대퇴경부 골절 환자의 입원 생활 (The Hospital Life of the Patient with Femoral Neck Fracture)

  • Nowerdays, the increase of traffic accidents and old age population make the Femoral Neck Fracture(FNF) patients increase. By the improvement of education and standard of living the patients demand better medical service than before. This study is designed to give practical help for the FNF patients by observing their hospital life and establish practical nursing strategies for the FNF patients. For these purposes the Ethnographic Participant Observation was adopted. By this study is focused on the hospital life patient's view. For this end, the field study adopted orthopedic ward in the C University Hospital with 400 beds in Seoul. The object patients of the study were twelve patients. The patients experienced five stages : Embarrassment, Conflict, Stability, Independent, and Extension Stage. The findings and prepared nursing strategies are stated as follows. First, in the Embarrassment Stage they suffered embarrassment, anxiety, pain, they could not do ordinary things. The patients who accidental fractures had anxiety from unfamiliar tests and from hospitalization itself. They lamented that they could not ordinary things, and do nothing but obeying the hospital, and endure the pain. They recognized the changed environment and resigned themselves to life in the ward. In this stage, full openness by the nurses is needed. Second, the attribute of the Conflict Stage were conflict, fear, curiosity, belief, reflection. When they sign the consentment form, they experience conflicts about the possibility of complication, fear of recovery from anesthesia, curiosity about the operation procedure, post - operation state, reflection on their past life, and promise to care for their family members after discharge and keep their religious life faithfully. And they accepted the operation depending on God, believing in modern medicine, and the surgeon. Asking for their changed informations, they expected positive results from the operation. In this stage, an empathic attitude by the nurses is needed. Third, the attribute of the Stability Stage were relief, gratitude, difficulty with excretion, and pain. When they awoke from anesthesia, they felt relief because of a the end of the operation, but they experienced extreme pain, difficulty of excretion in bed. They accepted the changed environment and expected recovery. In this stage, support by the nurses is needed. Fourth, the attributes of the Independence Stage were freedom, exercise, nurturing, anxiety, and discomfort. When they ambulated and exercised, they experienced freedom. They showed exhibited weakness of the digestive organs and discomfort hospital's space, structure, and facilities, the delay of medical certificate issue the lack of prompt response by the medical agents. They ate nurturious food and felt anxiety on the end of hospital life and returning to their ordinary life. They showed the independence of overcoming their environment by increasing exercise and expected their discharges. In this stage, respect by the nurses is needed for the patients to, overcome their environment and prepare for their independence. Fifth, the attributes of the Extension Stage were pessimism, isolation, dissatisfaction, and pain. Accompanied injury and old age made their ward life extend to over seven weeks. They exhibited weariness, melancholy, skeptisis, general pessimistic feeling, and desperation caused by their isolated life. They experienced the digestive discomfort caused by the prolonged medication and psycological pain caused by long-time hospitalization. As a, result, their dissatisfaction on the human, physical, and systematic environments had been increased. They acquired critical power and sought for something to do spending their time. They expected vaguely about the returning of their ordinary life. In this stage, counseling is needed by the nurse to overcome positively their psychological, social, and physical problems. The process of the FNF patient's ward life starts from the dependent state, when they are hospitalized, and gradually progresses to self-fulfillment in order to keep independent life. As a result, the FNF patients showed "Response in Challenge" or "Adaptation in Conflict" through their experiences of social, physical, and psychological difficulties.

한방 병원에서 발생한 중풍으로 인한 예상치 못한 사망에 관한 고찰 (Clinical Study on Unexpected Death in Stroke Patients)

  • Objective : Stroke is the second major cause of death in Korea. It is known that the survival and ultimate outcome in stroke depend on various factors, so it is not easy to predict unexpected death in stroke. This study was performed in order to find predicting factors of unexpected death in stroke. Methods : A retrospective study was accomplished by reviewing 21 medical records of stroke patients who expired in the ward of Kyung Hee Oriental Medical Center from January 1998 to December 2001. Data analyzed Were physical examination, laboratory findings, clinical charts and APACH III scoring system. Results : I. The number of unexpected deaths at the ward of Kyung Hee Oriental Medical Center from January in 1998 to December in 2001 were 21 patients (0.1%). 2. Major risk factors of unexpected death were age (${\geq}60$), high blood pressure on 1st admission day and acute stage (${\leq}30{\;}days$). 3. The most frequent time of unexpected death was from 6 a.m. to noon. 4. Major cause of unexpected death was recurrence of stroke (40%). 5. APACH III scores of 75% of unexpected death patients were over 30 points. 6. Clinical change of symptoms 3 or 4 days before the unexpected death were dyspnea, change of urination and defecation. Conclusion; This mortality study suggests that old age, high blood pressure on 1st admission day, acute stage, and high APACH III score are the major predictors of unexpected death in stroke patients and that intensive medical attention is necessary to reach a better outcome.

의대생들의 과제해결기반 병원 내 진료지원부서 현장체험에 관한 현상학적 분석 (Phenomenological Analysis of the Task-Based Field Experience for Medical Students: Focusing on the Medical Care Support Department in the Hospital)

  • 본 연구의 목적은 의과대학 1학년을 대상으로 병원 내 진료지원 부서에 대한 과제해결 기반의 현장체험 프로그램을 운영한 후, 학생들의 경험과 그 의미를 현상적으로 분석하는 것이다. 병원 내 진료지원 부서로 간호부(병동과 외래, 중환자실), 인공신장실, 의무기록실, 약제부, 진단의학검사실, 영상의학실, 원무팀, 고객상담센터, 장기이식센터, 국제의료센터를 선정하였다. 학생들은 부서를 방문하여, 인터뷰, 관찰, 직접 시행 등의 다양한 방법을 활용하여 주어진 과제를 해결하고, 프로그램 만족도와 자기성찰 에세이를 작성하였다. 그 결과 프로그램 만족도에서, 학생들은 병원 내에 많은 직종과 역할이 있고, 보건의료 직종의 역할을 이해하게 되었다는 것을 가장 높게 평가하였다. 학생들이 작성한 성찰일지에 나타난 현장체험학습 경험에 대한 본질적 주제는 현실을 인식에서는 '체험이 주는 이상과 현실의 괴리'로 나타났으며, 존중과 협력에서는 '다른 직종의 전문성 인정과 존중', '팀이라는 인식 형성'이 나타났으며, 경험으로부터의 자기성찰에서는 '내면의 변화'가 나타났다.

Model Determination of Delayed Causes of Analgesics Prescription in the Emergency Ward in Arak, Iran

  • Background: According to the reports of the World Health Organization 20% of world population suffer from pain and 33% of them suffer to some extent that they cannot live independently. Methods: This is a cross-sectional study which was conducted in the emergency department (ED) of Valiasr Hospital of Arak, Iran, in order to determine the causes of delay in prescription of analgesics and to construct a model for prediction of circumstances that aggravate oligoanalgesia. Data were collected during a period of 7 days. Results: Totally, 952 patients participated in this study. In order to reduce their pain intensity, 392 patients (42%) were treated. Physicians and nurses recorded the intensity of pain for 66.3% and 41.37% of patients, respectively. The mean (SD) of pain intensity according to visual analogue scale (VAS) was 8.7 (1.5) which reached to 4.4 (2.3) thirty minutes after analgesics prescription. Median and mean (SD) of delay time in injection of analgesics after the physician's order were 60.0 and 45.6 (63.35) minutes, respectively. The linear regression model suggested that when the attending physician was male or intern and patient was from rural areas the delay was longer. Conclusions: We propose further studies about analgesics administration based on medical guidelines in the shortest possible time and also to train physicians and nurses about pain assessment methods and analgesic prescription.

대학병원 병동 및 수술실 근무 간호사의 사회심리적 스트레스와 피로수준에 관련된 요인 (Factors Related to Psychosocial Stress and Fatigue Symptom Among Nurses Working at Ward and Operating Room in University Hospitals)

  • 본 연구는 근무특성이 다른 병동과 수술실에 근무하고 있는 간호사들의 인구사회학적 특성, 건강관련행위 특성, 직업적 특성, 직무스트레스 요인 및 사회심리적 요인에 따른 스트레스와 피로수준을 비교 분석하고자 시도하였다. 연구대상은 대전광역시에 소재하고 있는 4개 대학병원의 병동근무 간호사 220명, 수술실근무 간호사 147명, 합계 367명으로 하였으며, 2012년 7월 1일부터 8월 31일까지의 기간 동안에 구조화된 무기명 자기기입식 설문조사를 실시하였다. 연구결과, 사회심리적 스트레스에 관련된 요인으로 병동근무 간호사에서는 연령, 주관적인 건강상태, 근무경력, 업무에 대한 만족도, 업무에 대한 적성, 업무요구도, 업무자율성, 동료의 지지도, 자기존중감, 통제신념 및 A형 행동유형이 지적되었고, 수술실근무 간호사에서는 연령, 수면시간, 주관적인 건강상태, 근무경력, 업무의 신체적 부담정도, 업무에 대한 만족도, 업무에 대한 적성, 직업전환의사, 업무요구도, 업무자율성, A형 행동유형이 지적되었다. 피로수준에 관련된 요인으로 병동근무 간호사에서는 연령, 여가시간 여부, 주관적인 건강상태, 업무에 대한 만족도, 직업전환의사, 업무요구도, 통제신념, A형 행동유형이 지적되었고, 수술실근무 간호사에서는 연령, 주관적인 건강상태, 업무의 신체적 부담정도, 상사의 지지도, 동료의 지지도, 통제신념이 지적되었다. 이와 같은 결과를 볼 때 간호사의 근무부서에 따른 스트레스와 피로수준에 관련된 요인에 차이가 있음을 알 수 있으며 근무부서별 스트레스와 피로수준을 적절하게 통제할 수 있는 프로그램개발 및 실행이 필요하다고 생각된다.

경락마사지가 편마비 환자의 손부종과 손기능에 미치는 효과 (The Effect of the Meridian Massage on the Hand Edema and Function of the Hemiplegic Patient)

  • Purpose: The objective of the study was to identify the effects of the meridian massage therapy on hand edema and hand functions in patients with hemiplegia. Method: The experiment was conducted in the Oriental Medical ward of "K" hospital during the period of 2000. 12. 15 - 2001. 03. 15. Fifty five subjects with hemiplegia following a stroke participated in the study. Volumetric size of the hand and the circumference of the index finger of each patient was measured and functional points were recorded for pre and post experiment. Result: 1. Apparent decrease in volumetric size of the hand and the circumference of the index finger(p=.022. p=.021), and higher functional points(p=.003) in the experimental group in comparison to the control group. 2.Volume of hands & their functions appeared to be irrelevant(r=-.195, p=.154). 3. Circumference of the index finger & their functions also turned out to be irrelevant (r=-.110, p=.424). Conclusion: Meridian massage is an effective nursing interventional therapy to relieve hand edema and improve hand functions in hemiplegic patients and has great potential for use in a wide range of medical fields as an efficient supplementary treatment for stroke rehabilitation.

발반사 마사지가 뇌졸중환자의 중추성 통증, 피로감 및 수면에 미치는 효과 (The Effects of Foot Reflexology Massage on the Central Pain, Fatigue and Sleep in Stroke Patients)

  • Purpose: This study was to evaluate the effects of foot reflexology massage on the central pain, fatigue and sleep in stroke patients. Methods: The study utilized a non-equivalent control group pretest and posttest design. Subjects were contacted at the oriental medicine ward of D hospital in B city for strokes. The subjects were 25 in the experimental group and 23 in the control group. Collected data were analyzed central pain, fatigue and the change of sleep between pre and post foot reflexology massage using a t-test, $x^2-test$, Fisher's exact test by SPSS 12.0 Win program. Results: There were significant decrease in the central pain, fatigue and change of sleep between pre and post foot reflexology massage. Conclusion: The results proved that foot reflexology massage was an effective stroke patients to decrease in the central pain, fatigue and change of sleep. Therefore, this foot reflexology massage should be effective as a stroke patients.

감염병전문병원의 시설 가이드라인에 관한 연구 - 운영방식과 건축계획을 중심으로 (A Study on the Establishment of Facility Guidelines for Infectious Diseases Hospitals - Focusing on Operational Methods and Architectural Planning)

  • Purpose: In terms of efficiency and safety, this study attempted to organize data on the operation methods and architectural planning of infectious diseases hospitals. Methods: The results obtained through on-site and interview surveys with hospital officials and medical staffs at four infectious diseases hospitals under construction were summarized based on those original business plans and facility guidelines. Results: First, the operational methods to secure safety and operational efficiency were summarized for each department which are major hospital functions of infectious disease hospitals. Second, as the architectural planning, the characteristics of space and circulation of each department are summarized. For safety of medical staff, negative pressure and non-negative pressure zone have to completely separated. In addition medical staff wears PPE and enters the negative pressure zone and returns in the order of admiral, shower, and gowning in the PPE undressing room after patient treatment. In case of operational efficiency, flexible operation is required in normal and crisis situations. For example, it is important for The Ward to gradually switch to negative pressure beds in times of crisis from normal situation and the outpatient department considers the composition of negative pressure and non-negative pressure outpatient spaces that can operate in parallel even in crisis situations. Implications: Infectious disease hospitals require flexible operation and appropriate facilities for normal and crisis situations.

낙상예방 프로그램이 뇌졸중 환자의 보행, 균형 및 낙상효능감에 미치는 효과 (Effects of Fall Prevention Program on Gait, Balance and Falls Efficacy in Stroke Patients)

  • Purpose: This study was conducted to determine the effects of a fall prevention program on gait, balance and falls efficacy in stroke patients Methods: The study utilized a non-equivalent control group pretest and post test design. Subjects were contacted at the oriental medicine ward of D hospital in B city for strokes. The subjects were 25 in the experimental group and 25 in the control group. The fall prevention program consists of fall related exercises and fall related education. Collected data were analyzed using a t-test, Chi-Square test, Fisher's exact test by SPSS 14.0 Win program. Results: The fall prevention program with exercises and education used in this study was found to be very effective in increasing gait, balance and falls efficacy in patients with stroke. Conclusion: The fall prevention program used in this study seems appropriate for stroke patients. Thus, further research to refine the efficacy and application of this program is warranted.

척추마취수술 환자의 수술 후 일상활동이 경막천자 후 두통 발생에 미치는 효과 (Effect of Post Operative Daily Activity on Post-dural Puncture Headache after Spinal Anesthesia)

  • Purpose: The purpose of this study was to compare and analyze differences in effects of postoperative daily activity on headaches in patients who underwent surgery under spinal anesthesia. Methods: The study was conducted with 219 adults, 20 years or older. Official approval (Approval number: KMC IRB 1434-01) was received from K university hospital clinical trials review board. The research design was a nonequivalent control group non-synchronized design with a daily activities group and the bed rest group. Data was collected after receiving written consent from the participants. Results: There were no participants in either group who experienced headaches. Changes in a physiological index were also not significantly different between the daily activities group and the bed rest group. Conclusion: The results indicate that allowing daily activities in the ward, rather than maintaining bed rest for 6 hours, the existing method of nursing care for the prevention of postoperative headaches, in spinal anesthesia patients, is not detrimental to these patients post operatively.
