"의방류취(醫方類聚)"의 인용서(引用書)에 관한 연구(1) (A study of the reference books of ${\ulcorner}$Classified Collection of Medical Prescriptions${\lrcorner}$)

  • ${\ulcorner}$Classified Collection of Medical Prescriptions${\lrcorner}$(${\ulcorner}$醫方類聚${\lrcorner}$ is a medical book which classifies and edits the existing books. First of all, it is not until finishing the study of reference books that we can study the ${\ulcorner}$Classified Collection of Medical Prescriptions${\lrcorner}$ in earnest. We did not take traditional non-systematic methods to study the reference books of ${\ulcorner}$Classified Collection of Medical Prescriptions${\lrcorner}$ systematically. We investigated the title of 'reference book's name' as a reference book and did not investigate re-reference books. The number of reference books are one hundred forty-two volumes. Most of the reference books are medical books; some books are about Taoism and Buddhism. The title of the reference books are given a name by various methods. But these methods have a lot of problems. First, the same title book misapply an alias, the original text name and an abbreviated name. Second, a chapter name misapplies reference books. Third, reference books misapplies a chapter name. Forth, the writer and book name misapplies reference books and so on. From quotation collection about ${\ulcorner}$Classified Collection of Medical Prescriptions${\lrcorner}$'s characteristic point fellows below in three kinds. First, it emphasizes medical therapy. Second, most of Song(宋) period, Geum Yuan(金 元) period of China reference books take place in here. During this period it had accepted studying theory from clinical accumulation and the result of re-clinical studies reception based on theory research, and also had accepted Geum Yuan's expansive theory. Third, by adding technical books of a professional assortment, it has raised its profession of division.

의료 정보 검사코드 표준화를 위한 LOINC 자동 매핑 프레임웍 (An Automatic LOINC Mapping Framework for Standardization of Laboratory Codes in Medical Informatics)

  • 전자의무기록(Electronic Medical Record, EMR)은 모든 검사 과정이 텍스트 기반의 데이터 형태로 저장되는 의료 분야의 의무기록 시스템을 의미한다. 그러나 국내의 전자의무기록 시스템은 각 의료기관마다 고유한 의료정보검사코드 형태를 이용하여 기록하는 방식으로 정보를 저장하기 때문에 병원 간의 의료검사 기록 형태들의 공유, 해석, 분석에 많은 문제점들을 가진다. 위의 문제들을 해결하기 위하여 표준화 되어 있지 않은 병원들의 검사코드들을 LOINC (Logical Observation Identifiers Names and Code)로 표준화하려는 연구들이 많다. 현재까지의 연구들은 로컬 의료정보검사코드를 수동으로 LOINC로 변환하는 방법이 연구되었다. 또한 대용량 의학 정보들을 다루기에 적절하지 않은 파일 기반에서 코드들을 관리하는 연구들이 이루어져왔다. 기존의 문제점을 해결하기 위하여 본 논문에서는 의료 용어 표준화 알고리즘을 제안하고, 구현하여 해결하였다. 또한, 대표적인 상용시스템이 가졌던 문제점인 검색어를 의사가 직접 생성해야 했던 부분을 LOINC 의 여섯 가지 자동 속성 추출 및 검색어 자동 생성 기능을 구현하여 해결하였다. 또한, 기존의 시스템들이 고려하지 않았던 대용량 데이터의 매핑 부분을 파일 시스템 기반이 아닌 데이터베이스 기반 검색 프레임웍을 구축하였다.

  • 행복한 삶의 질을 목적으로 하는 의료소비자가 증가하면서 웹에 분산되어 있는 블로그의 의료 정보를 바탕으로 신뢰성 있는 의료 시설을 선택하고 고품질의 의료 서비스를 받음으로서, 시간과 비용을 절약할 수 있는 O2O 의료 마케팅 시장이 활성화 되고 있다. 인터넷, 모바일, SNS 등에서 증가하는 비정형 텍스트 데이터는 전문 의료 지식 이외에 작성자의 관심, 선호, 예상 등을 직간접적으로 반영하고 있기 때문에 의료정보의 신뢰성을 담보하기 어렵다. 본 연구에서는 빅데이터 및 MLP를 사용하여 의료정보 블로그를 분류 (의료블로그, 광고블로그)함으로서 사용자에게 보다 고품질의 의료정보 서비스를 제공하는 블로그 판단 시스템을 제안한다. 제안된 빅데이터 및 머신러닝 기술을 통해 인터넷상에 존재하는 국내의 다수 의료정보 블로그를 종합, 분석한 후 질환별 개인 맞춤형 건강정보 추천 시스템을 개발한다. 이를 통하여 사용자는 자신의 건강문제를 지속적으로 점검하고 가장 적절한 조치를 취함으로서 자신의 건강 상태를 유지하는 것이 가능할 것으로 기대된다.

조선시대 주요 의료 관련 쟁점과 관심사 - 시행법령을 중심으로 (Major Medical Issues and Interests in the Joseon Dynasty - Focusing on Enforcement Laws)

  • Through this paper, all the provisions of the enforcement statutes stipulated in the Joseon's law code were investigated and major medical issues and interests in the Joseon Dynasty were analyzed. The characteristics of each period reviewed in the text are as follows. The early Joseon Dynasty is divided into three periods. First of all, Joseon filled the gap in the law with the active acceptance of the Ming Dynasty's law code, Daemyeongrul, which conformed to Confucian virtue. Next, the completion of Gyeonggukdaejeon was an opportunity to prepare the basis for Joseon's medical laws. Lastly, from the late 15th century to the 16th century, the existing medicine promotion measures and emphasis on hyangyak(domestic herb) continued. it can be said that Joseon's politicians needed a medical policy based on Confucian virtues and maintained state-led promotion policies, but on the other hand, there was no other alternative to try newly by reflecting the limitations and failures of the policy. The late Joseon Dynasty is also divided into three periods. First of all, the period from the late 16th century to the early 18th century was marked by the growth of families in technical positions. The era of King Yeongjo can be said to be the period of reorganization of medical related laws. Finally, the period after the late 18th century is a period of passive regulation and supplementation. Lastly, the revision of the actual medical law was not made or reflected in era of King Jeongjo. In the case of the early Joseon Dynasty, the policy shifted from state-led to families in technical positions. However, in the 19th century, the weakening of the royal authority led to the weakening of the overall administrative system of the country, and the pharmaceutical policy had to be limited.

Comparison of Urologist Satisfaction for Different Types of Prostate MRI Reports: A Large Sample Investigation

  • Objective: To evaluate urologist satisfaction on structured prostate MRI reports, including report with tumor-node-metastasis (TNM) staging (report B) and with Prostate Imaging Reporting and Data System (PI-RADS) score with/without TNM staging (report C, report with PI-RADS score only [report C-a] and report with PI-RADS score and TNM staging [C-b]) compared with conventional free-text report (report A). Materials and Methods: This was a prospective comparative study. Altogether, 3015 prostate MRI reports including reports A, B, C-a, and C-b were rated by 13 urologists using a 5-point Likert Scale. A questionnaire was used to assess urologist satisfaction based on the following parameters: correctness, practicality, and urologist subjectivity. Kruskal-Wallis H-test followed by Nemenyi test was used to compare urologists' satisfaction parameters for each report type. The rate of urologist-radiologist recalls for each report type was calculated. Results: Reports B and C including its subtypes had higher ratings of satisfaction than report A for overall satisfaction degree, and parameters of correctness, practicality, and subjectivity (p < 0.05). There was a significant difference between report B and C (p < 0.05) in practicality score, but no statistical difference was found in overall satisfaction degree, and correctness and subjectivity scores (p > 0.05). Compared with report C-b (p > 0.05), report B and C-a (p < 0.05) showed a significant difference in overall satisfaction degree and parameters of practicality and subjectivity. In terms of correctness score, neither report C-a nor C-b had a significant difference with report B (p > 0.05). No statistical difference was found between report C-a and C-b in overall satisfaction degree and all three parameters (p > 0.05). The rate of urologist-radiologist recalls for reports A, B, C-a and C-b were 29.1%, 10.8%, 18.1% and 11.2%, respectively. Conclusion: Structured reports, either using TNM or PI-RADS are highly preferred over conventional free-text reports and lead to fewer report-related post-hoc inquiries from urologists.

파킨슨병의 한의학적 고찰 -병인병리(病因病理)와 침구요법(鍼灸療法)을 중심으로- (Literature Review on Parkinson's Disease in Oriental Medicine)

  • Objective: In order to find oriental medical therapies on Parkinson's disease and to make a fundamental basis for clinical application, this study was performed. Methods: We reviewed 35 kinds of the ancient and modern text, and related articles. Results: Parkinson's disease is an extrapyramidal disease characterized by akinesia, tremor at rest, rigidity, and slowness of movement. In old oriental medical text, Parkinson's disease is described as Jinjeon(振顫), Chi, Gyeong (痙), Gyejong, Ryeon(攣) and Pyeongo(偏枯), Tantan, Jungbu(中腑症) of Jungpung(中風). According to the text, major pathological causes were Wind(風), Fire(火), Phlegm(痰), and Blood stasis(瘀血). And Parkinson's disease can be classified into four clinical types as liver & kidney yin-deficiency, qi & blood deficiency, retention of phlegm-heat, and qi-stagnation & blood stasis. Standardized acupuncture points are GV20, GB20, GV14 on head, CV12, ST25 on abdomen, GV26, ST7, GB1, S14, LI20 on face, LI4, LI11, TE5, SI3, HT3, LI15, SI6 on upper extremity, and ST36, GB34, SP6, LR3, KI1, GB30, BL40 on lower extremity. Other methods, such as scalp acupuncture, electro-acupuncture, and herb-acupuncture, can be applied to treat Parkinson's disease. Conclusions: We find out that there are oriental medical concepts related with Parkinson's disease such as Jinjeon(振顫), Chi, Gyeong (痙), Gyejong, Ryeon(攣) and Pyeongo(偏枯), Tantan, Jungbu(中腑症) of Jungpung(中風), of which major causes are Wind(風), Fire(火), Phlegm(痰), and Blood stasis(瘀血). We can also apply many effective acupuncture points and acupuncture therapies according to differential diagnosis, for example, liver & kidney yin-deficiency. qi & blood deficiency, retention of phlegm-heat, and qi-stagnation & blood stasis.

텍스트 분류 기반 기계학습의 정신과 진단 예측 적용 (Application of Text-Classification Based Machine Learning in Predicting Psychiatric Diagnosis)

  • Objectives The aim was to find effective vectorization and classification models to predict a psychiatric diagnosis from text-based medical records. Methods Electronic medical records (n = 494) of present illness were collected retrospectively in inpatient admission notes with three diagnoses of major depressive disorder, type 1 bipolar disorder, and schizophrenia. Data were split into 400 training data and 94 independent validation data. Data were vectorized by two different models such as term frequency-inverse document frequency (TF-IDF) and Doc2vec. Machine learning models for classification including stochastic gradient descent, logistic regression, support vector classification, and deep learning (DL) were applied to predict three psychiatric diagnoses. Five-fold cross-validation was used to find an effective model. Metrics such as accuracy, precision, recall, and F1-score were measured for comparison between the models. Results Five-fold cross-validation in training data showed DL model with Doc2vec was the most effective model to predict the diagnosis (accuracy = 0.87, F1-score = 0.87). However, these metrics have been reduced in independent test data set with final working DL models (accuracy = 0.79, F1-score = 0.79), while the model of logistic regression and support vector machine with Doc2vec showed slightly better performance (accuracy = 0.80, F1-score = 0.80) than the DL models with Doc2vec and others with TF-IDF. Conclusions The current results suggest that the vectorization may have more impact on the performance of classification than the machine learning model. However, data set had a number of limitations including small sample size, imbalance among the category, and its generalizability. With this regard, the need for research with multi-sites and large samples is suggested to improve the machine learning models.

송원(宋元)시대 기후환경변화가 질병과 의학발전에 미친 영향 (The Influence of Change in Climatic Environment during the Song-Yuan Dynasty Had on the Development of Medical Science and Disease Emergence)

  • It is known that the development of medical science is influenced by various social environmental factors. Historically, Chinese Medicine developed the most during the Song Yuan dynasty, and the reason for this was known to be due to socio-political factors. According to recent studies, however, this period also had severe changes in climate and environment. Therefore, this study was conducted under the premise that this change in climate and environment influenced medical development. When looking at the coldness of the 11th~12th century and data indicating warming before and after this period, the Song Yuan dynasty went through drastic periods of climate change. Therefore, diseases related to heat such as bubonic plague, measles, and malaria were common. Furthermore, due to occasional wars during the Song Yuan dynasty, social unrest was aggravated and infectious diseases spread due to land development and environmental pollution. As the health of people were threatened due to these factors, the printing and distribution of medical text were encouraged, and during this process, the great 4 doctors of Jin-Yuan appeared. The reason why they studied cures for infectious diseases due to heat was related to climatic environment change. The development in medical science is closely related to socio-political factors, however the change in climatic environments are inevitably related to disease emergence as well. Therefore, it should always be taken into consideration as an important factor that promotes development in medical science.

아토피 피부염의 주증상에 대한 문헌적 고찰 (A Literal Study of the Main Symptoms of Atopic Dermatitis)

  • Objective : To find the main symptoms associated with atopic dermatitis and to get the utmost out of information about cause, treatment are contribute in the treatment of atopic dermatitis rather than looking for ways through how to connect modern atopic dermatitis and a listed symptom of classic literature. Method : In this research, we recorded about the main symptoms of atopic dermatitis such as itch, erythema, edema, pus and furuncle. Also we interpreted original text by extracting a sentence containing the terms like red[赤], pus[膿], flare[發赤], dermatosis[溞], swelling[洋], light red[紅], itch[瀁], worm running[蟲行], juice[汁], rash[癮疹], variegation[疹斑] and summarized cause of symptoms, disease sign, prescription. Results : There are the cause of many symptoms accompanied by atopic dermatitis in the body that recorded unpreparedness[虛], Wind[風], Fever[熱], unpreparedness & interruption of vitality hole[氣穴不通], unpreparedness & Wind, Wind Fever[風熱], Wind Coldness[風寒], Wind Humidity[風濕], Humidity Fever[濕熱], Wind Humidity Fever, Wind of Lungs[肺風], Wind of Lungs and Spleen[脾肺風], the Kidney Wind poison[腎臟風毒], Wind of Lungs Fever, Worm[蟲], miscellaneous vitality[雜氣], unpreparedness and Fever of Heart[心虛熱], old sore of kidney poison[久瘡腎臟風毒].

  • Aversion to wind is a feeling of cold when exposed to wind; aversion to cold is a feeling of cold. The distinction between aversion to wind and aversion to cold is ambiguous because in greater yang disease the two terms seem to be used indiscriminately. It is, however, worth noting that "aversion to wind" does not occur in the lines presenting disease of the three yin. In this text, we render as "heat effusion" rather than "fever," since the Chinese term is somewhat wider in meaning than familiar English term, Heat effusion is associated with many conditions and occurs both in externally contracted disease and miscellaneous disease (雜病), disease due to causes other than external evils). In externally contracted disease of the three yang channels, heat effusion is a manifestation of the struggle between right qi and evil qi; it does not necessarily indicate the presence of evil heat. In diseases of the three yin, right qi is not strong enough to counter evil qi; hence heat effusion is absent, and instead only aversion to cold is present. Sweating occurs in a variety of patterns. A distinction is made between spontaneous and night sweating(自汗). Spontaneous sweating is so called because it occurs spontaneously without exertion. it has numerous causes. Night sweating(盜汗) is sweating during sleep that ceases on awakening.
