본 연구는 장애자녀 부모가 인식한 돌봄역량, 돌봄경험의 평가(돌봄부담, 돌봄만족), 자조집단지지의 수준이 자녀의 장애유형(정신장애, 신체장애)에 따라 차이가 있는지 탐색하는데 목적이 있다. 이를 위해 전국 장애자녀 부모 301명을 조사대상자로 하였고, 자녀의 장애유형에 따라 정신장애 유형 180명과 신체장애 유형 121명으로 구분하였다. 두 집단의 잠재평균분석을 위해 형태동일성 검증, 측정동일성 검증, 절편동일성 검증, 요인분산동일성 검증을 실시하였다. 연구결과 정신장애 자녀를 돌보는 부모 집단은 신체장애 자녀를 돌보는 부모 집단에 비해 자조집단지지와 돌봄부담이 더 높게 분석되었고, 반대로 신체장애 자녀를 돌보는 부모 집단은 정신장애 자녀를 돌보는 집단에 비해 돌봄만족과 돌봄역량이 더 높은 것으로 나타났다. 이러한 결과를 바탕으로 실천적 서비스 개입 방안을 제시하였다.
본 연구는 정신질환자의 범죄와 치료에 대한 국내외의 주요 연구 문헌들에 대한 개관을 통해 정신질환의 특성에 따른 효과적인 범죄 예방과 치료 방법을 탐색 및 규명하고자 했다. 본 연구 결과 첫째, 정신질환과 범죄와의 관계에 대해 아직 명확한 결론이 내려지지 않고 있으며, 그 관계와 함께 정신질환자의 환경적 스트레스와 촉발 자극 등도 같이 연구해야 할 필요성이 강조되었다. 둘째, 정신질환의 정의에 여러 가지 개념이 혼재해 있으므로, 조현병이나 망상장애, 반복성 기분장애와 같은 중증 정신장애, 반사회적 성격장애, 지적장애, 분노·충동조절장애, 약물남용 등의 특성에 따른 범죄 예방과 치료에 대해 연구할 필요성을 기술했다. 특히 조현병 등의 중증 정신장애의 경우 약물치료와 사례관리만 잘 된다면, 범죄발생과 재범율이 낮아져 지역사회에서 정상인들과 함께 잘 생활할 수 있음이 많이 보고되어 왔다. 그러나 반사회적 성격장애, 지적장애, 분노·충동조절장애와 약물남용의 경우에는 지역사회에서의 적응과 생활이전에, 심리치료는 물론 강도 높은 의료적, 생물학적 개입과 격리 등이 우선적으로 이루어져야 한다고 할 수 있다. 본 연구결과 정신질환자의 범죄를 효과적으로 예방하고 치료하기 위해서는 정신질환의 특성에 따른 세분화된 연구와 관리가 필요하다고 할 수 있다. 또한 국가적, 사회적으로는 정신질환자에 대한 인식개선, 인권증진과 효율적인 지역사회 돌봄도 함께 이루어져야 할 것이다. 본 연구의 한계점과 향후 연구과제가 논의되었다.
본 연구는 구술자료 분석을 통해 이혼을 경험한 여성 정신장애인의 삶을 보다 깊이 이해하고 치유적 방안을 도출하는데 목적을 두었다. 연구에는 우울증과 공황장애 진단을 받은 여성정신장애인이 참여하였으며 생애사 분석을 위한 텍스트는 구술자료 인터뷰로 구성했다. 텍스트는 Mandelbaum(1973)이 제안한 생애사 분석틀에 의거하여 '삶의 영역', '전환점', '적응'의 3가지 개념틀로 분석했다. 자료분석 결과 '삶의 영역'은 "버려진 아이", "정신질환 발병", "종교", "범죄"로 분석했다. '전환점' 분석에서는 "이혼", "생이별"로 분석했으며, '적응'은 "합리화"로 도출됐다. 이러한 연구결과에 근거하여 이혼을 경험한 여성 정신장애인의 삶의 치유적 개입 방안을 제안했다.
본 연구는 장애인서비스 전문사서 양성 교과과정 모형개발을 위한 기초연구이다. 이를 위하여 문헌을 통한 이론적 연구를 수행하고 국내 문헌정보학과 교육과정 장애인서비스 담당사서의 계속 교육과정 및 서울시 공공도서관의 장애인서비스 현황을 조사 분석하였다. 조사 결과를 요약하면 다음과 같다. 첫째, 학부 대학원 과정의 경우 현재 각각 한 곳에 장애인서비스 전문사서 양성과 관련된 교과과정이 개설되어 있었다. 둘째, 장애인서비스 담당사서의 계속 교육과정은 국립중앙도서관에서 2010년 처음 강좌를 개설한 이후 매년 강좌를 개최하고 있으며, 강좌 이수 후 설문조사를 실시하여 이수자들의 의견을 다음 강좌내용 선정에 반영하고 있다. 셋째, 현재 공공도서관을 이용하는 장애인은 시각장애인보다는 지체장애, 정신지체, 정서 행동장애, 자폐성 장애가 있는 이용자가 많은 것으로 나타났다. 장애인서비스는 관외대출서비스가 대부분을 차지하고 있었으며 일반자료를 대상으로 이루어지고 있었다. 전화인터뷰에 의하면 현재 장애인서비스를 적극적으로 수행하고 있는 소수 기관의 담당사서들은 장애인에 대한 이해, 장애인 문화프로그램 운영 기획에 관한 정보를 필요로 하고 있었다.
Objectives : This study of oral health problems was conducted to 311 students in Special Schools in order to inquire into the state of their oral health behaviors, to find out the main obstacles against dental services, to secure dental reasonable basis for oral health promotion. Methods : A questionnaire survey was conducted in the Special School of the 311 students in the area of the metropolitan Seoul. By means of Chi-squared test and Fisher's exact test, oral hygiene habits for each type of the subjects, the contents of dental care services, the prerequisites to improve oral health, were inquired. To evaluate the effects to the current oral health status of types of disability, 2-way ANOVA was practiced. Results : The handicapped with Visual impairment, mental retardation, multiplicity with disabilities, answered negatively in their own oral health status. 47.4% of the deaf can do brush for theirselves without the inconvenience, but in other types of disability they showed that they were helped by others. Subjects did not use the secondary oral hygiene necessaries because, except brain damage, almost of types of disability impeded the convenient use. 60 to 88.2 percent of the total respondents were the recent visitors to dental clinique within 1 year, and the most common motives of the visiting dentist, is a routine medical examination, their movements to the dentist in all types of disabilities, needed helps of others. Most of them received medical treatment at a private dentist, the handicapped preferred the private care and place(49.8%). Only in the case of the brain disorders, extremely much of the disabled answered that they were needed the preventive treatment, and the another cases of disability were largely needed the treatment of the decayed teeth. To improve the oral health of the people with disabilities, at the opening of the clinics and hospitals over a certain size, the mandatory medical facilities for the care of the disabled should be preceded and followed by the improvement of dental insurance system so as to reduce the burdens of the cost of dental care. Conclusions : The improvement of oral health policy for the disabled are needed inevitably: Development of secondary oral hygienic easy to use for the disabled, the building systems of medical dental hospital with the disabled facilities, by the improvement of the insurance system, the reduction of the burden of payments.
The nutritional status is important for the physical and mental development of children. Children should have sufficient nutrient intake, specially for the developmentally disabled children. In spite of the importance not much research has been done for their food habit and nutritional status. In this study, nutritional status of the disabled children were examined, the number of children studied is 50 children in Seoul between 5 to 6 years old, of whom 11 were austism, 5 were mentally retarded children and 8 were down\\`s syndrome. In order to get the data questionnaires about the food habit and other related factors, and two days food records were used. Their mean height was 108.8$\pm$21.0cm and their average weight was 19.5$\pm$4.6kg. The proportion of study subject with problems of the food habits was 36%, without keeping the regular meal time was 16%, and having snacks 2-4 time daily was 56%. They chose confectionery and fruits as their favorite snacks. The proportion of study subject with the sensitivity to the food colors and smells when they ate was 54%, unskilled using spoon and fork was 58%, and having medications was 20%. When we asked them whether their parents used the food as compensation means or not, 32% of them said that they did it often or sometimes. For the nutrient intake, energy was 1,703 $\pm$ 511kcal, carbohydrate was 242.5 $\pm$ 69.3g, protein was 61.3 $\pm$ 17.5g, lipid was 50.0 $\pm$ 24.4g. As the nutrient intake, it was compared with RDA. The intake of vitamin A and calcium were below RDA. Intake of protein was more than 150% of RDA. The proportion of subject with more than 125% of RDA was 72% for the phosphorus, 62% for the protein, 60% for the Vitamin A and calcium were below 0.75 and that of the others was above 0.75. The mean adequacy ratio(MAR) was 0.88. As we sum up the results of the research, the developmentally disabled children were shorter and lighter than the reference, were having medications, had excessive appetites, and had unbalanced diets. Also they had the bad food habit. As for the nutritional intake, vitamin A and calcium intake were a little less than RDA, and protein and phosphorus intake were too much. With the results of the above research for the disabled children, we can conclude that there are need to take some necessary measures for those children. 본 연구는 노인이 되어 자신의 노화로 인한 어려움을 지니면서도 여전히 성인 장애자녀의 보호역할의 책임을 지니며 살고 있는 노인부모의 실태와 노후생활을 위한 보건복지 욕구를 고찰하여 향후 고령화가 지속되면서 복합화 되는 노인문제에 대한 체계적 연구와 정책적 관심의 필요성을 제기하고자 시도하였다. 성인 장애자녀를 돌보는 65세 이상의 저소득 노인부모를 대상으로 하였으며, 일반적 특성, 장애자녀특성, 노후생활 준비 및 보건복지 서비스 이용 및 필요성에 대한 욕구를 조사하였다. 성인 장애자녀를 돌보는 저소득 노인 부모는 신체적 노화와 함께 오랜 기간 동안 장애자녀를 돌보면서 치료 및 재활과 관련된 비용의 지속적인 지출, 사회 활동의 단절이라는 복합적 원인에 의해 노후생활 전반에 걸쳐 어려움을 경험하고 있었다. 이러한 노인부모의 경제적 어려움, 신체적 쇠약함, 그리고 여전히 끝나지 않은 자녀 돌봄의 역할은 이들의 노후생활을 위한 보건 복지 서비스 이용 및 욕구형성과 밀접한 관련성을 나타냈다. 성인장애자녀를 돌보는 대부분의 노인부모는 자신의 노후생활을 위한 준비를 전혀 하지 못하는 것으로 나타났으며, 이는 자녀의 장애기간과 관련이 있었다. 제공되고 있는 보건복지 서비스는 현물 형태의 직접적이고 수혜적인 성격이 강한 서비스를 주로 이용하였고, 좀 더 자발적이면서 추가적인 비용이 요구되는 사회정서적 영역에 대한 서비스 이용 및 필요도에 대한 인지는 낮게 나타났다. 따라서 향후 장애자녀를 돌보는 노인부모의 질적인 노후생활을 위한 대응방안 모색은 장애자녀의 사회통합이라는 목표와 함께 장기적인 측면에서 연구될 필요성이 있으며, 이러한 현실과 상황에 대한 이해를 바탕으로 이들을 위한 체계적인 지원프로그램 개발에 대해 지속적인 논의가 진행되어야 할 것이다.
Eater Seal, the child care experts, observes that children with disability significantly benefit from receiving appropriated care along side their peers. Typically developing children serve as role models from which children with disabilities learn age-appropriated communications and social behavior. Therefore the children with disability can actively participate in well-designed child care settings. But unfortunately in Seoul, Korea, only 2% of children with disability are taken care of in child care centers. And even the disabled children at the child care centers are mostly mentally or emotionally disabled because the building, programs and services of the centers are restrictive to the children with severe physical disabilities. This study reviewed Korean domestic physical environment of child care centers for children with disabilities. Questionnaire survey was conducted to 103 centers located in Seoul by mail. Our survey revealed that they need more areas for nursing room and special rooms and the accessibility to buildings has to be improved. Also, there are no elevators in all buildings surveyed. Accessibility to toilet is noted as significant problem, especially wet floors in toilets. In a nursing room, a space for psychological rest and special education is needed for children with mental disability. Technical specifications on how to make buildings and facilities accessible for the children with disability should be developed. The goal of this study is to provide basic information to develop domestic design guidelines to ensure that the child care centers are safe, convenient, and usable for everyone possible.
Background: The number of patients with disabilities who require general anesthesia for dental treatment is increasing. Methods: We reviewed the cases of 538 patients with disabilities who underwent outpatient general anesthesia for dental treatment at the clinic for the disabled in Seoul national university dental hospital according to the types of disabilities and comprehensive treatments. Results: The patients mean age was 17.4 years and fifty-eight percent of the patients had epilepsy, autism, mental retardation and cerebral palsy. About twenty-five percent of the patients were medically compromised such as brain disease, heart disease and genetic problem. In 142 cases, more than two dentists from the clinical departments systemically co-worked to provide the patients with comprehensive treatment as accurately and quickly as possible under general anesthesia. Conclusions: The number of medically compromised patients was high and the system of comprehensive dental treatment was well run.
