Correlation between Stress, Anxiety, Depression, and Quality of Life in Parents of Children with Disabilities during COVID-19 (COVID-19 발생 상황에서 장애아동 부모의 스트레스, 불안, 우울과 삶의 질의 관련성)

  • Objective : This study was conducted to investigate the stress, anxiety, depression, and quality of life that parents of children with disabilities have according to their general characteristics and determine the correlation between stress, anxiety, depression, and quality of life. Methods : The subjects were 242 parents of children with disabilities under the age of 13 receiving treatment at rehabilitation centers for the disabled, rehabilitation hospitals, and child development centers located in D, B, and U metropolitan cities. Results : According to the result of analyzing the correlation between stress, anxiety, and depression that the parents of children with disabilities felt and the quality of their lives, the quality of their lives correlated negative with their stress, anxiety, and depression. In the case of the results according to the parents' general characteristics, stress showed a significant difference depending on the education level, monthly income, and anxiety showed a significant difference depending on child rearing time, occupation, and public transportation. Additionally, depression and the quality of life had a significant difference depending on child rearing time, education level, occupation, and monthly income. Conclusion : The results of this study identified the importance of the mental health of the parents of children with disabilities and the correlation between their mental health and quality of life. Social distancing and quarantine rules that children with disabilities must follow have limited their opportunities to receive education and treatment, so the influence of stress, anxiety, depression that the parents of children with disabilities who should continue to take care of their children should be considered. Based on these results, it is necessary to develop support services for the parents of children with disabilities for enhancement in their mental health in preparation for other disasters in the future.

General Health Related Quality of Life and Associated Factors among Prostate Cancer Patients in Two Tertiary Medical Centers in Kuala Lumpur, Malaysia: A Cross-sectional Study

  • Measurement of quality of life among prostate cancer patients helps the health care providers to understand the impact of the disease in the patients' own perspective. The main aim of this study is to measure the quality of life among prostate cancer patients at University Malaya Medical Center (UMMC) and Universiti Kebangsaan Malaysia Medical Centre (UKMMC) and to ascertain the association factors for physical coefficient summary (PCS) and mental coefficient summary (MCS). A hospital based, cross sectional study using the Short Form-36 (SF-36) questionnaire was conducted over a period of 6 months. A total of 193 respondents were recruited. Their total quality of life score was $70.1{\pm}14.7$ and the PCS score was lower compared to MCS. The factors associated for PCS were: age, living partner, renal problem, urinary problem of intermittency, dysuria and hematuria. Factors associated for MCS were: age, living partner, renal problem, presenting prostatic specific antigen and urinary problem of intermittency and dysuria. Our prostate cancer patients had moderate quality of life in the physical health components but their mental health was less affected.

Status and Issues on Disaster Preparation Programs in Public Health Center (보건소의 재난관련 대비-대응 사업의 실태와 과제)

  • Objectives: The purpose of this study was to review the disaster preparation and response programs and the status of disaster preparation in public health center. Methods: In depth interview was performed in September 2017 using 5 open questions to the persons who are in charge of disaster response services in 5 public health centers of different levels in Korea. The questions included general characteristics of public health center, disaster programs and future issues. The research hired a quality method. Results: In general, the persons in charge recognized the cooperative agency of local government in disaster management. There were no disaster preparation programs developed by the public health centers. Most of the preparation were passive activities such as emergency support, crisis management on communicable disease and quarantine, participation in biological disaster response training, and education etc. The persons in charge emphasized necessity of disaster preparation programs. Conclusions: Disaster preparation and responsiveness is an evolving issue in public health centers in Korea. Medical support system and communicable disease management system are being set up in the national level. A comprehensive system covering health management, nutritional support, mental health, environment management of shelter, and volunteers supports on public health center level needs to be developed along with a easy-to-follow manual.

  • The aim of this study was to identify the performance and requirements of the visiting nursing care using Omaha system in public health center. The highest performance were 'personal hygiene', 'pain', 'medication regimen', 'nutrition', 'physical activity', 'sanitation', 'sleep and rest patterns', 'oral hygiene', 'mental health' in order. The lowest performance were 'sexuality', 'postpartum', 'income', 'family planning', 'pregnancy', 'spirituality', 'abuse', 'reproductive function', 'neglect' in order. Problems such as 'postpartum', 'pregnancy' and 'family planning' need to strengthen the role of visiting nurses according to the region. this result will be the basis for visitung nursing care.

The Response of the Seoul Municipal Hospitals against COVID-19 and Its Implications for Public Hospitals (서울시립병원의 코로나19 대응을 통해 본 공공병원의 시사점 고찰)

  • Purpose: The purpose of the study is to suggest the main functions and implications of public hospitals to effectively respond to the future epidemic crisis based on analyzing the accessibility to designated Coronavirus Disease 2019 (COVID-19) medical institutions of Seoul and examining the main features of the quarantine of Seoul municipal hospitals. Method: To analyze the response and function of Seoul municipal hospitals, we reviewed the Infectious Disease Control and Prevention ACT, 258 articles of Seoul Metropolitan Government press releases from January to the end of April, 48 articles of Seoul Metropolitan Government's daily newsletters, 2019 Health Bureau Budget report. We also referred to internal data of Seoul Children's Hospital, Seoul Seobuk Hospital, and Seoul Eunpyeong Hospital during the same period. Besides, the accessibility to medical institutions was analyzed by using the COVID-19 data which was announced daily basis. Results: The accessibility of COVID-19 patients living in the Southeastern part of Seoul to a medical institutions was 16.2km on a distance basis, and it was the lowest accessibility among four regions of Seoul since it took about 40 minutes by car. On the other hand, patients living in the Northeast part had the highest accessibility, as the access to medical institutions was 10.7km and 27 minutes by car. Also, the main functions of the municipal hospital of Seoul against COVID-19 were to shift the public hospital function to COVID-19 patients only hospitals, to perform the epidemiological investigation by medical doctors, and to support the operation of self-isolation facilities, community treatment centers and triage rooms of community health centers. Conclusion: Through the experience of COVID-19, we suggested that the functions of public hospitals will be reorganized as the reinforcement of infectious disease treatment and mental health for quarantined patients, cooperation with private hospitals, supporting for strengthening community health capacity and preparation for another epidemic.

Self-evaluative Awareness of Problem Solving Ability In Psychiatric Nurse Practitioners (정신보건간호사의 문제해결능력에 대한 인식정도)

  • This study was conducted to investigate the degree of PSI (self-evaluative awareness of problem solving ability) in PMHNP (psychiatric mental health nurse practitioners) and to identified the factors predicting problem solving ability of them, in order to provide basic data for the development of the specific education programs to improve problem solving ability. Data was collected from 355 subjects who have the certificates of PMHNP working in hospitals and public centers. Chun Seok Kyun's instruments (1993) based on the Heppner and Petersen's Problem Solving Inventory (1982) was used to determine PMHNPs' PSI. The results were as follows; 1. The average score of PSI of PMHNP was $2.81\pm0.23$. Of the three factors. there marked the lowest score in the factor 3. personal control $(2.56\pm0.43)$, and the highest score was in the factor 1. problem-solving confidence $(2.95\pm0.34)$. 2. The differences in PSI were significant for degree of education and career in psychiatric settings (p=0.000l, p=0.0187). 3. Job satisfaction was the highest factor predicting PSI of PMHNP$(17.9\%)$. When degree of education, career in psychiatric setting and marital status were added, the total predictors explained $24.3\%$. As a result, it is necessary to conduct further research in relation to the problem-solving process, the development of education program. skill acquisition and measurement of problem solving in nursing practice.

Correlates of Social Support, Family Function, and Conditions of Home Care Nursing in Family Caregivers (가정간호대상자 가족의 사회적 지지와 가족기능, 가정간호 특성 간의 관계)

  • Purpose: The purpose of this study was to determine the factors related to the social support, family and friend support as well as home care nurse support, in family caregivers of patients with home care service. Method: The participants were 111 family caregivers of patient, who were receiving home care services from home health care centers of 3 different general hospitals located in Seoul. The data was collected, using self-administered questionnaires. Result: The level of family and friend support varied significantly according to patients' mental status, period of home care nursing, frequency of home care service, caregiver's education level, family income, and family functioning level. On the other hand, home care nurse support varied significantly according to patients' mental status, caregiver's age, existence of interchangeable family caregivers, and family functioning level. There was a significantly positive correlation between the social support the family caregivers' perceived and family function while there was a negative correlation between family and friend support and the period of home care service. Conclusion: Thus, the establishment of nursing interventional program, with understanding of their social support, is needed for both patients and their caregivers.

Effects of a Stress Management Program on Perceived Stress, Depression and Somatic Symptom in the Elderly (스트레스 관리 프로그램이 노인의 지각된 스트레스, 우울 및 신체화 증상에 미치는 효과)

  • Purpose: This study was done to identify the effects of stress management program on perceived stress, geriatric depression, and somatic symptom in elderly Korean subjects. Methods: A pre-post test design with a nonequivalent control group was used. Fifty elderly Korean subjects with mild depression were recruited from the registerants at four public senior citizen centers within S-gu in Seoul. They were randomly allotted to the experimental group(n=25) and control group(n=25). The intervention was performed for one hour each week at each center for 5 weeks. The effects were measured by Perceived Stress Scale, Geriatric Depression Scale-Korean, and Patient Health Questionnaire-15. Results: Perceived stress and depression were significantly reduced in experimental group. The somatic symptom was not significantly different in the experimental group. Conclusion: The stress management program may have merit in improving mental health of elderly Koreans, although this conclusion is equivocal pending further well-controlled studies that include mediation techniques such as conversion therapy and health gymnastics.

Development and Application of a Nursing Assessment Tool for Senior Center Nurses (노인복지관 간호사를 위한 간호사정조사도구 개발 및 적용)

  • Purpose: The purposes of this study were to develop a nursing assessment tool for senior center nurses, and to test its feasibility and content validity. Methods: The study utilized a psychometric test design. Preliminary items were developed based on geriatric health needs and Gordon's 11 domains of functioning health. Initially, the tool was evaluated for content validity and feasibility. Then, it was administered among 195 older adults in a senior center by a gerontological nurse practitioner. Data were analyzed to describe the nursing problems of the senior center older adults. Results: The final version of the nursing assessment tool consisted of 27 items. The internal consistency, measured with the Cronbach's ${\alpha}$, was .74. The result of the assessment showed that senior center older adults had high nursing needs in the area of management of chronic diseases (hypertension, diabetes, and lipids), dental care, community relations, safety, elder abuse, health behaviors (such as drinking), mental health (depression, suicide, and cognitive function), and health consultations. Conclusion: The new tool was feasible for use with senior center participants, and it was evaluated as having high content validity by senior center nurses.

A Delphi Study for Constructing a Suicide Prevention Protocol for Male Baby Boomers (베이비붐세대 남성의 자살 예방 프로토콜 구성을 위한 델파이 연구)

  • Purpose: The purpose of this study was to construct a protocol for the prevention of suicide in male baby boomers. Methods: In order to construct the protocol, a three-step expert consensus process was carried out. The first step was to compose questions based on various research data. For this purpose, two professors of psychiatric mental health nursing and two advanced practitioner nurse of psychiatric mental health in the composition. The second step was to conduct the first Delphi survey on expert panelists. The survey period was from October 13, 2015 to November 1, 2015. The third stage, the second Delphi investigation was conducted from November 10 to December 4, 2015. Results: As a result of the survey, a total of 7 areas of protocol configuration items were derived. The derived items include the characteristics of male baby boomers, suicidal signs, suicide risk factors, suicide protection factors, suicide measurement tools, related organizations, and mental health promotion programs. Conclusion: This study can be used as a guideline when intervening suicide prevention of male baby boomers in community mental health centers.
