Correlations of Deficiency and Excess Patterns between Menstrual Symptoms and Whole Body Symptoms (월경통(月經痛) 증후(證候)와 전신 증후(全身 證候)의 허실(虛實) 상호 관련성 연구)

  • Purpose: Dysmenorrhea mostly depends on the causative factor, which usually falls under the categories of Deficiency and Excess pattern in traditional Oriental medical theories and diagnosis. Thus, we investigated menstruation symptom and sign related to dysmenorrhea and verified the validity of Deficiency and Excess pattern identification. Methods: We investigated menstruation symptom and sign related to dysmenorrhea in total 14 gynecology-medical books including the book ${\ll}$Exemplar Of Korean Medicine (Dongui Bogam)${\gg}$ and whole body symptom and sign identifying Deficiency and Excess pattern at the same time. A survey based on this investigation was carried out targeting women of childbearing age. Results: Total of 14 gynecology-medical books have mostly narrated pre-menstrual and mid & post-menstrual pelvic pain depending on the time of its manifestation for identifying Deficiency and Excess pattern. Dysmenorrhea in pre-menstrual period belonged to Excess pattern and dysmenorrhea in mid & post-menstrual period belonged to Deficiency pattern. Among a total of 343 women, 196 subjects suffered from dysmenorrhea. The number of dysmenorrhea in pre-menstrual period (Excess pattern) was 116 people and in mid & post-menstrual period (Deficiency pattern) was 80 people. Deficiency and Excess pattern of dysmenorrhea in menstrual period significantly correlated to Deficiency and Excess pattern of whole body symptom and sign in the statistics(P-value < 0.05). Conclusion: The results suggest that pre-menstrual and mid & post-menstrual pelvic pain depending on the time of its manifestation is preferentially utilized as symptom and sign related to dysmenorrhea identifying Deficiency and Excess pattern.

A Study on Menstrual Pattern of Patients with Cold Hypersensitivity on Hands and Feet (수족냉증 환자의 월경 특성에 관한 연구)

  • Objectives: This study was aimed to analyze clinical characteristics and menstrual pattern of female patients who have cold hypersensitivity on hands and feet. Methods: This study was performed from May, 2018 to April, 2019. We checked the patients' hand and feet's temperature and those who diagnosed with cold hypersensitivity filled in survey. Data were collected from 27 participants and we divided data into 2 group; Dysmenorrhea group and Non-dysmenorrhea group. Results: Throughout the study, we found out differences between 2 groups in age, height, weight, VAS score of cold hypersensitivity on hands and feet, menstrual pattern, upper and lower extremity temperature difference, pattern identification, VAS of menstrual pain, and pattern of leukorrhea. Conclusions: By analyzing of study, Dysmenorrhea group's VAS score of cold hypersensitivity on hands and feet was $6.67{\pm}1.73$, Non-dysmenorrhea group's VAS score of cold hypersensitivity on hands and feet was $7.78{\pm}1.72$ (p-value 0.015). This shows that the degree of hypersensitivity on hands and feet is not necessarily proportional to the degree of menstrual pain.

Results: According to 14 gynecology-medical books, polymenorrhea is relevant to Heat pattern, oligomenorrhea to Cold pattern and darkness of menstrual blood is relevant to Heat pattern. Among the total of 343 womens, 196 subjects suffered from dysmenorrhea. The number of dysmenorrhea with polymenorrhea(Heat pattern) was 6 person, with oligomenorrhea(Cold pattern) was 27 person. And the number of dysmenorrhea with darkness of menstrual blood(Heat pattern) was 39 person. As the result of checking correlations of menstrual symptom scores and whole body symptom scores, there was no significance of Cold and Heat pattern between menstrual symptoms and whole body symptoms. Conclusions: The results suggest that the period of menstrual cycle and the color of menstrual blood provides some informations of Cold and Heat pattern identification. But considering with other whole body symptom and sign is needed for more precise result.

  • Objectives In order to find out the relationship between the Eight Principle Pattern Identification of Cold-Heat, Deficiency-Excess and the Sasang constitution, we analyzed the clinical data from 500 women with menstrual pain and women without menstrual pain. Methods In the previous study, the subject's information of Typology Complexion Pulse and Symptom was collected, and Eight Principle Pattern Identification was executed based on this. Later, the relationship between the Sasang constitution and the Eight Principle Pattern Identification was statistically analyzed. Results and Conclusion 1. The obvious difference between the experimental group and the control group in the patterns of Cold-Heat and Deficiency-Excess is that patients who complain of menstrual pain do not maintain harmony with the yin-yang ratio, it can be said that the patterns of Cold-Heat and Deficiency-Excess can be a Identification standard that significantly obscures the condition of the disease. 2. There was a significant difference between the Sasang constitution and the Eight Principle Pattern Identification of Cold-Heat. There was no significant difference between the Sasang constitution and the Eight Principle Pattern Identification of Deficiency-Excess.

A Study on comparison of menstrual pattern according to Mibyeong Index of Korean women in their thirties and forties (30-40대 여성의 미병상태에 따른 월경양상 비교연구)

  • Introduction : The aim of this study was to investigate menstrual pattern according to Mibyeong Index(MBI) and compare Mibyeong symptom score according to severity of menstrual pain. Mibyeong Index was designed to measure inconvenience and resilience of subject's complains including four physical symptoms(fatigue, pain, low sleep quality, indigestion) and mental distress including anxiety, anger, depression. Method : We used the clinical data of Korean medicine Date Center(KDC) for subjects who participated in the study of 'Clinical research for collecting of clinical cases based on the personal type of Mibyeng' conducted in Seoul, from June 26, 2015 to June 26, 2017. A total of 566 fertile women aged 30 to 50 who completed the questionnaires were included in this study. In this study, we used items of Mibyeong Index and the menstrual pattern by self-report questionnaires. The date were analyzed through Kruskal Wallis test, Pearson's chi-square test, and one-way ANOVA using SPSS statistics 19.0. Results : Mibyeong status was significantly associated with severity of menstrual pain(p<0.001), worst day of menstrual pain(p<0.05), and type of menstrual pain(low back pain, nausea, depression, and none (respectively, p<0.05)). In addition, The 7-subtype score (fatigue (p<0.001), pain(p<0.001), low sleep quality(p<0.001), indigestion(p<0.001), anxiety(p<0.05), anger(p<0.05) and depression(p<0.05)) of Mibyeong index and total score of Mibyeong Index (p<0.001) showed significant difference for severity of menstrual pain. Conclusion : This result indicates that a close relationship between Mibyeong status and severity of menstrual pain. Further studies are needed but, we hope that this results will be used as the basic data to improve mibyeong status through health care to alleviate dysmenorrhea.

  • Purpose: The purpose of this study is to recognize menstrual pattern and dysmenorrhea related factor of first-year women student at college. Methods: From March 22nd to April 2nd, 2010 we researched 382 first-year women students at 00college, aged 19(born after March 1991 to February 1992) by questionnaires to investigate their menstruation and stress. Results: 1. There was statistical correlation between dysmenorrhea and weight and BMI. 2. There was statistical correlation between dysmenorrhea and menstrual amount. 3. There was statistical correlation between dysmenorrhea and menstrual blood clot. 4. There was statistical correlation between dysmenorrhea and stress. Conclusion: This study showed the menstrual pattern and relation between dysmenorrhea and stress. Dysmenorrhea is related to weight, BMI, menstrual amount, menstrual blood clot and especially stress is closely related to dysmenorrhea.

  • Premenstrual symptoms can begin at anytime after menache and usually remit with menopause. Numerous studies of the premenstrual symptoms have been reported, based on mature woman and clinic patients. As yet, however, no prospective study has specially examined the extent or nature of PMS in adolescents. The authors investigated the pattern of mood, behavior and physical symptom changes in the menstrual cycle for 230 high school students who reported premenstrual symptoms by using Baily Rating form(DRF), The scores on 21 symptoms of DRF were recorded daily for at least one menstrual cycle. The results as follows : 1) 1.7%(4/230) of the subjects who reported premenstrual symptoms met criteria for premenstrual dysphoric disorder(PMDD) by 30% change criteria. 2) The pattern of symptom changes in the menstrual cycle showed a clear increase in mean daily symptom score at premenstrual -1,-2 day. and reaching a maximum scores at the first day, rapidly decreased at day 4 during menstruation. 3) The mean daily symptom score in the menstrual phase was significantly higher than those in the premenstrual and postmenstrual phase, and significantly higher in the premenstrual phase than that in postmenstrual phase. 4) The scores of the subjects with longer duration of menstrual bleeding were higher than those of the subjects with the shorter duration. Particularly in the menstrual phase, there was a significant difference on the mean scores of physical symptom duster between the longer and shorter group. These results demonstrated that the mean daily pattern of symptom changes in the menstrual cycle for the subjects was similar pattern of change for the PMDD group, except high level of symptom scores if premenstrual phase. The adolescents might experienced more physical discomfort than the other symptoms in the menstrual phase Especially the subject with longer duration of mense flow were more likely to report more discomfort than that of shorter duration.

Systematic Review of Korean Medicine-related Study on Diagnostic Tools and Pattern Identification registered of Dysmenorrhea in the Korean Journal (국내 전자저널에 수록된 월경통 평가지표 및 변증에 대한 한의학적 임상연구 고찰)

  • The purpose of this review was to survey the Korean Medicine related papers about women with primary dysmenorrhea in order to develop the clinical protocol of the diagnostic medical device. We searched the literature from 2000 through April 2015 using 5 online databases including Oriental Medicine Advanced Searching Integrated Sysptem (OASIS), Research Information Sharing Service (RISS), DataBase Periodical Information Academic (DBpia) and Korean Medical Database (KMBase). We selected papers to meet the following inclusion criteria: the papers involved dysmenorrhea (excluding secondary dysmenorrhea), published papers (excluding textbook, educational materials, conferences, etc.) and the papers matched search keywords or scope, but excluded papers to meet the following exclusion criteria: the duplicative papers, get out of the keywords and scope and not in english or korean language. Finally we found 17 papers and classified the papers according to the three search purposes which were diagnostic tools for evaluating the menstrual pain, dysmenorrhea' pattern identification and menstrual phase. Out of the 16 studies, 4 studies were focused on the diagnostic tools including Visual Analogue Scale (VAS), Measurement of Menstrual Pain (MMP) and etc. Other 5 studies were aimed at menstrual phase, and the other 7 studies were studied for pattern identification. The VAS has been widely used in research and in clinical practice for the detection of the menstrual pain. Treatments for patients with primary dysmenorrhea can be prescribed in consideration of their patterns of sasang constitution or body constitution as following: Qi stagnation-Blood deficiency, cold dampness, Qi deficiency-blood deficiency and liver-kidney deficiency etc. This results of research will be used as a useful material during plan a clinical study of primary dysmenorrhea and acquisition of good clinical data.

The Research of Corelation between Sasang Constitution and Pattern Identification through Clinical Research of the Patients of Menstrual Pain (임상연구를 통한 월경통 환자의 사상체질, 변증간의 상관성 연구)

  • Objective We researched to analyze correlation between menstrual pain and Korean medical diagnosis - Sasang constitution and Pattern Identification - through clinical research of the patients of menstrual pain. Method Trial gruop who consists of women with menstrual pain is 193 people, and control group who consists of women without menstrual pain is 101 people. We producted analyzing of their Sasang Constitution and symptom with Diagnosis System of Oriental Medicine(DSOM). This study used chi-square test, two sample t-test, analysis of variance, Wilcoxon's rank sum test, Kruskal-Wallis test, correlation analysis. Results 1) In trial and control group, Soeumin was the largest and Soyangin was the least. There was'nt statistical significance between trial and control group. 2) In DSOM, deficiency of blood, stagnation of qi, blood stasis, coolness, spleen, kidney and phlegm pathogenic factors showed significant difference. Conclusion There were significant corelation between menstrual pain and Korean mdical diagnosis - Sasang constitution and Pattern Identification.
