"동의보감(東醫寶鑑)"을 이용한 "동의수세보원(東醫壽世保元)" 수록(收錄) 사상방(四象方)의 적응증(適應證) 확장에 관한 연구(硏究) - 보중익기탕(補中益氣湯)을 예(例)로 들어 - (Extended indications of Four-Constitution Medicine formula using "Dongeuibogam" - The case of Bojungyikgi-tang -)

  • Donguisusebowon(東醫壽世保元) doesn't have pretty large quantity of contents, so indications of Four-Constitution Medicine formulas on that medical book are small. Therefore, indications of Four-Constitution Medicine formula need to be extended for broader clinical use of Four-Constitution Medicine. There would be some methods to do that, and we could review the literatures that Four-Constitution Medicine formulas are based on. We reviewed Bojungyikgi-tang on "Dongeuibogam(東醫寶鑑)", and we found that if the Four-Constitution Medicine formula is based on Sanghan formula(傷寒方) or non-classical formula(後世方), it is worth adding more indications of base formulas to those of Four-Constitution Medicine formula. For exmaple, Indications of Bojungyikgi-tang for So-Eum type could be extended by indications of non-classical formula(後世方) Bojungyikgi-tang and modified formula(加減方) Bojungyikgi-tang on Dongeuibogam, We supposed 23 indications of non-classical formula(後世方) Bojungyikgi-tang, and 20 indications of modified formula(加減方) Bojungyikgi-tang as possible indications of Bojungyikgi-tang for So-Eum type.

  • Indications of Hyeongbangpaedok-san for Soyang-type on "Donguisusebowon" are small. Therefore, indications of Hyeongbangpaedok-san for Soyang-type need to be extended for broader clinical use. There would be some methods to do that, and we could review "Dongeuibogam" that Four-Constitution Medicine formulas are based on. We reviewed Hyeongbangpaedok-san, Insampaedok-san which is the origin of Hyeongbangpaedok-san, and their modified formula(加減方) on "Dongeuibogam". As a result, we supposed 12 indications of non-classical formula(後世方) Hyeongbangpaedok-san, 2 indications of modified formula of non-classical formula Hyeongbangpaedok-san, and 12 indications of Insampaedok-san and its modified formula as possible indications of Hyeongbangpaedok for Soyang-type.

119 구급대에 의해 이송된 환자들 중 직접적 의료지도가 필요한 범위와 그에 따른 수요 추정 (Indications and Estimations of the Needs for Direct Medical Control in the Patients Transported by 119 Rescuers)

  • 응급의료체계에서의 의료지도는 필수적인 요소로 인정되어지고 있다. 응급의료에관한법률에 의해서도 응급구조사에 의한 응급처치 시 의사의 지도를 필수적인 것으로 규정하고 있다. 그러나 법에서 정하는 응급처치의 범위 외에 119 구급대에서조차 의료지도가 필요한 상황에 대한 구체적인 기준이 마련되어 있지 않고 지도의사의 자격이나 역할, 책임과 권리 등에 대한 구체적인 내용이 없어 소방에서 2004년 1월부터 서울 119에서 서울종합방제센터에 지도의사를 두고 직접적 의료지도를 담당하고 있으나 그 이용이 활성화되어 있지 못하다. 이에 서울종합방재센터의 의료지도 사례를 바탕으로 의사의 직접적 의료지도가 필요한 범위를 설정하고 이를 토대로 의료지도의 수요를 추정하여 의료지도체계를 확립하는데 필수적인 자료를 제공하고자 이 연구를 시행하였다. 이에 2004년 11월 한 달 동안 서울종합방재센터 의료지도실에 의뢰되어진 의료지도 793건을 사례집단으로 직접적 의료지도가 필요한 기준의 타당성을 검증한 결과 직접적 의료지도가 필요한 기준으로 환자의 상태에 따른 조건, 응급처치에 따른 조건과 사고의 종류에 따른 조건으로 구분하여 각 항목의 직접적 의료지도 필요는 비정상적인 의식상태, 산소투여 등 6가지 응급처치를 시행한 경우, 교통사고 등 4가지 손상기전과 분만이 직접적 의료지도가 필요한 환자의 조건으로 설정되었고 서울 소방의 전체 구급대 이송환자 중 직접적 의료지도가 필요하다고 판단되는 환자는 45.9%에 달했다. 직접적 의료지도를 받아야 할 경우로 판단되었으나 실제 의료지도가 의뢰된 경우는 4.6%였다. 따라서 이상적인 직접적 의료지도 체계는 응급의학과 의사 등의 지도의사를 확보하고 각 응급의료기관과의 의사소통과정을 통해 구급대의 문제점을 개선하고 향상시키는 교육과 정책이 마련되어야 할 것이다.

한방물리요법에 대한 문헌적 고찰 (A Literatural Study on the Traditional Korean Medical Physiotherapy)

  • Objectives : The purpose of this study is to establish literatural evidence about traditional Korean medical physiotherapy through literatural review. Methods : Applicable paragraphs which were related to the traditional Korean medical physiotherapy were phrased from in "Hwang-je-nae-gyung(黃帝內經)", "Yumun-sa-chin(儒門事親)", "Geum-guei-yo-ryack(金匱要略)", "Eui-hack-ip-mun(醫學入門)", "Gyung-ack-jeon-seo(景岳全書)", "Eui-hack-gang-mock(醫學綱目)", "Dong-eui-bo-gam(東醫寶鑑)" where were archiving of Oriental or Korean medicine literatures. Searched paragraphs were analysed for establishing historical and theoretical bases of Korean medical physiotherapy. Results : 1. Theromtherapy is originated from yu(熨), hot hand, warm, water bath, warm earth, fumigation, lamp, torchlight and brazier. Matching indications are various musculoskeletal pain and diseases, skin diseases such as chronic inflammation, frostbite, gynecological and urogenital diseases such as uterine myoma, lower abdominal pain, metrorrhagia, breast diseases, digestive tract diseases such as vomiting, diarrhea, stomachache and also it treats cardiovascular disease. Other matching indications are sunstroke, rhinorrhea, psycosis and anal pain. And it used helping acupuncture therapy. 2. Cryotherapy is originated from lengfu(冷敷), lengtie(冷貼), cold stone and cold water. Matching indications are various fever disease, skin diseases such as acute inflammation, wound, fever skin disease, otorhinolaryngological diseases such as acute sore throat, otitis, stomatitis, nasal bleeding and contusions, abrasions, burn and dysuria. In addition, it treats extremity coldness, chillness, stomachache caused to accumulate fever. 3. Physiotherapy by circulating meridian system is originated from manipulation such as pushing, massage and beating to therapy point. Matching indications are musculoskeletal diseases such as various muscular pain, heavy sensation, convulsion, urogenital diseases such as dysuria, lower abdomen pain, digestive tract diseases such as stomachache, and paralysis due to blocked meridian system. And other matching indications are psycosis, leprosy, hypoesthesia, acute stroke. In addition it helps other therapy methods. In addition, the power of manipulation classify to strengthen and reduce. 4. Physiotherapy by balancing meridian system is originated from sunshine, magnet, horn, bamboo and tube. Sunshine helps improving self's care and cure inflammatory skin disease. Magnet's matching indications are paralysis, various injuries, abscess, fever and eye disease. Cupping therapy treats to skin diseases and internal diseases such as tuberculosis. Conclusions : Traditional Korean medical physiotherapy was widely used in traditional Korean medicine since ancient time based on the traditional Korean medical principle and anatomical knowledge. With modern physiotherapy and traditional Korean medicine together, effects of treatment are enhanced. And traditional Korean medical physiotherapy is appropriated to world physiotherapy's expectation.

Discrepancy between Clinical and Final Pathological Evaluation Findings in Early Gastric Cancer Patients Treated with Endoscopic Submucosal Dissection

  • Purpose: Early gastric cancer cases that are estimated to meet indications for treatment before endoscopic submucosal resection are often revealed to be out-of-indication after the treatment. We investigated the short-term treatment outcomes in patients with early gastric cancer according to the pretreatment clinical endoscopic submucosal resection indications. Materials and Methods: We retrospectively reviewed the medical records of patients with early gastric cancer that met the pretreatment endoscopic submucosal resection indications, from 2004 to 2011. Curative resection rate and proportion of out-of-indication cases were compared according to the pre-endoscopic submucosal resection indications. Pre-endoscopic submucosal resection factors associated with out-of-indication in the final pathological examination were analyzed. Results: Of 756 cases, 660 had absolute and 96 had expanded pre-endoscopic submucosal resection indications. The curative resection rate was significantly lower in the patients with expanded indications (64.6%) than in those with absolute indications (81.7%; P<0.001). The cases with expanded indications (30.2%) were revealed to be out-of-indication more frequently than the cases with absolute indications (13.8%; P<0.001). Age of >65 years, tumor size of >2 cm, tumor location in the upper-third segment of the stomach, and undifferentiated histological type in pre-endoscopic submucosal resection evaluations were significant risk factors for out-of-indication after endoscopic submucosal resection. Conclusions: Non-curative resection due to out-of-indication occurred in approximately one-third of the early gastric cancer cases that clinically met the expanded indications before endoscopic submucosal resection. The possibility of additional surgery should be empha-sized for patients with early gastric cancers that clinically meet the expanded indications.

데이터마이닝을 이용한 동의보감에서 경락의 주치특성 분석 (An Analysis of Indications of Meridians in DongUiBoGam Using Data Mining)

  • Objectives : DongUiBoGam is one of the representative medical literatures in Korea. We used text mining methods and analyzed the characteristics of the indications of each meridian in the second chapter of DongUiBoGam, WaeHyeong, which addresses external body elements. We also visualized the relationships between the meridians and the disease sites. Methods : Using the term frequency-inverse document frequency (TF-IDF) method, we quantified values regarding the indications of each meridian according to the frequency of the occurrences of 14 meridians and 14 disease sites. The spatial patterns of the indications of each meridian were visualized on a human body template according to the TF-IDF values. Using hierarchical clustering methods, twelve meridians were clustered into four groups based on the TF-IDF distributions of each meridian. Results : TF-IDF values of each meridian showed different constellation patterns at different disease sites. The spatial patterns of the indications of each meridian were similar to the route of the corresponding meridian. Conclusions : The present study identified spatial patterns between meridians and disease sites. These findings suggest that the constellations of the indications of meridians are primarily associated with the lines of the meridian system. We strongly believe that these findings will further the current understanding of indications of acupoints and meridians.

아토바스타틴의 새로운 약물 적응증 탐색을 위한 비정형 데이터 분석 (Analysis of Unstructured Data on Detecting of New Drug Indication of Atorvastatin)

  • Objectives: In recent years, there has been an increased need for a way to extract desired information from multiple medical literatures at once. This study was conducted to confirm the usefulness of unstructured data analysis using previously published medical literatures to search for new indications. Methods: The new indications were searched through text mining, network analysis, and topic modeling analysis using 5,057 articles of atorvastatin, a treatment for hyperlipidemia, from 1990 to 2017. Results: The extracted keywords was 273. In the frequency of text mining and network analysis, the existing indications of atorvastatin were extracted in top level. The novel indications by Term Frequency-Inverse Document Frequency (TF-IDF) were atrial fibrillation, heart failure, breast cancer, rheumatoid arthritis, combined hyperlipidemia, arrhythmias, multiple sclerosis, non-alcoholic fatty liver disease, contrast-induced acute kidney injury and prostate cancer. Conclusions: Unstructured data analysis for discovering new indications from massive medical literature is expected to be used in drug repositioning industries.

비정형 한의약텍스트 조제복용사항 정형화알고리즘연구 - 동의보감 처방정보를 중심으로 (A Study of the Algorithm that Standardizes Processing of Information and Taking Indications of East Asian Medicine Formula)

  • Currently, there are about 20,000 or so known ancient medical texts from the East Asian medical traditions. Although the most famous texts are widely known, many texts still exist only as original manuscripts. We are interested exploring these texts to uncover the potential benefits of their therapeutic knowledge. This study aims to develop a database program that automatically converts the treatment skills described in the text version into a more structured version. In the previous study, our team analyzed patterns in the way that treatment skills are described and then tried to design a database program algorithm that identified every meaningful keyword used to describe treatment skills and put that word in the right cell of a structured table. This study continues the development of this program. East Asian medical herbal treatment information is broken down into 4 elements: the first one is the name or title of treatment skills, and the second is the symptoms to which the treatment is applied, the third is ingredients used, the fourth is how information is processed and the indications taken. This study presents the algorithm's principles on how to analyze and structure the fourth element, the processing of information and taking of indications, which is described in a form of ancient natural language.

고압산소 치료에 대한 일개 권역 응급센터의 최근 10년 경험 및 제언 (Recent 10-Year Experience of One Regional Emergency Center and Recommendation for Hyperbaric Oxygen Therapy (HBOT))

  • Purpose: The aim of this study is to investigate current status, indications, and complications of hyperbaric oxygen therapy. Methods: A retrospective investigation of patients who underwent hyperbaric oxygen therapy at a university medical center from September 2004 to August 2013 was conducted based on patients' medical records and results of an email survey for 99 emergency centers. Results: During the study period, a total of 233 patients underwent hyperbaric oxygen therapy. Indications for hyperbaric oxygen treatment of illness or injury were as follows: 1) 151 cases of acute carbon monoxide poisoning(65.4%), 2) flap wound management, including 42 cases(18.2%), 3) skin care transplanted, including 23 cases(10.4%), 4) Burger's disease, including 5 five cases(2.1%), respectively. Total application time$^*$ frequency was 1,088 and total time was 1,239 hours. Among 233 patients who underwent hyperbaric oxygen therapy, 32 patients(13.7%) had complications: 1) otalgia in 21 cases(9.0%), 2) mastoiditis?in six cases(2.6%), 3) hemotympanum in five cases(2.1%), respectively. There were only 8 emergency centers that currently had an operational hyperbaric oxygen chamber in 77 emergency centers(10.4%). Conclusion: Indications identified through this study showed difference from current indications worldwide. It seems necessary that physicians' perception regarding application of hyperbaric oxygen therapy for more indications be changed and improved. A hyperbaric chamber capable of providing respiratory assistance and intensive care is also needed. A good network for sharing treatment experiences and a specialized team for administration of hyperbaric oxygen therapy is also required.
