Consideration of the Effect of Artifact during the Image Guided Radiation Therapy Using the Fiducial Marker (영상 유도 방사선치료 시 Fiducial Marker의 Artifact에 관한 연구)

  • Purpose: The effect of artifact was analyzed, which occurs from fiducial marker during the liver Image Guided Radiation Therapy (IGRT) using the fiducial marker. Materials and Methods: The size of artifact of fixed fiducial marker and length of mobile fiducial marker locus were measured using the On-Board Imager system (OBI) and CT simulator, and 2D-2D matching and 3D-3D matching were carried out, respectively, and at this time, the coordinates transition value of couch was analyzed. Results: The measurement of fixed fiducial marker artifact size indicated CT 4.90, 8.10, 12.90, 19.70 mm and OBI 5.60, 10.60, 14.70, 29.40 mm based on the reference CT slice thickness of 1.25, 2.50, 5.00, and 10.00 mm. Meanwhile, the measurement of mobile fiducial marker locus length indicated CT 42.00, 43.10, 46.50 mm, and OBI 43.40, 46.00, 49.30 mm. The coordinates transition of 1.00, 2.00, and 8.00 mm occurred between 2D-2D matching and 3D-3D matching. Conclusion: It was confirmed that the therapy error increased during IGRT due to the influence of artifact when CT slice thickness increased. Thus, it may be desirable to acquire the image less than 2.50 mm in slice thickness when IGRT is implemented using the fiducial marker.

  • In respiratory-induced proton therapy, the accuracy of tracking system and beam controlling is more important than photon therapy. Therefore, a high accuracy motion tracking system that can track internal marker and external surrogate is needed. In this research, our team has installed internal and external marker tracking system at our institution's proton therapy system, and tested the scanning with gating according to the position of marker. The results demonstrate that the developed in-house external/internal marker based gating system can be clinically used for proton therapy system for moving tumor treatment.

  • Purpose: The application of density override is very important to minimize dose calculation errors by fiducial markers of metal material in proton treatment plan. However, density override with actual material of the fiducial marker could make problem such as inaccurate target contouring and compensator fabrication. Therefore, we perform density override with surrounding material instead of actual material and we intend to evaluate the usefulness of density override with surrounding material of the fiducial marker by analyzing the dose distribution according to the position, material of the fiducial marker and number of beams. Materials and Method: We supposed that the fiducial marker of gold, steel, titanium is located in 1.5, 2.5, 4.0, 6.0 cm from the proton beam's end of range using water phantom. Treatment plans were created by applying density override with the surrounding material and actual material of the fiducial marker. Also, a liver cancer patient who received proton therapy was selected. We located the fiducial marker of gold, steel, titanium in 0, 1.5, 3.5 cm from the proton beam's end of range and the treatment plans were created by same method with water phantom. Homogeneity Index(HI), Conformity Index(CI) and maximum dose of Organ At Risk(OAR) in Planning Target Volume(PTV) as the evaluation index were compared according to the material, position of the fiducial marker and number of beam. Results: The HI value was more decreased when density override with surrounding material of the fiducial marker was performed comparing with density override with actual material. Especially the HI value was increased when the fiducial marker was located farther from the proton beam's end of the range for a single beam and the fiducial marker's position was closer to isocenter for two or more beams. The CI value was close to 1 and OAR maximum dose was greatly reduced when density override with surrounding material of the fiducial marker was performed comparing with density override with actual material. Conclusion: Density override with surrounding material can be expected to achieve more precise proton therapy than density override with actual material of the fiducial marker and could increase the dose uniformity and target coverage and reduce the dose to surrounding normal tissues for the small fiducial markers used in clinical practice. Most of all, it is desirable to plan the treatment by avoiding the fiducial marker of metal material as much as possible. However, if the fiducial marker have on the beam path, density override of the surrounding material can be expected to achieve more precise proton therapy.

Usability of 2D/2D Match for Image Guided Radiotherapy (IGRT) of Prostate Cancer with Fiducial Markers (전립선 암 환자의 영상유도 방사선 치료 시 Fiducial Marker를 이용한 2D/2D Match의 유용성에 대한 연구)

  • Purpose: To study the efficacy of marker match with using kilovoltage (KV) X-ray among multiple image guidance that referring tree fiducial marker in radiation therapy for prostate cancer patients. Materials and Methods: KV two-dimantional images (anterior-posterior, right-left) and cone-beam CT volumetric images were acquired after setup for patients with three fiducial markers. Compare the position of the fiducial marker of reference plan computed tomography (CT) and of KV, CBCT images; then decide the shift score of X, Y, and Z. This study executed 5 times on 10 patients and analyzed the shift value. Results: In the radiation therapy using fiducial marker, The function of marker match showed the same direction tendency as the CBCT, and showed X, Y, Z difference of about 0.6, 0.7, and 0.8 (unit: mm). Conclusion: Comparing to this, the result of shift value using 2D marker match showed less than 1.0 mm difference. The function of marker match is considered more useful in time-wise and effective dose rather than CBCT. Therefore, Both methods are used to treat patients for prostate cancer.

Evaluation of MT1XT20 Single Quasi-Monomorphic Mononucleotide Marker for Characterizing Microsatellite Instability in Persian Lynch Syndrome Patients

  • Background: Colorectal malignancies with high microsatellite instability (MSI-H), either hereditary (Lynch syndrome) or sporadic, demonstrate better prognosis and altered response to 5FU chemotherapy. It is now recommended to perform MSI testing for all new cases of colorectal cancer regardless of being categorized as hereditary or sporadic. For MSI detection, immunohistochemistry or PCR-based protocols using a cohort of various sets of STR markers are recommended. Here we aimed to evaluate a simplified protocol using just a single STR marker, MT1XT20 mononucleotide repeat, for detection of MSI in Lynch syndrome patients. A Promega five-marker MSI testing panel and immunohistochemistry (IHC) were used as the gold standard in conjunction with MT1XT20. Materials and Methods: Colorectal patients with a positive history of familial cancers were selected by evaluating medical records. Based on Amsterdam II criteria for Lynch syndrome 20 families were short listed. DNA was extracted from formalin fixed paraffin embedded tumour and adjacent normal tissues resected from the index case in each family. Extracted DNA was subjected to MT1XT20 mononucleotide marker analysis and assessment with a commercially available five marker MSI testing kit (Promega, USA). IHC also was performed on tissue sections and the results were compared with PCR based data. Results: Eight (40%), seven (35%) and five (25%) cases were MSI positive using with the Promega kit, IHC and MT1XT20, respectively. Among the markers included in Promega kit, BAT26 marker showed instability in all 8 samples. NR24 and NR21 markers showed instability in 7 (87.5%), and BAT25 and MONO 27 in 6 (75%) and 5 (62.5%). Conclusions: Although MT1XT20 was earlier reported as a valid standalone marker for MSI testing in CRC patients, we could not verify this in our Iranian patients. Instead BAT26 among the markers included in Promega MSI testing kit showed instability in all 8 MSI-H CRC samples. Therefore, it seems BAT26 could act well as a single marker for MSI testing in Iranian CRC patients.

  • The purpose of this study was to determine the possibility of sphingolipid as a diagnostic marker for Myocardiac Infarction(MI), atherosclerosis-related cardiovascular disease. Sphingolipids are known to playa role in the occurrence of atherosclerosis in human blood vessels. Component Analysis and Toxicity Study of Combined Cultivated Wild Ginseng Pharmacopuncture (산양산삼복합약침의 표준화 및 급성독성시험)

  • Objectives: The marker substances of cultivated wild ginseng pharmacopuncture that may not be detected during the process of steaming remain controversial. We developed a combined cultivated wild ginseng pharmacopuncture that contains all the marker substances. The aim of this experiment was to investigate the marker substances and test the toxicity of the combined cultivated wild ginseng pharmacopuncture. Methods: The marker substances were detected using HPLC. Intravenous injection toxicity studies were conducted at Medvill, an authorized institution for non-clinical studies, under the regulations of Good Laboratory Practice. We observed survival rates, abnormal behaviors, weight changes, gross findings in autopsy, blood biochemical properties, and histological abnormalities of organs such as the liver and kidney. Results: HPLC data showed that ginsenosides Rg1, Rb1, and Rg3 were detected at concentrations of 19.29, 47.64, and 3.02 μ g /ml, respectively. Administration of combined cultivated wild ginseng pharmacopuncture resulted in no dead animals or significant toxicological changes. Conclusions: The combined cultivated wild ginseng pharmacopuncture contains all the marker substances and is a relatively safe treatment medium. Further studies should be conducted to confirm the present findings.

The Effect of the Position Change of the Magnification Marker and the Stitching Range on the Image in the Examination of the Femur Radiography Using Stitching Technique (스티칭 기법을 이용한 대퇴골 방사선검사 시 확대 표식자 위치 및 스티칭 범위 변화가 영상에 미치는 영향)

  • In this study, we would like to examine the effect of the location of the magnification marker and the change in the stitching range on the magnification marker and the expansion of the anatomical structure in the examination of femoral stitching radiation using the stitching technique, and to find out the usability of the femoral stitching radiation examination in the preoperative examination for THRA. The measured values measured by 5 researchers were summed and presented through descriptive statistics. When the position of the magnification marker was changed, the average value of the measured value was in the range of 113.54 to 113.90 mm. The anatomical structures of the femur were measured in the range of 57.24 to 57.78 mm, 27.20 to 27.56 mm, and 70.11 to 70.81 mm, respectively. This showed a similar deviation from the expansion rate of previous related papers using markers. Therefore, it is considered that femoral stitching X-ray examination using magnification markers can be used for preoperative examination for THRA.

Consideration of the Effect according to Variation of Material and Respiration in Cone-Beam CT (Cone-Beam CT에서 물질 및 호흡 변화가 영상에 미치는 영향에 대한 고찰)

  • Purpose: Image Guided Radiation Therapy (IGRT) has been carried out using On-Board Imager system (OBI) in Asan Medical Center. For this reason, This study was to analyze and evaluate the impact on Cone-Beam CT according to variation of material and respiration. Materials and Methods: This study was to acquire and analyze Cone-Beam CT three times for two material: Cylider acryl (lung equvalent material, diameter 3 cm), Fiducial Marker (using clinic) under Motion Phantom able to adjust respiration pattern randomly was varying period, amplitude and baseline vis-a-vis reference respiration pattern. Results: First, According to a kind of material, when being showed 100% in the acryl and 120% in the Fiducial Marker under the condition of same movement of the motion phantom. Second, According to the respiratory alteration, when being showed 1.13 in the baseline shift 1.8 mm and 1.27 in the baseline shift 3.3 mm for acryl. when being showed 1.01 in 1 sec of period and 1.045 in 2.5 sec of period for acryl. When being showed 0.86 in 0.7 times the standard of amplitude and 1.43 in 1.7 times the standard of amplitude for acryl. when being showed 1.18 in the baseline shift 1.8 mm and 1.34 in the baseline shift 3.3 mm for Fiducial Marker. when being showed 1.0 in 1 sec of period and 1.0 in 2.5 sec of period for Fiducial Marker. When being showed 0.99 in 0.7 times the standard of amplitude and 1.66 in 1.7 times the standard of amplitude for Fiducial Marker. Conclusion: The effect of image size of CBCT was 20% in the case of Fiducial marker. The impact of changes in breathing pattern was minimum 13% - maximum 43% for Arcyl, min. 18% - max. 66% for Fiducial marker. This difference makes serious uncertainty. So, Must be stabilized breathing of patient before acquiring CBCT. also must be monitored breathing of patient in the middle of acquire. If you observe considerable change of breathing when acquiring CBCT. After Image Guided, must be need to check treatment site using fluoroscopy. If a change is too big, re-acquiring CBCT.
