy, and adverse events were analyzed. Risk of bias of included randomized clinical trials was assessed using a revised tool for assessing risk of bias in randomized trials (RoB 2). Results : A total of 6 randomized clinical trials and 2 case series involving 471 participants were included. Tender point or 'Ashi point' was the most commonly used treatment point, followed by LU4. Treatment frequency ranged from once a day to once a week. One to three outcome measures were used to evaluate the results of the studies, with the efficacy rate the most frequently used, followed by visual analogue scale. Overall risk of bias of all included randomized clinical trials was judged to have some concerns. Conclusions : All selected studies showed that fire needling and warm needling treatments for De Quervain syndrome were more effective than other clinical methods or acupuncture treatments. However, as the number of clinical studies is still too small and the risk of bias of the studies is not low, it is believed that more systematic and objective studies should be conducted.

수구(水溝; GV26) 위치와 주치증의 관련성 및 해부학적 상관성 고찰 (Review of GV26 Point Locations and Indications and Anatomical Correlation of the Point)

  • Objectives : This study examined the historical evolution of the GV26 acupuncture point, focusing on its location, therapeutic indications, and anatomical relevance to the philtrum region. Methods : We reviewed 18 historical acupuncture texts, analyzing the descriptions of GV26's location and associated indications. Results : The location of GV26 has varied over time. Early texts like the A-B Classics of Acupuncture and Moxibustion described it at the center of the philtrum groove, while some 16th-century texts shifted it closer to the nasal cavity. However, many sources continued to emphasize the central philtrum groove as the correct location. GV26 was historically indicated for conditions such as cold-type headache, epilepsy, edema, nasal and eye symptoms, respiratory issues, post-stroke symptoms, jaundice, pestilence, mouth-related symptoms, and low back pain. Epilepsy, edema, and post-stroke facial symptoms were consistently cited across texts, while other indications varied. Cadaveric studies show that the superficial orbicularis oris muscle divides into the upper nasalis and lower labialis portions. Needling the upper third of the philtrum stimulates the thinner, closer-to-maxilla upper nasalis portion, potentially providing stronger stimulation, while needling the center targets the lower labialis portion. Conclusions : Historical variations in GV26's location reflect an evolving understanding of its anatomical and therapeutic significance. The upper third of the philtrum groove may be more effective for acute conditions requiring strong stimulation, whereas the central groove may better address a broader range of conditions. These findings highlight the importance of historical and anatomical context in optimizing GV26's clinical use.

Ganoderma lucidum Pharmacopuncture for the Treatment of Acute Gastric Ulcers in Rats

  • Objectives: The gastric ulcer is a common disorder of the stomach and duodenum. The basic physiopathology of a gastric ulcer results from an imbalance between some endogenous aggressive and cytoprotective factors. This study examined whether Ganoderma lucidum pharmacopuncture (GLP) would provide protection against acute gastric ulcers in rats. Methods: Sprague-Dawley rats were divided randomly into 4 groups of 8 rats each: normal, control, normal saline (NP) and GLP groups. The experimental acute gastric ulcer was induced by using an EtOH/HCl solution and the normal group received the same amount of normal saline instead of ethanol. The NP and the GLP groups were treated once with injections of saline and GLP, respectively. Two local acupoints were used: CV12 (中脘) which is the alarm point of the Stomach Meridian, and ST36 (足三里), which is the sea point of the Stomach Meridian. The stomachs from the rats in each group were collected and analyzed for gross appearance and histology. Also, immunohistochemistry staining for BAX, Bcl-2 and TGF-${\beta}1$ was performed. Results: Histological observations of the gastric lesions in the control group showed comparatively extensive damage of the gastric mucosa and necrotic lesions had penetrated deeply into the mucosa. The lesions were long, hemorrhagic, and confined to the glandular portions. The lesions were measured microscopically by using the clear depth of penetration into the gastric mucosal surface. The length and the width of the ulcer were measured and the inhibition percentage was calculated. Wound healing of the acute gastric ulcer was promoted by using GLP, and significant alterations of indices in gastric mucosa were observed. Such protection was shown by gross appearance, histology and immunohistochemistry staining for BAX, Bcl-2 and TGF-${\beta}1$. Conclusion: These results suggest that GLP administered at CV12 and ST36 can provide significant protection to the gastric mucosa against an ethanol-induced acute gastric ulcer.

족삼리 애구가 고혈압 환자의 혈압 강하에 미치는 영향 ; 무작위배정 임상연구 (Effect of Choksamni(족삼리, $ST_{36}$) Moxibustion on Blood Pressure Elevation in Hypertensive Patients: A Randomized Controlled Trial.)

  • Objectives : The purpose uf this study was to evaluate the effect of Choksmni$(ST_{36})$ moxibustion in hypertensive patients who showed sudden elevation of blood pressure. Methods : Among patients admitted to Woosuk University Hospital from June to September 2004, sixty-one patients who had shown sudden elevation in systolic blood pressure over 100mmHg were chosen by random sampling and divided into a treatment group (Choksamni moxibustion group) and a control group. In the Choksamni $(ST_{36})$ moxibustion group, moxibustion was done at the point between tibial tuberosity and head of fibula where the $ST_{36}$ is known to be located. Direct moxibustion was practiced on the patients 5 times with an increase of size from a grain of rice to a bigger cluster. Male patients were chosen to practice on the left meridian and female patients were chosen to practice on the right meridian point. Changes in blood pressure after He moxibustion were checked 4 times at tine intervals of 30 minutes. In the control group, the patients took bed rest without my medical treatment. The two groups were compared in order to demonstrate whether then were any remarkable changes in depression of blood pressure. Results : There were significant decreases in the systolic and diastolic blood pressure before and after moxibustion. We found significant decreases in systolic blood pressure at 60 minutes, 90 minutes, and 120 minutes ana diastolic blood pressure at 120 minutes in the patient group compared with the control group. Conclusions : There was a statistically significant depressing effect on blood pressure elevation observed in the group with moxibustion at $ST_{36}$ versus the control group without any medical treatment.

백서의 경혈(해계, 곤륜, 양보) 침자가 Nitric Oxide Synthase 및 Nitric Oxide의 변화에 미치는 영향 (Effects of Acupuncture at ST41, BL60, GB38 on Changes of Nitric Oxide and Nitric Oxide Synthase in Rats)

  • Objectives : To observe the changes in the expression of nNOS, iNOS, eNOS and NO by the needle insertion on river points, one of the five transport points of three yang meridians of the foot. Methods : Based on rats, needle was inserted on both left and right sides of ST41, BL60 and GB38 and retained for five minutes. After the retention, blood was drawn via cardiac puncture and tissues from each point around meridian vessels were extracted to observe the changes in the expression of nNOS, iNOS, eNOS and NO. Results : In terms of the effect on expression of nNOS within tissues, ST41 showed significant decrease based on the results of immunohistochemistry. In terms of the effect on expression of iNOS within tissues, none of the experimental groups showed any significant change compared to the Normal group. Regarding expression of eNOS within tissues, GB38 showed significant increase based on the results of immunohistochemistry. In terms of the effect on NO creation in tissues, none of the experimental groups showed any significant change compared to the Normal group. Conclusions : The effect of needle insertion on the river points of three yang meridians of the foot on the function of nNOS, iNOS, eNOS and NO could be observed, and based on this study, it is considered that the effect of needle stimulation on the changes of nervous system could be found out through additional research.

족궐음간경(足厥陰肝經)의 화혈(火穴)과 수혈(水穴)의 침보사(針補瀉)가 자율신경계와 뇌파에 미치는 영향 (The effects of Fire point(LR2).Water point(LR8) through Reinforcement-Reduction acupuncture stimulation on ANS & EEG)

  • Objectives : The aim of this experiment was to investigate the influences of Autonomic Nervous System and EEG by conducting Reinforcement-Reduction(補瀉) acupuncture stimulation to compare the changes in acupoints on the body before and after treatment of acupuncture at Xingjian(LR2) being referred as Fire-point(火穴) and Ququan(LR8) being referred as Water-point(水穴) of Yin Liver Meridian(足厥陰肝經). Methods : This study was carried out on 30 healthy female volunteers in their 20's. There were four tests conducted throughout and the period for each test was between 2 to 5days. HRV and EEG were measured for 5 minutes before acupuncture stimulation was conducted on LR2-Reinforcement, LR2-Reduction, LR8-Reinforcement and LR8-Reduction at random. During the 20 minutes of acupuncture treatment, same subjects were measured simultaneously to observe any significant changes in acupoints. Again, the same subjects were measured for 5 minutes after removing the acupuncture in order to perform a comparative analysis. Results : The measurement of HRV showed that LF, LFnorm and LF/HF ratio increased significantly(p<0.05) while HF, HF norm decreased significantly in case of LR2-Reinforcement & LR8-Reduction. Both LR2-Reduction and LR8-Reinforcement induced a significant increase in HFnorm. EEG measurement indicated low $\alpha$ wave decreased and high $\beta$ wave increased significantly at LR2-Reinforcement during post-acupuncture period compared with acupuncture stimulation period. Both LR2-Reduction and LR8-Reinforcement developed significantly low $\alpha$ wave and high $\alpha$ wave. High $\beta$ wave increased significantly at LR8-Reduction during the acupuncture stimulation in comparison with pre-acupuncture period. Conclusions : The manipulation of acupuncture stimulation at LR2-Reinforcement and LR8-Reduction enhanced the activity of sympathetic nerves and the state of arousal while that of para sympathetic nerves declined. On the other hand, LR2-Reduction and LR8-Reinforcement developed the levels of para sympathetic nerves and relaxation.

흰쥐의 신경병증성(神經病症性) 통증(痛症) 모델에서 양로(養老) 자침(刺鍼)의 진통효과(鎭痛效果) (Analgesic effect of acupuncture applied to $SI_6$ in a rat model of neuropathic pain)

  • Objectives : The usage of acupuncture has gained popularity for certain chronic pain conditions. However, the efficacy of acupuncture in various diseases has not been fully established and the underlying mechanism is not clearly understood. In the present study, the effect of electroacupuncture (EA) applied to yangno$(SI_6)$ on the neuropathic pain was examined. Methods : A common source of persistent pain in human is a neuropathic pain. Neuropathic pain was induced by tight ligation of L5 spinal nerve. When rats developed pain behaviors, EA was applied for 30 min. under enflurane anesthesia with repeated train stimuli at the intensity of 10X of muscle twitch threshold. The foot withdraw latency of the hind limb was measured for an indicator of pain level after each manipulation. Results : EA increased the mechanical threshold of the foot in the rat model of neuropathic pain significantly for the duration of 1 hr. suggesting a partial alleviation of pain. EA applied to SI6 point produced a significant improvement of mechanical sensitivity of the foot lasting for at least 1 h. However, $ST_{36}$ point did not produce any significant increase of mechanical sensitivity. The improvement of mechanical threshold was interpreted as an analgesic effect. The analgesic effort was specific to the acupuncture point since the analgesic effect on the neuropathic pain model could not be mimicked by EA applied to a point, $ST_{36}$. In addition, this analgesic effect of EA is mediated by a adrenergic mechanism of descending control of spinal cord from the brain. Conclusions : The data suggest that EA produces a potent analgesic effect on the neuropathic pain model in the rat; and 2) that EA-induced analgesia is mediated by a adrenergic mechanism of descending control in a point specific manner.

수지요법이 복강경하 자궁절제술 환자의 통증과 복부팽만감에 미치는 효과 (Effects of Korean Hand Acupressure on Pain and Abdominal Bloating of Patients Receiving Laparoscopic Hysterectomy)

  • Purpose: This study aimed to investigate the effects of Korean hand acupressure on pain and abdominal bloating of patients receiving laparoscopic surgery. Methods: This study was a quasi-experimental design using a nonequivalent control group pre-post test. The experimental group and the control group were 39 patients each who were hospitalized at gynecology ward of a hospital located in Gyeonggi-do. The experimental group received Korean hand acupressure therapy on the meridian point: A5, A6, H3, H7, I38 for 48 hours immediately after the surgery. Pain and abdominal bloating were measured at 5 times. Data were analyzed using SPSS/Win 18.0. Results: The experimental group showed lower pain score than the control group (p<.001). While there was no significant difference in abdominal bloating (p=.528), the time effect was significantly different (p<.001). Conclusion: The findings indicate that Korean hand acupressure reduces pain and abdominal bloating of laparoscopic surgery patients.

간헐성 복통을 호소하는 아동에 대한 수지침의 효과 (The Effects of Hand-Acupuncture Therapy on Intermittent Abdominal Pain in Children)

  • Purpose: The purpose of this study was to identify the effects of hand acupuncture therapy on intermittent abdominal pain in children. Method: A quasi experimental pre-test and post (1,2)-test design was used. Data was collected from May, 2002 to February, 2003. Forty children were assigned to the experimental (20) or control group (20). The experimental group received Hand-Acupuncture therapy on the meridian point; A8, A9, A10, All, A12, E22, E45 for 20 minutes, while the control group rested on the bed. Data was analyzed using the SAS program with Fisher's Exact $x^2$ test, repeated measured ANOVA, and ANCOVA. Result: In the experimental group, pain intensity (f=63.26, p=0.00), A12(A)(F=60.40, p=0.00), and medication requirement ($x^2$=32.63, p=0.00) were significantly lower than that of the control group. Conclusion: These findings indicate that hand acupuncture therapy is effective for reduction of intermittent abdominal pain. Therefore, hand acupuncture therapy can be considered an independent nursing intervention for reducing intermittent abdominal pain.
