The Study for the Present Possession of CT and MRI and the Technologists's Awareness of Quality Control (대구지역 의료기관의 CT, MRI 보유 현황과 정도관리에 관한 담당 방사선사들의 인식 조사)

  • This study concerns the present possession of CT and MRI machines in Daegu and the technologists' awareness of Quality Control. the study was conducted by visiting forty medical institutions as well as a survey. The survey was carried out by technologists responsible for CT and MRI. CT and MRI machines have only been used in medical institutions in Daegu for a short period. The study revealed that most of them have been acquired by those institutions recently. Most of the technologists who participated in the study are aware of the (prescribed) Quality Control measures and conduct regular inspections in accordance with the quality control chart, however some of them do not.

The Accuracy of the ICD-10 Code for Trauma Patients Visiting on Emergency Department and the Error in the ICISS (응급센터에 내원한 외상 환자에 있어 ICD-10 (International Classification of Disease-10)입력의 정확성과 ICISS (International Classification of Disease Based Injury Severity Score)점수의 오류)

  • Purpose: We designed a retrospective study to measure the accuracy of the ICD-10 (International Classification of Disease-10) code for trauma patients. We also analyzed the error of the ICISS (International Classification of Disease based Injury Severity Score) due to a missing or an incorrect ICD-10 code. Methods: For the measuring the accuracy of the ICD-10 code for trauma patients in a tertiary teaching hospital's emergency department, two board certified emergency physician performed a retrospective chart review. The ICD-10 code was classified as a main code or a sub-code. The main code was defined as the code of the main department of treatment, and the sub-code was defined as a code other than the main code. We calculated and compared two ICISS for each patient one by using both the existing code and the other by using a corrected code. We compared the proportions of severe trauma (defined as an ICISS less than 0.9) between when the existing code and the corrected code was used respectively. Results: We reviewed the records of 4287 trauma patients who had been treated from July 2008 to November 2008. The accuracy of the main code, the sub-code of emergency department, main-code, the sub-code of hospitalized patients were 97.1%, 59.8%, 98.2% and 57.0%, respectively. Total accuracy of the main and sub-code of emergency department and of hospitalized patients were 91.4% and 58.6%. The number of severe trauma patients increased from 33 to 49 when the corrected code was used in emergency department and increased from 35 to 60 in hospitalized patients. Conclusion: The accuracy of the sub-code was lower than that of the main code. A missing or incorrect subcode could cause an error in the ICISS and in the number of patients with severe trauma.

A Study on Workers Exposed to Diatomaceous Earth Dust and Development of Pneumoconiosis in a Diatomite Factory (모 규조토 가공 업체의 규조토 분진 폭로에 의한 규조토폐증 유병에 관한 조사)

  • Diatomaceous earth, quarried from the remains of aquatic plants deposited millions of years ago, continues to be a very important raw material with many industrial uses. In its natural state, diatomaceous earth is an amorphous silica with no crystalline pattern. For many uses, however, it is calcinated and calcination converts a portion of the amorphous silica to a crystalline form, cristobalite which is far more fibrogenic. In a factory which produces calcinated diatomaceous earth, seven workers were proved as pneumoconiosis on 1991 and 1992. Authors reviewed medical chart and current status of them. Authors also examined thirty one subjects from the factory with questionnaire, physical examination, spirometry and chest radiography on August 13th 1993. The radiographs were independently interpreted by two radiologists and their findings were classified by International Classification of Radiography of Pneumoconiosis(ILO, 1980). Total and respirable dust of diatomaceous earth were measured on October 1993. The results were as follows; 1. Of 31 workers, 6 (19.4%) were diagnosed as diatomaceous earth pneumoconiosis. There was an increasing tendency in prevalence of pneumoconiosis as the duration of dust exposure gets longer. 2. There were no significant differences in age, smoking rate, alcohol drinking rate, and pulmonary function test results between cases and non-cases. 3. The means of total dust exposures at flour manufacturing, fire brick grinding and packaging, ceramic raws packaging processes exceeded Korean and ACGIH standards, $10mg/m^3$. Above results suggest that engineering controls, periodic environmental and medical surveillance are important for preventing pneumoconiosis in the diatomite factory.

THE RELATIONSHIP BETWEEN TRAUMA AND TEMPOROMANDIBULAR JOINT DISORDER (외상과 턱관절 장애 연관성에 관한 연구)

  • Objective : Trauma has been a controversial issue although it has been considered to be a major factor for the temporomandibular disorder(TMD). We evaluated the relationship between macrotrauma or microtrauma and TMD. Methods : This study was performed in patients with TMD undergoing treatment at SNUBH from October 2006 to January 2007. Sixty one male patients and 166 female patients(total 227) were included and the average age was 34 years(ranging from 14 to 85 years). We investigated the possible etiologic factors, diagnosis and treatment with the review of medical records and radiography. Chronic pain, depression, somatic score(including pain item), somatic score(excluding pain item) were evaluated on the basis of diagnostic index from the Research Diagnostic Criteria on TMD. Results : Eighteen patients(7.9%) out of 227 patients suffered from TMD as a result of macrotrauma. Ninety four(41.4%) patients had microtrauma and six patients(2.6%) had both macro- and microtrauma(etiologic factor). The main symptoms included pain. joint noise and mouth opening limitation while the other symptoms were headache and tinnitus. The patients had suffered from TMD for average 41 weeks (ranging from 1 to 480 weeks). 116 patients took splint as a major treatment. As a prognosis, 19 patients(8.4%) recovered completely. 26(11.0%) had improvement and 181(80%) had persistent symptoms. 1 patient(0.4%) underwent an arthroplasty. Diagnostic index from RDC chart showed that macrotrauma was the highest score(except depression score) among the other etiologic factors. Conclusion : This study showed that macro- and microtrauma can be considered to be the major etiologic factors of TMD, which also affect the chronic, depression and somatic discomfort.

Clinical courses and degradation patterns of absorbable plates in facial bone fracture patients

  • Background: Absorbable plates are widely used in open reduction and internal fixation surgeries for facial bone fractures. Absorbable plates are made of polyglycolic acid (PGA), polylactic acid (PLA), polydioxane (PDS), or various combinations of these polymers. The degradation patterns of absorbable plates made from different polymers and clinical courses of patients treated with such plates have not been fully identified. This study aimed to confirm the clinical courses of facial bone fracture patients using absorbable plates and compare the degradation patterns of the plates. Methods: A retrospective chart review was conducted for 47 cases in 46 patients who underwent open reduction and internal fixation surgery using absorbable plates to repair facial bone fractures. All surgeries used either PLA/PGA composite-based or poly-L-lactic acid (PLLA)/hydroxyapatite (HA) composite-based absorbable plates and screws. Clinical courses were confirmed and comparisons were conducted based on direct observation. Results: There were no naturally occurring foreign body reactions. Post-traumatic inflammatory responses occurred in eight patients (nine cases), in which six recovered naturally with conservative treatment. The absorbable plates were removed from two patients. PLA/PGA compositebased absorbable plates degraded into fragments with non-uniform, sharp surfaces whereas PLLA/HA composite-based absorbable plates degraded into a soft powder. Conclusion: PLA/PGA composite-based and PLLA/HA composite-based absorbable plates showed no naturally occurring foreign body reactions and showed different degradation patterns. The absorbable plate used for facial bone fracture surgery needs to be selected in consideration of its degradation patterns.

Comparison of Photobiomodulation Therapy Types for Adults with Chronic Pain

  • Purpose : Our study aimed to compare the effectiveness of low-level laser therapy (LLLT) and light-emitting diode therapy (LEDT) for chronic pain intensity reduction and body temperature increase in older adults with chronic pain. Methods : Overall, 144 of 332 participants' records were used in this retrospective chart review. The study was conducted at a private health center in Busan city and the integrative medical center of a tertiary care hospital in Daegu city, South Korea. Patients experiencing chronic pain for over 6 months were assigned to either the LLLT or LEDT group. Both groups underwent 16 sessions of phototherapy held twice a week for 8 weeks, with each session lasting 60 minutes. The primary outcomes for both groups were the mean visual analogue scale (VAS) scores and body temperatures in both groups. The secondary outcome was the correlation between changes in body temperature and pain intensity. Measurements were recorded at the baseline and at each follow-up session. Results : A decrease in pain intensity and an increase in body temperature (p<.001) were observed in both groups. There was a significant difference in the VAS scores and temperature changes between the groups (p<.001). Odditionally, there were significant differences in the patterns of change in the VAS score and body temperature between the groups as the sessions progressed (p<.01), and a strong inverse correlation between body temperature and pain intensity changes were observed (p<.01). Conclusion : The use of photobiomodulation therapy at a specific wavelength may improve pain severity and simultaneously increase the body temperature among elderly people with chronic pain.

Single-Center Retrospective Study on the Status of Korean Medicine Automobile Insurance Treatment and Usage of Main Pharmacopuncture (단일 한의원 교통사고 환자 258례에 대한 한방치료 효과 후향적 연구)

  • In South Korea, traffic accident patients can be treated under automobile insurance coverage. This study investigated the status of Korean medicine (KM) Automobile insurance system and usage status of main pharmacopuncture in KM clinic which reported that the largest number of automobile insurance fee claims among medical institutions. We surveyed 258 traffic accident patients who were treated at Namsangcheon KM clinic from 2014 to 2018 according to medical chart. The majority of the patients were male and thirties. In traffic accident situation, the highest distribution was car to car crash with 85.66% and 66.67% of the patients visited in the most acute phase. The most frequent treatment period was within 4 weeks and the number of treatments was 10 or less with 72.87%. In total results of treatment, the distribution was exellent with 10.08%, good with 46.90%, fair with 27.13% and poor with 15.89%, and the effective rate was 84.11%. The most frequent treatment period was within 4 weeks with 64.73% and the number of treatments was 10 or less with 72.87%. Of the 242 patients who received pharmacopunture, 91.5% were treated with HO, which was named after Honghwaja and TA, which was named after traffic accidents, and there were significant differences in the number of treatments and symptom improvement between the two groups. In this study, we confirmed the status of automobile insurance treatment and usage of main pharmacopuncture of single KM clinic with symptom improvement. This study can be regarded as one of the basis of KM treatment for the rapidly growing automobile insurance market.

Analysis of Treatment Effect According to the Period of Herbal Treatment in Cerebral Infarction Patients Admitted with Hemiplegia: Retrospective Medical Chart Review (편마비를 주소증으로 입원한 뇌경색 환자의 한방 치료 시기에 따른 치료 효과 분석 : 후향적 의무기록 분석)

  • Objective: This study was aimed at evaluating the effect and clarifying the treatment period of Korean medicine treatment for cerebral infarction. Method: This study was carried out on patients with hemiplegia who were hospitalized in the Department of Korean Internal Medicine of Jecheon Korean Medicine Hospital of Semyung University from June 2014 to May 2019. A retrospective study was performed on 253 patients who were diagnosed with cerebral infarction by brain CT or nuclear magnetic resonance imaging. Results and Conclusion: 1. Korean medicine treatment has a significant effect on improving the movement disorder and daily life independence of cerebral infarction. 2. In the group that started Korean medical treatment within one month after the onset of cerebral infarction, with the exception of MMSE-K, the indicators related to the movement disorder and daily life independence showed significant effect. 3. MMSE-K showed no statistically significant change in any of the patient groups. 4. Within three months after the onset date, the longer the period of treatment with Korean medicine, the better the symptom improvement of upper extremity movement disorder. 5. The longer the hospital stays, the better the symptom improvement of the lower extremity movement disorder. 6. The combination of Korean and Western medicine did not affect liver or kidney function.

Reversals in Decisions about Life-Sustaining Treatment and Associated Factors among Older Patients with Terminal Stage of Cardiopulmonary Disease (만성 심폐질환을 가진 말기 노인 환자의 연명의료 의사결정의 번복 및 관련 요인)

  • Purpose: The purpose of this study was to investigate the frequency, patterns, and factors of reversals in decisions about life-sustaining treatment (LST) among older patients with terminal-stage chronic cardiopulmonary disease. Methods: This was a retrospective correlational descriptive study based on medical chart review. De-identified patient electronic medical record data were collected from 124 deceased older patients with terminal-stage cardiopulmonary disease who had made reversals of LST decisions in an academic tertiary hospital in 2015. Data were extracted about the reversed LST decisions, LST treatments applied before death, and patients' demographic and clinical factors. Multivariate logistic regression analysis was used to identify the factors associated with the reversal to higher intensity of LST treatment. Results: The use of inotropic agents was the most frequently reversed LST treatment, followed by cardiopulmonary resuscitation, intubation, ventilator therapy, and hemodialysis. Inconsistency between the last LST decisions and actual treatments occurred most often in hemodialysis. One-third of the reversals in LST decisions were made toward higher intensity of LST treatment. Patients who had lung diseases (vs. heart diseases); were single, divorced, or bereaved (vs. married); and had an acquaintance as a primary decision maker (vs. the patients themselves) were significantly more likely to reverse the LST decisions to higher intensity of LST treatment. Conclusion: This study demonstrated the complex and turmoil situation of the LST decision-making process among older patients with terminal-stage cardiopulmonary disease and suggests the importance of support for patients and families in their LST decision-making process.
