Surgical Outcomes of Kommerell Diverticulum

  • Background: We aimed to assess the clinical outcomes of patients who underwent surgical repair of Kommerell diverticulum (KD) with individualized surgical methods. Methods: A retrospective analysis was performed of adult patients (aged ≥17 years) who underwent surgery to treat KD between June 2008 and October 2019. Results: Nine patients (median age, 45 years; range, 19-67 years; 7 men) underwent surgical repair. The indications for surgical therapy were acute aortic dissection in 2 patients, the presence of compressive symptoms due to dilated KD in 4 patients, and aneurysm growth in 3 patients. Various surgical techniques were used: (1) resection of the diverticulum stump and revascularization of the aberrant subclavian artery (n=3), (2) one-stage total-arch replacement including the diverticulum segment (n=3), and (3) hybrid repair (n=3). Early mortality occurred in 1 case of hybrid repair. Transient paraparesis occurred in a patient who underwent total arch repair as part of complicated acute aortic dissection. During follow-up (median duration, 30 months; range, 7-130 months), no late death or associated aortic complications were documented. All survivors were free from symptoms and had no abnormal findings on follow-up computed tomography. Conclusion: With a customized surgical approach and appropriate consideration of patient-specific anatomy and associated comorbidities, KD can be repaired with favorable outcomes.

Lumbar herniated disc: spontaneous regression

  • Background: Low back pain is a frequent condition that results in substantial disability and causes admission of patients to neurosurgery clinics. To evaluate and present the therapeutic outcomes in lumbar disc hernia (LDH) patients treated by means of a conservative approach, consisting of bed rest and medical therapy. Methods: This retrospective cohort was carried out in the neurosurgery departments of hospitals in KahramanmaraŞ city and 23 patients diagnosed with LDH at the levels of L3-L4, L4-L5 or L5-S1 were enrolled. Results: The average age was $38.4{\pm}8.0$ and the chief complaint was low back pain and sciatica radiating to one or both lower extremities. Conservative treatment was administered. Neurological examination findings, durations of treatment and intervals until symptomatic recovery were recorded. $Las{\grave{e}}gue$ tests and neurosensory examination revealed that mild neurological deficits existed in 16 of our patients. Previously, 5 patients had received physiotherapy and 7 patients had been on medical treatment. The number of patients with LDH at the level of L3-L4, L4-L5, and L5-S1 were 1, 13, and 9, respectively. All patients reported that they had benefit from medical treatment and bed rest, and radiologic improvement was observed simultaneously on MRI scans. The average duration until symptomatic recovery and/or regression of LDH symptoms was $13.6{\pm}5.4$ months (range: 5-22). Conclusions: It should be kept in mind that lumbar disc hernias could regress with medical treatment and rest without surgery, and there should be an awareness that these patients could recover radiologically. This condition must be taken into account during decision making for surgical intervention in LDH patients devoid of indications for emergent surgery.

난치성 간질에 대한 기능적 대뇌반구 절제술 및 Peri-insular 대뇌반구 절제술의 효과 (Surgical Results of Functional Hemispherectomy and Peri-insular Hemispherotomy)

  • Objective : To confirm the efficacy of functional hemispherectomy and peri-insular hemispherotomy on treatment of intractable epilepsy. Materials & Methods : From April 1997 to February 1999, we performed 1 functional hemispherectomy and 6 peri-insular hemispherotomy in 7 consecutive patients. These procedures result in completely disconnected hemisphere while maintaining the disconnected portion of the hemisphere intact within the surgical cavity. The indications were hemimegalencephaly in 2 cases, infarction with encephalomalacia in 2, Sturge-Weber syndrome in 1, hemiconvulsion hemiplegia epilepsy syndrome in 1, cortical dysplasia with leptomeningeal cyst in 1. Mean follow-up is 15.8 months(range 8-28 months). Results : Among 7 patients, 1 patient died immediately after peri-insular hemispherotomy. Five patients became seizure free with reduced doses of medications. One patient developed rare disabling seizure with medication. In 6 patients, there were improvements in the function of the hemiparetic limbs in the postoperative phase. A 3-year-old boy with infarction and encephalomalacia died few hours after surgery due to postoperative hypothermia. Two patients required shunt after surgery. Two patients developed postoperative brain swelling but were successfully managed with conservative care. Conclusion : In conclusion, functional hemispherectomy and peri-insular hemispherotomy may provide substantial seizure control in selected cases of young hemiplegic patients with intractable epilepsy.

노령인구의 요추질환에 대한 수술적 치료결과의 분석 (The Analysis of Surgical Results to the Lumbar Spinal Disorders of Aged Persons)

  • Objective : To retrospectively analyse the surgical outcome and efficacy of the lumbar spinal surgery in sixty to older patients who failed to conservative treatments. Methods : Between July 1990 and November 1996, the authors retrospectively investigated the medical records of 46 patients who over 60 years of age at the time of surgery. The clinical severity was assessed with Prolo's grade(economic and functional). Questionnaire was sent to each patient regarding long-term effect, satisfaction, and side effects. Results : In 46 patients, 2 patients(1 case died of lung cancer, 1 case lost in follow-up) were lost. Among 44 patients (28 men, 16 women ; mean age 64 years), 22 patients underwent partial or total laminectomy, 17 spinal fusion with instruments, 2 chemonucleolysis, 2 adhesiolysis for failed back surgery syndrome, and 1 automated percutaneous lumbar discectomy. Although postoperative complications were observed in 5 patients, they were successfully managed. No deaths were documented in the perioperative periods. The average Prolo's economic and functional grade improved from 2.98 to 3.48 and 2.81 to 3.75, respectively. Conclusion : In overall, the favorable surgical outcome was obtained. This results indicated that with appropriate preoperative selections and indications, careful intraoperative monitoring, and attentive postoperative care, the surgical treatment of eldery patients for the lumbar spinal disorders, significant improvement with acceptable levels of morbidity and mortality can be achived.

열다한소탕(熱多寒少湯)으로 양수(兩手) 진전(振顫), 안면(顔面) 상열감(上熱感), 어미삽(語微澁)을 치료한 태음인(太陰人) 환자 1예(例) (A Case Study of 1 Taeeumin Patient Experiencing Hand Tremor, Heat Flash, and Dysarthria Treated Successfully with Yeoldahansotang)

  • 1. Objective This case study reports a Taeeumin patient experiencing hand tremor, heat flash, and mild dysarthria who improved with Yeoldahansotang medication. 2. Method The patient's subjective and objective symptoms were observed daily, and the VAS scores for the main symptoms (hand tremor, heat flash, and mild dysarthria) were recorded daily throughout hospitalization period. 3. Result The patient's general conditions improved by the end of hospitalization, and VAS score dropped dramatically from VAS 10, VAS 10, VAS 10 to VAS 0, VAS 0.5, VAS 3 for hand tremor, heat flash, and mild dysarthria, respectively. 4. Conclusion The patient responded well to Yeoldahansotang, one of the main medicaments for the Joyeol (dry-heat) symptomatic pattern, which indicates that Yeoldahansotang can be used to treat the Joyeol symptomatic pattern even when the textbook indications are not evident as long as the Joyeol pathology is recognizable from other symptoms.

아편 유래 약물에 대한 문헌적 고찰 (Literary investigation on the origin of poppy and other narcotics Research Articles)

  • Objectives : This study was performed to developing orally administered analgesics and locally injected pharmacopuncture analgesics like opioids. Methods : Literary investigation on the origin of poppy (Papaver somniferum L) and other narcotics was conducted to examine the potential of developing orally administered analgesics and locally injected pharmacopuncture analgesics. Opium is a gum-like mass derived from air-dried white fluid of immature fruit of the poppy. Opium contains approximately 20 types of alkaloids including morphine, codeine, thebaine, papaverine and others. Natural opioids and synthetic alkaloid derivatives are the constituents of opioid analgesics and their effects and side-effects depend on the peculiarities of receptors. An extreme caution is required in the selection of proper dosage, proper analgesic types, and indications for successful pain management. Results and Discussion : With the enactment of "Narcotic control protocol", herbs such as cannibis and poppy are no longer available for use by Korean medicine doctors, and these doctors are faced with difficulty in managing severe pain in the clinical environment. A systematic consideration is inevitable for overcoming the limitation on these analgesics.

Three-dimensional intraoperative computed tomography imaging for zygomatic fracture repair

  • Objectives: Zygomatic complex (ZMC) fractures comprise up to 40% of all facial fractures. Misaligned bone fragments and misplaced fixation hardware traditionally detected postoperatively on plain radiographs of the skull might require re-operation. The intraoperative O-Arm (Medtronic, USA) is a three-dimensional (3D) computed tomographic imaging system. Materials and Methods: This retrospective single-center study evaluated the utility of O-Arm scanning during corrective surgeries for ZMC and zygomatic arch (ZA) fractures from 2018 to 2020. Three females and 16 males (mean age, 31.52 years; range, 22-48 years) were included. Fracture instability (n=6) and facial deformity (n=15) were the most frequent indications for intraoperative 3D O-Arm scan. Results: The images demonstrated that all fracture lines were properly reduced and fixed. Another scan performed at the end of the fixation or reduction stage, however, revealed suboptimal results in five of the 19 cases, and further reduction and fixation of the fracture lines were required. Conclusion: Implementation of an intraoperative O-Arm system in ZMC and ZA fracture surgeries assists in obtaining predictable and accurate results and obviates the need for revision surgeries. The device should be considered for precise operations such as ZMC fracture repairs.

First line Treatment of Traumatic Carotid Cavernous Fistulas Using Covered Stents at Level 1 Regional Trauma Center

  • Objective : The widely accepted treatment option of a traumatic carotid cavernous fistula (TCCF) has been detachable balloon or coils based fistula occlusion. Recently, covered stent implantation has been proving an excellent results. The purpose of this study is to investigate our experiences with first line choice of covered stent implantation for TCCF at level 1 regional trauma center. Methods : From November 2004 to February 2020, 19 covered stents were used for treatment of 19 TCCF patients. Among them, 15 cases were first line treatment using covered stents. Clinical and angiographic data were retrospectively reviewed. Results : Procedures were technically successful in all 15 cases (100%). Immediate angiographic results after procedure were total occlusion in 12 patients (80%). All patients except two expired patients had image follow-up (mean 15 months). Recurred symptomatic three patients underwent additional treatments and achieved complete occlusion. Mean clinical follow-up duration was 32 months and results were modified Rankin Scale 1-2 in five, 3-4 in five, and 5 in three patients. Conclusion : The covered stent could be considered as fist line treatment option for treating TCCF patients especially in unstable vital sign. Larger samples and expanded follow-up are required to further develop their specifications and indications.

한방이학요법 중 경피경근온냉요법에 대한 문헌적 고찰 (A Literatural Study on the Evidence of Using Thermotherapy and Cryotherapy of Cutaneous and Muscle Meridian in Korean Medical Physiotherapy)

  • Objectives : The purpose of this study is to establish literatural evidence about thermotherapy and cryotherapy for Korean medicine through literatural review. Methods : Applicable paragraphs which were related to the thermotherapy and cryotherapy of cutaneous and muscle meridian were phrased from in "Yibujicheng(醫部集成) and "Dongyibaojian(東醫寶鑑)" where were archiving of Oriental or Korean medicine literatures. Searched paragraphs were analysed for establishing historical and theoretical bases of thermotherapy and cryotherapy in Korean medicine. Results : Thermotherapy of cutaneous and muscle meridian(經皮經筋溫熱療法) such as hot pack, warm water therapy, paraffin bath, ultrasound is originated from yu(慰) warm water(溫水) hot water(熱水). Matching indications are various pain conditions(caused by coldness(寒), hard-work(僗若), extravasated blood(瘀血), inflammatory skin disease, frostbite and several internal diseases. It also treats gynecological diseases and facial palsy. Diathermic therapy on acupuncture points(穴位照射溫熱療法) such as infra-red, microwave, shortwave is originated from huolu(火爐), wenlu(溫爐), xianglu(香爐), lamp light(燈火). Its objective is to improve the effects of herb medicine by aiding sweating or to treat the residual symptoms of fever disease or to care skin disease and pain from bone fracture, contusion. Cryotherapy of cutaneous and muscle meridian(經皮經筋溫寒冷療法) such as ice pack, ice spray, iced whirpool, cool water bath is originated from lengfu(冷敷), lengtie(冷貼), lengshiyu(冷石熨). Matching indications are contusions, animal bite injury, corn(肉刺) and (淋病), eye disease, nasal bleeding, hemorrhoid, inflammatory skin disease and chicken pox. Conclusions : Thermotherapy and cryotherapy of cutaneous and muscle meridian(經皮經筋溫冷療法) are the treatments which were used in Korean medicine from the ancient Korean medicine. As scientific equipments were originated from yu(慰), huolu(火爐), wenlu(溫爐), xianglu(香爐), lamp light(燈火). lengfu(冷敷), lengtie(冷貼), lengshiyu(冷石熨). It can be said that these are elements of Korean medicine. More rigorous studies are needed to establish clinical evidence about not only thermotherapy and cryotherapy but also the other physiotherapy of Korean medicine.

화질의 국소적 변화를 고려한 의용화상처리 (Medical Image Processing with Local Variati on of the Image Quality)

  • 잡음을 포함한 저화질의용화상의 배경과 목적대상정을 분할하는 환경영역은 중요한 정보로 의학상 진단에 큰 의의를 갖고 있다. 이 논문의 목적은 화상의 농도변화를 정총화하여 통계적수법에 의해 환경영역을 결정하는 threshold를 구하는 방법을 제시하고 이를 비 scintigram에 적용하여 실험을 행했다. 전화상을 64개의 소영역으로 나누고 경계영역이 존재하는 부분온 선택하며 이 부분에 maximum likelihood법을 적용하여 threshold를 결정한뒤 내삽법에 의해 전화소에 대한 threshold를 구하고 수곽을 포함한 2식화면을 구했다. 이의 결과는 인간의 인식과 거의 같은 결과로 동적해석방법의 유효성이 증명되었다. The boundary has been one of the most important information in radiographic images and the degrees of difficulty involved varies greatly with the quality of the picture. These Buantifications are the means to diagnoses. The purpose of this paper is to quantify intensity variation and the threshold decision which is based on statistical principles and is developed to detect limits in liver scintigrams the entire picture is devide4 into 64 small regions. The kurtosis and variances for each smal region are used as indications to select the histograms the thresholds are computed according to the method o(maximum likelihood which minimizes the probability o( misclassification. Therefore Ive have demonstrated the applicability of the boundary detection and proved good agreement with human recognition, and we can use it for the diagnosis data of liver disease.
