병원의 자본조달 행태와 투자효율 (Raise Capital Type and Investment Efficiency of Hospital in Korea)

  • This research analyzed Raise Capital type and Investment Efficiency for non-profit hospitals in Korea. 152 cases of financial information from 2007 to 2010 were utilized for analysis. As a result of analysis, Raise Capital for Borrowings to total assets was primarily used, taking around 40% on average, and the method of Raise Capital with significant difference among Medical Institutions was Liabilities in Excluded Borrowings to Total Assets and Capital Stock & Capital Reserves to Total Assets. Besides, the relation between Invested capital and Investment efficiency was opposite each other in the non-profit hospitals, and Region was an important element influencing over Productivity per Value Added. In addition, in the investment activity of non-profit hospitals in the light of Investment Efficiency, only hospitals among Medical Institution types had a character of Capital Intensive, and General Hospital and Geriatric & Long-term Medical Care Hospital among Medical Institution types showed a character of Labor Intensive in the light of Performance.

서비스산업별 서비스본질과 직무성과와의 영향 관계 연구: 인구통계학적 특성을 중심으로 (A Study on the Correlation between Service Nature by Service Industry and Job Performance: Focusing on Demographic Characteristics)

  • 본 연구에서는 서비스산업별 서비스본질과 직무성과와의 영향 관계를 확인하고, 인구통계학적 특성에 따라 서비스본질과 직무성과간의 영향관계에 차이가 있는지를 검증하고 그 결과를 논의하였다. 서비스본질인 관계성, 상호작용성, 수평성, 조화성의 증대가 직무성과의 향상에 영향을 미치는지 분석하였다. 또한 OECD의 서비스산업 분류를 반영하여 대표적 서비스산업 유형에 대해 서비스본질과 직무성과간의 관계를 분석하였다. 실증분석을 통해 도출된 본 연구의 결과는 다음과 같다. 먼저 서비스본질과 직무성과와의 영향 관계를 분석한 결과, 서비스본질의 상호작용성, 수평성, 조화성이 직무성과에 정(+)의 영향을 미쳤으나, 관계성과 직무성과와의 영향 가설은 기각되었다. 한편 대표적 서비스산업별로 서비스본질이 직무성과에 미치는 영향 관계를 분석한 결과, 운수업에서는 상호작용성과 조화성이 직무성과에 정(+)의 영향을 미쳤으며, 관계성과 수평성은 직무성과에 영향을 미치지 못하는 것으로 나타났다. 금융·보험업에서는 수평성과 상호작용성이 직무성과에 정(+)의 영향을 미쳤지만, 조화성과 관계성은 직무성과에 유의미한 영향을 미치지 못하는 것으로 나타났다. 숙박·음식업에서는 상호작용성과 수평성 및 조화성이 직무성과에 정(+)의 영향을 미쳤지만, 관계성은 영향을 미치지 못하였다. 의료·보건업에서는 상호작용성과 수평성이 직무성과에 정(+)의 영향을 미쳤지만, 관계성과 조화성은 직무성과에 영향을 미치지 못하는 것으로 나타났다. 인구통계학적 특성에 의한 분석의 경우, 금융·보험업에서 나이에 따라 상호작용성과 조화성에서 유의한 차이를 보였지만, 근속연수에 따라서는 수평성만이 차이를 보였다. 숙박·음식업에서는 근속연수에 따라 수평성만이 차이를 보였다. 의료·보건서비스업에서는 근속연수에 따라 관계성, 수평성, 조화성에서 차이를 보였으며, 결혼여부에서는 수평성에서만 유의한 차이가 나타났다. 향후 전체 산업군에 대해 국가별 비교 연구가 필요하다.

구급대원의 의복관리를 위한 감염관리 인지도와 수행도 (Infection control awareness and practice for clothing management in 119 emergency medical technicians)

  • Purpose: A descriptive study design was used to investigate infection management awareness and infection management performance for clothing management in 119 emergency medical technicians (EMTs). Method: 188 paramedics and basic EMTs completed questionnaires between March 1 and April 30, 2013 in Jeollabuk-do. Results: Participants' average score for infection control awareness was 2.69 and that for performance was 1.58, with the differences being statistically significant (t = 931.455, p = .003). Participants' general characteristics resulted in differences in infection management performance. Women (1.63) have scored higher than men (1.28), and the difference was statistically significant (t = 11.174, p = .001). Participants who had clinical experience (1.63) had higher scores than participants who did have clinical experience before (1.53) and, again, the difference was significant (t = 7.167, p = .009). Conclusion: Education program for infection management is need to be developed for more effective infection management.

무선랜 기반 e-Healthcare 시스템에서의 의료용 트래픽의 서비스 품질 보장을 위한 채널 접속 차별화 방안 (Differentiated Channel Access Scheme for Assuring QoS of Medical Traffic in WLAN-based e-Healthcare Systems)

  • The IEEE 802.11e EDCA (Enhanced Distributed Channel Access) mechanism has been proposed to improve the QoS (Quality of Service) of various services in WLANs (Wireless Local Area Networks). By differentiating the channel access delay depending on ACs (Access Categories), this mechanism can provide the relative service differentiation among ACs. In this paper, we consider that WLAN is deployed in medical environments to transfer medical traffic and we reveal that the quality of the medical traffic (in particular, ECG signals) is significantly deteriorated even with the service differentiation by IEEE 802.11e EDCA. Also, we analyze the reason for performance degradation and show that IEEE 802.11e EDCA has difficulty in protecting the transmission opportunity of high-priority traffic against low-priority traffic. In order to assure medical-grade QoS, we firstly define the service priority of medical traffic based on their characteristics and requirements, and then we propose the enhanced channel access scheme, referred to as DIFF-CW. The proposed scheme differentiates CW (Contention Window) depending on the service priority and modifies the channel access procedure for low-priority traffic. The simulation results confirm that the DIFF-CW scheme not only assures the QoS of medical traffic but also improves the overall channel utilization.

선택진료제도 개편에 대한 중요도-성취도 분석(IPA) - 서울시내 상급종합병원 의료종사자 인식을 중심으로 - (An Importance-Performance Analysis of the Healthcare Reform on a Doctor-Designation System - Focused on the Perception of Medical Providers' at Tertiary Hospital in Seoul -)

  • Purpose: The purpose is to understand the perception of medical providers at tertiary hospitals in Seoul on the importance-performance of the reform on a doctor-designation system, and to provide the policy suggestion of the perception on such a system. Methodology: To achieve the purpose, this study utilized structured survey tools to conduct a questionnaire survey of nursing, administrative and medical technology professionals at six tertiary hospitals in Seoul. An importance-performance analysis was utilized for an analysis that showed the perception of the reform on a doctor-designation system. Findings: First, it was identified that those medical providers had the highest perception of the importance about the performance of the reform on a doctor-designation system. Second, according to the result of action grid of an importance-performance analysis (IPA), it can be identified that in the 2nd stage of 'Concentrate here' included are the items as to design and effect of medical care quality and subsidies & patient infection and safety and control fees, effect of income security of the method for compensating for loss established by the government and offering sufficient information on general medical services and doctor-designation medical services. In the 1st stage of 'Keep up the Good Work', and the 2nd stage of 'Concentrate here' included is the item as to the performance of patients of the reform on a doctor designation system. Practical Implications: There could be identified the effect of the reform bill on the perception of medical providers. It is expected that a better environment would be provided for patients to use a hospital and for medical providers to offer medical services, if the goverment makes efforts to improve methods for compensating for loss and continuous monitoring of the performance of patients.

메디컬 환경에서 HTK를 이용한 환자 진료 정보서비스 설계 및 구현 (A Study on the Design and Implementation of Information Service of Patients using HTK in a Medical Environment)

  • 19세기 이후 다양한 분야의 과학기술이 거듭 성장하면서 의료분야에서도 보다 혁신적인 기술들이 높은 수준에까지 발전하고 있다. 융복합 기술의 촉진으로 인하여 실버계층을 위한 환자만족도 증진 시스템이 국내 메디컬 프로세스에 도입되고 있지만 기존 대형병원을 중심으로 한정적인 운용성을 유지하고 있으며, 서비스 시스템의 환경 여건 및 높은 경제적 비용으로 인하여 중소병원에서의 활용이 불가피한 실정이다. 그로인해 환자 만족도의 취약성 및 실버계층에 대한 서비스 접근성이 낮아지는 현상이 발생하게 된다. 본 논문에서는 HTK(Hidden Markov Model Toolkit)를 이용하여 일반 사용자는 물론 실버 계층과 같은 취약 계층까지 메디컬 서비스에 대한 사용자 접근성을 높일 수 있도록 하고 중소병원에서의 효율적인 환자 만족 서비스 관리가 가능하도록 하는 안드로이드 기반의 저비용 스마트 진료 정보서비스 시스템(Smart Medical treatment Information Service System)설계 기법을 제안한다.

119구급대 1급 응급구조사의 전문기도유지술과 자기효능감과의 관계 (The Relationship between Advanced Airway Management and Self-Efficacy by Level 1 Emergency Medical Technicians(EMT) in 119 Emergency Medical Service)

  • 본 연구는 119 구급대의 1급 응급구조사에 의한 심장 마비 환자에 대한 전문기도유지술 실시횟수의 관련 요인을 파악하였다. 광주소방 본부의 1급 응급구조사 95명을 대상으로 구조화된 설문지를 이용하여 일반적 특성, 직업관련 특성, 자기효능감, 전문기도유지술을 시행하는데 저해요인을 조사하였고, 2010년 1월 1일부터 2011년 6월 30일까지 전문기도유지술 실시한 자료는 광주 소방본부에서 제공받았다. 본 연구의 결과로, 전문기도유지술 관련 특성과 실시횟수와 상관관계를 분석한 결과, 필요를 느낀 횟수(r=0.397, p<0.01)와 자기효능감(r=0.419, p<0.01)이 상관관계를 보였고, 저해요인과 실시횟수와 상관관계를 분석한 결과 자신감(r=-0.036, p<0.01)과 경험문제(r=-0.405, p<0.01)가 상관관계가 있었다. 결론적으로, 전문기도유지술의 필요성을 제대로 인식하도록 하고, 자기효능감을 향상시킬 수 있도록 현장위주의 교육 프로그램과 인적자원관리에 대한 대책 마련이 필요할 것이다.

DRGs(Diagnosis Related Groups)별 환자집중도 수준에 따른 입원진료비와 재원일수의 차이 분석 (Is the Hospital Caseload of Diagnosis Related Groups Related to Medical Charges and Length of Stay?)

  • This study analyzes the effects of hospital caseload on medical charges and length of stay for inpatients. Hospital caseload, representing the level of concentration of patients, was measured with the Internal Herfindal Index for three diagnosis related group (DRG) codes (appendectomy, operations on anus, and operations on uterus and adnexa). Ordinary least squares regression was used for analysis. Results showed that medical charges per inpatient and average length of stay significantly differed with respect to hospital concentration indices, and that hospital caseload was inversely related to operational performance for appendectomy and operations on uterus and adnexa. The significant negative relationship between concentration index and length of stay may decrease the total medical charges. The results imply that the expansion of the DRG payment system to hospitals will have a negative influence on their gross sales.

보건의료정보관리 전공 학생의 임상실습 수행능력과 실습 만족도 (Clinical Practice Ability and Satisfaction of Clinical Training of Health-Medical Information Management Major Students)

  • Objectives : This study aimed to investigate the clinical practice ability and satisfaction of clinical training of health-medical information management major students. Methods : The data were collected from 68 persons from students finished clinical training at medical record (information) team using self administered questionnaires. The data were analyzed using t-test, ANOVA and correlation with SPSS 22.0 version. Results: Performance of data collection, data management, and data analysis were analyzed in three areas of the job area. In terms of academic characteristics and correlation, they were not related to the level of satisfaction with the practical experience. Conclusions : Research on a virtuous cycle clinical practice program that analyzes the factors by assessing the satisfaction level of clinical practice in each area of health care information management will be conducted continuously.

재가노인복지시설 급식소의 위생·영양관리 실태조사 연구 (Current Status of Sanitary and Nutritional Food Service in Elderly Day Care Center)

  • Objectives: This study was conducted to examine the status of foodservice management, with special interest on sanitary and nutritional food service in elderly day care centers. Methods: A total of 79 employees who managed foodservice facilities in elderly day care centers were included in the survey. The contents of the questionnaire consisted of general characteristics, importance and performance of sanitary and nutrition management, the reasons for poor performance, factors necessary for improvement, and the employee's demand for support. Data analysis was conducted using the SPSS v25.0. Results: Sanitary management showed an average importance score of 4.84 ± 0.40 and a performance score of 4.70 ± 0.61 (t-value: 8.260). The item with the lowest performance score was personal sanitary management (4.58 ± 0.71). In nutrition management, the average importance score was 4.52 ± 0.68, and the performance score was 4.20 ± 1.00 (t-value: 9.609). There were significant differences between the average score of importance and performance in both areas. As a result of an Importance-Performance Analysis, items that were recognized as important but had relatively low performance was "personal hygiene", "ventilation" and "food storage". Also in the nutritional management area, "menu planning for disease management" and "checking the saltiness in the soup" etc. had very low performance with low importance recognition. The items shown in the "low priority" quadrant were those that required professional management skills. In the areas that demanded support in foodservice management, education about sanitary and safe institutional food service had the highest score (4.42 ± 0.74), and all other items showed a demand of 4 points or more. Conclusions: Foodservice managers recognize the importance of foodservice facility management but performance is relatively low. Institutional support is, therefore, needed to improve performance. For items with low importance, it seems necessary to improve awareness of the necessity of these items and to provide education in this regard. To gradually improve foodservice management, continuous provision of education and training in these areas are of great importance.
