Effects of Contralateral Seventh Cervical Nerve Transfer on Upper Extremity Motor Function in the Patients with Spastic Hemiplegia after Stroke: a Retrospective Cohort Study

  • Objective: Contralateral seventh cervical nerve transfer (contralateral C7 transfer) is a newly attempted method to restore upper extremity motor function in the patients with spastic arm paralysis. The aim of this study was to investigate the effects of contralateral C7 transfer on upper extremity motor function in the patients with spastic hemiplegia after stroke. Design: A retrospective cohort study. Methods: Thirty-four patients with spastic hemiplegia after stroke was investigated. All patients registered between January 2020 and February 2021. The subjects were assessed on upper extremity motor function, cognition, and spasticity before and after contralateral C7 transfer. The upper extremity motor function was measured using the Fugl-Meyer upper extremity scale and box & block test. The cognition and spasticity were assessed by Korean version mini mental state examination (K-MMSE) and modified Ashworth scale from baseline to 8 weeks after the surgery. Results: The Fugl-Meyer upper extremity scale and modified Ashworth scale were significantly improved after contralateral C7 transfer (p<0.05). However, box & block test and K-MMSE were no significant changes after the surgery (p>0.05). Conclusions: This study suggested that the contralateral C7 transfer was a feasible and practical approach to improve upper extremity motor function in the patients with spastic hemiplegia after stroke, but further study is required to identify the long-term effects after the contralateral C7 transfer.

  • This is a literature Study on ophthalmologic disease remedies of the four famous physicians in Jin Yuan period. The results were as follows: 1. Yu Wan So regarded the cause of ophthalmologic disease as the wind and heat(風熱), the blood deficiency(血少), or the deficiency of vital function and essence ,of the kidney(腎氣虛). For each treatment he used the method to remove wind and heat(除風散熱), the tranquillizing and tonifying method to nourish the heart and to relieve mental strain due to dificiency of vital essence(養血安神). the method to nourish Yin in the kidney(補腎水). The important nature of the these medicines is bitter and cold. 2. Jang Jong Jeung regarded the cause of ophthalmologic disease as the blood excess(血太過). or the blood deficiency(血不及). or the fire and heat(火熱). For each treatment he used the method to extract blood(出血療法). the method to nourish the liver and the kidney(補肝腎療法). or the method of vomiting and diarrhea(吐不法). The medicines are usuallly classified into external treatment medicines(外用藥). medicines to disintegrate mass(破積聚藥). purgating medicines(攻下藥). 3. Lee Dong Won regarded the cause of ophthalmologic disease as the spleen and the stomach weakness(脾胃虛弱), excessive fire in the heart(心火太盛). For each treatment he used the method to tranquillize the spleen and the stomach (調理脾胃). the tranquillizing and tonifying method to nourish the heart and to relieve mental strain due to deficiency of vital essence(養血安神). 4. Ju Dan Gye regarded the cause of ophthalmologic disease as the wind and heat(風熱). the blood deficiency(血少). the mental exhaustion(神勞). the kidney weakness(腎弱). For each treatment he used the method to remove wind and heat(除風散熱), the method to nourish blood and yin water(養血補水), the method to relieve mental strain and to remove spiritual heat(安腎瀉神火). the method to nourish yin water(養陰水). These drugs are usually classified into the medicines to remove endogenous heat(淸熱藥). the medicines to nourish yin(補陰藥).

Electroconvulsive Therapy for Psychiatric Disorders in Elderly Adults (노년기 정신장애의 전기경련치료)

  • Electroconvulsive therapy (ECT) is indicated for various mental disorders (e.g., major depressive disorder, schizophrenia, and bipolar disorder) and the behavioral and psychological symptoms of dementia in elderly patients. Furthermore, ECT is a useful first-line treatment in emergency and crisis situations such as suicide risk, violent behavior, catatonia, and food refusal, which are more frequent in elderly patients. ECT is also effective in the treatment of the motor symptoms of neurological disorders, such as Parkinson's disease and Huntington's disease. Due to the high risk of various physical diseases, the comorbid physical conditions of elderly patients should be individually controlled to optimize ECT treatment. Compared to young adults, in elderly patients the seizure threshold is higher, the seizure duration is shorter, and the anesthetic dose is lower. On the contrary, the response rate in the elderly is both faster and higher. Considering potential cognitive decline and the prevention of further deterioration of cognitive function in elderly patients, in the absence of significant comorbidities, twice weekly sessions and right unilateral electrode placement with a lower seizure threshold and less cognitive effect are preferred to bilateral electrode placement, which has a high risk of adverse cognitive effects. After an acute course of ECT, continuation and maintenance of ECT, combined with prescription of therapeutic drugs, may prevent possible relapse or recurrence of mental disorders. In conclusion, ECT can be used to treat mental disorders in elderly adults, with safety and effectiveness comparable to that in young adults.

Vocational Rehabilitation of People with Mental Disabilities by Repeated Training of Horticultural Activities

  • The purpose of this study was to develop an effective horticultural activity program for the vocational rehabilitation of people with mental disabilities. The subjects of this study were seven persons with mental disabilities in the vocational rehabilitation team of a public health center located in G city, Gyeongsangbuk-do. A total of 10 horticultural programs were held from October 10 to December 11, 2018. Five programs were selected among those that were easy for the persons with mental illness to handle and their preference was high. In each session, the same task was repeated 10 times and finally changes in hand functions and working speed were measured. As a result, the assembly task of the Purdue Pegboard test was 23.6 (SD = 10.5) before the program, 26.6 (SD = 11.5) after planting, 27.7 (SD = 12.2) after wrapping pots, and 28.0 (SD = 13.9) after making mini flower baskets, 26.9 (SD = 12.0) after wrapping a single flower, and 29.1 (SD = 11.9) after making corsages. There was a significant difference between the five programs (p = .016). As a result of measuring the time taken to produce the same 10 products, the average planting time was 9.9 (SD = 4.0) minutes in the first class and 6.0 (SD = 2.3) minutes in the second class (p = .018), and making mini flower baskets was reduced from 35.2 (SD = 10.1) minutes to 21.0 (SD = 7.7) minutes (p = .018), wrapping a single flower from 23.3 (SD = 7.9) minutes to 16.2 (SD = 7.3) minutes (p = .043), and making corsages from 53.6 (SD = 15.9) to 40.8 (SD = 16.8) minutes (p = .043). The working time was significantly shortened in the second class compared to the first class. The class for wrapping pots was shortened from 52.7 (SD = 11.7) to 49.6 (SD = 17.8) minutes, but there was no significant difference (p = .398).

  • Objectives: Dementia is a kind of chronic, progressive, degenerative disease. The symptoms results in troubles in intellectual functions. memory, orientation, intelligence, judgement, common sense and calculating ability. The Korean version of Mini-Mental State Examination(MMSE-K) has been used widely to screen cognitive impairment of the elderly in Korea. This instrument has been specifically developed and evaluated for the assessment of cognitive function in older Korean populations. Methods: Cognitive function was evaluated by the Korean version of Mini-Mental State Examination(MMSE-K). This patient was diagnosed as Vascular dementia and We treated her cognitive impairment with Gagambosim-tang(Jiajianbuxin-tang). Results: After treated with Gagambosim-tang(Jiajianbuxin-tang), Cognitive function was improved. Conclusion: This report may have a meaning to treat Vascular dementia.

Sexual Dysfunction in Patients with Polycystic Ovary Syndrome in Malaysia

  • Background: Polycystic ovary syndrome (PCOS) is a combination of chronic anovulation, obesity, and hyperandrogenism and can affect sexual function in women of reproductive age. It is also associated with endometrial cancer. Our aim was to evaluate the frequency and predisposing factors of sexual dysfunction in PCOS patients. Materials and Methods: In this cross-sectional study, 16 married women with a definite diagnosis of PCOS were recruited. Sexual function was assessed in the domains of desire, arousal, lubrication, orgasm, satisfaction and pain using the female sexual function index (FSFI) questionnaire. Patients were also assessed for mental health using the depression, anxiety and stress (DASS-21) questionnaire. Presence of hirsutism was assessed using the Ferriman-Gallwey (FG) scoring system. Demographic data were obtained from patients during in-person interview. Results: Sexual dysfunction was present in 62.5% of patients with the domains of arousal and lubrication particularly affected (93.8% and 87.5%, respectively). Patients with symptoms of depression and anxiety were significantly more likely to suffer sexual dysfunction than those without these symptoms (p=0.04 and p=0.03 respectively). Patients with stress symptoms reported higher orgasm dysfunction than those without (p=0.02). No significant difference in any of the FSFI score domains was observed between patients with and without hirsutism. Conclusions: PCOS patients markedly suffer from sexual dysfunction and therefore it seems appropriate to be screened for intervention. Poor mental health conditions that may be the result of infertility or other complications of PCOS should also be considered as curable causes of sexual dysfunction in these patients.

Study on the Utilization of the Child-Adolescent Functional Assessment Scale in Mental Health Service (정신건강서비스 영역에서 아동청소년 기능평가척도의 활용에 관한 연구)

  • Objectives: This study aims to supplement the limitations of the preliminary study and expand the use of the Child-Adolescent Functional Assessment Scale by providing standardized T-scores for measuring function in different areas, as well as guidelines for their interpretation. Methods: The Child-Adolescent Functional Assessment Scale and Korean-Child Behavior Checklist were administered to a total of 623 individuals, including 467 parents of emotionally and behaviorally disturbed elementary school students, 45 parents of emotionally and behaviorally disturbed middle school adolescents and 111 parents of elementary school students with developmental disorders, as well as to a control group consisting of 186 parents of mainstream elementary school children in the Seoul metropolitan area. Results: In contrast to the preliminary study, the emotional domain was disassociated into two factors, negative affect and emotion regulation, and the coefficient of determination for misconduct was increased significantly. Also, the overall reliability and validity of the Child-Adolescent Functional Assessment Scale were similar to those in the preliminary study, and the item structure was consistent. This study provides standardized T-scores of function in different areas based on the revised scale, and provides guidelines for their interpretation. Conclusion: The Child-Adolescent Functional Assessment Scale can assist the Child Global Assessment in Functioning in providing the information necessary for case management and treatment planning by comparing the impairment severity in each area of functioning, and also determining changes in behavior and function after treatment interventions. On the other hand, this scale has yet to clearly distinguish between rebellious behavior and misconduct. Further research is necessary to provide standards for more diverse age groups and for its utilization.

  • Recently, with increasing the number of patients with head injury and cerebrovascular accident, there has been an increasing need for the useful assessment tools of brain dysfunction and it's localization. With the advances in the neuroscience since the mid-1970s, particularly in the areas of neuroanatomical tracing, neuroimaging, and improved behavioraltest design, it has been possible to develop a more precise understanding and localization of brain dysfunction. However, these equipments are not readily available in the private clinics and too expensive to use as a screening tool to all suspected patients with brain dysfunction. Although several screening tests such as Mini-Mental States Examination(MMSE) or Brief Cognitive Rating Scale(BCRS) are simple in use and useful for the brief assessment of brain dysfunction, these are also limited in using for localization of brain dysfunction because of their simplicity. With increasing need of the assessment tool which is able to localize the dysfunction more precisely in the clinical practice, we planned to develop the new assessment tool, the Bedside Neurocognitive Function Localization Test(BNLT) which is suitable for this purpose. The BNLT was designed to be utilized for localizing brain dysfunction effectively and readily in the clinical practice. We introduced the whole process of designing the BNLT in this manuscript.

Effect of Gait Exercise Program on the Self-Efficacy and Parenting Attitude of Adolescent Spastic Cerebral Palsy - Case Study (보행운동프로그램이 청소년 경직형뇌성마비아동의 자아효능감 및 부모의 양육태도에 미치는 영향 - 사례연구)

  • Purpose : The purpose of this study was to investigate the effect of increased gait function enhancement exercise in three adolescent convulsive cerebral palsy sinuses on children's large movement function, balance and gait function, self-efficacy, and guardian attitude. Methods : The purpose of this study was to conduct a program to strengthen trunk muscles and strengthen walking ability 5 times a week for 8 weeks in 3 children with convulsive cerebral palsy in adolescence. The main reinforcement of the program was 20 minutes of muscle strength and 20 minutes of walking on a treadmill five times a week. Exercises were focused on the reinforcement of the flexor muscles of the proprioceptive neuromuscular promotion (PNF) and the extension of the legs in the bridge posture exercise and squat movements. Results : The results of GMFM-88 to determine the effects of this functional enhancement program on the body of children with convulsive cerebral palsy, and PBS & TUG to determine the effects of balance and gait ability were improved. In addition, parenting attitudes of guardians, self-efficacy of children, and self-efficacy of parents increased positively due to mental consequences. Gender Differences in the Cognitive Function and Nutritional Status in Older Age: A Representative Nationwide Data of Korean Elders (노인의 성별에 따른 인지기능과 영양관리상태의 차이)

  • Purpose: The purpose of this study was to identify the gender differences in the cognitive function and nutritional status among elderly people living in the community. Methods: This study used data from the 2014 National Survey on the Elderly and focused on 10,054 respondents who answered that they did not have dementia. The Korean version of Mini-Mental State Examination for Dementia Screening and the Nutrition Screening Initiative were used. IBM SPSS Statistics 23.0 was used to conduct statistical analyses. Results: The cognitive function score was 25.30 for men, which was higher than for women. The percentage of subjects who showed cognitive decline was 25.5% for males and 50.2% for females, which was statistically significant (p<.001). There were statistically significant differences in the nutritional status between males and females (p<.001). There was a statistically significant negative correlation between the cognitive function and nutritional status in both men and women (p<.001, p<.001). Conclusion: These results showed that the intervention to maintain cognitive functioning should be provided to elderly women with a low education level or high number of chronic diseases and medication, and an intervention to prevent the cognitive decline of the elderly should include nutritional management for health and function maintenance.
