양한방 진찰행위의 상대가치 비교분석 (A Comparative Analysis of RBRVS for a Doctor's Consultation in Western and Oriental Medicine)

  • This paper analysed the RBRVS for a doctor's consultation by measuring the time consumed in outpatient consultation, and compared the time among medical doctors, dentist, and oriental medical doctors. The time used in consultation could be a proxy for measuring RBRVS for medical services because it is the only common factor we observe in three different clinical settings. The results show that the optimal RBRVS for consultation is 183.22 for medical doctor, 99.12 for dentist, and 236.17 for oriental medical doctor. This implies the current fee schedule for consultation should be revised as 10,740 Won for a visit to medical doctor, 5,808 Won for dentist, and 13,832 Won for oriental medical doctor.

응급구조(학)과 학생들의 상담현황 및 상담요구도 분석 (An analysis of the actual status and need for counseling in paramedic students)

  • Purpose: This study aimed to examine the consultation status and request level of paramedic students. The findings were expected to offer basic data for establishing consultation plans that meet the demands of students. Methods: Questionnaires were distributed among 153 students at two universities, and analyses were conducted based on the responses of 140 students. Results: The preferred counselor was the advising professor. The most common response was that holding two consultation sessions per semester was the most ideal. Regarding topic of consultation, career and employment scored the highest preference. Conclusion: Students had high request levels for consultation. Given the diversity in the themes of consultation, schools, should improve the capacities of advising and other professors in the department for professional consultation.

In addition, we reviewed what psychiatrists should do during consultation.

Telemedicine for Real-Time Multi-Consultation

  • We introduce a new multimedia telemedicine system which is called Telemedicine for Real-time Emergency Multi-consultation(TREM), based on multiple connection between medical specialists. Due to the subdivision of medical specialties, the existing one-to-one telemedicine system needs be modified to a simultaneous multi-consulting system. To facilitate the consultation the designed system includes following modules: high-quality video, video conferenceing, bio-signal transmission, and file transmission. In order to enhance the operability of the system in different network environment, we made it possible for the user to choose appropriate data acquisition sources of multimedia data and video resolutions. We have tested this system set up in three different places: emergency room, radiologist's office, and surgeon's office. All three communicating systems were successful in making connections with the multi-consultation center to exchange data simultaneously in real-time.

정신과자문에 대한 타과 의사들의 태도 (Nonpsychiatric Physicians' Attitudes Toward Psychiatric Consultation)

  • The authors investigated 131 nonpsychiatric physicians' attitudes toward psychiatric consultation, using questionnaires. A comparison was made as regards psychiatric consultation from nonpsychiatric physicians over position(staffs vs. residents) and department(medical vs. surgical). These findings are as follows ; 1) 51.9 percent of nonpsychiatric physicians estimated that less than 30 percent of their patientshadpgychiatricproblems. 2) The percentage of the patients referred for psychiatric consultation were 30 or less than 30 percent 3) Staffs tried to refer their patients to psychiatric department for psychiatric consultation more frequently than residents. 4) Medical physicians tried to refer their patients to psychiatric department for psychiatric consultation more frequently than surgeons. 5) Psychiatric consultation was estimated to be most frequently requested for overt psychiatric symptoms(23.0%) and past history of psychiatric treatment(20.8%). 6) The most frequent causes of not referring to department of psychiatry were found to be the patients' rejection(46.8%) and non-psychiatric physicians' dissatisfaction with the results of consultation (22.2%). 7) Medical physicians tended to explain the reasons for psychiatric consultation more adequately than surgeons. 8) Residents more specifically wrote the reasons for psychiatric consultation on the chart than staffs. The results suggest that staffs are more active in psychiatric consultation than residents, whereas medical physicians are more active than surgeons. Thus, education should be more emphasized for surgeons and residents, especially for the latter for effective consultation-liaison activity. On the other hand, psychiatrists should try to improve nonpsychiatric physicians' dissatisfactions with the results of psychiatric consultations, which will positively change their attitudes toward psychiatric consultation.

  • 본 연구는 비대면 진료의 개념과 제도적 도입과정에 대해 살펴보고 비대면 진료를 긍정적으로 인지하는 기대 편익 변인과 부정적으로 인지하는 위험 변인들이 이용의도에 어떠한 영향을 미치는지를 PLS-SEM을 사용하여 각 잠재변수의 영향력을 실증적으로 검증하였고 연구 결과는 다음과 같이 요약될 수 있다. 첫째, 의료서비스의 질은 비대면 진료의 인지된 유용성과 인지된 용이성에 유의한 영향을 미치는 것으로 나타났다. 둘째, 접근성은 비대면 진료의 인지된 용이성과 비용 절감에 유의한 영향을 미치는 것으로 나타났다. 셋째, 비대면 진료의 기대 편익들은 이용의도에 유의한 영향을 미치는 것으로 나타났다. 넷째, 기능적 위험과 서비스 위험은 의료진 위험에 유의한 영향을 미치고 의료진 위험은 비대면 진료의 이용의도에 부(-)의 영향을 미치는 것으로 나타났다. 본 연구는 비대면 진료의 이용의도에 영향을 미치는 긍정적 요인과 부정적 요인을 고려하여 이용 의도에 미치는 영향을 도출하여 의료소비자들의 행태를 실증적으로 규명하였다는 점에서 그 의의를 찾을 수 있다.

응급의료센터로 전원된 환자의 진료의뢰서 표준화 및 충실도에 관한 연구 (A study on standardization & completion of transfer consultation record for patients transferred to emergency medical center)

  • The purpose of this research which was conducted by surveying the transfer consultation records from 360 medical institutions such as general hospitals, hospitals, clinics to the Emergency Medical Center at E University Hospital for six months(Jan. 1, 2000 - Jun. 30, 2000) are to standardize & complete transfer consultation record of hospitals at the 1st & 2nd referral level and to give patients transferred emergency medical center medical information services on a better quality. The conclusions and suggestions from this study were summarized as follows; (1) Examing the distribution of the referral medical consultation(transfer) sheet type, surgery part local clinic sheet types were 34.4%, medical part local clinic sheet types were 26.7%, undifferentiated local clinic sheet types were 23.9% and hospital level sheet types were 15.0%. (2) The items of the transfer consultation records had been standardized more than 75% in the order of patient's name, date, doctor's name, diagnosis, patient's status, impressions. (3) That the degree of recording completion on these items is in the order of patient's name, date, diagnosis, impressions was revealed. (4) Because the standardization and the degree of recording completion are very low in the patient's gender, age, address, electronic recording system was needed for more perfect input of initial patient informations. (5) This standardizing & complete recording on examination and medication will prevent re-examination and abuse of medication for patients transferred emergency medical center. (6) EMT Transfer System should be fixed in all medical institute for the standardizing & complete recording on care period and departure time will give many emergency patients the proper treatments at the proper time. (7) It was revealed that developing new standardized transfer consultation record & using electronic recording system are needed. (8) The complete recording & Fast Track System were needed for higher rate of bed operation at emergency medical center and more hospital profit.

정신과 자문이 의뢰된 내과 환자들의 추적치료 결정요인 (Significant Variables Affecting Follow-up after Psychiatric Consultation in General Medical Units)

  • This study was performed to identify the significant variables affecting Follow-up after psychiatric consultation in General medical units and the trends of psychiatric consultation at the Hallym University hospital in Chunchon. Subjects of this study were 107 patients from April 1, 1998 to August 31, 1998, who were referred for psychiatric consultation during hospitalization. The results were as follows ; 1) Referral rate of General medical units was 7.5% and ordered Gastroenterology, Cardiology, nephrology, Other part. 2) The rate of total follow-up was 58% and the rate of No-follow-up was 42%. 3) In the cases of referral timing over admission 8th days, the days hospitalized after consultation performed were lengthened, significantly. 4) Common reasons for no follow-up of consultation were 'early- discharge' and 'patient's unwillingness to psychiatric reconsult. 5) Significant variables affecting follow-up were 'treatment recommendation', 'concordance with psychiatric drug recommendation', 'concordance with laboratory recommendation', 'total days hospitalized', 'Days hospitalization after consult was performed' and 'referral reason'.

Development of a knowledge-based medical expert system to infer supportive treatment suggestions for pediatric patients

  • This paper discusses the design, implementation, and potential use of an ontology-based mobile pediatric consultation and monitoring system, which is a smart healthcare expert system for pediatric patients. The proposed system provides remote consultation and monitoring of pediatric patients during their illness at places distant from medical service areas. The system not only shares instant medical data with a pediatrician but also examines the data as a smart medical assistant to detect any emergency situation. In addition, it uses an inference engine to infer instant suggestions for performing certain initial medical treatment steps when necessary. The applied methodologies and main technical contributions have three aspects: (a) pediatric consultation and monitoring ontology, (b) semantic Web rule knowledge base, and (c) inference engine. Two case studies with real pediatric patients are provided and discussed. The reported results of the applied case studies are promising, and they demonstrate the applicability, effectiveness, and efficiency of the proposed approach.

섬망 및 우울장애로 자문 의뢰된 입원환자의 임상적 특징 및 의뢰 형태에 관한 연구 (A Study on the Characteristics and Consultation Type of Inpatients Referred for Delirium and Depressive Disorder)

  • 연구목적 본 연구는 섬망 및 우울장애로 정신건강의학과에 의뢰된 환자들의 인구학적 특성과 자문 의뢰 형태, 재협진 여부에 따른 차이를 알아봄으로써, 종합병원 입원환자에 있어 섬망 및 우울장애의 특징 및 자문 조정 정신의학의 개선 방향을 위한 자료를 얻는 것을 목표로 한다. 방 법 건국대학교병원에 2005년 8월 1일부터 2011년 12월 31일까지 입원한 환자들 중 정신건강의학과로 자문 의뢰된 4966명의 환자를 대상으로 전산화된 의무기록을 후향적으로 분석하였다. 자문의뢰형태는 정신과 과거력, 입원 시 주호소, 의뢰된 증상의 특성 등에 따라 각각 상호보완형, 병렬형, 보수형, 선행 과거력형, 오진형, 단순 재의뢰형의 총 6가지로 분류되었다. 결 과 우울장애 환자의 평균 나이는 57.0세, 섬망 환자의 평균 나이는 68.6세로 섬망 환자군에서 유의하게 더 높았다. 성비의 경우 우울장애 환자에서 여성이 66.7%, 섬망 환자에서 남성이 60.8% 로 각각 더 높았다. 입원환자의 과별 분포는 우울장애의 경우 내과계 환자의 비율이 67.7%, 섬망의 경우 외과계 환자의 비율이 50.1%로 각각 더 높은 비율을 차지하였다. 자문 의뢰 형태에서는 우울장애의 경우 병렬형-상호보완형-보수형의 순서를 보였고, 섬망 환자의 경우 보수형-병렬형-상호보완형 순서였다. 우울장애 환자군 내에서 재협진군과 1회 협진군을 비교해 보았을 때 재협진군에서 남성 비율이 유의하게 높았다. 자문 의뢰 형태 분석에서 재협진군은 병렬형-보수형-상호보완형의 빈도 순을 보였으며 1회 협진군에서 병렬형-상호보완형-보수형의 순서를 보였다. 섬망 진단 환자군 내에서 재협진군과 1회 협진군을 비교해 보았을 때 재협진군에서 외과계의 비율이 유의하게 높았다. 자문 의뢰 형태 분석에서 재협진군과 1회 협진군 모두 보수형-병렬형-상호보완형의 순서를 보였다. 결 론 정신과 자문 의뢰 형태를 분석한 결과 우울장애 환자군과 섬망 환자군에서는 연령, 성비, 자문 의뢰 형태, 재협진률 등에서 유의한 차이를 보였다. 이 같은 결과를 바탕으로 의료진은 환자를 입원시키는 시점부터 우울장애 및 섬망과 같이 호발하는 정신과적 공존 질환에 대해 정신건강의학과 자문을 통해 적극적으로 치료계획을 논의함으로써, 환자에게 이차적으로 발생할 수 있는 우울장애와 섬망 등의 위험 요인을 파악하고 조기에 개입하여 의료 서비스의 질을 향상시킬 수 있을 것이다.
