  • The use of Radio Frequency Identification technology (RFID) in medical context enables not only drug identification, but also a rapid and precise identification of patients, physicians, nurses or any other health care related staffs. The combination of RFID tag identification with structured and secured Internet of Things (IoT) solutions enables ubiquitous and easy access to medical related records, while providing control and security to all interactions. This paper surveyed a basic security architecture, easily deployable on mobile platforms, which would allow to establish and manage a medication prescription service in mobility context making use of electronic personal health records.

  • 본 연구에서는 약 관리를 효율적으로 할 수 있는 IoT 기반의 스마트 약상자를 개발하였다. 약을 정기적으로 먹어야 하는 사용자에 대해 약 복용 현황 및 남아 있는 약의 상황 등을 모니터링 하여 효과적으로 관리할 수 있다. 이를 위해 약을 보관할 수 있는 상자를 3D 프린터를 이용하여 제작하였으며, 사용자의 복약 현황을 나타내주는 LCD를 전면에 배치했다. 또한, 약상자 내부에 있는 서보모터를 활용하여 정해진 복용 시간에 정해진 양만큼의 약을 제공할 수 있도록 하였다. 제안하는 스마트 약상자를 이용하여 사용자의 복약을 효율적으로 관리할 수 있으며 복약 순응도를 높이고 약물의 오남용을 막는데 도움을 줄 수 있다.

  • 한 첩씩 복용하는 알약은 복용 여부를 정확히 판별하기 어렵다. 그래서 이러한 상황에서 벗어나고자 만든 스마트 약통을 제안하고 있다. 이 스마트 약통은 약의 오남용을 방지하고, 날짜별 복용 여부를 휴대폰 어플로 알려주는 장점을 가지고 있다. 장기간 복용하는 약은 한 번의 내원으로 많은 양의 약을 처방받아 오기 때문에 기억력이 좋지 않은 어른이 사용하기에 유용하다. 처방 받은 약통에 있는 QR코드를 최초 입력함으써 약 3일 정도의 데이터 수집기간을 통해 평균 복용시간을 인식하고, 평균 복용시간을 인식한 후에 약 먹을 시간을 알려주는 기능을 탑재하였다. 평상시에는 잠금장치를 통해 걸어 열 수 없게 프로그램을 설정하고, 복용시간에만 잠금장치를 해제하여 환자의 약물 오남용을 막고 안전하게 복용할 수 있을 것이다.

  • 의약품 추천 방법은 환자의 현재 건강 상태가 주어졌을 때, 해당 환자에게 효과적이면서도 안전한 의약품들을 추천해주기 위한 방법이다. 최근 의약품 추천 방법들은 환자에게 효과적이면서도 안전한 의약품들을 추천해주기 위해, 환자의 방문 정보(즉, 환자에게 진단된 질병, 수술 정보) 뿐만 아니라 의약품들 간의 (1) 동시 처방 여부 관계와 (2) 부작용 유발 관계를 추가적으로 활용하고 있다. 구체적으로, 이들은 의약품들 간의 두 관계를 각각 임베딩으로 나타낸 뒤 하나의 임베딩으로 융합하여 활용한다. 그러나 기존 의약품 추천 방법들은 이 두 임베딩을 융합하는데 있어서, 두 임베딩 간의 관계를 깊게 고려하지 않고 단순히 뺄셈 연산을 적용하고 있다. 따라서 본 논문에서는 실세계 데이터 집합을 활용하여, 의약품들 간의 두 관계에 대한 임베딩들을 다양한 방법을 통해 융합한 뒤각 방법이 추천 정확도에 어떠한 영향을 미치는지 비교 및 분석하여 더욱 정확한 의약품 추천을 제공하고자 한다.

A Case Study of Implementation of Concurrent Drug Utilization Review System at a General Hospital (동시적 의약품 사용평가(cDUR) 시스템 구축 및 적용 사례 연구 : 국내 한 대학병원을 중심으로)

  • Medical errors such as adverse drug event, improper transfusion, wrong-site surgery, mistaken patient identity and so on commonly occur at health care practice. Information technology, like Drug Utilization Review(DUR) system which reviews, analyzes, and interprets medication data when prescribing, can play a key role in reducing such medical errors and improving patient safety. Korean Government has guided all hospitals to implement concurrent DUR(cDUR) system, which is the first case worldwide in that all healthcare providers have to use cDUR system when prescribing. This paper introduced a case study that a tertiary hospital has integrated the cDUR system into its comprehensive Hospital Information System(HIS) and analyzed the whole prescription data during a week right after adoption of cDUR system. Considering technical strength and weakness, the cDUR system was integrated into the HIS, using Broker Servers for minimizing doctors' anxiety. As the quantitative analysis of the whole prescription data, DUR conflict events, which mainly included duplicate medications and contra-indicated drug interactions for outpatients, were 2.77%. Although only 0.7% is for the contra-indicated drug interactions, it will be greatly devoted to achieve the purpose of DUR such as improving patient safety.

Readability of the Product Labelling Information of Over-The-Counter Pharmaceuticals in Convenience Store (약국 외에서 판매되는 안전상비의약품 설명서의 난이도 평가)

  • Background: Since November 2012, some of over-the-counter (OTC) medications have been sold in convenience store without pharmacist' s supervision. We purposed to examine if the product labels of OTCs provide sufficient information that is appropriate for consumers who may have low health literacy. Methods: We compared the difficulty of words that are utilized in pharmaceutical product labels of interest (intervention) with those in the $6^{th}$ grade textbook (control). Pharmaceutical products of interest were comprised of 13 OTCs which have been sold currently in convenience stores. We grouped words into the 4 levels of difficulty based on the Korean Vocabulary Classification for Education, and statistically tested words frequency in each level between OTCs and control. Results: The 13 OTC labels included lay language (easier or equal to language used in primary school) about 10% less; professional language about 10% more (p < 0.001 in all). Labels for analgesics had the longest and most difficult information, followed by common cold preparations, muscle pain relievers as plaster or cataplasma and digestives. Conclusion: The 13 OTC labels might fail to provide appropriate information for safety use by consumers in terms of the difficulty level of words. The improvement of labels of OTC medications and consumer education strategies are called for safety use of OTC medications sold in convenience stores.

  • Medical data sharing is increasing due to treatment duplication which increases the cost of medication. Medical healthcare system has been improved to combine with cloud computing. It reduces treatment delay and the medical data error. However, the concern about the privacy protection of medical information is also significant. Medical information is more sensitive than other information because involuntary disclosure can affect in both personal and social life. Privacy cloud brokerage has conquered great attention for solving these problems. Our method provides a security model in the cloud computing environment that facilitates the exchange of medical records between assigned custodians. It allows doctors to obtain a complete patient medical records which can help to avoid duplication, reduce the medical error and healthcare cost as well. In addition, our method offers a trustworthy solution against the privacy violence.

Health Behaviors Related to Hypertension in Rural Population of Korea (우리 나라 농어촌지역 성인의 고혈압 관련 행태)

  • Objectives : To describe health behaviors related to hypertension in rural population of Korea and focused to identify inappropriate awareness, treatment, and control of hypertension. Methods : We surveyed 5,517 adults (2,288 males, 3,229 females) older than 30 years in 58 rural areas, purposely sampled nationwide from December 1996 to February 1997. Blood Pressure was checked twice af the time of the first visit. For those who showed high blood pressure using the JNC-6 criteria at their initial visit, we followed up their blood pressure one week later. Also information on the health behavior related to hypertension was collected through the person-to-person interview using structured questionnaire at the first visit. Results : For the past one year, females had more experiences of checking their blood pressure than males (77.3% versus 69.5%, p=0.001). Through the results of consecutively checked blood pressure, only 51.7% of the hypertensives were aware of their condition. Of the hypertensives who aware of their condition, 44.4% did not receive any medication and/or recommendation. And 50.4% of the hypertensives who had anti-hypertensive medication were classified as still having hypertensive blood pressure by 160/95 mmHg criteria. Of the medicated, 54.8% were found to take medication regularly for the past six months. Among the medicated, only 11.4% knew the name of anti-hypertensive drug they had. Conclusions : 'Rule of halves', which works in the situation of no special efforts for hypertension control, was identified. This study showed that much efforts to control hypertension would be required in the rural population of Korea.

Evaluation of Therapeutic Differences of Angiotensin II Receptor Blockers and Calcium Channel Blockers Among Hypertensive Patients Classified by Oriental Traditional Way (한국적 의학 기준에 근거한 고혈압환자의 Angiotensin II Receptor Blockers와 Calcium Channel Blockers의 약물 평가)

  • Background: Oriental lifestyle for treating diseases has been developed and well-accepted for a long time among Koreans. Sasang Constitution theory, originated from Korean traditional medicine, suggests that medication treatment should be differentiated by each patient's body classification (So-yang [SY], So-eum [SE], Tae-yang [TY], and Tae-eum [TE]), in contrary to western medicine's theory that medication should be applied equally by disease indication without such classification. However, the pharmacotherapeutic outcomes of these theories have not been compared to date. In this study, we aimed to compare the two theories by evaluating blood pressure (BP), which is lowered as a therapeutic outcome, among hypertensive patients taking angiotensin II receptor blockers (ARBs) or calcium channel blockers (CCBs), two most commonly used antihypertensive classes in Korea. Methods: From April 2006 to June 2012, we retrospectively collected data on hypertensive patients with Sasang Constitution classification at Kyunghee University Hospital at Gangdong, one of the East-West collaborative medical centers in Korea. We collected information on age, gender, underlying diseases, antihypertensive drugs (ARB, CCB, ARB+CCB), and BP by reviewing the electronic medical records. We excluded patients with missing blood pressure at baseline or follow-up, or those who had a change in their antihypertensive drug class during follow-up. Results: We selected a total of 573 patients (SY: 165, SE: 158, TY: 0, TE: 250). Baseline BPs were on average 139.0/82.0 mmHg for SY, 137.8/78.5 mmHg for SE, and 138.7/79.2 mmHg for TE. In all three groups, CCBs were the most prescribed, followed by combination therapy with ARB+CCB, then ARBs. BP reduction after 1 month of initial medication was significantly different among the drug classes, but not in Sasang constitutional classification (ARB [SY: -12.4/-4.7, SE: -12.3/-2.5, TE: -8.6/-1.8], CCB [SY: -12.3/-5.4, SE: -13.0/-2.3, TE: -10.8/-6.0], ARB+CCB [SY: -15.6/-6.7, SE: -18.4/-8.1, TE: -20.2/-6.7], drug [$P{\leq}0.05$/P>0.05], constitutional type [P>0.05/P>0.05]). Conclusion: We observed significant differences in reduction of blood pressure by classes of drugs (ARB+CCB>CCB>ARB) but not by Sasang constitutional classification. Therefore, current approach of antihypertensive pharmacotherapy assisted by Western medicine is appropriate for treatment of hypertension. However, further larger scale or prospective studies are required in order to confirm these results.

Korean Medication Algorithm for Panic Disorder 2008 : Diagnosis, Treatment Response and Remission of Panic Disorder in Korea (한국형 공황장애 약물치료 알고리듬 2008 : 공황장애의 진단, 치료 반응과 관해의 평가)

  • Objective : This article is a part of the Korean Medication Algorithm Project for Panic Disorder, which aims to build consensus regarding the diagnosis, treatment response and achievement of clinical remission for patients with panic disorder in Korea. Methods : The questionnaire used in this article had parts : 1) diagnosis, 2) treatment response, and 3) remission for patients with panic disorder. The questionnaire was completed by each of 54 Korean psychiatrists who had much experience in treating patients with panic disorder. We classified the experts' opinions into 3 categories (first-line, second-line, and third-line) using the ${\chi}^2$-test. Results : Five factors were considered in this research : panic attack, anticipatory anxiety, phobic avoidance, severity of illness, and psychosocial disability. Most reviewers agreed that the presence of a panic attack was the most important factor in the diagnosis of patients with panic disorder. Phobic avoidance was included in the first-line category, whereas the severity of illness and psychosocial disability were included in the second-line category. Most reviewers also agreed that the presence of a panic attack was the most important factor in determining the appropriate treatment response, and it was included in the first-line category along with several other items. To determine remission status, the patients' scores on tests pertaining to the severity of panic attack, anticipatory anxiety, phobic avoidance, severity of illness and psychosocial disability should be less than 3.0-3.3 on a 9-point Likert scale. Conclusion : We suggest useful information for making a diagnosisof panic disorder, determining the appropriate treatment response and identifying remission in panic disorder patients on the basis of the results of a nationwide survey of experts in Korea.
