Comparison of the Graduate Medical School Student's Perception Structure about 'Happy Doctor' by Clerkship Experience (임상실습 경험에 따른 의학전문대학원생들의 '행복한 의사' 개념 인식 비교)

  • The purpose of this study is to provide direction of medical education by analysing medical school student's perception structure about 'happy doctor'. In particular, this study compared perception structure between two groups of students before clerkship and after clerkship. The subject of this study were 1~4 academic year students in medical school. Students' text about 'happy doctor' were collected by open-ended questionnaire and analyzed by using sematic network analysis. Based on the result of network analysis, perception structure of each groups were confirmed. The network of each groups have 'Professionalism' group including words such as 'patient', 'treatment', 'worthwhile' in common. Three groups, 'Professionalism', 'Quality of life' and 'Self-realization' constituted the before clerkship network. And five groups, 'Professionalism', 'Time with family', 'Balance between work and household', 'Interpersonal relationship', 'Physical and psychological health' constituted the after clerkship network. The results of this study is expected to contribute for developing the basic medical education curriculum for 'happy doctor'.

Redox Factor-1 Inhibits Cyclooxygenase-2 Expression via Inhibiting of p38 MAPK in the A549 Cells

  • In this study, we evaluated the role of apurinic/apyrimidinic endonuclease1/redox factor-1 (Ref-1) on the tumor necrosis factor-$\alpha$ (TNF-$\alpha$) induced cyclooxygenase-2 (COX-2) expression using A549 lung adenocarcinoma cells. TNF-$\alpha$ induced the expression of COX-2 in A549 cells, but did not induce BEAS-2B expression. The expression of COX-2 in A549 cells was TNF-$\alpha$ dose-dependent (5~100 ng/ml). TNF-$\alpha$-stimulated A549 cells evidenced increased Ref-1 expression in a dose-dependent manner. The adenoviral transfection of cells with AdRef-1 inhibited TNF-$\alpha$-induced COX-2 expression relative to that seen in the control cells ($Ad{\beta}gal$). Pretreatment with $10\;{\mu}M$ of SB203580 suppressed TNF-$\alpha$-induced COX-2 expression, thereby suggesting that p38 MAPK might be involved in COX-2 expression in A549 cells. The phosphorylation of p38 MAPK was increased significantly after 5 minutes of treatment with TNF-$\alpha$, reaching a maximum level at 10 min which persisted for up to 60 min. However, p38MAPK phosphorylation was markedly suppressed in the Ref-1-overexpressed A549 cells. Taken together, our results appear to indicate that Ref-1 negatively regulates COX-2 expression in response to cytokine stimulation via the inhibition of p38 MAPK phosphorylation. In the lung cancer cell lines, Ref-1 may be involved as an important negative regulator of inflammatory gene expression.

  • We can adapt telemedicine systems in advancement of information technology capabilities and increase of network bandwidth. The telemedicine service can be applied to a public health center, a school, a prison and islands in lacks of medical equipments and medical staffs. The telemedicine services which can be provided high quality medical services. We designed the multiple control server system consisting 3 sub-function, patients and doctors name list, network types, connection states and computer equipments. The telemedicine link configuration was decided as 'Flowing', or 'By-passing' in accordance the network type and bandwidth of patient systems or doctor systems. The multiple control server system was performed the best communication configuration over heterogeneous networks. This system was achieved high quality telemedicine services through dynamic wired and wireless networks at any time. This study represented a hybrid multimedia telemedicine system over heterogeneous networks. We expected that the designed system could provide not only the high quality services, tele-diagnosis and tele-consultation, but also the effective emergency telemedicine services to multi-patients in the heterogeneous network environments.

  • Purpose Interactive 3D visualization system through remote data transmission over heterogeneous network is growing due to the improvement of internet based real time streaming technology. Materials and Methods The current internet's IP layer has several weaknesses against IP spoofing or IP sniffing type of network attacks since it was developed for reliable packet exchange. In order to compensate the security issues with normal IP layer, we designed a remote medical visualization system, based on Virtual Private Network. Results Particularly in hospital, if there are many surgeons that need to receive the streaming information, too much load on the gateway can results in deficit of processing power and cause the delay. Conclusion End to end security through the network method would be required.

A Study on the Architectural Planning of the Spaces Related With Digital Network System in General Hospital (병원 정보화 관련 실(室) 계획에 대한 연구)

  • The digital network system is one of the most important features in the resent hospital buildings. This study is to lay out a guideline of architectural planning according to the hospitals with digital network system. First, this study develops the concept of hospital building digital network system and reports the survey results of domestic general hospitals for this matter. Required spaces by digital network system in general hospital are classified into two categories. One is equipment room, main equipment room and a telecommunication closet, to accommodate network facilities. The other is the spaces related with medical information, medical archives and the department of radiology, because digital network system makes a change the form and use of information. In result, this study proposes models for each equipment room and raises the possibility of size change of information-spaces.

  • Recent generation of wireless computing has focus on the integrating of exisitng technologies to enhance the mobile capabilities and developing a new approaches to meet the needs of the growing pool of applications. This paper describes an integrated IEEE802.15.4 wireless CDMA based healthcare system that interacts and received the data wirelessly from wireless medical devices of patient and forward to medical center by using the cellular network. Mobile application had been developed not only as the middle ware to handle the receive and transmit of medical data between wireless sensor network and cellular network but also provides the interface for monitoring and analyzing the health condition of patients continuously at cellular phone regardless of its physical location. This system thus enables the remote healthcare monitoring and supports medical data seamlessly roams between IEEE802.15.4 wireless network and CDMA network beyond and outside the hospital environment.

Protein-protein Interaction Network Analyses for Elucidating the Roles of LOXL2-delta72 in Esophageal Squamous Cell Carcinoma

  • Lysyl oxidase-like 2 (LOXL2), a member of the lysyl oxidase (LOX) family, is a copper-dependent enzyme that catalyzes oxidative deamination of lysine residues on protein substrates. LOXL2 was found to be overexpressed in esophageal squamous cell carcinoma (ESCC) in our previous research. We later identified a LOXL2 splicing variant LOXL2-delta72 and we overexpressed LOXL2-delta72 and its wild type counterpart in ESCC cells following microarray analyses. First, the differentially expressed genes (DEGs) of LOXL2 and LOXL2-delta72 compared to empty plasmid were applied to generate protein-protein interaction (PPI) sub-networks. Comparison of these two sub-networks showed hundreds of different proteins. To reveal the potential specific roles of LOXL2- delta72 compared to its wild type, the DEGs of LOXL2-delta72 vs LOXL2 were also applied to construct a PPI sub-network which was annotated by Gene Ontology. The functional annotation map indicated the third PPI sub-network involved hundreds of GO terms, such as "cell cycle arrest", "G1/S transition of mitotic cell cycle", "interphase", "cell-matrix adhesion" and "cell-substrate adhesion", as well as significant "immunity" related terms, such as "innate immune response", "regulation of defense response" and "Toll signaling pathway". These results provide important clues for experimental identification of the specific biological roles and molecular mechanisms of LOXL2-delta72. This study also provided a work flow to test the different roles of a splicing variant with high-throughput data.

Use of an Artificial Neural Network to Predict Risk Factors of Nosocomial Infection in Lung Cancer Patients

  • Statistical methods to analyze and predict the related risk factors of nosocomial infection in lung cancer patients are various, but the results are inconsistent. A total of 609 patients with lung cancer were enrolled to allow factor comparison using Student's t-test or the Mann-Whitney test or the Chi-square test. Variables that were significantly related to the presence of nosocomial infection were selected as candidates for input into the final ANN model. The area under the receiver operating characteristic (ROC) curve (AUC) was used to evaluate the performance of the artificial neural network (ANN) model and logistic regression (LR) model. The prevalence of nosocomial infection from lung cancer in this entire study population was 20.1% (165/609), nosocomial infections occurring in sputum specimens (85.5%), followed by blood (6.73%), urine (6.0%) and pleural effusions (1.82%). It was shown that long term hospitalization (${\geq}22days$, P= 0.000), poor clinical stage (IIIb and IV stage, P=0.002), older age (${\geq}61days$ old, P=0.023), and use the hormones were linked to nosocomial infection and the ANN model consisted of these four factors. The artificial neural network model with variables consisting of age, clinical stage, time of hospitalization, and use of hormones should be useful for predicting nosocomial infection in lung cancer cases.

The 18-kDa Translocator Protein Inhibits Vascular Cell Adhesion Molecule-1 Expression via Inhibition of Mitochondrial Reactive Oxygen Species

  • Translocator protein 18 kDa (TSPO) is a mitochondrial outer membrane protein and is abundantly expressed in a variety of organ and tissues. To date, the functional role of TSPO on vascular endothelial cell activation has yet to be fully elucidated. In the present study, the phorbol 12-myristate 13-acetate (PMA, 250 nM), an activator of protein kinase C (PKC), was used to induce vascular endothelial activation. Adenoviral TSPO overexpression (10-100 MOI) inhibited PMA-induced vascular cell adhesion molecule-1 (VCAM-1) and intracellular cell adhesion molecule-1 (ICAM-1) expression in a dose dependent manner. PMA-induced VCAM-1 expressions were inhibited by Mito-TEMPO ($0.1-0.5{\mu}m$), a specific mitochondrial antioxidants, and cyclosporin A ($1-5{\mu}m$), a mitochondrial permeability transition pore inhibitor, implying on an important role of mitochondrial reactive oxygen species (ROS) on the endothelial activation. Moreover, adenoviral TSPO overexpression inhibited mitochondrial ROS production and manganese superoxide dismutase expression. On contrasts, gene silencing of TSPO with siRNA increased PMA-induced VCAM-1 expression and mitochondrial ROS production. Midazolam ($1-50{\mu}m$), TSPO ligands, inhibited PMA-induced VCAM-1 and mitochondrial ROS production in endothelial cells. These results suggest that mitochondrial TSPO can inhibit PMA-induced endothelial inflammation via suppression of VCAM-1 and mitochondrial ROS production in endothelial cells.

  • One of current trends of medical technology is analog to digital. Moreover, The Government put a premium on PACS(Picture Archiving and Communication System) for Medical Insurance in November, 1999. Therefore many hospitals are installing(or planning to install) PACS facilities on their buildings. However most of hospitals didn't consider PACS facilities in the beginning of construction. In particular, the Networking is one of the most difficult things for PACS installation. There is no space for network equipments, and no path for network lines, etc. To solve these problems, wireless network is proposed. The wireless network is divided to three parts mainly. One is the wireless communication for Local Area Network, another is the data transmission through a satellite for Tele-Radiology, and the last one is the technology using Bluetooth for each sub-system of PACS. But there are other problems in wireless systems, i.e., network bandwidth, system stability, interference with other devices. The purpose of this paper is to suggest the future and new trend of PACS including all the problems.
