Intra- and inter-rater reliability of muscle thickness measurement of the tibialis anterior using different inward pressures

  • Objective: This reliability study examined the effects of applying varying induced inward pressures using a transducer placed at 0° neutral ankle position (NEU) and 15° ankle dorsiflexion (DF) on tibialis anterior (TA) muscle thickness using a custom-made device with a force indicator during rehabilitative ultrasound imaging. Design: Cross-sectional study. Methods: Twenty-four healthy subjects were recruited in this study. Two examiners measured the muscle thickness of the TA at 0° NEU and 15° DF in 3 conditions of inward pressures (1.0 N, 2.0 N, and 4.0 N) using a custom-made holder. The muscle thickness was measured three times for each of the conditions arranged in random order. For intra- and inter-rater reliability, the intraclass correlation coefficients (ICCs) with 95% confidence intervals, standard error of measurement, minimal detectable change, and coefficient of variation were analyzed. One-way repeated measures analysis of variance was conducted for investigating changes of TA muscle thickness according to the inward pressures of the transducers. Results: The intra-rater reliability of TA muscle thickness measurement was excellent (ICC3,1: 0.92-0.96) for all conditions (at both ankle joint angles per varying inward pressure). Likewise, the inter-rater reliability of TA muscle thickness measurement was excellent (ICC2,1: 0.89-0.97) under same conditions. The mean of TA thickness showed the trend of decreasing significantly with increased inward pressures at all ankle joint angles (p<0.05). Conclusions: Use of this custom-made device with a force indicator is useful to accomplish the high intra- and inter-rater reliability of TA muscle thickness measurement at both ankle joint angles in reducing the measurement error.

Existing 3D imaging devices are expensive devices, and there is a limit to their universalization. Due to the recent spread of inexpensive 3D cameras and the development of various measurement methods, various possibilities are being shown. It is expected to have a great impact on the medical device market that requires accurate data collection. Various medical device products using artificial intelligence are emerging, and accurate data collection is the most important to develop accurate artificial intelligence algorithms. Collection equipment using 3D cameras is expected to act as a major factor in the development of artificial intelligence algorithms using 3D images.

Performance evaluation study of a commercially available smart patient-controlled analgesia pump with the microbalance method and an infusion analyzer

  • Background: Patient-controlled analgesia (PCA) has been widely used as an effective medical treatment for pain and for postoperative analgesia. However, improper dose errors in intravenous (IV) administration of narcotic analgesics from a PCA infusion pump can cause patient harm. Furthermore, opioid overdose is considered one of the highest risk factors for patients receiving pain medications. Therefore, accurate delivery of opioid analgesics is a critical function of PCA infusion pumps. Methods: We designed a microbalance method that consisted of a closed acrylic chamber containing a layer and an oil layer with an electronic balance. A commercially available infusion analyzer (IDA-5, Fluke Co., Everett, WA, USA) was used to measure the accuracy of the infusion flow rate from a commercially available smart PCA infusion pump (PS-1000, UNIMEDICS, Co., Ltd., Seoul, Korea) and compared with the results of the microbalance method. We evaluated the uncertainty of the flow rate measurement using the ISO guide (GUM:1995 part3). The battery life, delay time of the occlusion alarm, and bolus function of the PCA pump were also tested. Results: The microbalance method was good in the short-term 2 h measurement, and IDA-5 was good in the long-term 24 h measurement. The two measurement systems can complement each other in the case of the measurement time. Regarding battery performance, PS-1000 lasted approximately 5 days in a 1 ml/hr flow rate condition without recharging the battery. The occlusion pressure alarm delays of PS-1000 satisfied the conventional alarm threshold of occlusion pressure (300-800 mmHg). Average accuracy bolus volume was measured as 63%, 95%, and 98.5% with 0.1 ml, 1 ml, and 2 ml bolus volume presets, respectively. A 1 ml/hr flow rate measurement was evaluated as 2.08% of expanded uncertainty, with a 95% confidence level. Conclusion: PS-1000 showed a flow accuracy to be within the infusion pump standard, which is ± 5% of flow accuracy. Occlusion alarm of PS-1000 was quickly transmitted, resulting in better safety for patients receiving IV infusion of opioids. PS-1000 is sufficient for a portable smart PCA infusion pump.

  • A micro-computer based system has been designed to evaluate the performance of infant incubators. The measurement system used a hybrid of analog electronics for amplification, integration and switches and micro-computer for data storage, data display, and control of relay. This approach has been applied to measure the warm-up time, temperature stability, temperature change due to open/close of door and portholes, and temperature distribution on the mattress. The micro-computer provides on-line access of multi-point temperature data.

  • Magnetoencephalography (MEG), the measurement of magnetic fields produced by neuronal current associated with normal and pathologic brain activities, is a totally noninvasive method for localizing functional regions of the brain. During the past several years, many clinical research centers are working to expand various fundamental functional brain regions, which can be easily localized, as well as to characterize magnetic abnormalities which accompany a wide variety of cerebral disease. At present, MEG is used in a number of clinical centers throughout the world for the presurgical functional localization of eloquent cortex, and for the non-invasive localization of epileptiform activity. And also, non-invasiveness means that it can be used for screening and repetitive follow-up measurement without concern for adverse effects. As procedures for activating various functional brain regions are standardized, and as the effects of specific cerebral diseases on the MEG are carefully documented in controlled studies, the number of routine neurological applications for MEG will increase significantly. In this paper, the basic principles of MEG are reviewed briefly with its clinical application to neurologic disease.

Trends in research on the image of emergency medical technicians in Korea (국내 응급구조사 이미지에 대한 연구동향 고찰)

  • Purpose: This study aimed to analyze the research trends on the image of emergency medical technicians (EMTs) in studies published in Korean journals. Methods: Electronic databases were searched, including RISS, KMbase, and KCI. Eight studies from 21 references screened were included in the analysis. Results: The number of studies related to the image of EMTs is severely lacking. The research methods and participants are limited to questionnaire surveys and EMT students, respectively. In addition, most of the studies used modified tools developed for image measurement of other occupations. Conclusion: Image research for EMTs should include various methods and participants. Standardized measurement tool development and image-related research should be steadily conducted.

  • An account book of medical treatment is a form of collection materials for diagnostic standardization, and it is a basis of standardization, standardization of medical records is a preconsideration of each standardization. But an account book of medical treatment is only a kind of form for recording medical treatment, therefore standardization of medical treatment eventually holds the key to the standardization of recording charts. However until now we have gradually reformed medical records in accordance with individual characters of medical treatment, and didn't have even standard sheme of medical records, also medical terms for medical records had an inconsistency of redescription and reiterative representation for an identical terms in all parts of the East learning, medical terms for medical records didn't unity. To make better this realities, standardization study used orginated system in the process of existing study, it can get ready the basis of discussion between O.M.D and O.M.D. it can make analysis of diagnostic course and can clearly understand usable information by diagnostic course. for that reason we hope that the basis of standardization is accomplished. And in advance of study for this standardization we have to analysis the course of medical treatment with demonstration of roof, first of all we have to study term definition by diagnostic course and prepare basis by diagnostic course. because this study have limits of indivisual study, it needs to long and synthetic investigation in Association levels. Although we cann't completely alternate with methods of measurement which relyed on individual mastery, if we exclude erroes of individual measurement through mechanization and verify results of diagnosis through keynotes, we can realize standardization of medical treatment with demonstration of proof and in this process we can use medical records as a tool collecting exact data, also we can realize standardization of drawing up medical records.
