사망진단서(사체검안서) 상의 선행사인으로부터 사망통계의 원사인이 선정되는 비율: (3개 대학병원에서 교부된 사망진단서를 중심으로) (The rate that underlying causes of death for vital statistics are derived from the underlying causes of death recorded at death certificates: (a study on the death certificates issued from three university hospitals))

  • Background : To examine the problems involved in writing practice of death certificates, we compared the determination of underlying cause of death for vital statistics using recorded underlying cause of death in issued death statistics. Methods : We collected 688 mortality certificates issue in year of 2,000 from 3 university hospitals. And we also collected vital statistics from ministry of statistics. The causes of death were coded by experienced medical record specialists. And causes of death determined at ministry of statistics for national vital statistics were mapped to causes of death recorded at each death certificates. The rate that underlying causes of death for vital statistics were derived from underlying causes of death recorded at issued death certificates were analysed. Results : 64.5% of underlying cause of death for could be derived from underlying cause of death recorded at issued death certificates, 8.6% derived from intermediate cause of death, and 3.9% derived from direct cause of death. In 23% of cases, underlying cause of death could not be derived using issued death certificates. The rate that underlying cause of death for vital statistics could be derived from underlying cause of death recorded at death certificates was different between 3 university hospitals. And the rate was also different between death certificates and postmortem certificates. We classified the causes of death using 21 major categories. The rate was different between diseases or conditions that caused death too. Conclusion : When we examined the correctness of death certificate writing practice using above methods, correctness of writing could not be told as satisfactory. There was difference in correctness of writing between hospitals, between death certificates and postmortem certificates, and between diseases and conditions that caused death. With this results, we suggested some strategy to improve the correctness of death certificate writing practice.

연세대학교 의과대학 학습공동체 교육과정 개발 및 운영 분석 (Development and Implementation of a Learning Community in the Curriculum for Undergraduate Medical Students)

  • Learning communities in medical education have demonstrated favorable outcomes in terms of students' learning, professional development, and wellness. Despite these strengths and the widespread adoption of learning communities in US medical schools, there has been little interest in medical learning communities in Korea. In this context, the present study examined the development and implementation of the Yonsei Medical Learning Community (YMLC) and analyzed its outcomes and areas of improvement. The Yonsei University College of Medicine has operated a learning community as part of the undergraduate medical education curriculum since 2014. The YMLC is the first program of its type in Korea. The overall structure of the YMLC consists of four distinct communities (pillars), which are named after four distinguished alumni, and each pillar is organized into five learning community classes. Each class is vertically integrated across students in different medical school years, and one faculty advisor is matched to about 30 students. As the YMLC focuses on fostering reflective practice in students and providing them with opportunities to build teamwork and experience social relatedness, two educational approaches have been adopted: reflective writing and mentoring and community activities. In this study, we obtained and analyzed second-year students' feedback on the YMLC curriculum and identified its achievements, merits, and areas that need improvement. The results have shown that over 75% and 60% of respondents reported satisfaction with reflective writing and mentoring and community activities, respectively. The educational activities of the learning community helped students regularly reflect on their learning and progress and establish close relationships with faculty advisors. However, several areas of improvement regarding content, format, and logistical issues were also identified. The present findings may provide valuable information for other institutions to develop learning communities relevant to their own context.

필담(筆談)을 통한 조일(朝日) 의원(醫員) 간 소통의 방식 - 1763년 계미사행(癸未使行)의 필담을 중심으로 - (A study on the communication between the medical staff of Chosun and Japan to conversation by writing)

  • 본 논문은 1763년 계미사행(癸未使行) 중 의원필담집(醫員筆談集)인 키타야마 쇼우[北山彰]의 "계단앵명(?壇?鳴)"과 야마다 세이친[山田正珍]의 "상한필어(桑韓筆語)"와 이 빈케이[(井敏卿]의 "송암필어(松菴筆語)"를 연구 대상으로 삼아 18세기에 의원을 포함한 조일(朝日) 지식인들의 소통 방식 및 대립과 갈등, 교류와 협력의 상황을 확인하는 데 그 목적이 있다. 그들은 미지(未知)와의 조우를 통해 나누고, 시문(詩文)을 창화(唱和)했으며, 해부학 등 당시 서양의학에 눈뜬 일본의 상황을 가늠해보기도 했다. 또한 각 나라의 풍습을 접하고 비교하는 기회를 가졌으며, 아울러 한글에 대한 일본인의 증폭된 관심을 확인할 수 있다. 대립과 갈등은 주로 두 나라의 역사 인식과 학술적 차원의 관점 차이에서 비롯되었는데, 특히 당시 일본의 고학(古學)적 학풍이 여기에 상당한 영향을 끼쳤다고 할 수 있다. 따라서 금고문(今古文) 논쟁이나 두 나라의 음악 비교, 일본 신공황후(神功皇后)의 삼한(三韓) 정벌설 및 오규 소라이[荻生?徠]와 다자이 준[太宰純]의 학문에 대한 논쟁 등으로 상호 날선 공방이 오고갔다. 다만 간과하지 말아야 할 것은 이 중에는 한문에 의한 필담이란 제한 때문에 빚어진 상호 오해도 존재한다는 점이다. 교류와 협력은 전문 지식과 기술을 지닌 의원들을 중심으로 인삼 제법(製法)이나 질병의 치료 방법을 확인하는 절차로 구체화되었다. 또한 일본에서의 서적 구입 방법 문의 및 실제 치료 등을 통해 양국 우호의 분위기를 연장시켜 나갔다고 볼 수 있다. 결국 이들 사이에는 간혹 오해와 불신도 있었지만, 양국의 인사들은 한문이란 동아시아 공통문자를 통해 소통했고, 심도 있는 정보와 의견을 교환했다.

『침구경험방』을 통해 본 17세기 조선 의료와 침구기법 (The 17th century Medical Service and Acupuncture & Moxibustion Technique in the period of Joseon Dynasty viewed through 'Chimgugyeongheombang')

  • All this while, 'Chimgugyeongheombang' has been rated as the level of Im Heo's personal writing. However, Huh, m, in his introduction of 'Chimgugyeongheombang' made it clear that his purpose of publishing this book was to criticize the doctors of the day who were tied down by one or two curative methods, and at the same time, ultimately to improve the quality of national medical service by enlightening them. It seems that such incompetent doctors' engagement in medical service at that time was attributable to insufficient medical education system unlike today as against much social demand for acupuncture & moxibustion therapy. Im Heo, in an effort to enlighten the medical practitioners of the day, put emphasis on an eclectic method, and contained the theories on internal organs and meridian on a systematic basis. In addition, he made his updated medical skills known to medical circles by integrating and wrapping up the Acupuncture & Moxibustion techniques at that time. Lastly, he tried to provide the optimized information for clinical trials in his own language by using a preposterous writing style and form. In such a context, 'Chimgugyeongheombang' is not a special medical book gripped with a sentence, or a simple medical formula with an emphasis on empiricism, but rather it might be proper to say that his 'Chimgugyeongheombang' was a serious attempt to embrace all the merits of both parties.

  • Objectives : First, the time of publication of the writing Jinjeong(陳情), was identified with consideration of current affairs at the time in Hamgyeongdo. Second, the relevance between agricultural policies and military/defense policies were identified. Third, based on these findings, the phrase "洋醜之漁獵頻泊汊 港, 峩人之耕牧恒據沿䐉" was throughly examined within its historical context. Methods : First, the time of publication of the writing Jinjeong(陳情), was identified with consideration of current affairs at the time in Hamgyeongdo. Third, based on these findings, the phrase "洋醜之漁獵頻泊汊 港, 峩人之耕牧恒據沿䐉" was throughly examined within its historical context. Results & Conclusions : 1. The Jinjeong(陳情) of the Dongmuyugo(東武遺稿) is a writing on crossing borders into the Maritime Province of Siberia, which was written between the end of September and December of 1870. 2. The phrase "洋醜之漁獵頻泊汊港" is related to the incident when a handover line was at anchor in the Unggi harbor three times in 1869. 3. "我人之耕牧恒據沿海" refers to the rumor spread in 1870 about Russia being an ideal country that practiced ideal politics, to the extent of cattle and horse being gifted to immigrants into the country.

마진편(麻疹篇) 저자와 저술 시기에 대한 고찰 (A Study on the Author and Writing Year of the Marjinpyeon)

  • A thesis insisting that Sugjong's medical doctor Yoo Itae(劉以泰) and the author of Marjinpyeon Yoo Itae(劉爾泰) are not a same person and his activities were in Heonjong(헌종) period, and his writing Marjinpyeon is in 1846, is published. In this study, I'd like to reveal that Yoo Itae(劉爾泰), the Marjinpyeon's author and Yoo Itae(劉以泰), Sugjong's medical doctor are a same person through analysis of Geochang Yoo's genealogy, articles, The True Record of the Joseon Dynasty, Seungjeonilgi, Taeweonseonsaengan, Sancheong-gun's paper, Sancheong Hyanggyo's paper, Sancheong-gun's paper, The geographic paper, Dongyuhagan, Sawoo Munjib, and analysis of folk story, the wrote year of his Marjinpyeon and Inseomunkyunrok and his birth year. According to my analysis, Yoo Itae(劉爾泰), the Marjinpyeon's author and Yoo Itae(劉以泰), Sugjong's medical doctor are a same person. And Marjinpyeon is written in 1696, the year of Byongja, and other his books, InseoMungyonlok at 1709, the year of Gichug. In conclusion, Yoo Itae(劉以泰.劉爾泰) was a famous medical doctor at Sancheong, born in 1652 (HyoJong the $3^{rd}$ year) and passed in 1715 (SugJong the 41th year), left books of Marjinpyeon, Silheomdanbang, and Inseomunkyunrok.

우리나라 의학전문직업성 교육과정에서의 '전문직 정체성 형성' 교육 현황 (Current Status of 'Professional Identity Formation' Education in the Medical Professionalism Curriculum in Korea)

  • This study examined the current status of the medical professionalism curriculum in Korea to suggest a plan to move towards the formation of a professional identity. Professionalism education data from 28 Korean medical schools were analyzed, including the number of courses, required or elective status, corresponding credits, major course contents, and teaching and evaluation methods. Considerable variation was found in the number of courses and credits in the professionalism curriculum between medical schools. The course contents were structured to expand learners' experiences, including the essence and knowledge of professionalism, understanding of oneself, social interaction with others, and the role of doctors in society and the healthcare system. The most common teaching methods were lectures and discussions, while reflective writing, coaching, feedback, and role models were used by fewer than 50% of medical schools. Written tests, assignments and reports, discussions, and presentations were frequently used as evaluation methods, but portfolio and self-evaluation rates were relatively low. White coat ceremonies were conducted in 96.2% of medical schools, and 22.2% had no code of conduct. Based on the above results, the author suggests that professional identity formation should be explicitly included in learning outcomes and educational contents, and that professional identity formation courses need to be added to each year of the program. The author also proposes the need to expand teaching methods such as reflective writing, feedback, dilemma discussion, and positive role models, to incorporate various evaluation methods such as portfolios, self-assessment, and moral reasoning, and to strengthen faculty development.
