미국 정신보건제도의 개혁 방안에 대한 고찰 - 정신보건위원회의 활동을 중심으로 -

  • 최근 정신질환의 심각성과 정신보건의 증진에 대한 세계적 관심이 고조되면서 세계보건기구(WHO)와 미국 보건복지부(DHHS)에서는 잇달아 정신보건에 대한 연구 결과들을 발표하였다(United States Public Health Service Office of He Surgeon Central, 1999 & 2001, World Health Organization, 2001). 이러한 가운데 지난해 미국 정부는 대통령 직속의 정신보건위원회를 설립하여 현재 정신보건서비스제도가 드러내고 있는 다양한 문제점들을 파악, 분석하여 개선 방안들을 제안해 줄 것을 요청하였다. 미국 사회의 정신질환 및 정신보건과 관련된 서비스시스템을 개선하기 위한 이러한 획기적인 노력은 1997년 Carter대통령 쥐임 직후 구성된 정신보건위원회(President's Commission on Mental Health) 이후 약 25년 만에 이루어진 일로서 정신보건 분야뿐만 아니라 미국 사회 전반에 걸쳐 비상을 관심을 끌고 있다. 그리고 이러한 노력과 결과는 최근 들어 심각하게 노출되는 정신질환 및 사회병리 현상으로 어려움을 겪고있는 한국 사회에도 많은 시사점을 줄 수 있으리라 기대된다. 따라서 본 연구는 정신보건위원회의 활동을 중심으로 미국 사회의 정신보건에 대한 다양한 문제를 인식하고 이를 해결하기 위한 정신보건서비스제도 개선 방안을 고찰하는 것을 목적으로 한다. 연구의 주요 내용으로는 위원회의 설립 배경과 활동 경과, 정신보건제도의 다양한 문제점과 개혁 방안으로 구성되었다. 그리고 이러한 고찰은 우리나라 정신보건제도에 대한 인식과 발전 방향에 대한 새로운 조망을 도출해낼 수 있으리라 기대한다. 연구의 주요 방법으로는 각종 연구보고서와 기존 문헌을 이용한 문헌 연구를 활용하였으며, 기존 출간 자료나 통계에 밝혀지지 않은 사항은 정신보건위원회의 위원과의 직접 인터뷰를 통해 보완되었다.

THE ADDITIONAL POINT SYSTEM OF NATIONAL HEALTH INSURANCE FOR DENTAL TREATMENT IN PATIENTS WITH A SPECIAL HEALTH CARE NEED IN KOREA (한국의 장애인 환자 치과 진료를 위한 국민 건강 보험 가산제도의 종류 및 청구 현황)

  • In order to increase the accessibility of dental care for people with disabilities, National Health Insurance Service has implemented an additional point system of National Dental Insurance for dental treatment of patients with a special health care need (AID). The purpose of this study is to investigate the types and status of AID in Korea using data of the Health Insurance Review and Assessment Service from 2011 to 2017. The basic consultation fee is increased by 9.03 points (713 won) for brain disorder, intellectual disability, mental disability, or autistic disorder. From 2011 to 2015, the number of claims with a basic consultation fee increased from 90,456 to 141,179. Dental treatment and surgical treatment fee is increased by 100% of the defined insurance score for each of the 15 items. During the five years from 2012 to 2016, the number and amount of claims for each item increased steadily. Of the total claims for 5 years, endodontic treatment was highest, with 107,477 cases, followed by 51,641 cases of scaling. There are two types of dental safety observation fee, simple and complex. The simple safety observation fee is 10,370 won per day, and the complex safety observation fee is 20,750 won per day. Dental safety observation fees were charged 34 times in 2015, 14 times in 2016, and 41 times through May 2017. From 2011 to 2017, the number and amount of claims using AID for dental care for people with disabilities increased. However, considering that the number of registered dental users with disability was about 560,000 in 2016, the number of claims using AID is 1-20,000, which is less than 2% of registered dental users with disability. Therefore, it is necessary to expand dental services for people with disabilities including AID.

The factors associated with suicidal ideation among the elderly living alone received the elderly care service in a rural area (일부 농촌지역 노인돌보미 대상 독거노인의 자살생각 관련요인)

  • Objectives: The objective of this study was to identify the factors associated with suicidal ideation among the elderly living alone received the elderly care service in a rural area. Methods: This study was surveyed 206 the elderly living alone who received the elderly care service in G-gun, Jeollanamdo. The collected data were socio-demographic and health-related characteristics, family support and suicidal ideation. The used methods for statistical analysis were used t-test, analysis of variance, Pearson's correlational analysis and the multiple logistic regression analysis. Results: In univariate analysis, the suicidal ideation of the elderly living alone was statistically significant associated with age, monthly income, sufficiency of sleeping, depression, stress, activities of daily living, instrumental activities of daily living, perceived health status and family support. As a result of multiple regression analysis, the associated factors with suicidal ideation were age, depression, stress and the activities of daily living. Conclusions: The suicidal ideation of the elderly living alone in the rural area was associated with age, mental health and independency of living. It will be needed the multidisciplinary approaches to prevent the suicidal ideation and attempts of the elderly living alone.

Factors Affecting the Downward Mobility of Psychiatric Patients: A Korean Study of National Health Insurance Beneficiaries

  • Objectives: The purpose of this study is to examine the magnitude of and the factors associated with the downward mobility of first-episode psychiatric patients. Methods: This study used the claims data from the Korean Health Insurance Review and Assessment Service. The study population included 19 293 first-episode psychiatric inpatients diagnosed with alcohol use disorder (International Classification of Diseases, 10th revision [ICD-10] code F10), schizophrenia and related disorders (ICD-10 codes F20-F29), and mood disorders (ICD-10 codes F30-F33) in the first half of 2005. This study included only National Health Insurance beneficiaries in 2005. The dependent variable was the occurrence of downward mobility, which was defined as a health insurance status change from National Health Insurance to Medical Aid. Logistic regression analysis was used to assess factors associated with downward drift of first-episode psychiatric patients. Results: About 10% of the study population who were National Health Insurance beneficiaries in 2005 became Medical Aid recipients in 2007. The logistic regression analysis showed that age, gender, primary diagnosis, type of hospital at first admission, regular use of outpatient clinic, and long-term hospitalization are significant predictors in determining downward drift in newly diagnosed psychiatric patients. Conclusions: This research showed that the downward mobility of psychiatric patients is affected by long-term hospitalization and medical care utilization. The findings suggest that early intensive intervention might reduce long-term hospitalization and the downward mobility of psychiatric patients.

Survey Analysis for Resting Space Design of Subway Driver (지하철 기관사 휴게 공간 디자인을 위한 요구조사 분석)

  • There is an increasing trend each year in the number of subway users due to increase in the traffic volume. As the number of train service has risen, the subway drivers are given heavy work and from the same, are put at risk of mental health deterioration due to stress. As this is directly connected to safety, there is an urgent need for improvement of the situation. Therefore, as a way to appropriately relieve such stress, resting space where mental and physical energy can be recharged is necessary. Thus, this research conducted a survey of demand with subway drivers as subjects in order to provide resting space that can offer healthy rest and increase work effectiveness. 50 subway drivers were the subjects of this survey. As a result of analyzing the contents of the survey, environmental satisfaction was shown to be the lowest compared to psychological and morphological satisfaction. In the case of sleeping room, they wanted more comfortable and individual space. And the waiting room had the highest rate of usage frequency. The storage space was where the subway drivers showed the strongest attachment and was shown to be the only personal space. Most were shown to be satisfied with the outdoor space, but it had the lowest usage frequency. In addition, it is nessasery customized storage cabinet designed for subway drivers should be produced, which can effectively arrange the limited space. I hope that these direction of improvement can increase the healthy rest and work effectiveness of the locomotive engineers as it is directly being connected to the health and safety of the citizens.

Relationships between university students' Dental Fear and Mental Hardiness, subjective well-being (대학생들의 치과공포도와 심리적 강인성 및 주관적 안녕감과의 관련성)

  • The purpose of this study was to investigate the association between university students' dental fear and their psychological hardiness and subjective well-being, promote positive awareness of and access to dental service, and develop a plan for continuous oral health care. To collect data, convenience sampling of college students in North Jeolla Province was performed, and they were asked to complete a self-administered questionnaire from May 1 to 30, 2017. Dental fear was negatively correlated with both psychological hardiness and subjective well-being, and psychological hardiness and subjective well-being were positively correlated with each other. University students' dental fear, psychological hardiness, and subjective well-being were found to be correlated with one another; therefore, dental clinics need to apply a positive style of coping, taking into account psychological responses suitable for clinical competence on the basis of satisfactory communication, with the objective of promoting oral health.

  • The people with disabilities living in residential facilities have more difficulty in caring oral hygiene than those living at home. The purpose of this study is to evaluate the recent 2014 dental treatment records of free mobile dental clinic service for disabled people in Korea. 203 disabled living in residential facilities participated in mobile dental clinic. Patients classified according to types of disability. Mental retardation were 75.3%, mental disorder were 6.0%, crippled disorder were 7.4%, brain disorder were 6.5%, visual disorder were1.4%, auditory and speech disorder were 2.3% and autism disorder were 0.9%. Performed treatments were 99 scaling and curettage, 88 fluoride varnish and TBI, 4 extraction, 1 endodontic treatment, 16 caries control (resin filling, GI filling), 1 denture repair and 8 refuse the treatment. Free mobile dental clinic can not provide complex dental treatment. So, the organization should systemize advanced dental treatment and regular preventive programs. Furthermore, we need to have a more concerns about the people with disabilities living in residential facilities and constantly participate on a dental voluntary work.

A Study on the Relationship between Firefighters' Smartphone Addiction and Stress (소방공무원의 스마트폰 사용과 스트레스간의 상관관계)

  • The use of smartphone improves convenience and efficiency in modern people's lives but threatens the mental health of individuals by manifesting an addiction phenomenon that is difficult for everyday life without a smartphone. In particular, it is suggested that the effects of the smartphone addiction of firefighters who have a high level of job stress due to work environment exposed to disaster and fire have a very negative effect on their personal well-being and job performance. Therefore, the analysis of the relationship between firefighters' smartphone addiction and stress will be helpful in examining the problems of stress management. This study analyzed the relationship and influence between smartphone addiction and stress of firefighters. Total study subjects were 230 and SPSS 20.0 was used. Statistical methods are group analysis (t-test and ANOVA) and multiple regression analysis. The results of this study show that firefighters' household type and monthly income have a significant effect on stress. Of the sub-factors of smartphone addiction, withdrawal experience has a statistically significant effect on stress. The results of the study will provide empirical data on the smartphone addiction of fire service employees.

  • The purpose of this study is to evaluate the usability of the mobile app 'Allive' to improve accessibility to resources and mental health services among sexual violence survivors. Participants include survivors of sexual violence as well as service providers at sexual violence crisis intervention centers. The usability test was conducted as a quantitative and qualitative evaluation after study participants used the app in a real-life environment. The results of usability testing highlighted the positive and beneficial impact on mental health function as well as accessibility to essential resources for those who have experienced sexual violence.

Understanding the Categories and Characteristics of Depressive Moods in Chatbot Data (챗봇 데이터에 나타난 우울 담론의 범주와 특성의 이해)

  • Influenced by a culture that prefers non-face-to-face activity during the COVID-19 pandemic, chatbot usage is accelerating. Chatbots have been used for various purposes, not only for customer service in businesses and social conversations for fun but also for mental health. Chatbots are a platform where users can easily talk about their depressed moods because anonymity is guaranteed. However, most relevant research has been on social media data, especially Twitter data, and few studies have analyzed the commercially used chatbots data. In this study, we identified the characteristics of depressive discourse in user-chatbot interaction data by analyzing the chats, including the word 'depress,' using the topic modeling algorithm and the text-mining technique. Moreover, we compared its characteristics with those of the depressive moods in the Twitter data. Finally, we draw several design guidelines and suggest avenues for future research based on the study findings.
