A Philological Study on The Pyrite (자연동(自然銅)에 대(對)한 문헌적(文獻的) 고찰(考察))

  • Objectives: The aim of this study is to find the therapeutic meaning of the Pyrite in herbal medication. Methods: About the origin, the component, the processing the drug, the properties and tastes of drugs, the meridian tropism, the effects, the treating disease, the contraindication and the method of administration. We have researched thirty three literatures to mention the pyrite in time sequence. Results: 1. The pyrite belongs to the metallic herbs and it consists of Iron sulfide, sometimes containing small amounts of cobalt, nickel. silver, and gold. 2. The processing the drug are the tempering, the annealing with vinegar. the refining drug with water or RADIX GLYCYRRHIZAE and et cetra, and accoring to these methods, the pyrite classified into three groups - the rough pyrite, the tempered pyrite, the annealed pyrite. 3. The properties and tastes of drugs is pungent and regular. If pyrite is well refine, it is nearly safe form the metallic virulence. the meridian tropism is mainly liver meridian. 4. From old times, pyrite has come into general use to treat the injury of muscles and tendons and bone facture because it is effective on dissipating blood stasis, alleviating pain and reunion of bone, muscles and ligament. 5. When the symptom is gone, a medicine to contain pyrite must be stoped taking. Because of pyrite's effect on regulateing Gi and promoting blood flow. Conclusions: This study showed that the pyrite is useful herb to treat the injury of muscles and tendons and bone facture.

Therapeutic Efficacy of Extracts from Root of Acnthopanax Sessiliflorus as Anti-cancer Drug ; in vivo and in vitro Study

  • The cortex and root of Acnthopanax sessiliflorus, a herbal medicine, have been used for several diseases including cancer in Oriental countries. In the previous study, we showed that the cortex of this plant have anti-cancer activity. But its therapeutic efficacy of CORTEX ACANTHOPANAX RADICIS (CAR) is not clarified. For these reasons, we investigated immuno-potentiating and anti-cancer properties of CAR compared with CA, in terms of body and tumor weights, proliferation of thymocytes and tumor cells, and nitric oxide production from macrophages through in vitro and in vivo studies. In our results, administration of CAR reduced tumor mass and increased body weights. CAR also inhibited proliferation of tumor cells in vivo and in vitro dose-dependently. Thymocyte proliferation was accelerated by treatment with CAR and NO production was also promoted by CAR in vivo and vitro. In conclusion, we demonstrated that CAR is useful to treat for cancer as complementary or alternative medicine to Western medication, its therapeutic efficacy is involved in direct inhibition of tumor growth and immuno-potentiating activity.

Physicians and Pharmacists' Perceptions about the Goal Achievement of the Separation Policy of Drug Prescribing and Dispensing, and Benefit and Loss Caused by the Policy in Busan (부산시 개원 의사와 개국 약사의 의약분업 정책목적 달성도와 손익에 대한 인식도)

  • The purpose of this study was to identify health care providers' perceptions about the goal achievement and benefit/loss caused by the separation policy of drug prescribing and dispensing after the policy implemented on July 1, 2001. Uslng stratified sampling method based on the administration area, Ku, 315 physicians and pharmacists were sampled from the rosters of physician and pharmacist association in the city of Busan on 2001. There were 122 and 115 responses from physician and pharmacist sample, respectively. 78.3% of physicians and 50.4% of pharmacists evaluated that the goal of the policy was not achieved. Moreover, 75.3% of physicians and 40.7% of pharmacists did not support the policy. Most physicians and pharmacists considered preventing the citizens with drug abuse and misuses as the most important benefit derived from the policy. However, physicians and pharmacists concerned over raising health care cost that could be patients' burden. The most important physicians' benefit derived from the policy was free choice of all possible medicine that might result in effectiveness of medication. In physicians' the most important loss, most physicians worried about that breaking traditional patient and physician relationship might cause physicians' authority in treating diseases to be damaged. Pharmacists considered the most important policy benefit as hiked social status resulted from enforcement of profession due to the policy whereas they considered the most significant loss as expected financial problems of small pharmacies compared to that of large pharmacies or pharmacies adjacent to hospitals. In the current problems of the policy, physician and pharmacists blamed the government for inadequate preparations of the policy implementation. Physicians and pharmacists also considered citizens' mature attitudes toward the policy as a crucial success factor.

A Study on the Factors Influencing Injury Prevention Practices of the Elderly (노인의 사고예방 실천정도와 그 영향요인에 관한 연구)

  • The purpose of this study is to identify the relationship among injury prevention practices, health locus of control, and response patterns to HLOC of the elderly. Subjects were 121 healthy elderly. The data had been collected from November 5 to 18 in 2001 and analyzed with descriptive statistics, t-test, ANOVA, Pearson's correlation and stepwise multiple regression. Health locus of control and injury prevention practices were measured by using MHLC scale and an instrument created by the researcher on the basis of the results of literature review respectively. The results of this study were as follows: The mean score of injury prevention practices was 2.80 and the mean scores for the health locus of control were internal health locus of control : 17.25, external health locus of control : 16.09, and chance health locus of control : 14.26. The response patterns of the HLOC identified were six types; pure internal, pure powerful others, pure chance, double external, believers in control, and complex control. The 'pure internal' was the largest group(35.5%), and the 'believers in control' was the next(31.4%). The relationship between internal health locus of control and the injury prevention practices of the elderly revealed a significant correlation(r=.215, p=.018). The relationship between external health locus of control and the injury prevention practices of the elderly revealed a significant correlation (r= .208, p=.022). There was significant difference between response patterns of the health locus of control and injury prevention practices(F=2.393, p=.042). There were significant differences between injury prevention practices and general characteristic factors, which were education, family type, administration of medication, injury experience, ADL, and self-directed search for health information. Self-directed search for health information, injury experience, and education explained 16.7% of the variance for injury prevention practices. The above results may be used as the basic data for seeking more efficient way of improving safety of the elderly.

Medication for Benign Prostatic Hyperplasin in a Dog (개에서 발생한 양성 전립선 비대증의 약물 치료 1례)

  • An 8-year-old, male Shih-tzu dog with clinical signs of hemorrhage from penile orifice and pain on lifting hindlimb was brought to the Veterinary Medicine Teaching Hospital, Chungnam National University. The patient was shown polyuria/polydipsia (PU/PD), more than 10 times per day, and gasterointestinal signs such as intermittent vomiting and melena. On laboratory examination, BUN, creatinine and amylase level were all highly increased. Radiographic and ultrasonographic findings included that both kidneys were abnormal, and both prostates were hyperplastic bilaterally and cysts were presented in parenchyma. Additionally, hyperechoic pancreas and intact urinary bladder were observed from ultrasonographic examination. The dog was diagnosed as pancreatitis, acute renal failure and benign prostatic hyperplasia (BPH). After the appropriate treatment for pancreatitis and acute renal failure, He dog was treated with administration of azasteroid for BPH. On day 71 after azasteroid treatment, good response to the treatment was defined by ultrasonography and clinical signs cured completely.

Sinoatrial Reentrant Tachycardia in a Yorkshire Cross Dog (요크셔테리어 잡종견에서 나타난 동방회귀성 빈맥증)

  • An 8-year-old intact male Yorkshire cross dog (7.5 kg of body weight) was referred with the primary complaint of exercise intolerance and occasional syncope. Initial cardiological examination could not identify any abnormalities except mild mitral regurgitation. Exercise stress test revealed chronotrophic incompetence. Furthermore the 1 hr-digital event recording found the sudden onset of paroxysmal sinus tachycaridas (156-172 bpm) lasting few minutes and stopping abruptly. In addition, the tachycardia terminated by vagal maneuver and verapamil administration. Based on this finding, the case was diagnosed as sinoatrial reentrant tachycardia (SART). The dog was treated with diltiazem and enalapril. Although the dog still has exercise intolerance, no syncope has been observed after medication.

A case of steroid-induced psychosis in a child having nephrotic syndrome with toxic epidermal necrolysis (신증후군 환아에서 발생한 독성표피괴사용해 치료를 위해 사용된 고용량 스테로이드로 인한 정신질환 1례)

  • Toxic epidermal necrolysis (TEN) and Stevens Johnson syndrome (SJS) are rare, life-threatening mucocutaneous -diseases, usually attributable to drugs and infections. Corticosteroids have been used in the management of TEN for the last 30 years. This remains controversial and is still much debated. TEN can occur despite administration of high doses of systemic corticosteroids. The psychiatric side effects of corticosteroids can include headache, insomnia, depression, and mood disorders with or without psychotic episodes. Steroid-induced psychosis is dealt with by tapering or discontinuing the steroid; antipsychotics are also sometimes used. We report a case of an 11-year-old boy who was admitted with TEN. He had also been diagnosed as having nephrotic syndrome in the past. Remission was achieved through induction therapy and by maintaining the use of steroids. After a full-dose intravenous dexamethasone for TEN, he showed psychotic symptoms. We diagnosed him as having steroid-induced psychosis. We tapered the steroid use and initiated an atypical antipsychotic medication, olazapine and intravenous immunoglobulin (IV-IG). His symptoms dramatically improved and he was discharged.

Selecting QA Items & Guidelines for Hospital Safety Management (병원내 안전관리 향상을 위한 항목 및 지침 선정)

  • Background : The goal of this study was to establish the QA items and guidelines for preventing and improving of safety management. Therefore we investigated the nurses' recognition and knowledge of the safety and risk procedures and policies, and the agreement between the nurses beliefs on the degree of importance of those procedures and policies, with actual implementation in hospitals. Method : The subjects of this study were 201 nurses who participated in a program called continuing education for nurses, which held in December, 1993. Result: The results of this study were as follows: 1. Among 18 types of hospital risks, the items that scored highest or the need of closer attention in safety management was the needle stick, medication errors, falling, and bed sores. 2. In most questions of the 18 incidences, the nurses showed that the estimated result would have positive signs except for hospital infections, burns, and bed sores. 3. Even though the survey shows that incidences and types of occurences varies according to the person's age and the time of incident, they mostly occur between midnight to 6AM. Falls and bed sores can be seen more in the elderly. Medications errors, hospital infections and burns are frequently found between the ages of one through twenty. 4. There was a higher mean score for recognizing the importance of those items than the importance of implementing them. Conclusion : In summary, nurses did perceive the need of safety management but the hospital policy for proper safety management was not established. So we recommended that the hospital administration would undertake an early detection and proper management system for hospital precautions, based on QA items & guidelines presented in this study.

Evaluation of Nivolumab Use and Factors related to Treatment Outcomes in a Cancer Center of a Top Tier General Hospital (상급종합병원 암센터에서 Nivolumab 사용평가와 치료성과에 미치는 영향인자)

  • Background: We strived to evaluate the status of nivolumab use and associated factors on the clinical efficacy of the drug. Methods: The study was retrospectively conducted in patients who had been administered nivolumab at least once at the cancer center of Seoul National University Hospital from June 2015 to April 2017. Data were collected from electronic medical records. A medication-use evaluation was performed based on the American Society of Health-System Pharmacists mediation-use guidelines. Results: Sixty-six of the 74 patients (89.2%) showed indications approved for nivolumab use by the Korean Ministry of Food and Drug Safety (MFDS; n=55) or the US Food and Drug Administration (FDA; n=11). Approximately 73.0% of the patients were administered the approved dose of 3 mg/kg but 25.7% were administered an unapproved fixed dose of 100 mg. The overall response rate was 21.7%, and the response rate of non-small cell lung cancer patients, who accounted for the largest number of indications, was 18.8%. Adverse reactions were found in 90.1% of the patients and were mostly mild (86%). The expression of programmed death-ligand 1 (PD-L1) was analyzed as a factor affecting treatment response (p=0.028, odds ratio [OR]=11.331). Conclusion: PD-L1 expression was found to affect treatment response. However, caution is required while using an unapproved dosage and in the absence of monitoring for effectiveness and safety. Therefore, an effective protocol or instruction manual for the proper use of nivolumab should be considered.

The Effects of Pilocarpine in Patients with Orofacial Movement Disorder (구강안면운동장애에 대한 필로카핀의 적용)

  • Orofacial movement disorders (OMD) are uncontrolled movement of the muscles involving the face, tongue, lip and mandible. Due to variable oral and lingual muscles affected, the patients with OMD are interfered with the appropriate performance such as chewing, swallowing and talking. In this study, there are 4 OMD cases with oral dryness that saliva flow rate is decreased or not. The symptoms are improved after oral administration of pilocarpine to 4 patients with OMD. Therefore, we suggest that objective or subjective oral dryness could be etiologic factor in OMD and pilocarpine could be regarded as medication for OMD.
