Technical Aspects of Lung Transplantation: Pediatric and Lobar Transplantation

  • Fewer patients undergo pediatric lung transplantation (PLT) than adult lung transplantation. Size mismatch is the key factor that limits the availability of potential donors. Every candidate for PLT is in a different scenario in terms of age, height and weight, size of structures, indications for PLT, the concomitant presence of a cardiac anomaly, and other individual-specific factors; thus, a thorough understanding of pediatric patients' medical problems is essential. Living-donor lobar lung transplantation (LDLLT) has only been performed once in Korea to date. However, since each step in the LDLLT is a well-established procedure, including intrapericardial lobectomy, lung procurement, and lobar lung transplantation, qualified surgeons and lung transplantation teams are competent to perform LDLLT in clinically necessary situations.

Current Status and Tasks of Faculty Development Programs for Medical Education in Korea (국내 의학교육 교수개발 프로그램 현황과 과제)

  • This study aimed to investigate the current status of faculty development (FD) programs operated by medical colleges and institutions in Korea, and to suggest future-oriented directions for FD. A survey was conducted targeting medical colleges and medical education institutions that operate FD programs. We investigated the reasons for selecting topics, program themes, program operation methods, longitudinal program status, program improvement and quality control methods, the evaluation of the program effects, the outcomes and problems of the programs, and opinions on the latest trends. Twenty-nine out of 40 medical colleges and three out of six institutions responded. Topics were selected based on an analysis of medical education trends and the educational environment in both groups. The most common program themes were assessments in medical colleges, and teaching/learning and curriculum themes in institutions. FD was perceived to induce professors' and administrators' interest in medical education and improve the quality of medical education. The most common program method was workshops. Three medical colleges and one institution had longitudinal programs. Participant surveys constituted the most common method of evaluating programs' effects. Difficulties in publicizing programs and inducing voluntary participation were the most common problems in both groups. New attempts for FD were perceived as the role of external institutions. Based on the results, it is necessary to develop a framework and quality improvement indications for FD programs in the future, and FD programs are expected to be developed through new initiatives, such as longitudinal programs and those focusing on the community of practice.

Corticosteroid-induced Avascular Necrosis of Talus after Cardiac Transplantation (심장이식 후 부신피질호르몬 투여로 인한 거골의 무혈성 괴사 - 1예 보고 -)

  • A vascular necrosis of the talus has frequently been reported following trauma because talus has no muscle insertions, sixty percent of the surface of the talus is covered by hyaline cartilage, takes only a small area for entrance of a blood supply. Osteonecrosis is also associated with a variety of nontraumatic disorders. There are many indications for steroid usage, patient with rheumatoid arthritis, systemic lupus erythematosus, chronic obstructive pulmonary disease, and status- post renal or cardiac transplantation may be on long- term steroid usage, osteonecrosis may develop. A vascular necrosis of the talus secondary to chronic steroid usage is an unusual case. Delay in detection of osteonecrosis may lead to fragmentation and collapse of the talar body. When pain on range of motion is present and conservative treatment have been exhausted, surgical treatment is indicated, that is, fusion of the ankle joint. However it is important that conservative treatment may prevent its various sequelae with early diagnosis because steroid - treated patients have a more operative risk and increased risk for postoperative infection. We report a rare case of corticosteroid induced avascular necrosis of talus after cardiac transplantation.

Radiotherapy for Ovarian Cancers - Redefining the Role

  • Radiation therapy in ovarian cancers has been considered an outdated concept for many years, mainly due to its toxicity and failure to show benefit in terms of survival. Chemotherapy has been extensively used after surgery for these cancers and it has almost replaced radiation therapy as an adjuvant treatment. Nevertheless, failures in ovarian cancers continue to occur even with the use of newer and effective chemotherapy regimens. About 70% patients demonstrate recurrence in the abdomen or pelvis after first line chemotherapy in ovarian cancers. With advances in technology and sophistication of radiation techniques, along with the molecular and biological knowledge of distinct histological subtypes, there is a need to redefine the role of radiation therapy. This review article focuses on the literature on use of radiation in ovarian cancers and its rationale and indications in the present day. For this, a literature pub med/medline search was performed from January 1975 to March 2014 to redefine the role of radiotherapy in ovarian cancers.

  • Objective : The Blood-Letting Therapy which gives an acupuncture and lets blood on the surface of body by using various needles is quite effective in chronic disease. SoMoon lays the theoretical foundation of Korean Medicine. This article could help improve the understanding of the blood-letting therapy in SoMoon and also paves the way for applying the therapy to treatment of the various diseases. Method : This study selected the texts correlated to indications of blood-letting, after collecting the sentences from SoMoon with the search words 'blood', 'stagnated blood' and so on. Conclusion : This study on the Blood-Letting Therapy includes the way and location to give an acupuncture, the general rules of prescription, needles, amount of blood-letting, running mechanism and attentions. The purpose of Blood Letting Therapy is to treat and cure the disease through Improve vital energy and Expel harmful energy by improving meridian function and harmonizing Um-Yang and Ki-Blood.

  • The Meridian and Collateral Diagram is one of the most important 圖像s of Traditional Korean Medicine. A 圖像 is a picture made on a two-dimensional surface using lines and colors to portray an object or an image. Meridian and Collateral diagram is a 圖像 of the human body with indications of acupoints and meridian passageways and have different names such as 經穴圖, 輸穴圖, 鍼灸圖, 明堂圖, 銅人圖, in accordance with its classification. The documental basis of the Meridian and Collateral Diagram is the Internal Classic and the very first Meridian and Collateral Diagram confirmed through textual evidence can be found in 葛洪's "抱朴子 雜應". The Korean 동인도 in existence today, called '銅人明堂之圖', exists in two versions; a hand-copied version and a wooden engraving block version. It displays all the locations of the acupoints located on the anterior side of the human body, labels the names of the acupoints, and specifically distinguishes 起始穴 and 終止穴 of the eight meridian vessels.

Rotator cuff repair: what questions are patients asking online and where are they getting their answers?

  • Background: This study analyzed questions searched by rotator cuff patients and determined types and quality of websites providing information. Methods: Three strings related to rotator cuff repair were explored by Google Search. Result pages were collected under the "People also ask" function for frequent questions and associated webpages. Questions were categorized using Rothwell classification and topical subcategorization. Webpages were evaluated by Journal of the American Medical Association (JAMA) benchmark criteria for source quality. Results: One hundred twenty "People also ask" questions were collected with associated webpages. Using the Rothwell classification of questions, queries were organized into fact (41.7%), value (31.7%), and policy (26.7%). The most common webpage categories were academic (28.3%) and medical practice (27.5%). The most common question subcategories were timeline of recovery (21.7%), indications/management (21.7%), and pain (18.3%). Average JAMA score for all 120 webpages was 1.50. Journal articles had the highest average JAMA score (3.77), while commercial websites had the lowest JAMA score (0.91). The most commonly suggested question for rotator cuff repair/surgery was, "Is rotator cuff surgery worth having?," while the most commonly suggested question for rotator cuff repair pain was, "What happens if a rotator cuff is not repaired?" Conclusions: The most commonly asked questions pertaining to rotator cuff repair evaluate management options and relate to timeline of recovery and pain management. Most information is provided by medical practice, academic, and medical information websites, which have highly variable reliability. By understanding questions their patients search online, surgeons can tailor preoperative education to patient concerns and improve postoperative outcomes.

Study on the Application of Established Korean Medical Classics in the Korean Pharmaceutical Affairs Act (약사법규 중의 기존한약서 활용에 대한 고찰)

  • Purpose: There is an issue in applying various principles introduced in established Korean medical classics to "Pharmaceuticals Approval, Notification and Review" of "herbal medicinal preparations" and "new drugs from natural products" that are used for western forms of medical treatment. Thus, an analysis of the origin, purpose, and application of established Korean medical classics in the Korean Pharmaceutical Affairs Act is essential. Methods: We collected data regarding the origin, purpose, and application of established Korean medical classics in the Korean Pharmaceutical Affairs Act, and classified them by periodical change and subjects. Results: Established Korean medical classics are applied as follows: 1) as criteria for Korean medicine distributors' sales of mixed herbal drugs (Pharmaceutical Affairs Act; since 1953), 2) as official compendiums for pharmacists' preparation of Korean medicine (Ministry of Health and Welfare's authoritative interpretation; from early 1970s to 1993), 3) as standards for oriental pharmacists' quality measurement of preparations (notification of the Ministry of Health and Welfare; since 1995), 4) as criteria for "Pharmaceuticals Approval, Notification and Review" of herbal medicinal preparations and crude drug preparations (notifications regarding drug approval process by the Ministry of Health and Welfare and the Ministry of Food and Drug Safety; since 1978), and 5) as standards for the quality of materials of health functional food (from 2004 to 2011). Conclusion: The application of Korean medical classics has been closely related with the change of the laws, regulations, and systems that are relevant to Korean medicine, and it seems to be more favorable for pharmacists than oriental pharmacists. Meanwhile, regulations that apply prescriptions that are recorded in Korean medical classics - dosage, indications, and preparation methods - as criteria for the approval of crude drug preparations for western medical treatment should be abolished.

  • An empirical formula that has been empirically shown to be effective for a particular condition can be said to add medicinal materials to an existing formula or to make a new prescription by the Korean medical doctors' empirical clinic. This dissertation aims to study the prescriptions according to A Confidential Document of A Distinguished Medical Family written by the writer's ancestor and to contribute to the development of Korean medicine. A Confidential Document of A Distinguished Medical Family is a medicinal recipe that is an heirloom of the Gyeongju Kim family for the eighth generation. And the date and the writer of the book are unknown. The contents of the book consist of 18 parts and are made up of about 300 medical recipes. Of these, 158 formulas and indications of cold damage, wind, urination, stool, jaundice, and internal injury are found in the book. From among these, 61 formulas was directly quoted out of The Precious Mirror of Oriental Medicine, 97 formulas are adjusted in proportion or combined. By the parity of reasoning of the above, I think that this book was published after The Precious Mirror of Oriental Medicine. I will continue to study other formulas except the formulas above mentioned.

Surgical Treatment of Spontaneous Pneumothorax (자연기흉의 외과적 치료)

  • The spontaneous pneumothorax occurs subsequent to a disruption in the continuity of the visceral pleura with escape of free air into the pleural space included primary & secondary pneumothorax that is unrelated to identifiable etiologies such as trauma. In. the 33 year period 1960 to 1993, the 230 cases of open thoracotomy were carried out for definitive treatment of spontaneous pneumothorax, at the Dept. of Thoracic & Cardiovascular Surgery, National Medical Center, Seoul, Korea. There were 193 men & 37 women. They ranged in age from 15 years old to 72 years old. The lesion site was on the right side in 117 and on the left in 97, the 16 cases were in bilateral lesions.Surgical indications included recurrence in 98 cases, persistent air leak in 68 cases, nonexpansion of the lung 37 cases, roentgenologically apparent bullae & blebs in 23 cases, bilateral lesions in 16 cases,combined hemothorax & prevent for recurrence in each 2 cases. The types of operation were bullectomy in 207 cases, wedge resection in 13 cases, decortication & B.P.F. closure in 6 cases,lobectomy in 2 cases, pneumonectomy, plication in each I case. The post operative complication developed in 18 cases[7.8 %], there was I case of death due to sepsis. We believed that open thoracotomy with resection or obliteration of blebs & pleurodes is provided the best protection against recurrence.
